OK, we knew the human gut had one of these, but the vagina too? What's actually going on in there? Microbiomes are incredibly diverse ecosystems: we are more bacteria than we are human. Yep. Read that again — there are more bacteria in us than human cells. You, my friend, are a moving, breathing microcosm that is incredibly cooperative when you're healthy. Don't panic. Katy Bradbury is back to explain what all this means and how we can use this knowledge to be healthier. Things to know: A healthy gut is NOT free from bacteria. We need the friendly bacteria to survive. Yes, it's a bit icky to think about, but it's important. “Bad” bacteria is opportunistic, like weeds in a garden, says Katy. You need to keep your “good bacteria” healthy and plentiful so the bad can't get a foothold. We want a diversity of good bacteria in both the gut and the vagina, but these are not the same critters. Lactobacilli are best for the vagina, so knowing how to protect lactobacilli by keeping the vaginal environment at the right pH is a good thing. This is important for fertility but also for avoiding infections like yeast infections and UTIs, so care is a life-long need. Douching and using other “cleaning products” is NOT vaginal care unless you have a specific medical reason. These products disrupt the natural vaginal pH and aren't necessary for this self-cleaning organ. There's so much good information in this episode, so please give it a listen and share with others. While it may be difficult to overcome the traditional shame around our bodies, this shame is, frankly, not helpful. Sharing information and talking openly are the best way to erase the shame and be healthier. Ready to learn more? Listen up! Join us as we discuss the vaginal microbiome. Jacques explains what it is, how to support it, and what to stop doing so that you can continue to have a healthy vaginal microbiota. This is a fascinating conversation that reveals so much about what we know so far and what we have yet to learn in this unique microbiome. Some background on Dr. Jacques Ravel, he is a professor in the Department of Microbiology and Immunology, and the Associate Director for Genomics at The Institute for Genome Sciences, University of Maryland, School of Medicine in Baltimore Maryland. Over the past 15 years, he has developed a research program focused on applying modern genomic technologies and ecological principles to characterize the role and dynamics of the microbial communities inhabiting the human body in health and disease and better define the interactions between the host, the microbes and the environment that drives these ecological systems. Specifically, he is interested in the role of the vaginal microbiome in women's health in order to develop improved strategies to manage gynecological and obstetrics conditions. He has published over 225 peer-reviewed publications and has attracted grant income of more than $30 million. His work is currently being translated by developing novel live microbiome-based biotherapeutic formulations targeted to restore vaginal health, treat conditions such as bacterial vaginosis, urinary tract infections and improved fertility. His work earned him to be elected to the fellowship of the American Academy of Microbiology, and in 2015, he was awarded the Blaise Pascal international research chair. He is the founder and editor-in-chief of the journal Microbiome. He joined the Institute for Genome Sciences at The University of Maryland Medical School in 2007 as an associate professor. The vaginal microbiome is just like the gut, it's a collection of bacteria that live together to perform a function that is beneficial. If it lives in the body, it's often beneficial to the host. When it comes to the vaginal microbiota, there are five types of women, four of these types are dominated by lactobacillus, one of the four species (lactobacillus crispatus, lactobacillus iners, lactobacillus gasseri, lactobacillus jensenii). And one of them is not and it's represented, it's more like the gut, this type is very diverse. So you can see it's diverse and very even, a lot more even than what you see in the others. Those bacteria, they acidify the vagina, the pH of the vagina is about 3.6-4, so it's very acidic Those that have an acidified environment are those that you would call very well-protected 25-30% of women that don't have lactobacillus How does the composition of the vaginal microbiome impact a woman's overall health? Without a healthy vaginal microbiome, women are subject to many different risks Sexually transmitted infections A woman being able to get pregnant - the implantation of the egg Chronic yeast infections Vaginal dryness which can make intercourse very painful Can lubricants affect the vaginal microbiome negatively? They are very hypertonic, that's a big problem because what happens is it dries the cells Most lubricants, especially in medicine, contain antimicrobial agents, so it kills even the good bacteria Not every woman is affected by the application of lubricant What are some of the things that people are doing in day to day life that seem to be hurting the vaginal microbiome? Douching - you're basically washing your vagina with chemicals Smoking has a major impact on the vaginal microbiota Poor diet A really heavy dose of azithromycin has a really detrimental effect on the vaginal microbiota What can women start doing right now to help support a healthy vaginal microbiome? Why would you want to use a feminine wash, why would you want to douche, in essence why would you want to wash your vulva, your vagina with substances, solutions, soaps and what not. Why would you want to scrub vigorously all in the name of “never to be caught un-fresh” There has been several stories, people inserting garlic in their private parts, yes garlic, onions, detergents, paw paw, watermelon – because they want it to have that scent when the guy goes down there. These are the same people that will not let you rest in consulting rooms. Okay, leave those ones. How about the ones that use exotic products, the so called ph balanced feminine products? Now some of the common reasons why women douche or use feminine wash are 1. Common reasons women report they use a douche include: • eliminating unpleasant odors • preventing pregnancy • washing away menstrual blood after a period or semen after sex • avoiding a sexually transmitted infection Douching doesn't accomplish any of these things. 