Can ketogenic therapy help college students living with depression? Jeff Volek, PhD, RD, is investigating this question in a pilot study at The Ohio State University, where he is a professor in the Department of Human Sciences. Dr. Volek has been a world-renowned expert in ketogenic diets for more than two decades. He has pioneered critical work raising awareness of how the keto diet can reverse diabetes and metabolic syndrome and enhance athletic performance and recovery. Eirin Winje har type 1 diabetes og klarer etter nesten 28 år endelig å trene uten at diabetesen ødelegger treningseffekt og treningsglede - fordi hun er i fettforbrenning/ketose. Hun begynte å løpe i januar, og bestemte seg samtidig for å løpe Oslo Maraton allerede i september! Eirin er en ekte baktropper - og hun fullførte halvmaraton med perfekt blodsukker! Her forteller hun hvordan hun spiser til hverdags og før hun skal løpe, hvordan et ketogent kosthold virker på kroppen og hvordan det gjør at hun kan nå løpe med sin diabetessykdom. Men dette er ikke bare for dem med type 1 diabetes! Eirin er psykologspesialist med spesialisering i rus og avhengighetslidelser. I tillegg har hun doktorgrad i årsaker til spiseforstyrrelser. Dr. Jeff Volek has been investigating how humans adapt to ketogenic—and carbohydrate-restricted diets for the past 30 years.  Today, Jeff returns to STEM-Talk to discuss a growing accumulation of studies supporting a ketogenic diet as a way to improve metabolic health, as well as research confirming the relative safety of dietary fat. Jeff is a professor in the Department of Human Sciences at Ohio State University. He is known for his research on the clinical application of ketogenic diets in the management of insulin resistance and type-2 diabetes. His research particularly aims to understand individual variability, including how well-formulated ketogenic diets alter fatty acid composition, lipoprotein metabolism, gut microbiome and overall metabolic health. Jeff has performed several prospective diet studies that demonstrate that well-formulated ketogenic diets result in substantial improvements in (if not complete reversal of) metabolic syndrome and type-2 diabetes. Ken mentions that Jeff, who appeared on episode 43,  has perhaps published more research on the ketogenic diet and its effects on humans than anyone. While most STEM-Talk listeners are familiar with Jeff's research, Ken points out that many people might not know that Jeff was once an accomplished powerlifter, achieving impressive numbers for his body weight. Ken asks Jeff what his best lifts were, and if his background in powerlifting inspired him to study exercise physiology. [00:05:25] Dawn mentions there is a paradigm shift in terms of low-carb diets and the public perception regarding the relative safety of dietary fat.  Americans have long been led to believe that saturated fats lead to obesity and heart disease. Dawn goes on to explain that in the last 20 years, there has been a steady accumulation of studies supporting carbohydrate restriction as well as the relative safety of dietary fat. Jeff addressed this in a paper in Science titled “Dietary Fat: From Foe to Friend?”, and also a paper in the Journal of the American College of Cardiology titled “Saturated Fats and Health: A Reassessment and Proposal for Food Based Recommendations” Dawn asks Jeff to talk about this research and what listeners should take from it. [00:08:37] Ken mentions that Jeff at one point in his life demonized fat, and was a strong advocate for low-fat diets. Ken asks what changed his mind on this issue. [00:10:04] Dawn mentions that when Jeff was interviewed back in 2017, he was in the early stages of launching Virta, a company that was founded in 2014 to address the type-2 diabetes epidemic that we're seeing in the United States and across the world. (6:00) It also discusses how various nutrients play the part in various ways in human evolution.  The big question Vinnie and Stephen discuss is: are you taking care of your brain? Vinnie's interest in the brain comes from taking years of beatings playing football; as a result, he's curious about CTE (chronic traumatic encephalopathy). (16:00) Stephen says it's never too late to be able to make improvements; the question becomes "what can you do"? (21:25) Part of the challenge is the parameters given to doctors that make them hesitant to suggest low-carb. Mark Cucazzella is brought up as an example. The two discuss the movie "First Do No Harm", by Jim Abrams of the Charlie Foundation. (25:05) Charlie's doctors wanted to give him a lobotomy for his seizures; instead, they controlled his epilepsy with the ketogenic diet. They also discuss Eric Westman, Jeff Volek, Steve Phinney, Tim Noakes, and others and what an important part they play in low-carb science. They switch to discuss certain