Why did my symptoms of Raynaud’s syndrome seem to get worse when I started eating keto? Hey Jimmy and The Doc, I went gluten free years ago and Paleo was the next natural step. I have had autoimmune issues my whole life and my doctors couldn’t figure it out. One of them once told me, "we usually only see these symptoms in overweight people", but overweight I am not. When first going Paleo, I made a drastic mistake without knowing it. The Paleo crowd was all about the bananas and potatoes back then so that's what I did. Even though I am not big fans of either of these they were the least offensive carbs and it seemed to help. A few months ago I decided to go with a ketogenic approach doing it the correct way this time. I still have fruit in the house for my kids but I have not touched it. I never had the low-carb flu and within a week was sleeping like a rock for the first time in years despite living the stressful life of being a single mom. Joint pain did not flare up as much and my migraines are not showing up as often. My question for you guys is this: I have always been plagued with Raynaud's syndrome. It lasts all year long, not just in the winter. I have noticed that since I do not feel hungry on a regular basis it is easy to go many hours without food while out running errands without noticing how long it has been. My Raynaud’s then rears it's ugly head and I feel it. I'm not sure if it's going too long on an empty stomach or something else happening, but the Raynaud's is the only thing that has gotten worse since I started eating keto. Do you know why this is happening? I am already taking a magnesium supplement to help. Even if you guys have no solution for me, I will just suck it up buttercup and deal with it as I have in the past because the benefits I am getting from nutritional ketosis far outweigh the cold hands and feet that I have always had to deal with anyway. Thank you both for your time and effort and keep up the great work. Sarah 2. Why did I start experiencing very sore spots on my skin when I decided to cheat on my low-carb, ketogenic diet? Hey Jimmy and The Doc, I recently found your podcast and have greatly enjoyed getting caught up on all the episodes! I have a question that might actually be a new one for you as I haven’t heard it discussed on your show before. I have been eating low-carb, ketogenic with weekly cheat days as Tim Ferriss outlines in his book The Four-Hour Body. After about six months of doing this, I noticed I would wake up with very sore spots on my skin following a cheat day. It seemed to be concentrated on my torso and sometimes on my upper arms, neck, and scalp. This soreness was on the surface of the skin, not the muscles or joints. The best way I can describe it is it felt like a giant bruise that was only painful when you pressed on it. My skin would be so sensitive that I would skip running or other exercise that day because the mere bouncing of my skin was too uncomfortable to bear. The surface area of skin that would be sore seemed to correlate positively with how much crappy carbage I splurged on the previous day. Interestingly, the first areas to become sore were the areas in which I tend to store fat, such as the love handles, and the soreness would radiate out from there. If I had a really obscene cheat day, then even my upper leg skin would get sore. Very surprisingly, this phenomenon did not go away when I went back to the high-carb Standard American Diet for a period of time. It seemed anytime I indulged in something particularly sugary/carby, my skin got sore. This soreness would only subside after a day of eating low-carb keto again. So my questions for you guys is this: What could be causing this? Is it just a rapid-onset generalized edema resulting from a significant increase in carbs? Why did it not begin occurring until a good 6+ months into eating low-carb and why did it persist after I went back on a high-carb diet? Have I permanently altered how my body reacts to carbs by going low-carb for so long? Finally, should I be concerned about this? While trying to find answers to these questions online, I saw this “sore skin” phenomenon mentioned by a couple of people on low-carb diet forums who claimed they never experienced this before going low-carb. Can Adderall be having an adverse effect on getting into ketosis? And does keto do anything to help improve adult ADHD? Dear Keto Talk hosts, I am a 62-year old female and have been moving to keto through the usual progression—it started 20 years ago trying to eat local and real food, then on to WAPF, then Paleo, and now low-carb, ketogenic consuming 20-40g carbohydrates daily. Most of my carbs come from salad, kefir, cheese, and kombucha. My blood sugars have been coming down but I am still in the high 90s for fasting blood sugar in the morning. I have only seen .5 once on the blood ketone meter. I am trying to figure out what might be preventing ketosis for me and I have a couple of suspects. Because I have some of the MTHFR family of variations, my doctor has me on 1000 mcg of L-methylfolate and adenosyl/hydroxy B12. I am also taking 