EP-119 Scientific Approach Needed - Gender-Affirming Care Highlighting the UK's NHS Cass Review on gender-affirming care with a focus on the often cited “suicide ideation” propaganda in Washington State politics. Beginning in 2020 the National Health Service (NHS) in England commissioned a comprehensive multi-year review of gender-affirming care for children and adolescents. The scientific review, published in 2024, was a complete rebuke of the prevailing approach to gender-affirming care in the UK. As a result of the review, the UK medical establishment and policy makers changed their approach to treatment for gender dysphoria in youth. Despite the updated science, activists and policymakers in Washington State continue to use emotional propaganda and pseudoscience to guide their campaigns and legislation. The science has been reviewed. We must change our ways. “She blinded me with science!” - Thomas Dolby ‧ 1982 Information war “Save the children” has been a manipulative political ploy for decades. Both major political parties have used it as a vehicle for inserting ideology into policy. It may seem trite from an experienced lens of political analysis, but the effectiveness proves its longevity as a tactic. There is a pernicious use of junk science fueling political propaganda regarding “youth gender care” in our society. The “save the children” slogan has been tied into a gordian knot of misinformation. The claims of science are used to tighten this twisted knot weaving through our community. It's often hard to know who to trust when information outlets contradict each other with claims of factual science: “Not that science, this science. Not this science, that science.” How do parents and community members inform themselves when the information space is littered with manipulation and a constant barrage of conflicting data sets? How are they to participate when each extreme faction emotionally invokes the language of “save the children” at every turn? In the end, the very object of manipulation must be the guiding light forging a path forward to truth and reason - science. Pseudoscience, popularized through political propaganda, states that youth gender-affirming care is necessary to combat teen suicide rates. This basic emotional plea invokes the time-honored manipulation of the “save the children” mantra. But the NHS Review debunked the pseudoscience; and it presented a peer-reviewed scientific assessment of youth gender-affirming care and the holistic issue of gender dysphoria. National Health Service Commissioned Cass Review The Cass Review was an independent review of the UK's NHS gender identity services for children and young people. The scientific endeavor culminated with a lengthy 388-page review led by Dr Hilary Cass. The mission for the Cass Review as stated in the document: “The Review was commissioned by NHS England to make recommendations on how to improve NHS gender identity services, and ensure that children and young people who are questioning their gender identity or experiencing gender dysphoria receive a high standard of care, that meets their needs, is safe, holistic and effective.” Trust the Science! Advocates for gender-affirming care often cite research to support their stance that such interventions, including puberty blockers and hormone treatments, are crucial in reducing suicide risk among transgender youth. The often-quoted study is titled “Pubertal Suppression for Transgender Youth and Risk of Suicidal Ideation.” This study suggested that transgender and gender non-conforming youth who received puberty blockers exhibited lower rates of suicidal ideation compared to those who did not. However, the Cass Review highlights a significant methodological flaw in the study: The study did not adequately account for the underlying mental health conditions of the participants. This oversight could lead to a false correlation between the treatment and reduced suicide ideation. Other factors or concurrent treatments might be affecting the outcomes. The Cass Review notes that many studies, including this one, fail to control for these psychiatric comorbidities, which are known to influence suicide risk independently of gender dysphoria. The Cass Review delves into the actual data concerning suicide risks among transgender youth, offering a more nuanced perspective. It points out that while there is a narrative suggesting an elevated suicide risk specifically due to gender dysphoria, the evidence does not support such a direct causation. Instead, the review finds that: "Suicide risk appears to be comparable to other young people with a similar range of mental health and psychosocial challenges." (page 186). This suggests that the increased risk of suicide in transgender youth might be more aligned with the general profile of mental health issues rather than solely due to gender dysphoria. The review underscores that studies reporting reduced suicidality with gender-affirming treatments often had significant methodological limitations, such as small sample sizes, lack of long-term follow-up, and absence of controls for concurrent mental health treatments. Quality of Evidence: The review found that only 2% of the studies examined were of high quality, with the majority suffering from issues like selection bias, lack of control groups, and inadequate statistical analysis. This has led to an overemphasis on potentially misleading results. Longitudinal Data: There's a stark absence of long-term follow-up data for those who have undergone gender-affirming treatments. This lack of data makes it impossible to assess the actual impact on mental health outcomes and suicide risk associated with gender-affirming medical treatments. Psychosocial Interventions: The review notes that while there's a focus on medical interventions like puberty