Stephanie Buttermore is a fitness influencer who in 2019, popularized going “All In”.   She'd spent years tracking macros, cutting and bulking, and working to be as lean and muscular as possible.   But she was struggling with extreme hunger, irregular periods, and constant thoughts about food.   The “All In” approach, for her, meant eating to complete satiety, every meal.   Eating to satiety means eating until you feel completely content. You genuinely don't want to eat anything else.   Stephanie Buttermore did this for several months and at the beginning was eating over 5000 calories per day.   Her Youtube videos showed her VERY physically full, with a distended belly…but feeling satisfied and like she actually could stop thinking about food between meals.   She gained weight rapidly in the beginning, then hit a plateau a few months in, and about a year later seemed to lose some body fat naturally.   If you're struggling with binge eating, thinking about food and feeling hungry all the time…   …or you've lost your period and have a similar background to Stephanie Buttermore, you may wonder if this approach is right for you.   As an RD who works with women along the disordered eating/eating disorder spectrum, and have guided MANY clients through the all in process…   …I have some thoughts.   Who “All In” is right for: People who lost weight, got fit and are considered the “picture of health” - and that's when these symptoms started: People who've grown an enjoyment for nutritious food, an understanding of macros, and are in the habit of creating “balanced” meals. People with OK mental health, not dealing with severe mental health issues OR are working with a therapist that you find very effective and helpful at providing skills to cope with difficult emotions.   Who “All In” is NOT right for: Someone who is currently struggling at a very high body weight. You may have heard before that in order to get your period back with Hypothalamic Amenorrhea (aka a missing period due to under eating, overexercise and/or STRESS)...you need to exercise less.   With my clients, I actually recommend taking a FULL exercise break.   I believe this to be proven by the data we have on women who lose their periods due to HA, and the BEST path to regaining optimal hormonal health + become truly healthy and fit.   I took a 3 month exercise break back in 2018, and have walked hundreds of clients through this process.    I break down the what, how and why behind exercise breaks in this episode.   An exercise break = Temporarily stopping all structured exercise and only doing low-intensity exercise IF truly desired in the moment.   Taking an exercise break helps you get your period back by: Reducing cortisol (not suppressing the hypothalamus) Creating the proper calorie surplus to increase estrogen, leptin and insulin (sends signals to the hypothalamus: “we are fed”)

In today's episode Dani Sheriff shares her experience in the FAMM program, and how it has enhanced her professional practice. She also shares her birth story. Dani was pregnant during the FAMM program, and gave birth to her beautiful baby daughter during covid/lockdown times. Dani Sheriff is an Australian-Born, Austin based Podcaster, YouTuber and digital illustrator. She is the host of The Hypothalamic Amenorrhea Podcast and founder of The HA Society – a podcast and online community for women who care about getting their periods. She's just like every other person in the world, she struggles with her body image and self worth. Being obsessed with looking ‘the correct way' took so much energy both physically and mentally. She had absolutely nothing to show for it. No one was congratulating her or thanking her for impacting their lives. She didn't have an enjoyable social life or a menstrual cycle. Nothing. She lost both her time and her fertility. So she realized she needed to change. She started eating and resting and her body started to heal. With all this new-found time and energy, she picked up a pen and started to draw women with real bodies, having real thoughts and struggling through real barriers. And people liked it. They started thanking her for the impact that her work was having on them. That's when Dani realized she was starting to make an impact and found her calling. She then started podcasting and YouTubing to keep the message spreading. Since then, she's grown a following of women (and men) who are trying to free up mental and physical space so that they can actually do real work. She does this by creating art, podcasts and YouTube videos. That's her mission. And none of it had anything to do with making herself smaller or hungry. Samantha is an exercise and mindset coach, helping women take control of their fertility through the mind body connection so they can level up their life. My personal work is with orthorexia, and as a result we see a ton of HA clients and use No Period Now What as a recommendation for our clients to read while we work together. I find people with orthorexia can use this scientific understanding of what’s going on in their bodies to focus on recovery. A lot of my work is focused on how exactly to eat more and exercise less, which sounds simple but is incredibly complex. But for today’s episode I really want to dive into the bodily processes and science behind what is happening in your body when  your period is missing, which is why I asked the main author of this incredible resource, Dr. Nicola Rinaldi to come on the show today.   