limitations of the current healthcare systems. (29:50) Certain low-carb doctors are walking away from standard practice so they can actually practice health care and not sick care. ESSENTIAL NUTRIENTS Vinnie and Stephen also discuss what zinc and other minerals are optimal for the brain and how they help. (34:10) They discuss iodine, zinc, selenium, and certain deficiencies. They chat about the best sources of various nutrients. More is not always better, and Dr. Cunnane explains why. Don't over-intellectualize it: just eat sensibly. Our interview today is with Dr. Vyvyane Loh, a board-certified physician in obesity and internal medicine. She is the founder and leader of Transform Alliance for Health, a Boston preventive-care practice that  specializes in weight management and the treatment of chronic metabolic diseases such as diabetes, hypertension and dyslipidemia. She and her staff are known for helping people lose 50 pounds or more and getting their type-2 diabetic patients off their many medications. Vyvyane has spent her medical career developing expertise in immunology, metabolic syndrome, fat metabolism, clinical nutrition, and preventive medicine. In today's interview, we discuss how abdominal, or visceral, fat is linked to a wide range of metabolic disorders. Vyvyane goes on to explain how there's a clearcut association between obesity and decreased brain volume that rarely gets discussed. When her overweight patients complain about their behavior around food and how they consistently give in to snacks that patients know are bad for them, Vyvyane explains how the challenges they are facing is often a result of the brain struggling with decreased blood flow and the shrinkage of neurons. Vyvyane also shares how a patient asked Vyvyane if she knew anything about the Atkins diet, and although she didn't, Vyvyane ended up doing the diet along with her patient. This led Vyvyane to start seriously researching whether a ketogenic diet could help people not only lose weight, but also reverse chronic disease. Toward the end of today's interview, we explore Vyvyane's interest in macrophages, which are specialized cells involved in the detection and destruction of bacteria and other harmful organisms. We also have a nice discussion about how Vyvyane took some time off from practicing medicine to enroll in the writing program at Warren Wilson College in North Carolina in 1999. She spent the next two years writing a novel, “Breaking the Tongue.”  Set in Singapore during World War II, her book was nominated for the prestigious International IMPAC Award in fiction and was selected by the New York Public Library as one of its top 25 books of 2004. His recent research collaboration with Dr Jeff Volek looked into the benefits of ketogenic diets for women with metastatic breast cancer.  Like many others, Dr David thought it was healthy to have a high-carb, low-fat diet.  Dr David's book, BioDiet, was made for a layperson to understand the benefits of ketogenic diets [05:05] Debunking High-Carb, Low-Fat Diet  The high-carb, low-fat diet is an experiment conducted by health bureaucrats on millions of North Americans for years.  Exercise is good for the body, but it's not the key to losing weight. Instead, people must focus on their diet to shed off some pounds.  Obesity is your body's physiological reaction to excess carbohydrates in your diet. Tune in to know how David came to this realisation through a radio show!  [09:06] Why Ancel Keys Is Wrong Ancel Keys is an American physiologist who discovered that, supposedly, high saturated fat and cholesterol cause heart diseases.  While his hypothesis seems intuitive, there is no evidence to prove Keys' statement.  It's incorrect to consider calories as the primary contributor to weight loss or gain. It's the food's effect on your metabolism that matters.  The health industry is now revisiting the high-fat, low-carb model due to recent studies revealing sugar and smoking as the primary causes of heart disease and not fat.     [13:56] Obesity in America The rise of obesity rate in America happened during the 1980s when it released its first dietary guidelines on a high-carb, low-fat diet The benefits of keto are therapeutic; it's not a fad. It's difficult to gather all relevant data debunking fat as unhealthy; Proving a nutritional intervention prevents disease means controlling the diet of 1000 people for decades. Studies show that a ketogenic diet is an effective intervention for some chronic illnesses.   [19:12] On Starting a Ketogenic Diet  Always consult with your physician first before trying a ketogenic diet.  Some might experience mild side effects because of their changing metabolism.  There are multiple benefits of keto: weight loss, fair skin complexion, protection from chronic diseases, and more. [22:29] The Factors of Illness Chronic diseases and cardiovascular diseases are interrelated.  