blockers, psychosocial interventions, which have a robust evidence base for treating mental health in general adolescent populations, are often overlooked or underutilized in this cohort. Scientific Recommendations Given the findings, the Cass Review offers several recommendations to address the care of transgender youth more holistically and scientifically: Evidence-Based Approach: There is a call for more robust, long-term studies that can accurately assess the benefits and risks of all types of interventions for gender dysphoria, including but not limited to medical treatments. Comprehensive Care: The review recommends a shift towards a model of care that includes broader mental health support, acknowledging that gender dysphoria often coexists with other mental health issues that require attention. Research and Monitoring: Establishing a research program to monitor outcomes of transgender care, ensuring that interventions are based on high-quality evidence rather than anecdotal or poorly supported claims. Professional Training: Enhancing the skills of professionals in this field to deal with the complexity of gender dysphoria within the context of adolescence and mental health. Science Wins Last year, the NHS in England did a complete about-face regarding gender-affirming care and the way children and adolescents are treated for gender dysphoria. The Cass Review discovered that established policies and guidelines were developed from pseudoscience that created fear bias based on child suicide ideation and ignored fundamental underlying factors associated with gender confusion. By debunking flawed research and highlighting the lack of high-quality evidence, the Cass Review challenges the narrative that has driven much of the policy and public discourse on transgender youth care. However, Washington State policymakers remain in a blindfolded bubble of ignorance regarding the science associated with gender dysphoria. While the Cass Review suggests a scientific, evidence-based approach, urging caution against rushing to medical interventions without considering the full spectrum of an individual's mental health needs, Washington State political leaders have rushed to protect a mill of treatments and policies steeped in misinformation. The scientific recommendations by the Cass Review not only promise better outcomes for transgender youth, but also align with ethical medical practices that prioritize long-term well-being over short-term relief or ideological pressures. The UK has changed direction as a result of the scientific Cass Review. Washington State Democrat policymakers must adhere to their sloganeering to “trust the science.” A clear-eyed reassessment of the basic care for children and adolescents afflicted with gender dysphoria must be undertaken. Yes. This all must occur in Washington State to …. “Save the Children.” Outro Music Credit - Boomcrickets 2001 - Vancouver Washington

A Supreme Court case on medical care for transgender youth could have major ramifications – not only for children who have gender dysphoria and their families but also for how other statutes are reviewed under the Equal Protection Clause. In this episode, Amanda and Holly examine the oral arguments in U.S. v. Skrmetti, breaking down key moments in the heated courtroom exchanges, examining the specific constitutional question in this case, and discussing the broader implications of the possible ruling. While the specific question in this case involves the Equal Protection Clause of the Fourteenth Amendment and not the Religion Clauses of the First Amendment, religion and religious arguments often loom large in cases that involve sexual orientation or gender identity. In this episode, Therese Markow and Dr. Ximena Lopez discuss the challenges faced by transgender youth, emphasizing the importance of gender-affirming care. Dr. Lopez explains the difference between sex and gender, noting that gender dysphoria is distress caused by a mismatch between one's gender identity and sex assigned at birth. She describes treatment options, including puberty suppression and hormone therapy, which can significantly improve mental health and reduce suicidality, particularly in teens. Dr. Lopez also criticizes state bans on gender-affirming care for minors and cites numerous studies showing its benefits, as well as discusses the inconsistencies in hormonal therapy for children and teens. Key Takeaways: In medicine and psychology, gender and sex are two different things. While aligned in most people, they are not aligned in those individuals on the transgender, nonbinary, or intersex spectrum. Adults who transitioned later in life typically knew they were different when they were young, but didn't have the language or awareness. When a child comes out as transgender, most parents are typically in denial. It is not until their child or teen is depressed and often suicidal, that the parents are willing to take the next steps with their child. Gender-affirming care at the beginning of puberty can help to pause the puberty of the incorrect gender where changes happen that cannot easily, if at all, be reversed later in life. Puberty suppression can be reversed on the off chance the individual changes their mind. "Most of the stress comes from the adult world, and if the adults are transphobic and influence their kids to be transphobic, then we can also see kids who are transphobic, and then they can bully and discriminate. If it's a very affirming school where there are policies to protect transgender students, and the teachers and all the staff are on board, then that promotes well-being." — Dr. Ximena Lopez Episode References: TEDMED Talk: How one pediatrician is supporting transgender youth: https://www.youtube.com/watch?v=ViqvPknY4HE Connect with Dr. Ximena Lopez: Professional Bio: https://profiles.ucsd.edu/ximena.lopez Connect with Therese: Website: www.criticallyspeaking.net Threads: @critically_speaking Email: theresemarkow@criticallyspeaking.net Audio production by Turnkey Podcast Productions. Florence Ashley, "Parental Rights Over Transgender Youth—Furthering a Pressing and Substantial Objective?" (2024) 62(1) Alberta Law Review 87–119 Abstract: Parental rights are increasingly being invoked to oppose the growing inclusion of trans youth in education. Recently, some provinces have proposed or adopted laws and policies predicated on the belief that parents have a right to be informed of their child's choice of name and pronouns at schools and that trans youth should not be allowed to change the names and pronouns they use at school without parental consent, which I term “blanket veto and disclosure laws.” In this article, I explore whether blanket veto and disclosure laws can be justified under two dominant conceptions of parental rights — parental authority and parental entitlement. Using the framework provided by section 1 of the Canadian Charter of Rights and Freedoms, I argue that blanket disclosure and veto laws cannot be justified under either conception of parental rights. Conceived as protection of parental authority, blanket veto and disclosure laws are unjustified because they are not rationally or narrowly tailored to their objective. Conceived as protection of parental entitlement, the laws are unjustified because their objective is inconsistent with the values of a free and democratic society. Regardless of the conception of parental rights we adopt, blanket veto and disclosure laws are constitutionally and politically deficient. (Link to article)

There's fresh calls to investigate the use of puberty blockers for Kiwi youth who identify as transgender following research out of the UK. The Ministry of Health has been reviewing puberty blockers since 2022- but there's been no word confirming they plan to follow the UK's lead and block use of the drug for under-18s. Starship paediatric endocrinologist Dr Ben Albert says puberty blockers can serve a purpose in helping younger users- and that banning them isn't the way to go. "It's really important if a young person chooses to use medication like this, they have a really good understanding of what the effects will be, and the uncertainties that exist around those treatments." "There's something beautiful, almost magical about the realization that what you are doing is participating in a procreative action. That is something that same sex intercourse cannot do. The point of this episode isn't to say that people with same-sex attraction should be demonized, hated, or unjustly treated. Rather, the point is that awareness needs to be raised of the health risks, both psychologically and biologically, that are associated with homosexual and bisexual lifestyles. There are people who are born with same-sex attraction and can live holy and healthy lives. If we love them, then we ought to help them, not by forcing them to do anything, but by providing resources and information that they could willingly choose to accept or reject." Around the country, Republican-led states have cracked down on gender-affirming care services for minors. Twenty states have banned such care — including North Dakota, South Dakota and Iowa. Meanwhile, 13 states – including Minnesota – have gone the opposite direction and enacted legal protections for patients seeking gender care and for providers. On Sunday, National Public Radio is set to air a special feature that delves into the realities on the ground for transgender youth and their families, along with medical providers in two states – Utah, which has adopted restrictions, and Minnesota, which has added legal protections for care. The Kremlin said the attack was foiled and the alleged drones destroyed. Video that appeared on social media shows a bright flash and a puff of smoke over a part of the Kremlin, the official residence of the Russian president and the most potent symbol of power in Moscow. In a statement, the Kremlin said it regarded the alleged attack as terrorism and a deliberate attempt on Putin’s life. “Russia reserves the right to take retaliatory measures where and when it sees fit,” it added. Ukraine denied involvement in the alleged strike. US officials said they were still assessing the incident, and had no information about who might have been responsible. Whatever the truth, any admission of a security breach at the heart of the Kremlin is remarkable. Moscow said the alleged attack took place in the early hours of Wednesday. The Russian president was not in the building at the time, Kremlin spokesperson Dmitry Peskov said. analysis of video showing the incident support the Kremlin’s claim that two drones were flown above the Kremlin early Wednesday, but did not show evidence of Ukrainian involvement: A video that appeared to show smoke rising from the Kremlin surfaced on a local neighborhood channel on social media platform Telegram at 2:37 a.m. local time Wednesday. The first reports of the incident citing the Kremlin came via Russian state media TASS and RIA around 2:33 p.m. local time – around 12 hours later. Shortly after the first media reports, another video appearing to show the moment a drone exploded above the Kremlin began circulating widely on social media. In the video, the drone appears to fly towards the building’s domed roof, followed by what looks like a small explosion. In this video, two people appear to be climbing on the dome holding flashlights, and can be seen ducking down just before the moment of the explosion. The people climbing the dome are not present in the first of these videos, but appear in the second, suggesting they were responding to the fire caused by the first drone at the time the subsequent drone