Nico has a PhD in computational biology from MIT. After graduating she worked for a biotechnology company while pursuing her dreams of a family, which were thwarted by a diagnosis of hypothalamic amenorrhea (no periods). Eighteen months of trying to conceive followed. She spent hours on research, using the knowledge gained to work on recovery. Concurrently, she tried the medical route to pregnancy, with multiple doctor visits, injections, and ultrasounds resulting only in failure. Ultimately, she was able to achieve a natural pregnancy. Since that time, Nicola has shared her knowledge of the path to recovery, helping hundreds of others achieve their dreams of womanhood and pregnancy. Jess openly shares her inspirational journey through HA, TTC, pregnancy/postpartum, body image, starting a business, being a mom, and all the in-between on her personal blog and Instagram, @thehealthysweetpotato. In this interview, Emma sat down with Jess to hear more details of her experience recovering from HA. Jess discusses the challenges of having to shift her lifestyle away from overexercising and undereating in order to recover her period in the hopes of conceiving. Dr. Rinaldi is a pioneer in the field of recovery from HA, an expert fertility consultant and such an amazing human being. She has set up multiple support groups for women trying to get pregnant, trying to get their period back and goes out of her way to spread information about HA so women can heal. In our experience there are five factors in acquiring HA. Eating habits, exercise, weight/weight loss, stress, and genetics. The good news is that HA is reversible in most cases." As I continue to navigate my recovery from years with an eating disorder I have made it a priority to share my learning with others because of how much time I've spent researching to support my own healing. This episode provides some information on the signs/symptoms of eating disorders, without emphasis on the labels for the various disorders (i.e: orthoexia, anorexia, bulimia, ED-NOS). We spend a lot of time diving into some side effects of the full recovery process! Then we move into a review of the Minnesota Semi-Starvation Experiment, which provided great scientific insight into how bodies respond to under-nourishment- which is a prevalent concept in the diet-culture all around us. This episode concludes with some tips for those who are currently struggling with disordered eating and/or an eating disorder, that I found really helped me commit and see through my recovery. It happens often when someone is under-fueling their bodies. It is a common condition with atheletes, but can affect anyone who menstruates and does not take in enough energy. While it might sound nice to not have a period, the hormones that cause menstruation affect many aspects of health and can lead to infertility if not addressed. Nicola Rinaldi, PhD has done a lot of research on HA, and experienced it herself. She now spends her time spreading the word about the causes and treatments for HA. Since experiencing hypothalamic amenorrhea (missing periods) herself, Dr. Rinaldi has been on a mission to spread awareness of the condition and how to recover. This book is a comprehensive resource that includes much of the medical and scientific research that underlies our current understanding of the triggers for amenorrhea, what steps to take for recovery, and treatments to use for recovery and pregnancy as needed. In addition, Dr. Rinaldi performed the largest survey to date of women who likewise experienced amenorrhea, and includes results from the survey answering many of the common questions women have, such as "How long will it take to recover?" In today's episode of the "Life. Unrestricted." podcast, you’ll hear my conversation with Lindsay Meisel from San Francisco. I’ve known Lindsay from the Facebook groups we're both in that are specifically for women who are dealing with Hypothalamic Amenorrhoea (or – in layman's terms – women who are missing their periods). Lindsay is one of the strong voices in those groups, and I always wanted to talk to her, since she has recovered from "HA", as we call Hypothalamic Amenorrhoea (to not have it be such a mouthful), and she is clearly very knowledgeable about this. Lindsay works as the head of content for Ava, a women's health technology company that makes a sensor bracelet that monitors the menstrual cycle. She was a long distance runner for over a decade, but gave it up to get her period back. You'll hear Lindsay talk about: – What set off her disordered eating and exercise behaviors – Why the lure of "reaching energy deficit" can be so addictive and dangerous – How she was only able to break that compulsion by actively changing her behaviors towards "reaching energy surplus" – Why she is angry at people in the medical field who give women terrible advice when it comes to issues like missing periods – Why her own doctor encouraged her to dismiss her HA, which kept her in her disorder for a number of years to come – How the stages of her disordered eating/exercise morphed over the years – What her breaking point was – How she started to wake up to the fact that she wasn’t healthy – Why, when she decided to start recovering, she unknowingly went about it in the completely wrong way – Why it was only when she read Nicola Rinaldi’s book "No Period. Now What?" that she finally got an idea of what real recovery looks like and what she had to do to "go all in" – How long it took her to break free (and feel free) from her exercise addiction – Why we often compensate not "formally exercising" by compulsively ramping up low-level movement – How she reframed her thought-patterns and taught herself to think about her recovery as her goal and mission to get healthy – How she started filling up her time and using her energy for different things than exercising – Why recovery without the mental work doesn’t work, and why a person with HA is never at "a healthy point“ with exercise and eating – Why for women with HA (coming from a history of overexercising, undernourishing) it takes a phase of letting the pendulum swing in the other direction (full rest and complete replenishment of the body’s depleted energy stores) – Why it’s important to stay "all in" for 4-6 months more after getting the menstrual cycle running again – How there really is no "cheating" in HA recovery since the "feel good hormones" of exercising are exactly what cause the hypothalamus to stay shut down – Why it’s completely normal to experience exhaustion and pretty extreme hunger in recovery – Why it is important to work on body image while recovering – How HA is impacting a person’s health in worse ways than one might suspect at first glance (bone loss, depression, anxiety, thin hair/nails, mood swings, irritability, low libido, lowered body temperature, sleep issues, etc.) – Why (even though many physicians might recommend it!) going on the birth control pill is NOT solving the problem of HA, but, in fact, only masking the problem by producing a so-called "withdrawal bleed" – Why she says that life "after recovery" is a million times better than before... ... Nicola is the author of the book, No Period. Now What? A book that came out of her own experience trying to conceive over an 18 month period. After receiving a diagnosis of hypothalamic amenorrhea, she took matters into her own hands