Another one of the benefits of keto is as a therapeutic intervention for neurological disorders. Insulin resistance, inflammation, and obesity are the axes of illness. Listen to the whole episode to learn how these three factors result in chronic diseases.  Most doctors treat the symptoms and not the disease. The root of diseases is at the cellular metabolic level.  [27:00] Poor Metabolic Health Amid the Pandemic Most people who contracted COVID-19 are those with poor metabolic health. If we could solve the public's diet problem, we could reduce the burden on hospitals.  There are no definitive findings that keto can help decrease the risk of contracting COVID-19. However, studies show promising results on how it can boost one's immune system against the virus.  [33:52] The Benefits of Keto  Most postmenopausal women with chronic illnesses found that the benefits of keto include working against brain fog. Ketone is a cleaner brain fuel and causes less inflammation.  Switching to a keto diet activates the essential proteins for a healthy brain.  [38:18] On Alzheimer's Disease Alzheimer's is a form of type three diabetes or insulin resistance of the brain.    Suzanne de la Monte's study found that exogenous ketones called MCT oil can improve the cognitive function of people with Alzheimer's disease.  Ketones improve health and brain function.  [42:16] The Global Burden of Disease Diet is the primary global burden of disease.  Approximately a sixth of the calories young people take in America is fructose. To maintain a healthy life, avoid highly processed food and sugar.  [45:08] Why Natural is Not Always Good For You Not all plants are good for our bodies and minds.  Plants don't exist for good health. They're there to do whatever it takes to survive.  The plants we eat today are not the same as our ancestors ate 50 years ago. [48:56] Getting the Trend Right  Reduce your sugar, high glycemic index carbohydrates, flour, and the like to see great results on your health.  It's okay to eat some treats, but you have to be conscious about it. There must be a limit set to maintain a healthy lifestyle.  [53:32] Keto Flu  Keto flu involves having cravings for food that you previously consumed.  In the first phase, it's good to take MCT oil to help familiarise your body with the ketones.  It's good to take lots of magnesium and potassium iIf you don't have any kidney problems.  Some might experience side effects when starting the keto diet. These include nausea, changes in stomach acid, brain fog, sleep disturbance, and the like.  Adapting to a keto diet decreases your insulin level that subsequently reduces your body water.   [55:57] Fully Adapting to Keto The side effects of the keto diet may last for a couple of days. However, you'll feel a sudden boost of energy once you get over the keto flu.  It's not advisable to cycle in and out of keto.  The best lifestyle decision you can make every day is on what's the end of your fork.  7 Powerful Quotes [10:48] ‘When you consider the calories to be the cause of weight loss or gain, you're going down the wrong road. It's the effect of the foods you eat on your metabolism—in particular, I would argue, on your hormone balance and insulin.'    [23:07] ‘If you're insulin resistant, you become more inflamed and more obese. If you're more obese, you become more insulin resistant. So they make each other worse…eventually, it results in a state of poor metabolic health that will then manifest itself…as one of these chronic diseases.'    [24:05] ‘The thing is you're treating the symptom, not the disease. So the disease is at the cellular metabolic level.'    [27:00] ‘If you look at the people that are having bad COVID outcomes, those are people with poor metabolic health—that is absolutely known. So if we can improve the overall metabolic health of the public, we would reduce the burden on our hospitals again.'   [35:44] 'What happens is you put these stressors on your cells to change its metabolism and that turns the genes on.'   [40:32] 'So we're realising that the ketones don't just provide optimal health. They might actually even improve brain function over and above what you expect.'   [45:52] ‘Plants aren't there for good health. They're there to do whatever it is that makes the plant more successful.' About Prof David Harper Prof David Harper is a health educator, researcher, and technology CEO. The theory is that supplementing with additional beta-hydroxybutyrate (BHB) would result in muscle preservation as well as an increase in body fat loss over keto alone. But that’s not what happened. ⁣ There was no difference in weight loss or fat loss between the groups and after the first two weeks there was no difference in blood ketone concentrations. In other words, the endogenous production took over after adaptation and provided all the benefits they needed. Gee, imagine that! Anyone pushing exogenous ketone supplements on you as a weight loss tool REALLY need to read this study and realize just how much they are lying to their clients and customers. Don’t fall for the marketing gimmicks and the sad sob stories of how their life has been changed because they drink ketones. Intermittent and extended water fasting has a great deal of momentum on the internet and I wanted to share with you some of the concepts to consider exploring in your life to gain the benefits associated with such strategies to improve your vitality and overall wellbeing. I share four books and authors that you can search on the internet and especiall youtube as they all have a stong presence on social media outlets. First some concepts Intermittent Fasting is Eating Less Often and giving your body a shorter window of consuming caloric foods and a longer period of rest and digest. An example would be to consume food and caloric beveraes within an 6, 8 or 10 hour period leaving 18, 16 or 14 hours for you digestive system to rest. Intermittent fasting is the beginning of Metabolic Rehabilitation to restore your metabolism to burn and efficiently utilize Ketones (a by product of burning fat) as a fuel source; especially prefered by the Brain and Heart Diet Variation: Once adapted to fat burning and utilizing Ketone we may want to mix up our diet variation with a fasting and feasting cycle. We may have an intermittent cycle of 6 hours of feeding and 18hours of fasting 3x per week, 2 feast days where we consume a little more carbohydrates of more protein and say two days of 1 meal only or a 24 hour fasting day. Remember this is an artistic practice and you can play around with this concept. Extended water fasting is ideally consuming water only for 3,5 , 7, or more days with 5 being scientifically accepted as the best amount of days to shoot for especially for beginners. Also, it would be wise to fat adapted before venturing into the extended water fast and therefore, practicing intermittent fasting for 5, 6 or more weeks. To help you know if you are becoming fat adapted it is necessary to measure blood glucose levels and ketone levels. I use a keto-mojo for this purpose. During my fast this week I measured BG -81 and Ketones - 3.0 with a Keto Index of 1,5 which is a deep state of autophagy or nutritional ketosis. Extended water fasting has three phases The preparatory phase - get you mind right, get your home and environment right and get your support right The fasting phase - listen to your body and mind. You may need to make some modifications so you can roll with this and not create a huge stressful event where you will yourself through it in pride. The goal is practice not perfection The post phase - very important as to not consume lots of food or carbohydrates or sugars! This could be a disaster leading you to be really sick. It is all about metabolic rehabilitation and down regulating leptin and insulin surges thus blood sugar control, increasing insulin sensitivity, down regulating inflammation, improving gut and immune system function, anti-aging, and numerous other benefits. If you find that you have difficulty with either intermittent fasting or extended water fasting consider you might have high levels of stored cellular toxicity. And this may need to be addressed along the way. Researcher and elite athlete Brianna Stubbs is back on the podcast today, checking in before her recent Ironman competition in Santa Cruz, California. Since we last talked with Brianna she’s left HVMN and joined the Buck Institute for Research on Aging as Lead Translational Scientist. He recently led research and clinical operations for San Francisco-based Virta Health, a nationwide medical provider that delivers the first clinically-proven treatment to safely and sustainably reverse type 2 diabetes without medications or surgery. Rachelle and I studied our Masters in Human Nutrition at Deakin University together and she alludes to our many interesting discussions and debates during that time. I thoroughly enjoyed this episode with Rachelle; her attention to detail and depth of knowledge is apparent in the interview. Rachelle explains the difference between the Paleo and Keto diets, both of which are low carb diets but are significantly different in terms of the ratios of fats, proteins and carbs consumed. Given the current popularity of the diets with celebrities and the like, it can be tempting to lump them into the fad category. However, in relation to the Keto diet, Rachelle explains its scientifically documented anti-inflammatory effects and the numerous benefits that flow from that. In her nutritional practice Rachelle has seen weight loss, the reversal of Type 2 Diabetes, improvements in symptoms of patients