appeared. The Kremlin Press Service has called the purported drone attack an “attempt on the President’s life,” said it was an “act of terrorism” and blamed Ukraine. https://www.outkick.com/audrey-hale-nashville-police-release-update-litigation/ POLICE ANNOUNCE MANIFESTO OF NASHVILLE TRANSGENDER MASS KILLER WON’T BE RELEASED SOON The release of transgender mass killer Audrey Hale’s manifesto has been put on hold. The Nashville Police Department had previously told Fox News Digital last week the manifesto was being revealed and would be released. People are interested to see what drove Hale to murder six innocent people, including three children, at the Covenant School in late March. Now, it appears that the process has hit a roadblock. Metro Nashville PD announced Wednesday afternoon that “Due to pending litigation filed this week, the Metropolitan Nashville Police Department has been advised by counsel to hold in abeyance the release of records related to the shooting at The Covenant School pending orders or direction of the court.” The update is in response to a lawsuit filed by former Hamilton County Sheriff Jim Hammond and the Tennessee Firearms Association against the Nashville city government and the police to release the manifesto, according to TimeFreePress.com. As of early May, the public still doesn’t have any idea what is in the manifesto. It’s been more than a month since Audrey Hale walked into the Covenant School to unleash her rampage of terror against children and staffers. Despite the significant time that has passed, government officials have seemingly dragged their feet to make sure the manifesto doesn’t come out. For most of the time, it appeared that Hale’s writings would never be released. Metro Nashville Council Member Courtney Johnston previously told the New York Post, “What I was told is, her manifesto was a blueprint on total destruction, and it was so, so detailed at the level of what she had planned … That document in the wrong person’s hands would be astronomically dangerous.” https://www.cnbc.com/2023/05/03/fed-rate-decision-may-2023-.html Fed increases rates a quarter point and signals a potential end to hikes The Federal Reserve on Wednesday approved its 10th interest rate increase in just a little over a year and dropped a tentative hint that the current tightening cycle is at an end. In a unanimous decision widely expected by markets, the central bank’s Federal Open Market Committee raised its benchmark borrowing rate by 0.25 percentage point. The rate sets what banks charge each other for overnight lending but feeds through to many consumer debt products such as mortgages, auto loans and credit cards. The increase takes the fed funds rate to a target range of 5%-5.25%, the highest since August 2007. Markets, though, are more focused on whether the Fed will pause here, particularly with lingering concerns over economic growth and a banking crisis that has rattled nerves on Wall Street. Stocks rose slightly and Treasury yields were mostly lower immediately following the Fed news, but stocks struggled to hold on to the gains. During Wednesday’s news conference, Chairman Jerome Powell said “a decision on a pause was not made today” but noted the change in the statement language around future policy firming was “meaningful.” The post-meeting statement had only offered some clarity on the future pace of rate hikes — and not by what it said but what it didn’t say. The document omitted a sentence present in the previous statement saying that “the Committee anticipates that some additional policy firming may be appropriate” for the Fed to achieve its 2% inflation goal. The statement also tweaked language to outline the conditions under which “additional policy firming may be appropriate.” Previously, the FOMC had framed the forward guidance around how it would determine “the extent of future increases in the target range.” The statement reiterated that the Fed “will take into account the cumulative tightening of monetary policy, the lags with which monetary policy affects economic activity and inflation, and economic and financial developments.” Taken together, the moves are at least a tenuous nod that while tight policy could remain in effect, the path ahead is less clear for actual interest rate hikes as policymakers assess incoming data and financial conditions. Wednesday’s decision comes amid U.S. economic fragility and over the objections of prominent Democratic lawmakers, who urged the Fed this week to stop rate hikes that they insisted could cause a recession and excessive loss of jobs. However, the labor market has remained strong since the increases started in March 2022. At the same time, inflation is still well above the 2% target that policymakers consider optimum. Multiple officials have said rates probably will need to stay elevated even if the hikes are put on hold. https://www.theepochtimes.com/irs-spent-10-million-on-weapons-since-covid-19-pandemic-began-watchdog_5238319.html?utm_source=partner&utm_campaign=BonginoReport IRS Spent $10 Million on Weapons Since COVID-19 Pandemic Began: Watchdog The Internal Revenue Service (IRS) has been splashing out millions stocking up on guns, ammunition, and combat gear since 2020, according to the findings of Open The Books, a watchdog group that tracks government spending. A report by the watchdog, published on April 27, found that the agency has spent a total of $10 million on weaponry and gear since the COVID-19 pandemic began, including $2.3 million on duty ammunition, $1.2 million on ballistic shields, and another $1.3 million on “various other gear for criminal investigation agents.” Additionally, the agency has spent $474,000 on Smith & Wesson rifles, $463,000 on Beretta 1301 tactical shotguns, and $243,000 on body armor vests since 2020. Another $467,000 was