Some ambulance crews may not have the best understanding of how a fire crew is going to run a call, with everyone assigned specific roles – often the fire department will be allowing a new crew member to lead the callI always struggled with arriving first on scene on the ambulance because that role is more work and more pressure What does fire like from the ambulance crews when they arrive first?First on scene should be allowed to lead the call and ask for help where needed, second on scene should not be pushing their way in and trying to take over the callSometimes the providers that take over lack experience or are not yet comfortable enough with their own skills to allow someone else to leadIf you have another provider on scene constantly trying to interrupt, give them something to do - often this applies to a disruptive family memberDoes the ambulance paramedic have to attend in the back if the fire paramedic rides in?As a previous ambulance paramedic, I viewed the ambulance as my space, meaning I always appreciated it when the fire paramedic had the respect to treat it as such, asking to ride into the hospital as opposed to telling me they were riding in. As a general rule, if the fire paramedic believes they need to ride in due to acuity, the ambulance paramedic should also attendKash, as a medical director, gives his opinion on this situationI really appreciated it when the fire crews respected our ambulance because the front is truly our officeEMT's can ride in too on low acuity where more hands, not ALS treatment, is neededI've talked before that a paradigm shift is needed for the paramedics at times, where they are more likely to have to attend more calls then their EMT partners -  easy for me to say from outside the field now – but transporting the patient is almost always the safest, lowest liability option, we shouldn't be trying to get out of transports just because it's less workAlways treat the patient like they are a family memberWe are looking for proof that the patient is not sick, as opposed to assuming they are not sick from the outset, our approach is different in emergency medicineWe have, historically, reversed hypoglycemia or opiate OD, and the patient has refused when maybe transport to the hospital is warranted despite the fact that we have temporarily fixed a major problemSupport the showFull show notes can be found here: Episodes - Practical EMS - Content for EMTs, PAs, ParamedicsMost efficient online EKG course here: Practical EKG Interpretation - Practical EMS earn 4 CME and learn the fundamentals through advanced EKG interpretation in under 4 hours. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Melissa: Hi Dr. Cabral - on my food sensitivity test call, the coach said that there would be concern if a person didn't have any immune reactions to food because food is a foreign invader of the body. I see what she's saying to a certain extent but this was confusing to me because I thought the point of taking the test was to figure out your sensitivities and remove them for a certain period of time then reintroduce them to see if you're still sensitive…the goal being to remove a negative immune reaction and reduce stress on your body. Maybe I was misunderstanding what she was saying but this just sounded counterintuitive to me. Can you please explain? Could the sensitivities be affecting the IgG part of your immune system in which wouldn't you want to remove them?     Dylaini: (Pronounced: Delaney) Hi Dr. Cabral, thanks for all of the knowledge and information you share. I am an RN hoping to make the jump to IHP soon! My son is 13 months old and he's already been on antibiotics 3x… once for fever of unknown origin at 50 days old and 2 other for ear infections. I know you and your coaches don't see kids until three years old. Are there any tips you have for kids under three to rebuild gut Microbiome because of these antibiotics and any suggestions of what to do to instead to avoid them in the future.     Jackie: Hi Dr. Cabral, thank you for all you do. Your team and the work you do have been life changing for me. I struggle with garlic and onions - they cause me to burp & have gas for 48hours. It also take 3-4 days for me stop stinking (even with daily sauna/cardio)- very unpleasant. I don't eat them unless at family or friends or out of my direct control as a common cooking ingredient. Is there anything I can do to help with digestion. For context did big 5 no food intolerances other than eggs/Brazil nuts - FMD 21, HVM, CBO, CBO FIN, & FMD 7. I also ran allergy tests and nothing came back. I take HCL + digestive enzymes with meals to support digestion. I probably have some insulin resistance (years of high carb/ processed diet) GD during pregnancy (failed by 2 pts) tinnitus acquired during third trimester. Modern medicine is primarily influenced by big pharma and big food. The medical industry is also overspecialized, and nutrition and food therapy training is virtually nonexistent. Check out these 9 common medical industry lies! 1. You don't need a gallbladder. The gallbladder stores bile salts that help you extract fats from the digestive system. If you don't have a gallbladder, you won't be able to concentrate bile. 2. Statins have minimal side effects. Statins have major side effects and increase your risk for type 2 diabetes and liver damage. 3. Hypertension is caused by too much salt in the diet. Instead of trying to lower sodium, increase potassium. Focus on adequate potassium intake to support healthy blood pressure levels. 4. Low amounts of vitamin D will fix a deficiency. If you're low in vitamin D, 600 to 800 IU will not be enough. You need around 10,000 IU every day. 5. Red meat causes cancer. Grass-fed red meat is one of the healthiest foods you can eat. It's a high-quality protein source and contains all of the B vitamins, zinc, selenium, copper, iron, creatine, l-carnitine, coenzyme Q10, and glutamine. 6. Avoid saturated fats. Unsaturated fats are a much bigger problem. Dangerous seed oils and ultra-processed foods are full of unsaturated fats—not saturated fats. 7. Your weight is causing your health issues. Your weight is a symptom, not the cause of your problems. The root cause of your weight problems and the associated health issues is likely insulin resistance. 8. Plant-based diets are best. Doctors and dieticians often falsely claim that plant-based diets are cleaner. Most fake meat products contain many ultra-processed ingredients. 9. Hypoglycemia is a complication of diabetes. If you have hypoglycemia, you have too much insulin. Instead of focusing on lowering insulin, doctors will advise patients to eat glucose pills or candy if their blood sugars drop too low.