with Alzheimer’s disease, polycystic ovarian syndrome and insulin resistance. Dr. Dominic D’Agostino returns to STEM-Talk to give Ken and Dawn an update on his research into ketogenic nutrition. Dom was the guest on episode 14 back in 2016 when ketogenic diets didn’t even show up on a list of the top-10 diets that people Googled. Since then, the search term “ketogenic diet” has risen to the top of the list. In today’s episode, Dom talks about his past 10 years of research into ketogenic diets and what he is learning about the physiological benefits of nutritional ketosis. Dom is tenured associate professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine.He also is a research scientist here at IHMC.Throughout his career, Dom has been a researcher with a diverse background in neuroscience, molecular pharmacology, nutrition and physiology. Show notes: [00:02:55] Dawn begins the interview mentioning that when she and Ken started hosting STEM-Talk, the ketogenic diet wasn’t on the list of the top-10 most Googled diets of 2015. Today, however, Dawn points that ketogenic diet is number one on the list. She asks Dom if he foresaw sudden mass interest in a ketogenic diet coming. [00:04:12] Ken asks Dom for his thoughts on how the ketogenic diet has went from being very obscure to becoming a household term. [00:06:04] Ken comments on the evolutionary component of the ketogenic diet and how our ancestors must have gone in and out of ketosis based on the availability of food. He also comments on the unique aspect of the ketogenic diet, being that it has an objective measurement, and asks Dom to talk about that. [00:06:59] Dawn comments on the cynicism regarding the ketogenic diet, particularly from nutritionists. She asks Dom to address the criticism and pushback that the ketogenic diet receives from so many nutritionists. [00:10:02] Ken mentions that some fields are resistant to change and new science due to the emotion behind established theories. Dom agrees and then talks about how people, even doctors, are resistant to new data and new science. [00:11:13] Dom talks about the most common misconceptions and overrepresentations of the ketogenic diet. [00:12:54] Ken discusses his dissatisfaction with the term “ketogenic diet” since the word diet implies the mandated consumption of certain food items. He goes on to say that if one is in ketosis, then, by definition, they are doing a ketogenic diet, even though they may be in ketosis because they have been fasting and haven’t eaten anything.  Ken and Dom discuss how knowledge about ketogenic nutrition has changed over time and that it is certainly possible to eat an unhealthy ketogenic diet. [00:15:35] Dom and Ken talk about the results of a recent Megan Roberts paper, “A Ketogenic Diet Extends Longevity and Healthspan in Adult Mice,”that showed a 13% increase in the lifespan of the mice along with remarkably improved healthspan. [00:20:26] Dom shares his thoughts on the potential of exogenous ketones in the context of blood glucose regulation. [00:27:07] Ken asks if Dom has been tracking Virta Health, which was founded by Dr. Jeff Volek who was interviewed in STEM-Talk episode 43. Virta Health has been publishing impressive results of its trials that show  people reversing type-2 diabetes via a well-formulated ketogenic diet. Today we have a double header with 2 great figures as well as athletes in the low carb community. Zach Bitter is the 100 mile American record holder and 12 hour world record holder for ultramarathons. He hosts the Human Performance Outliers podcast with Dr. Shawn Baker and has learned many lessons from amazing people along the way. He has wealth of knowledge of fat adapted athletics and fueling himself with meat while healing his body. But first we have Dr. Mark Cucuzzella who is a Professor of Medicine at West Virginia University medical school, Fellow of the American Academy of Family Physicians, a Family physician for 20 years, and a Lt Col in the US Air Force Reserves. He’s doing such great work in his hospital and community and has implemented some very beneficial programs. He’s a great guy and was a big supporter of the film early on. It seems to be dependent on how soon you’re going to train again. If it’s only once per day it seems carbs aren't really necessary. 2 sessions per day, probably yes. He only needs carbs when doing intensity plus high volume. Either one separately he doesn't. How he got into the carnivore world He’s done 73 podcast episodes with various experts and has learned a lot 2 big things are that vegetables aren’t really necessary it seems and there’s a lot of myths about protein that are turning out to not be true. There’s probably no such thing as too much protein My ultra simplistic theory is that animal foods are +1, processed foods are -1, and plant foods are just neutral (or 0). