I have recently started having significantly higher blood pressure readings than usual (138/75 upon waking). Hi Jimmy and Megan, I am one of those people who ends up with watery stools during and after I break a fast. I’ve spent a lot of time thinking about why. I know Jimmy mentioned that he was also puzzled as to what is coming out when nothing is going in during an extended fast. Is it possible that some of the bowel bacteria (which I understand make up a good deal of stool) are dying from not being fed? And since they are dead, they are eliminated from the body while extended fasting? It certainly smells like something died in there. I also wonder whether some of the health improvements of fasting occur because of the elimination of carb-loving bacteria and replacing them with the ones that survive better while fasting. Does pain symptom improvement while extended fasting mean the damaged cells are removed or is it simply the inflammation going down?   Hey guys,   I have been suffering with awful post nasal drip for over a year along with a sore Achilles heel and I thought my running career was over (I’m a long distance runner and keto athlete). I began fasting in January consuming water, tea, and bone broth. By the 36-hour, my post nasal drip had vanished and some tooth pain I was having had disappeared as well. By Day 5, my Achilles pain was 50% more tolerable. I felt so good on Day 7 that I continued my fast. I’m currently on Day 9 and ran 10 miles yesterday. I noticed my Achilles pain was minimal during that run and the morning stiffness I had been experiencing has vanished. Finally, my nagging tennis elbow in my right arm is now pain-free. I initiated what I wanted to be a four-day fast and maybe longer recently. I made it to the 32-hour mark before experiencing signs of hypoglycemia. I was shaky, dizzy, faint, and very weak despite my heart rate at around 100 bpm. When I checked my blood sugar, it was down to 33. Because I didn’t feel safe to drive at the moment, I decided to drink 8 ounces of pomegranate juice since that was the only thing I could find to bring my blood sugar back up. I assume it improved because the symptoms decreased a bit and I made it to work safely. I'm on my second time through a 6 day cycle of 3 days fasting followed by 3 days feasting. Before I started, I was eating keto to satiety and slowly gaining weight. I was eating a lot at night, eating lots of macadamias, and drinking wine. So, I was primed for a change. My first day of a 3-day fast started with keto coffee (lots of coconut oil and a pinch of cayenne). Midday I had a mug of bone broth with a Tbsp of butter melted in it. I know I overate a couple times with plenty of butter and macadamia nuts for lots of fat—lots. I was poaching burgers in butter, eating rib eye steaks, fried fish with keto breading, and an entire jar of tartar sauce. Still, I didn't gain an ounce in the 3 days that I feasted. Yesterday was my first day back to fasting. I started the day with keto coffee again, and my salt-water concoction with 1 tbsp of butter, and a couple more of those without the butter later in the day. This morning I had lost another 3 pounds. So, that's 10 pounds in 7 days—the most weight I've ever lost in a week. Ever. Also, last night my blood sugar was the lowest I've ever recorded it at 66. The first day of fasting is challenging, even the second time around. Last night I was dreaming of fast food. But, I got over it, and it was smooth sailing again here on Day 2. But thyroid specialist and neurologist Dr. Datis Kharrazian says that one of the 10 mistakes that Hashimoto’s patients make is the following: “When blood sugar gets too low it raises the inflammatory messenger IL-6 and promotes autoimmune flare-ups. Symptoms of low blood sugar are most noticeable between meals or if you skip meals and include shakiness, blurred vision, crankiness and irritability, and loss of function. If you feel a jump in your function and energy after eating it confirms your blood sugar was low—when your blood sugar is stable the only thing you should feel after eating is not hungry. Constantly skipping breakfast and missing meals will aggravate your autoimmune response and promote autoimmune flare-ups.” I know Dr. Fung has said there is no impact to the thyroid when fasting. But do we know what impact fasting has on Hashimoto’s? It seems the autoimmune disease is made worse by fasting, not better. I am under the care of a functional medicine doctor and have adopted all of his proposed lifestyle changes to heal my Hashimoto’s. I tried a two-day fast and saw ketones rise to 7.7 mmol and blood glucose drop to 58 before I broke the fast. It was a water-only fast with sea salt and occasional bone broth. Two days later, I didn’t feel great and developed a sore throat with excessive sleeping. Did I have a flare up of my Hashimoto’s from fasting? Thanks, Maria 3. I grew up in Orlando and ate a typical diet throughout the first 25 years of my life. I did cut out soda/pop in my teens and noticed that I lost some weight without any change to my physical activity but I was still overweight with a poor body composition. I stayed up late, ate crap junk food and drank in university and grad school. My weight peaked while on holiday in Germany as a graduation trip, 235 pounds at 6'2". I noticed new stretch marks, a marked drop in my energy levels and my clothes were tight! I was starting to get concerned about my future health and what could happen given my family medical history of diabetes, high blood pressure, heart attack, and Alzheimer's. I'm a scientist and I enjoy researching, so I went to work. With the limited nutritional knowledge I had and novice workout experience, I started reading various sources online. There are some great fitness subreddits that really helped me to understand what I needed to do. I started tracking my calories, lowered my fat intake, upped the protein and began a weight training and cardio regiment. After about 9 months of exercise and diet, my weight was about 220 pounds and I was significantly stronger in the gym, but I still had plenty of fat. This is where my story changes, I met Megan Christie (Ramos). After we had been dating for a few months, treating every trip to see each other as a vacation and eating junk, Megan explained the IDM program to me in detail and I was blown away. I began to see that by eating tons of processed/refined carbohydrates, I was only hurting myself. I also began fasting when I started eating an LCHF diet. Our cavemen ancestors certainly didn't get lucky and have a feast every day, so why do I need 3 square meals a day. I started out doing 24-hour fasts from dinner to dinner and was quickly surprised that it was much easier than I thought. After about a week, my body wasn't angry at me if at 1 pm I haven't eaten anything. I began to see weight fall off over the course of a few months. I had to buy almost an entirely new wardrobe because none of my clothes fit. I'm proud to say that I've lost 70 pounds and now weigh about 165 pounds. There’s no way I could get over 1,000 calories in one meal. I had my blood lipids drawn after a 5-day water-only fast as prescribed in The Complete Guide To Fasting and my numbers were wrecked as you’ll see in my results below. I have plenty of energy to walk and lift light things, but I’m not ready for eight hours of hard labor during a water fast. When I’m in nutritional ketosis, I also have no hunger but I’m able to do heavy lifting and do a full eight hours of hard labor work without trouble. I'm guessing the body fat burns better when we eat fat and protein on eating days than when we water fast. But we have plenty of stored body fat to fuel us when we fast. So my question is this: why can't I do my normal routine that includes a very physically demanding job on just a water fast and have the same amount of energy as I experience in nutritional ketosis? I have been playing around with intermittent fasting since 2015 but decided to give an extended fast a go in 2017. My goal was to fast for as long as I could, but by Day 4 I started to feel nauseous and threw up after taking my salt in the morning. By Day 5 it got worse and I broke the fast. I actually felt so horrible that I called my doctor who recommended taking pain medications and rest. My blood pressure got very high during the fast and I felt so uncomfortable from this experience. Within a few days I felt better and was back to normal again. My blood ketones were 3.8 mmol/L and blood glucose was 3.4 mmol/L (61 mg/dL) this afternoon and I feel great during the day without cravings or hunger pangs. The rest of my calories come from fat like butter, MCT oil, coconut oil, rendered bacon fat and olive oil. I only eat clean and avoid nuts, nut flour, artificial sweeteners, and all processed foods, and only eat 100% grass-fed and pastured meats, poultry, pork and eggs and wild fish. My blood glucose is excellent and I test my blood ketones regularly with readings always between 1.0-2.0 mmol. Should I add more of something (probably fat) to bring up my calories on days I intermittent fast so as not to lower my basal metabolic rate? Should I be concerned about developing gallstones while intermittent fasting? I'm one of the genetically unlucky people whose blood sugar spikes on anything—even eating low-carb. I've lived a low-carb, gluten-free, sugar-free, Paleo, Specific Carbohydrate, and GAPS diet for many years but still have consistently high fasting sugars (6.5-7 mmol or 115-125 mg/dL) and am so insulin resistant with abdominal obesity rearing its ugly head. I'm now eating within a 9-hour window but have in the past had acute gallstones on very low-calorie, rapid weight loss plans. My health takes a big dive if I go to extremes like fasting with my thyroid issues (Hashimoto’s) and liver problems (extreme Gilbert's disease). Are people like me just not suited for fasting? A 24-hour water fast is necessary to determine the low border and a 96 hour fast is necessary for the high border of my personal ketosis. I felt so good after four days that I decided to keep it going. Today is Day 6 and I’ve been having some problems since Day 3, including a general feeling of cold, pain in my calves, brain fog, and more. Is this normal? ADAM NALLY’S KETO LIVING SUPPLEMENTS Jimmy and Jason respond to Dr. Phinney's concerns about fasting in Episode 5: - Labels fasting as a “fad” saying it will produce major pushback from the health authorities when people start dying suddenly - Getting adequate fluid, potassium, and sodium with an intermittent fast up to 24 hours is benign - After 24 hours of no calorie intake, the body starts breaking down protein from lean tissue - By Day 3 of fasting, the rate of lean tissue loss is equivalent to 3/4 pound per day - The data on extended fasting in THE COMPLETE GUIDE TO FASTING is misstated - If someone fasts for three days a week, the amount of lean tissue lost is greater than the amount of lean tissue they can regain in the other four days - Doesn’t believe that my 10-pound lean tissue loss (according to DXA) during my 28 of 31 day fast in January 2016 was water, claims it would take 40 days of eating again to recover this loss (actually a follow-up DXA two weeks later showed all of that 10-pound “lean tissue” loss was back on my body again—so he’s mistaken) - When someone fasts for a prolonged period of time, you’re vulnerable to “refeeding syndrome” (WWII prison camp prisoners suddenly died when they were fed copious amounts of food following extended fasting) - There’s an acute shift in minerals out of the bloodstream into the cells causing an impairment of heart function - The Liquid Protein Diet is used as an example of sudden death from refeeding syndrome that was reported as happening because of ketones…it shut down the science on ketosis for two decades - For some people, a 36-hour fast is fine as long as the other 5 1/2 days of eating have adequate minerals (apparent contradiction of his earlier statement) - We need research that shows long-term, positive, sustainability for people who are fasting to reverse diabetes and reduce body fat that goes 1-2 years - With two-week fasting or longer, the rate of protein breakdown comes down markedly. Hey guys, I’m 71 years old with a low BMI and no medical or health conditions to deal with. I resistance train twice weekly lifting heavy and have been ketogenic for the past four years.  Every couple of months, I do a water-only fast for about 3 days and have noticed that my blood sugar goes down to the upper 50’s and my blood ketones reach a very high level of 8.0 mmol on the Precision Xtra testing device. Is this a problem? I feel fine so I do not allow it to interrupt my fast. But I can’t help but wonder if the blood ketones can go too high for a non-diabetic perhaps impacting blood pH levels. Any input on this would be greatly appreciated!  Thank you again for your great new podcast. Richard 2. Why do blood sugar levels increase during a fast when no food is consumed? Is it muscle wasting? Hi Jimmy and Megan, I am on Day 3 of a 5-7 day fast, but I seem to be struggling with higher than normal blood glucose levels. I started fasting after a trip when I definitely fell out of ketosis from indulging a bit too much on carbs. On the morning of Day 2, my fasting blood glucose was 80. But by midday, it was up to 110 and only down to 99 by bedtime. When I woke up the next morning, it was still 99 and stayed elevated all day long despite the fact my blood ketones are 2.3 mmol. What’s going on here? Is this muscle-wasting that I need to be concerned about since no food is being consumed? I feel great otherwise and am doing much better than I have on other fasts.  Thanks so much! Robin  3. Should I be concerned about my blood sugar levels going too low while fasting? Hi Jimmy and Megan, I’m on Day 4 of my fast and my blood sugar has been reading very low the entire time. It’s been below 70 since Day 1 and I registered 45 this morning. I had a serving of kombucha and two hours laterthat brought my blood sugar back up to 60. All I’ve been consuming is water, black coffee, and tea along with bone broth to get through the hard days. Interestingly, despite the very low blood glucose levels, I have physically and mentally felt the best I have since I began this fast. Can you shed some light about what’s happening and whether I should be concerned about this super-low blood sugar while fasting? Thank you,  Jennifer 4. How is it possible I haven’t lost a single pound while fasting for eight days? Hey guys, One of my goals is to do a 21-day fast and I’m currently in Day 8 of my extended fast. I drink homemade bone broth when I need it and a Bulletproof Coffee if I really need something—typically when my hands and feet feel like ice. I’ve been eating keto for over a year and typically intermittent fast daily. I workout hard in the gym virtually everyday and have done a 6-day fast and a 10-day fast previously. On the 10-day fast, I lost a lot of weight and gained it all back while on the 6-day fast, I lost some and gained it back too—but I felt amazing! On my current fast, I haven’t lost a single pound! So what’s going on with my weight? How is it possible I haven’t lost a single pound when all I’ve consumed is Ketoaide and 1 cup of Bulletproof Coffee? I’m athletic and rather muscular for a female and workout hard to help manage my multiple sclerosis. Does consuming a small amount of calories (300 daily) slow down fat metabolism while fasting? Will intermittent fasting with less than 1,000 calories send my body into starvation mode? And will unsweetened coconut milk in my coffee technically break the fast? I’ve been doing some intermittent fasting, mainly by skipping breakfast on weekdays, for the past couple of months. My concern, however, is that when I intermittent fast, and also eat no starches, I'm worried that this is similar to a crash diet, because I'm often eating maybe 800-900 calories a day because it's all vegetables and proteins. But eating these things make me full after dinner. I'm 37 years old and and only 5'2" tall and I have maybe 20 pounds more I want to lose. I've spent years being told by experts warning me about not getting starvation mode. How do I know I'm not negatively affecting my metabolism by doing this fasting? By the way, when I do eat breakfast, I'm hungry all day, even when it's just a protein shake. My second question after the metabolism concern is that although I skip breakfast, I do have a small cup of coffee with unsweetened coconut milk in it. Is this technically preventing me from getting the benefits of intermittent fasting or is that ok? Thanks! Tara 2. What’s the best strategy for breaking an extended fast to avoid problems? Hey guys, I definitely appreciate all your work on The Complete Guide To Fasting. I have done intermittent fasting in the past but I'm concerned about coming off of my two to three day extended fast. Other than using a small snack before a meal coming off too fast, are there any other issues that I should be aware of? Thanks again for this awesome book and podcast. James 3. What can I do about diarrhea when I attempt an extended fast? What’s causing this to happen? I've been very interested in doing an extended fast, but at approximately the 72-hour mark I get frequent diarrhea. I'm looking for specific information that can help with my situation. This doesn't seem to be a topic many people want to discuss. Will this go away and get better at some point? Is there such a thing as too much water? Is there anything I can do to control it? Working in an office setting, this feels like a disaster waiting to happen and has always stopped me from progressing further in an extended fast. I want to know how other people handle it. Jay 4. Why did my blood sugar levels skyrocket when I engaged in fasting for a few days? Thank you for this new Fasting Talk podcast. I am an avid listener to The Livin’ La Vida Low Carb Show and Keto Talk, so this new show will be added to my regular podcasting playlist. I have been eating low-carb for about a year now and have really seen an improvement in my blood tests. I haven't really tracked blood glucose consistently during the year but it has always been below 100.  Recently I tried a couple of extended fasts (60 and 96 hours) about three weeks apart.  My goal was to get deeper into ketosis as I have not been successful in maintaining ketosis with a carb level of 30 grams per day.  I know it is probably my protein intake keeping me out of ketosis where I am consuming around 150 grams per day. I thought the fasts would help me with this. But after each fast, I measured my blood ketones and blood glucose. Ketones never registered above 1.5 and my blood glucose was 155 to 170.  Have you ever seen this high blood glucose in folks after moderate length fasts like I’ve done? I have no history of diabetes and am a bit concerned about these numbers. Thanks, Dan 5. Can I do a 500-calorie bone broth fast and get all the benefits of fasting? How about adding a prebiotic in almond milk to my fast? Hi guys! Thank you for taking time to help me out with my fasting question. In The Obesity Code, Dr. Fung discusses doing 24 hour fast has no food.  He said coffee with milk is okay. In the 24 hour fast in the book by Dr. Kellyann Petrucci called The Bone Broth Diet, she says 300 to 500 calories per day of bone broth is okay. Dr. David Perlmutter of Grain Brain fame endorsed the book, but he also recommends a water-only 24 hour fast in his books. Is the 500 calorie bone broth diet correct? Would it interfere with ketosis, weight loss, and all the other health benefits that come from fasting? Why does the notion that fasting is not good for women continue to linger? Hey Dr. Fung, I’ve heard several prominent health experts like Mark Sisson and others online who say women should probably not fast. I don’t understand why this idea of fasting being detrimental to women is so commonplace because I’ve seen nothing but success using it. I’m 48 years old and a mom of three who has used a low-carb, high-fat, ketogenic diet with regular exercise and yoga to be healthy. But my weight and fasting blood glucose levels had been going up. That’s when fasting changed all of that for me enabling me to lower my weight, reduce my waist size, and lowering my blood sugar, insulin, and A1c levels.
