Thomas Seyfried said If you want to prevent cancer, go on a one week fast once a year. People on a ketogenic or low-carb diet will have an easier time with intermittent fasting. Intermittent fasting won’t work on sugar burners aka people who regularly eat large amounts of carbs. It’s best to eliminate the idea of “snacks” in between meals to start. Compensate by eating enough during actual meal times. Some of the benefits of intermittent fasting include cancer prevention, reduces the onset of certain illnesses, lowers insulin levels, and improves mood. The ketogenic diet and fasting complement each other when it comes to resetting your body’s systems and optimizing its functions. Some of the common mistakes of people who start with intermittent fasting are consumption of artificial sweeteners thru “diet” beverages, people take it easy during fasting instead of living their normal lives, and some try to fast before the holidays which causes them to “flip out” and crash. Fasting revs up your metabolism by 10%. It also won’t make you lose muscle if you have fat, even as low as 8% body fat is enough to be used as fuel. I used to be a woman who struggled with fasting but it was because I was making one major mistake that is more common among women and less common among men. Women are much more likely to under-eat by a significant amount of calories, then they overtrain in their workouts, and on top of that try to incorporate fasting into their health strategy. When you under-eat and also fast, the body thinks you are starving and I have heard Dr. Fung speak about this inducing starvation mode. Hi Jimmy and Megan, I’m a 56-year old woman who started fasting with a goal of lowering my blood sugar and losing weight. I just finished a 24-day water and black coffee fast and ended my goal of 30 days because of extreme nausea. By Day 8, I started to drag a bit and so I began drinking some homemade bone broth. But then I became nauseous from drinking the broth a couple of days later and ditched that. A few days after that the coffee I was drinking made me nauseous, too. By Day 15, I was only drinking water and pushed my way to Day 24 before ending it. Why did I have such extreme nausea while fasting? Thank you for helping me with this! Does fasting make the symptoms of the autoimmune disease Hashimoto’s better or worse? Hi Jimmy and Megan, From what I read about fasting, it seems to be a potential therapy for autoimmune conditions. But thyroid specialist and neurologist Dr. Datis Kharrazian says that one of the 10 mistakes that Hashimoto’s patients make is the following: “When blood sugar gets too low it raises the inflammatory messenger IL-6 and promotes autoimmune flare-ups. Symptoms of low blood sugar are most noticeable between meals or if you skip meals and include shakiness, blurred vision, crankiness and irritability, and loss of function. If you feel a jump in your function and energy after eating it confirms your blood sugar was low—when your blood sugar is stable the only thing you should feel after eating is not hungry. Constantly skipping breakfast and missing meals will aggravate your autoimmune response and promote autoimmune flare-ups.” I know Dr. Fung has said there is no impact to the thyroid when fasting. But do we know what impact fasting has on Hashimoto’s? It seems the autoimmune disease is made worse by fasting, not better. I am under the care of a functional medicine doctor and have adopted all of his proposed lifestyle changes to heal my Hashimoto’s. I tried a two-day fast and saw ketones rise to 7.7 mmol and blood glucose drop to 58 before I broke the fast. It was a water-only fast with sea salt and occasional bone broth. Two days later, I didn’t feel great and developed a sore throat with excessive sleeping. Did I have a flare up of my Hashimoto’s from fasting? Thanks, Maria 3. Does fasting lead to the development of the “keto rash?” Hi Jimmy and Megan, I have been in nutritional ketosis for over two years and have successfully trained for and ran in two marathons over the past year in a fasted state. I have read The Complete Guide To Fasting and have undertaken my first extended fast which lasted four days. During the fast I had a recurrence of what appears to be a so-called “keto rash” which I have had on and off since I've been eating ketogenic. I am wondering if Dr. Fung has seen this in his fasting and ketogenic patients and what could be causing this. Is this something to be concerned about when fasting? But there are so many other benefits of fasting that are extremely crucial for all of us, not just those with weight and blood sugar issues. I am in the minority and feel like a minority because I am not overweight and I have pretty much have been the same weight forever. What kind of fasting if any would be advised for someone like me who is not dealing with excess weight but would like to get the other benefits of fasting such us immune system reset? It seems to me our ancestors, who were not overweight, were fasting without any major negative impact on their health. Is fasting just for overweight people? So while fasting you can lose about half a pound of body fat a day. On a recent episode of Fasting Talk, I heard Megan says it is detrimental to your health to not get enough calories in on your feeding days. She noted that patients who only eat 500-800 calories a day are the hardest ones to help because their metabolism has been slowed down for so long. But then in last week’s episode from Low Carb Breckenridge with Dr. Fung answering questions, he said it’s okay to eat one meal a day when you intermittent fast and that he doesn’t really care about how many calories you get in that meal. There’s no way I could get over 1,000 calories in one meal. So if you are doing this six days a week then aren’t you getting to that place where Megan says it is detrimental with the low calorie intake? Please help me with this. Leota 4. Do I need to do extended fasting to help reduce eczema symptoms or can keto/IF give me relief from this? Hello Jimmy and Megan, Thank you so much for the information you are providing the public. From what I’ve read, it seems fasting is beneficial, but more so for doing an extended fast of more than three days. I'm not sure I want to fast for that long since I don't have any weight to lose. I'm 5'3", 123 pounds, 52 years old and very active with trail running 2x/week, Crossfit 2x/week and ballroom dancing 2-3 times per week. Would shorter or intermittent fasting reduce the eczema symptoms or do I need to suck it up and do an extended fast of a few days to get the most benefits? He completed medical school and internal medicine at the University of Toronto before finishing his nephrology fellowship at the University of California, Los Angeles at the Cedars-Sinai hospital. He joined Scarborough General Hospital in 2001 where he continues to practice. We cover things from fasting as an application to obesity, to diabetes, especially Type II, to then obviously just weight loss and the health maintenance and even in athleticism as well, you know, what are the precautions around it, what should be looking for, is fasting hard, can we eat whatever we do outside of fasting and so forth, and why should we do it. Jason, welcome to the show. Jason Hey, how are you? [inaudible 00:02:04] be here. Guy Thank you, mate. Did I pronounce your surname right? I should have asked you before we started Jason Yeah. Guy Yeah? Beautiful. Look, the first question I ask everyone on the show is if a complete stranger stopped you on the street and asked you what you did for a living, what would you say? Jason [00:03:00] I’m a kidney specialist by trade, so I’m a physician. I trained very conventionally, through internal medicine, and then I did a couple of years in Los Angeles for my nephrology. About 10 years ago now, 8 years ago now, I became very interested in the question of nutrition, obesity, because that’s really the core problem of what faces us in the medical world. A lot of the problems that we face are not what we used to face, which is infections and so on. They’re all metabolic problems, that is, Type II diabetes and all the problems that go along with obesity such as sleep apnea, high blood pressure, high cholesterol, and all those sorts of problems. They really take up probably about 40%–50% of the health care budget, so you know, modern Western nations, it’s a huge problem, and worse than that, it’s a growing problem. You’ve all seen the statistics on obesity and Type II diabetes kind of rises right along with that. [00:04:00] That’s where I really got interested in, trying to see where we kind of went wrong, because obviously what we’re doing was not working. We told everybody, you know, cut calories, eat less, move more. We’ve been saying the same thing for 30 years, we’ve been singing the same song, and nothing has worked. I don’t know why we would keep using it because we knew it didn’t work. That’s really where it came from, that I really started looking into, first, the problems with obesity and the very much related problems of Type II diabetes and how our treatments are really quite incorrect, and really how