reportedly spent on duty tactical lighting, $354,000 on tactical gear bags, and $267,000 on ballistic helmets in the same time. The report, called “The Militarization of Federal Bureaucracy,” contains updated data through the end of March 2023. Since 2006, the IRS has splashed out $35.2 million (adjusted for inflation) on guns, ammunition, and military-style equipment. The report found that 2020 and 2021 saw them spend increased amounts. In a statement to Open The Books in 2021, an IRS spokesperson said IRS Criminal Investigation special agents “have been using weapons throughout their history as they have consistently found themselves investigating the most dangerous criminals involved in organized crime, drugs, and gangs.” The IRS Careers website for criminal investigation special agents states that applicants must be willing to “carry a firearm, must be prepared to protect him/herself or others from physical attacks at any time and without warning and use firearms in life-threatening situations; must be willing to use force up to and including the use of deadly force.” https://www.dailyfetched.com/san-francisco-loses-another-retail-giant-as-shops-continue-to-close-due-to-crime/ San Francisco Loses Another Retail Giant as Shops Continue To Close Due to Crime According to a memo from Chief Stores Officer Jamie Nordstrom to employees, the company has decided not to renew its leases for its San Francisco Centre Nordstrom store and the Market Street Rack store. “We’ve spent more than 35 years serving customers in downtown San Francisco, building relationships with them and investing in the local community,” Nordstrom wrote. “But as many of you know, the dynamics of the downtown San Francisco market have changed dramatically over the past several years, impacting customer foot traffic to our stores and our ability to operate successfully.” The San Francisco Centre location will close at the end of August, and the Market Street Rack will close on July 1. the owner of the Westfield mall, told the San Francisco Standard that such closures “underscores the deteriorating situation in Downtown San Francisco.” A spokesperson for the mall added: “A growing number of retailers and businesses are leaving the area due to the unsafe conditions for customers, retailers, and employees, coupled with the fact that these significant issues are preventing an economic recovery of the area.” The statement continued: [Unibail-Rodamco-Westfield] has actively engaged with city leaders for many years to express our serious concerns, which are shared by our customers and retailers. We have urged the city to find solutions to the key issues and lack of enforcement again.” The news comes not long after San Francisco flagship Whole Foods retail store closed after just a year due to 568 emergency calls over 13 months amid soaring crime in the city. Incidents included vagrants starting fights, throwing food, and defecating on the floor. Twenty retailers have closed stores in the Union Square area since 2020, The Standard reports. Last month, CashApp founder Bob Lee was fatally stabbed in the streets of San Fransisco, while former fire commissioner Don Carmignani was recently beaten over the head with a pipe by a vagrant. Last October, The Daily Fetched reported that commuters were resorting to arming themselves with baseball bats and stun guns due to droves of violent drug addicts. Residents of the SoMa, in northeast San Francisco, said the recent opening of the SoMa RISE drug sobering center, which opened in June, has plagued the neighborhood, making it unsafe. Residents say the center, which aims to temper drug usage, has done nothing but draw heavy users to the neighborhood. Residents said the influx of drug users had brought soaring crime, making the neighborhood more dangerous, Fox News reported. San Francisco Mayor London Breed touted the center as a safe haven for addicts looking to rebuild their lives. But according to SoMa resident and business owner Mark Sackett, the reality of the center in the neighborhood is much different. https://www.dailyfetched.com/joe-biden-offers-500k-grant-for-teachers-in-pakistan-to-help-transgender-youth/ Joe Biden Offers $500K Grant for Teachers in Pakistan to Help Transgender Youth According to the State Department grant, it will teach English language skills to Pakistani transgender youth so they “better participate in the global community and prepare them for success in the workplace.” The grant report aims to focus on three components: (1) Professional Development for English Language Teachers from Non-Mainstream Institutions (2) Professional Development for novice Pakistani English language teachers (3) Professional Development for Transgender Youth and for Afghan Teachers, Students, and Young Professionals Residing in Pakistan.” The program component that includes a focus on transgender youth accepts proposals from applicants “for a minimum of $25,000 and a maximum of $75,000 to implement: (1) intensive professional development courses for Pakistani transgender youth from the ages of 13-25, and (2) and intensive professional development courses for Afghan teachers, students, and young professionals residing in Pakistan.” The components aim to improve English language communication skills and connect trainees to a professional alumni network. Applicants to the Biden grant program are encouraged to address, “What is the most effective way to reach the greatest number of Pakistani transgender youth and Afghan teachers, students, and young professionals from diverse locations across Pakistan?” A spokesperson for the State Department said the education project from the agency focuses on transgender youth like it does with other “marginalized communities.” However, the spokesperson added that the grant funds won’t be used for gender transitions.