doing research and using the knowledge she gained to eventually recover and bring her period back. She tried the medical route to pregnancy, with multiple doctor visits, injections, and ultrasounds but was not successful in her efforts. Missed, delayed, and irregular periods are often pushed aside, even by medical professionals. Doctors and other medical professionals are not taught about hypothalamic amenorrhea and the five factors that cause a loss of a menstrual cycle. An energy deficit is the biggest one, and can cause significant damage to bone health in both men and women, but it is not the only cause. Running, stress, weight loss, and genetics can all play a role in why running causes periods to stop. In this episode, author of No Period. Now What?, Dr Nicola Rinaldi is on the podcast to talk about why this is more common than we realize, and how our desire to be healthy has caused the reproductive system of many women to shut down. As running is a high intensity activity, it can be stressful enough on the body that it feels it needs to reserve energy to keep moving. Birth control pills can mask a deeper problem, and today we talk about how even if you are having regular periods on the pill, you might still have an issue, which will go undetected until you come off the pill. We also have Heidi Greenwood, former professional runner, on the show. Heidi shares her story of transitioning away from running to start a family because of her amenorrhea. Heidi shares her powerful and heartbreaking story of how she had to say goodbye to her daughter Jules at 28 weeks because of anencephaly. This is a long episode, but so important to discuss and bring attention to. You or someone close to you could have struggled with missed, irregular, or complete loss of a menstrual cycle. This episode shows just how prevalent amenorrhea is, and how birth control pills may be masking the problem for many seemingly healthy women. Inspirational Quotes The hypothalamus is one of the master regulators in your body and when it senses a lack of energy or too much stress, which can be either physical or psychological, it will shut down your reproductive system and other systems to try to conserve energy...it will use the energy to the things you need to stay alive.-Nicola I feel like there is a huge stigma, that if you don't get a period, you have an eating disorder, so what does the person do who doesn't get their period? Stay quiet, because they don't want people to think they have an eating disorder.-Heidi There are 4-5 factors involved in HA. You can be extreme on any one of those factors, and shut down your hypothalamus and menstruation…for most of us, it is not just one of those aspects, but a whole bunch of things together.-Nicola It is important to be eating freely, not restricting any food groups, definitely eating enough to support whatever exercise you are doing.- Nicola Women, especially young women, don't realize that we need to ask questions. Heidi I have not seen any impact on fertility from having HA. Once you ovulate and get your period, you have the same chance of getting pregnant as anyone else, in fact in some ways, it might even be better. -Nicola High intensity exercise has been demonstrated to increase cortisol, and an increase in cortisol shuts down the hypothalamus, so I really recommend people do walking, gently yoga, and maybe easy biking and elliptical. -Nicola I typically suggest cutting out high intensity exercise completely.- Nicola Even if you take 6 months to a year off to restore your cycle, you can always go back to it later.- Nicola I would have never waited till after the trials to start a family with my husband...time is too precious for things like starting a family, things that bring you joy. -Heidi Everyone thinks, "oh, I am going to be different, I have read the stuff, but thats not really true for me". I hear that over and over again.-Nicola We have been taught for so many years that you need to be thin, you need to eat right, you need to exercise as much as possible. To recover from HA, you need to do the opposite of that, which is really hard. For many of us, for decades we have been taught this "healthy lifestyle" that turns to not be healthy when you take it too far even if its unintentional. It is really hard to wrap your mind around that.- Nicola We become numb to the workload we make our bodies do...we train our body to withstand long amounts of exercise, its like brushing our teeth, we just go do it, we don't even think about it. We forget that actually, we are making our body work!-Heidi When you don't have your cycle, you are almost assuredly not ovulating because you are not having the monthly hormonal fluctuations.- Nicola Any more than one period missing, I would be concerned. The sooner you take care of whatever the issue is, the easier it is to do. The longer you let something like that go, the more ingrained you get in the habits that have caused your periods to go missing, and the harder it is to recover. Birth control pills can completely mask this, as you bleed every month. That is not any indicator of a healthy menstrual cycle. Nicola Clomid or femera are probably not going to work, unless you have started making lifestyle changes.- Nicola I have learned so much through the process of recovery. Learning to cope with stress in other ways, other than through exercising and to realize how limiting my food restrictions were. I think that is a common feeling among women who have recovered from HA, they are really thankful for the process and for having to go through it, as hard as it is at the time.- Nicola We are in communities where this is quite prevalent, but in the overall population, doctors don't see this that much...for a typical OBGYN, they are not seeing that much of this, they are seeing more PCOS, so it is not on their radar screen, and they were not taught about this in medical schools.-Nicola Don't let running determine when you want to start a family. Running will always be there.- Heidi Don't have fear foods. Every food can have a place in what you eat. If you have a cookie, it's not the end of the world. It is really important to avoid any kind food restriction and eat what you feel like, eat what your family is eating, eat what your friends are eating. let your food be your fuel for the rest of your life, as opposed to feeling like you have to control it.- Nicola Resources Mentioned No Period.