to properly treat them. I felt fine, had plenty of energy, needed very little sleep, watched some scary stuff come out of my body, dropped 30 pounds, and kept off 18 of it since ending the fast. Gary HOT TOPIC: Does fasting raise or lower your cholesterol numbers? Hey Jimmy and Megan, Here’s a hot topic idea for your Fasting Talk podcast. How exactly does fasting impacts your cholesterol? I’ve heard you say how it lowered your cholesterol, but my doctor said fasting would raise mine. And he was right. I had my blood lipids drawn after a 5-day water-only fast as prescribed in The Complete Guide To Fasting and my numbers were wrecked as you’ll see in my results below. I was ketogenic in the before starting fasting, so this is a shock. I would love to hearing a discussion about this on your podcast. Maybe it only works for people with already high cholesterol levels? Here are my numbers after fasting compared with keto: Total cholesterol 255 -- previously 197 Triglycerides 169 -- previously 84 HDL 41 -- previously 69 LDL 180 -- previously 121 Any thoughts? I have plenty of energy to walk and lift light things, but I’m not ready for eight hours of hard labor during a water fast. When I’m in nutritional ketosis, I also have no hunger but I’m able to do heavy lifting and do a full eight hours of hard labor work without trouble. I'm guessing the body fat burns better when we eat fat and protein on eating days than when we water fast. But we have plenty of stored body fat to fuel us when we fast. So my question is this: why can't I do my normal routine that includes a very physically demanding job on just a water fast and have the same amount of energy as I experience in nutritional ketosis? It seems they are accomplishing the same thing and should work equally. What say you? Steve 2. Does the autophagy that fasting produces cause any pain? And can that process prevent weight loss? Hi Jimmy and Megan, I’m currently in Day 7 of my fast and for the past three days my lower body, including my calves, quads, glutes, hips, and lower back, has been so sore that it has prevented me from getting adequate sleep. Full disclosure, these areas in particular are stressed in my daily life already. But fasting seems to make it worse. Could this pain be associated with the autophagy that is happening while I’m fasting? Could this be why I haven’t lost any weight during my fast? I have been playing around with intermittent fasting since 2015 but decided to give an extended fast a go in 2017. My goal was to fast for as long as I could, but by Day 4 I started to feel nauseous and threw up after taking my salt in the morning. By Day 5 it got worse and I broke the fast. I actually felt so horrible that I called my doctor who recommended taking pain medications and rest. My blood pressure got very high during the fast and I felt so uncomfortable from this experience. Within a few days I felt better and was back to normal again. I don’t know why I reacted so badly, but was wondering if my gastric sleeve was the reason why my extended fasting attempt was an abysmal failure. I know that fasting would be extremely useful for controlling my weight and my anxiety, but I’m guessing the rise in adrenaline and cortisol are what make me feel so tense. Do you have any tricks for alleviating this feeling? I really want to keep fasting. I find that when I intermittent fast for at least 18-24 hours, I usually only eat 500-1000 calories on those days. I eat about 46 grams of protein between one or two meals and less than 25 grams of carbs all from green leafy vegetables. The rest of my calories come from fat like butter, MCT oil, coconut oil, rendered bacon fat and olive oil. I only eat clean and avoid nuts, nut flour, artificial sweeteners, and all processed foods, and only eat 100% grass-fed and pastured meats, poultry, pork and eggs and wild fish. My blood glucose is excellent and I test my blood ketones regularly with readings always between 1.0-2.0 mmol. Should I add more of something (probably fat) to bring up my calories on days I intermittent fast so as not to lower my basal metabolic rate? Is it a problem to only get 500-1000 on these days I do IF? I'm just not that hungry but could eat more if I had to. Thank you in advance for your help and thank you for all you guys do everyday! Should I be concerned about developing gallstones while intermittent fasting? I'm one of the genetically unlucky people whose blood sugar spikes on anything—even eating low-carb. I've lived a low-carb, gluten-free, sugar-free, Paleo, Specific Carbohydrate, and GAPS diet for many years but still have consistently high fasting sugars (6.5-7 mmol or 115-125 mg/dL) and am so insulin resistant with abdominal obesity rearing its ugly head. I've been a Jimmy Moore fan for ages but recently found Dr. Fung’s work and love what he’s doing because it makes so much sense. I'm now eating within a 9-hour window but have in the past had acute gallstones on very low-calorie, rapid weight loss plans. Needless to say, I’m nervous about doing this or going longer in my fasting because of these fears. My health takes a big dive if I go to extremes like fasting with my thyroid issues (Hashimoto’s) and liver problems (extreme Gilbert's disease). Are people like me just not suited for fasting? And if fasting is helpful for me, then what would you recommend to avoid these gallstone problems? Thanks for helping me with this issue! Jayne KEY QUOTE: “Fasting is not a fad diet with lots of rules to follow. It's simple, just don't eat.” – Jimmy Moore 3. Why am I not showing ketone production after four days of fasting? I have been fasting for 96 consecutive hours and my Ketonix breath ketone meter is registering little to no ketones. After 96 hours, I must have depleted the glucose in my system by now. In my mind I have to be producing ketones but it’s not showing up on this meter. What could be going on here? By the way, I’ve lost 39 pounds and 2 inches off my waist since September 15, 2016. I did enjoy three days of holiday eating with my family and fell out of ketosis after the third day. But I’ve been implementing fasting ever since and cannot get the Ketonix to register any ketones. Thanks in advance if you can offer any insights. Dennis 4. How do I deal with the negative side effects that happen to me during an extended fast? I have been on a ketogenic diet since April 2015 and decided to do a water-only fast in December 2016 to calibrate my new Ketonix breath ketone device. A 24-hour water fast is necessary to determine the low border and a 96 hour fast is necessary for the high border of my personal ketosis. I felt so good after four days that I decided to keep it going. Today is Day 6 and I’ve been having some problems since Day 3, including a general feeling of cold, pain in my calves, brain fog, and more. Is this normal? How can I resolve these issues and should I be concerned? Should I be supplementing my extended fast with salt, bone broth, and iodine? I appreciate your answering my questions! Dirk JIMMY AND DR. I have done five rounds of it over the past years to lose weight that otherwise just doesn’t come off, but each time the weight came back on faster. And now not only can I not lose weight, I am continuing to gain and there doesn't seem to be an end in sight. I suspect HCG slowed my metabolism because of the low-calorie intake putting my body is survival mode holding on to fat. Is this what is happening? ADAM NALLY’S KETO LIVING SUPPLEMENTS Jimmy and Jason respond to Dr. Phinney's concerns about fasting in Episode 5: - Labels fasting as a “fad” saying it will produce major pushback from the health authorities when people start dying suddenly - Getting adequate fluid, potassium, and sodium with an intermittent fast up to 24 hours is benign - After 24 hours of no calorie intake, the body starts breaking down protein from lean tissue - By Day 3 of fasting, the rate of lean tissue loss is equivalent to 3/4 pound per day - The data on extended fasting in THE COMPLETE GUIDE TO FASTING is misstated - If someone fasts for three days a week, the amount of lean tissue lost is greater than the amount of lean tissue they can regain in the other four days - Doesn’t believe that my 10-pound lean tissue loss (according to DXA) during my 28 of 31 day fast in January 2016 was water, claims it would take 40 days of eating again to recover this loss (actually a follow-up DXA two weeks later showed all of that 10-pound “lean tissue” loss was back on my body again—so he’s mistaken) - When someone fasts for a prolonged period of time, you’re vulnerable to “refeeding syndrome” (WWII prison camp prisoners suddenly died when they were fed copious amounts of food following extended fasting) - There’s an acute shift in minerals out of the bloodstream into the cells causing an impairment of heart function - The Liquid Protein Diet is used as an example of sudden death from refeeding syndrome that was reported as happening because of ketones…it shut down the science on ketosis for two decades - For some people, a 36-hour fast is fine as long as the other 5 1/2 days of eating have adequate minerals (apparent contradiction of his earlier statement) - We need research that shows long-term, positive, sustainability for people who are fasting to reverse diabetes and reduce body fat that goes 1-2 years - With two-week fasting or longer, the rate of protein breakdown comes down markedly. Will intermittent fasting with less than 1,000 calories send my body into starvation mode? And will unsweetened coconut milk in my coffee technically break the fast? I’ve been doing some intermittent fasting, mainly by skipping breakfast on weekdays, for the past couple of months. My concern, however, is that when I intermittent fast, and also eat no starches, I'm worried that this is similar to a crash diet, because I'm often eating maybe 800-900 calories a day because it's all vegetables and proteins. But eating these things make me full after dinner. I'm 37 years old and and only 5'2" tall and I have maybe 20 pounds more I want to lose. I've spent years being told by experts warning me about not getting starvation mode. How do I know I'm not negatively affecting my metabolism by doing this fasting? By the way, when I do eat breakfast, I'm hungry all day, even when it's just a protein shake. My second question after the metabolism concern is that although I skip breakfast, I do have a small cup of coffee with unsweetened coconut milk in it. Is this technically preventing me from getting the benefits of intermittent fasting or is that ok? Thanks! Tara 2. What’s the best strategy for breaking an extended fast to avoid problems? Hey guys, I definitely appreciate all your work on The Complete Guide To Fasting. I have done intermittent fasting in the past but I'm concerned about coming off of my two to three day extended fast. Other than using a small snack before a meal coming off too fast, are there any other issues that I should be aware of? Thanks again for this awesome book and podcast. James 3. What can I do about diarrhea when I attempt an extended fast? What’s causing this to happen? I've been very interested in doing an extended fast, but at approximately the 72-hour mark I get frequent diarrhea. I'm looking for specific information that can help with my situation. This doesn't seem to be a topic many people want to discuss. Will this go away and get better at some point? Is there such a thing as too much water? Is there anything I can do to control it? Working in an office setting, this feels like a disaster waiting to happen and has always stopped me from progressing further in an extended fast. I want to know how other people handle it. Jay 4. Why did my blood sugar levels skyrocket when I engaged in fasting for a few days? Thank you for this new Fasting Talk podcast. I am an avid listener to The Livin’ La Vida Low Carb Show and Keto Talk, so this new show will be added to my regular podcasting playlist. I have been eating low-carb for about a year now and have really seen an improvement in my blood tests. I haven't really tracked blood glucose consistently during the year but it has always been below 100.  Recently I tried a couple of extended fasts (60 and 96 hours) about three weeks apart.  My goal was to get deeper into ketosis as I have not been successful in maintaining ketosis with a carb level of 30 grams per day.  I know it is probably my protein intake keeping me out of ketosis where I am consuming around 150 grams per day. I thought the fasts would help me with this. But after each fast, I measured my blood ketones and blood glucose. Ketones never registered above 1.5 and my blood glucose was 155 to 170.  Have you ever seen this high blood glucose in folks after moderate length fasts like I’ve done? I have no history of diabetes and am a bit concerned about these numbers. Thanks, Dan 5. Can I do a 500-calorie bone broth fast and get all the benefits of fasting? How about adding a prebiotic in almond milk to my fast? Hi guys! Thank you for taking time to help me out with my fasting question. In The Obesity Code, Dr. Fung discusses doing 24 hour fast has no food.  He said coffee with milk is okay. In the 24 hour fast in the book by Dr. Kellyann Petrucci called The Bone Broth Diet, she says 300 to 500 calories per day of bone broth is okay. Dr. David Perlmutter of Grain Brain fame endorsed the book, but he also recommends a water-only 24 hour fast in his books. Is the 500 calorie bone broth diet correct? Would it interfere with ketosis, weight loss, and all the other health benefits that come from fasting? I’m also using a prebiotic in almond milk that totals about 80 calories. Should I skip this on fasting days? Thank you so much for your help with my questions.