2. DefinitionsDouching: technically refers to cleansing the vaginal canal with liquidEnema: delivers water or a solution directly into the rectum to loosen fecal matter and stimulate a bowel movement (i.e. pooping). Can be done as a way of relieving constipation or as a way to “clean house” before anoreceptive sexWays to douche1. Bulb enema: a common “over the counter” option you can find at drugstores. Includes a bulb (filled with fluid) attached to a nozzle which can be inserted into the rectumCan buy reusable rubber enemas that you fill with tap water, or can buy brand names (such as Fleet enemas) that come pre-filled with solutions designed to help stimulate a bowel movement - more on these solutions later.Bag enema: A rubber bag with a small tube attached. You fill the bag with water, lock the tube shut with a small plastic piece and insert the other end of the tubing into the anusA great option for folks who need to be in certain positions while douching or have mobility differences Shower nozzle: What it sounds like. Beware of the high velocity and large volumes that come with shower nozzle douching. These can be damaging to the rectum!Best practices – or, how to ace your douching exam1. Dietary fiberWhat to buy: something with psyllium husk or high soluble fiber as the active ingredient. No need to buy brand names (all you're paying for is the muscular torsos on the labels, which...fair)The dose: scale up slowly (every other day) to avoid bloating, cramping and other side effects. Increase gradually, and goes best with water2. The actual gymnastics – er, mechanicsPositioning: bent at the waist (this straightens the rectum, making it easier to get liquid where it needs to go)Place lube on the tip of the device to prevent local trauma to the areaInsert the device into the rectum but not too far (otherwise water goes too far in and you pull down unformed stool into the very area you want to cleanse)Hold a Coke can-volume of water for about a minuteExpel into the toiletMyth-busting“The harder and faster I douche, the better”Douching at high volumes/speeds (e.g. with shower nozzles) can can physically damage the rectum Water can travel too far and risk pulling extra feces into the rectum“I can douche as often as I'd like”Aim to douche no more than 1x/day and 3x/week When we say once a day, we mean the entire process of clearing the rectum, knowing that you may need more than one rinse per sitting, so to speakSome experts have pointed out that douching runs the risk of stripping the colon of its mucosal layer – which plays an important protective role“Douching with ~fancy~ commercial solutions is better than douching with plain water”Douching with water - tap water - is preferable over other solutions (such as those found in brand-name enemas)No need to look for “pH balanced” solutions – the anus is a neutral pH with the rest of the bodyMoreover, some of these solutions are designed to irritate the colon/rectum to help you poop, which – while useful for constipation – should be avoided if you plan on inserting something else into your rectum shortly thereafter“Using warmer water helps cleanse more effectively”Remember: the tips of our fingers can handle water temperatures a lot higher than than what our insides canLukewarm water is the way to go (since cold water, while safe, is not terribly pleasant) “Douching after sex will help me avoid sexually transmitted infections”In fact, the opposite may be true, as further local mucosal injury and introduction of pathogens may increase the risk of injury and/or acquiring an infection. Overall, douching after sex does not decrease the risk of getting a sexually transmitted ifnection and has no proven health benefit.Douching and sexually transmitted infectionsThere's very little scientific literature on this subject. However, the few studies we've found suggest that those who douche > 1x per week may have an increased association with getting an STI, such as chlamydia, gonorrhea, or HIVThe reasoning behind this: in those who douche, the outer protective layer of cells in the rectum can sometimes be removed. This means that bugs that cause STIs have increased access to their port of entry, so to speak.For people who have sex without condoms, remember that part of being a responsible sex partner is getting screened for STIs in the sites you use for sex (including your butt and throat)Is there anyone who should think carefully about douching?Patients with high-inflammatory states affecting their rectum: Particularly underlying colitis, active rectal or anal infections, active HPV. In these cases, douching may exacerbate the underlying condition by further irritating the rectum.Patients whose immune systems aren't running at 100% (such as those with HIV and low CD4 counts): These folks can be susceptible to a greater number of infections. Important to make sure filtered or bottled water is being usedPatients with chronic kidney disease:Folks with chronic kidney disease have a hard time clearing phosphate from their systems.Here, the pro-tip is to avoid using phosphate solutions (such as those found in Fleet enemas) since this introduces extra phosphate to your retum which can then be absorbed into your body.

In 2015, Lolo was asked by Jennifer Hopelezz to organize Superball, a fierce battle of drag kings and queens. Today, this legendary event continues to influence queer and drag culture throughout Holland as well as Europe (No, really. It is now the biggest international Drag Ball in Europe), organized by Lolo, DjPrisz (Priss), Darling Peter, and Maayan Bengal. Lolo is the mother of the Vipers Squat through which she creates performance spaces and acts for many parties and festivals (Wasteland, Lowlands, Oerol festival). She is also a resident queen for Club Church and the House of Hopelezz, resident stylist for the team at Funhouse (Rapido Events), and has performed at parties in Paris, Istanbul, Italy, Belgium and, basically, all over Europe. It's been a minute since we had an episode. But John and Dean are trying to get back at it. And what better way then to explore both queer bating as well as the "Over 30 Curse" in the same episode we talk about K-Pop! John has a theory that when a female artist hits 30 they are either excluded or cast out of popularity. Whether it's by the actions of stations, labels, or critics, or just some social force that we all somehow cling to, we've seen it time and time again. Christina, Britney, and now Katy Perry. Who's album John and Dean absolutely loved, and yet was somehow both too much like the old Katy and somehow also not as good as the old Katy. And therein lies our problem. As much as John and Dean loved Katy's album, there were some very interesting moments where she is either expressing unstated bisexual desires, or is queer bating us because that's a thing that money people do to get more gay dollars. In two videos there are definite references to either Katy being queer, or Katy being in a relationship with someone queer. And while we love to see it, we also hate to see it. Dean muses on what Shawn Mendes was or was not saying in his latest song/video, and then launches into a rant about queer bating and queer representation in both old Hollywood and new. (John says yes a lot.) Finally we end on a great note, the rise of K-Pop in popular culture and the amazing (almost) new Mini-Album from one of our favorite groups, Blackpink. While they disagree on "Ice Cream" (and whether not liking is the same as disliking,) they overall love the the new release and can't wait to hear more!