10 mg daily of Adderall. Would any of these be having an adverse effect on getting into ketosis? And does ketosis have any positive effect on improving adult ADHD? I’d really love to get off of that medication if at all possible. Thanks for any insights you can provide. Stephanie KETO TALK MAILBOX - Why can’t I stay in ketosis eating what seems to be a well-formulated low-carb, moderate protein, high-fat, ketogenic diet? Hey Jimmy and The Doc, I listen to your Keto Talk podcast all the time and I have been doing keto for nearly 2 months. I am 6'7" and weigh 220 pounds with 16% body fat. I started keto for all the health benefits, energy, and disease prevention it provides. I eat about 100g protein, less than 20g carbs, and 200g of fat. This all seems right to me and I am being very strict with it. I get lots of sleep, am not stressed, and have experimented with exercising a little and not doing any exercise. There’s only one problem: I JUST CANT STAY IN KETOSIS! I bought a blood ketone monitor and the highest reading I have seen is 1.2 for a very short 2-day period and most of the time I am less than 0.5. Every morning I wake up I am 0.2 and then it’ll rise to 0.6 mid day and once in awhile I'll get to 0.8 in the evening or sometimes it goes the other way and I'll drop back down to 0.3. I eat bacon and eggs in the morning with coffee, MCT oil, and full fat whipping cream as well as high-fat meats and cheeses or cream cheeses for lunch and supper. The frustrating thing for me is my wife is getting 2.5-3.5 on the ketone meter eating the exact same thing I am. HELP! I hope you have an idea why I can't stay in ketosis. Is it normal to experience stomach pain that disrupts sleep when fasting for more than 24 hours? Hey guys, I love listening to Fasting Talk and reading your book. I’ve been IF for a year now. Started with 16/8 and then have been doing 18/6 for a long time. I’ve done a few 24 hour fasts, a 48 hour fast, and most recently a 42 hour fast for the purposes of fat loss. My question for you guys is this: Is it common to get stomach pain when fasting for more than 24 hours? It's not hunger pangs, but feels like gastritis pain. Hi Jimmy and Megan, I have been doing IF and keto for about a month and half and I love the combination because it allows me to enjoy real food when I eat and gives me excellent mental clarity. My purpose in IF and keto is to prevent Type 2 diabetes since my A1c has been 5.8 the past couple of years. I’m already seeing my blood sugar and A1c drop, so I’m very happy with my nutritional choices. However, I do have a concern that I’ve noticed. Before my IF and keto protocol, my periods usually come every 38 days. But since I changed my diet and added fasting, it has been 46 days since my last menstrual cycle and I still have no period. Does fasting raise blood pressure levels? Hi guys, For the last month, I have been doing the 5:2 version of fasting as a warm-up for some of the protocols you set out in your fasting book. I’m 46, not diabetic but I am overweight with a BMI of 28, and I’m hoping fasting will prevent the development of Type 2 diabetes, lead to weight loss, and re-calibrate my eating habits. So far I’ve lost 4 pounds, my clothes are looser, and I no longer feel polluted inside. Fasting days are not always easy, especially in this cold northern England climate, but I still look forward to them! I have recently started having significantly higher blood pressure readings than usual (138/75 upon waking). I’m not completely sure why this is happening, but I did recently complete a course of prednisone for asthma and a chest infection. I also have a busy job with two lively young children. Enough said, right? But I have heard from people on a 5:2 fasting forum that fasting itself may have caused my elevated blood pressure. I’ve just started alternate day fasting and I’m fasting on 500 calories (M, W and F) and then eating normally the other 4 days. It’s going pretty well for me so far, but I’m finding that on the “normal eating” days my appetite has reduced a fair bit and I’m not wanting to eat the same amount as I did prior to beginning alternate day fasting. My concern is if I reduce the amount of food I eat to fit with my lowering appetite levels that it will begin to creep downward to where it is on my non-fasting days. Am I at risk for slowing down my metabolism doing this? As someone who has spent most of her life as a yo-yo dieter, I’m really worried about rebound weight gain from doing this. Can you offer any tips on what I can do to prevent this from happening? I am loving alternate day fasting and really getting into the groove with it. Help me optimize the results I’ll get from it. Do the watery stools/diarrhea that can happen during and after a fast be an indication of and come from gut bacteria dying off? Hi Jimmy and Megan, I am one of those people who ends up with watery stools during and after I break a fast. I’ve spent a lot of time thinking about why. I know Jimmy mentioned that he was also puzzled as to what is coming out when nothing is going in during an extended fast. Is it possible that some of the bowel bacteria (which I understand make up a good deal of stool) are dying from not being fed? And since they are dead, they are eliminated from the body while extended fasting? It certainly smells like something died in there. Incidentally, I did eat a small meal of 3 ounces of ham and 8 ounces green beans about 3-4 before bed, so possibly that is the culprit in my higher reading. Hi guys, I've read The Complete Guide to Fasting and have been loving the Fasting Talk podcast. As a science teacher, I really appreciate how you present the information. I initiated what I wanted to be a four-day fast and maybe longer recently. I made it to the 32-hour mark before experiencing signs of hypoglycemia. I was shaky, dizzy, faint, and very weak despite my heart rate at around 100 bpm. When I checked my blood sugar, it was down to 33. Because I didn’t feel safe to drive at the moment, I decided to drink 8 ounces of pomegranate juice since that was the only thing I could find to bring my blood sugar back up. I assume it improved because the symptoms decreased a bit and I made it to work safely. I'm generally healthy except for elevated cholesterol and a bit of excess weight and I'm hoping these things will be fixed by fasting and keto. I'm on my second time through a 6 day cycle of 3 days fasting followed by 3 days feasting. Before I started, I was eating keto to satiety and slowly gaining weight. I was eating a lot at night, eating lots of macadamias, and drinking wine. So, I was primed for a change. My first day of a 3-day fast started with keto coffee (lots of coconut oil and a pinch of cayenne). Midday I had a mug of bone broth with a Tbsp of butter melted in it. At night I went right for salt-water with a dash of Chipotle chili pepper powder. I use Morton lite-salt so I can get some Potassium. I know I overate a couple times with plenty of butter and macadamia nuts for lots of fat—lots. I was poaching burgers in butter, eating rib eye steaks, fried fish with keto breading, and an entire jar of tartar sauce. Still, I didn't gain an ounce in the 3 days that I feasted. Yesterday was my first day back to fasting. I started the day with keto coffee again, and my salt-water concoction with 1 tbsp of butter, and a couple more of those without the butter later in the day. This morning I had lost another 3 pounds. So, that's 10 pounds in 7 days—the most weight I've ever lost in a week. Ever. Also, last night my blood sugar was the lowest I've ever recorded it at 66. The first day of fasting is challenging, even the second time around. Last night I was dreaming of fast food. But, I got over it, and it was smooth sailing again here on Day 2. I grew up in Orlando and ate a typical diet throughout the first 25 years of my life. I did cut out soda/pop in my teens and noticed that I lost some weight without any change to my physical activity but I was still overweight with a poor body composition. I stayed up late, ate crap junk food and drank in university and grad school. My weight peaked while on holiday in Germany as a graduation trip, 235 pounds at 6'2". I noticed new stretch marks, a marked drop in my energy levels and my clothes were tight! I was starting to get concerned about my future health and what could happen given my family medical history of diabetes, high blood pressure, heart attack, and Alzheimer's. I'm a scientist and I enjoy researching, so I went to work. With the limited nutritional knowledge I had and novice workout experience, I started reading various sources online. There are some great fitness subreddits that really helped me to understand what I needed to do. I started tracking my calories, lowered my fat intake, upped the protein and began a weight training and cardio regiment. After about 9 months of exercise and diet, my weight was about 220 pounds and I was significantly stronger in the gym, but I still had plenty of fat. This is where my story changes, I met Megan Christie (Ramos). After we had been dating for a few months, treating every trip to see each other as a vacation and eating junk, Megan explained the IDM program to me in detail and I was blown away. I began to see that by eating tons of processed/refined carbohydrates, I was only hurting myself. I also began fasting when I started eating an LCHF diet. Our cavemen ancestors certainly didn't get lucky and have a feast every day, so why do I need 3 square meals a day. I started out doing 24-hour fasts from dinner to dinner and was quickly surprised that it was much easier than I thought. After about a week, my body wasn't angry at me if at 1 pm I haven't eaten anything. I began to see weight fall off over the course of a few months. I had to buy almost an entirely new wardrobe because none of my clothes fit. I'm proud to say that I've lost 70 pounds and now weigh about 165 pounds. But then in last week’s episode from Low Carb Breckenridge with Dr. Fung answering questions, he said it’s okay to eat one meal a day when you intermittent fast and that he doesn’t really care about how many calories you get in that meal. There’s no way I could get over 1,000 calories in one meal. So if you are doing this six days a week then aren’t you getting to that place where Megan says it is detrimental with the low calorie intake? Please help me with this. Leota 4. From what I’ve read, it seems fasting is beneficial, but more so for doing an extended fast of more than three days. I'm not sure I want to fast for that long since I don't have any weight to lose. I'm 5'3", 123 pounds, 52 years old and very active with trail running 2x/week, Crossfit 2x/week and ballroom dancing 2-3 times per week. Would shorter or intermittent fasting reduce the eczema symptoms or do I need to suck it up and do an extended fast of a few days to get the most benefits? I was concerned because I didn't think I could do an extended fast because of my very physical job. Well, long story short, I completed a 26-day fast running solely on water, green tea, and KetoAide (which is a blend of water, light salt for sodium and potassium, and magnesium citrate). I felt fine, had plenty of energy, needed very little sleep, watched some scary stuff come out of my body, dropped 30 pounds, and kept off 18 of it since ending the fast. I could have physically continued to fast, but it was somewhat mentally taxing more than physically—and I just lost the will to continue it. I’ve heard you say how it lowered your cholesterol, but my doctor said fasting would raise mine. And he was right. I had my blood lipids drawn after a 5-day water-only fast as prescribed in The Complete Guide To Fasting and my numbers were wrecked as you’ll see in my results below. I was ketogenic in the before starting fasting, so this is a shock. I would love to hearing a discussion about this on your podcast. Maybe it only works for people with already high cholesterol levels? Hey Jimmy and Megan, I have water fasted for 21 days and many shorter ones for spiritual reasons. I know what it's like to have zero hunger after 3 days of no food. I have plenty of energy to walk and lift light things, but I’m not ready for eight hours of hard labor during a water fast. When I’m in nutritional ketosis, I also have no hunger but I’m able to do heavy lifting and do a full eight hours of hard labor work without trouble. I'm guessing the body fat burns better when we eat fat and protein on eating days than when we water fast. But we have plenty of stored body fat to fuel us when we fast. So my question is this: why can't I do my normal routine that includes a very physically demanding job on just a water fast and have the same amount of energy as I experience in nutritional ketosis? Full disclosure, these areas in particular are stressed in my daily life already. But fasting seems to make it worse. Could this pain be associated with the autophagy that is happening while I’m fasting? Could this be why I haven’t lost any weight during my fast? Can a gastric sleeve making longer fasting contraindicative? Hi Jimmy and Megan, I had a gastric sleeve put on three years ago and I have been LCHF off and on for most of that time shifting to keto three months ago. I have been playing around with intermittent fasting since 2015 but decided to give an extended fast a go in 2017. My goal was to fast for as long as I could, but by Day 4 I started to feel nauseous and threw up after taking my salt in the morning. By Day 5 it got worse and I broke the fast. I actually felt so horrible that I called my doctor who recommended taking pain medications and rest. My blood pressure got very high during the fast and I felt so uncomfortable from this experience. Within a few days I felt better and was back to normal again. I don’t know why I reacted so badly, but was wondering if my gastric sleeve was the reason why my extended fasting attempt was an abysmal failure. I'm trying to use an eating plan Megan posted where she broke her fast with bone broth, salad with little or no protein, and green tea for lunch and then a small serving of meat with cooked non-starchy vegetables for dinner. My question stems from something Dr. Fung said about fat. Will having tea with MCT oil in the morning mess up my fast and produce an insulin response? I’m asking because obviously I don’t need insulin to not be released when I’m not eating. I feel an increased amount of tension when I’m trying to fast that it depletes me of my willpower so much that I give up on it. I’ve suffered from varying degrees of clinical anxiety most of my life and being in ketosis is the closest to calm I’ve ever felt. I know that fasting would be extremely useful for controlling my weight and my anxiety, but I’m guessing the rise in adrenaline and cortisol are what make me feel so tense. Do you have any tricks for alleviating this feeling? I really want to keep fasting. I hope to move into an intermittent fasting protocol for long-term use and don't want to spend a couple of days per week with insomnia. I’m not having any withdrawals from caffeine or anything else that would be keeping me awake. And I’ve been in a state of ketosis for over six months, so I’m already in fat-burning mode. My blood ketones were 3.8 mmol/L and blood glucose was 3.4 mmol/L (61 mg/dL) this afternoon and I feel great during the day without cravings or hunger pangs. But eating these things make me full after dinner. I'm 37 years old and and only 5'2" tall and I have maybe 20 pounds more I want to lose. I've spent years being told by experts warning me about not getting starvation mode. How do I know I'm not negatively affecting my metabolism by doing this fasting? By the way, when I do eat breakfast, I'm hungry all day, even when it's just a protein shake. My second question after the metabolism concern is that although I skip breakfast, I do have a small cup of coffee with unsweetened coconut milk in it. Is this technically preventing me from getting the benefits of intermittent fasting or is that ok? Thanks! Tara 2. What’s the best strategy for breaking an extended fast to avoid problems? Hey guys, I definitely appreciate all your work on The Complete Guide To Fasting. I have done intermittent fasting in the past but I'm concerned about coming off of my two to three day extended fast. Other than using a small snack before a meal coming off too fast, are there any other issues that I should be aware of? Thanks again for this awesome book and podcast. James 3. What can I do about diarrhea when I attempt an extended fast? What’s causing this to happen? I've been very interested in doing an extended fast, but at approximately the 72-hour mark I get frequent diarrhea. I'm looking for specific information that can help with my situation. This doesn't seem to be a topic many people want to discuss. Will this go away and get better at some point? Is there such a thing as too much water? Is there anything I can do to control it? Working in an office setting, this feels like a disaster waiting to happen and has always stopped me from progressing further in an extended fast. I want to know how other people handle it. Jay 4. Why did my blood sugar levels skyrocket when I engaged in fasting for a few days? Thank you for this new Fasting Talk podcast. I am an avid listener to The Livin’ La Vida Low Carb Show and Keto Talk, so this new show will be added to my regular podcasting playlist. I have been eating low-carb for about a year now and have really seen an improvement in my blood tests. I haven't really tracked blood glucose consistently during the year but it has always been below 100.  Recently I tried a couple of extended fasts (60 and 96 hours) about three weeks apart.  My goal was to get deeper into ketosis as I have not been successful in maintaining ketosis with a carb level of 30 grams per day.  I know it is probably my protein intake keeping me out of ketosis where I am consuming around 150 grams per day. I thought the fasts would help me with this. But after each fast, I measured my blood ketones and blood glucose. Ketones never registered above 1.5 and my blood glucose was 155 to 170.  Have you ever seen this high blood glucose in folks after moderate length fasts like I’ve done? I have no history of diabetes and am a bit concerned about these numbers. Thanks, Dan 5. Can I do a 500-calorie bone broth fast and get all the benefits of fasting? How about adding a prebiotic in almond milk to my fast? Hi guys! Thank you for taking time to help me out with my fasting question. In The Obesity Code, Dr. Fung discusses doing 24 hour fast has no food.  He said coffee with milk is okay. In the 24 hour fast in the book by Dr. Kellyann Petrucci called The Bone Broth Diet, she says 300 to 500 calories per day of bone broth is okay. Dr. David Perlmutter of Grain Brain fame endorsed the book, but he also recommends a water-only 24 hour fast in his books. Is the 500 calorie bone broth diet correct? Would it interfere with ketosis, weight loss, and all the other health benefits that come from fasting? I’m also using a prebiotic in almond milk that totals about 80 calories. Should I skip this on fasting days? Thank you so much for your help with my questions. Hey guys, What about fasting when you have had your gallbladder removed? I've heard this stresses the liver the Whole30 book says fasting is not recommend for people without a gallbladder. I'm curious if that's true or if there is a difference between intermittent and extended fasting. I've tried to find some research about this online but no good answers have come up. I asked my gallbladder doctor about this, he had no options for me either. Any information you can provide would be appreciated. Thanks, Leah 3. Why does the notion that fasting is not good for women continue to linger? Hey Dr. Fung, I’ve heard several prominent health experts like Mark Sisson and others online who say women should probably not fast. I don’t understand why this idea of fasting being detrimental to women is so commonplace because I’ve seen nothing but success using it. I’m 48 years old and a mom of three who has used a low-carb, high-fat, ketogenic diet with regular exercise and yoga to be healthy. But my weight and fasting blood glucose levels had been going up. That’s when fasting changed all of that for me enabling me to lower my weight, reduce my waist size, and lowering my blood sugar, insulin, and A1c levels.
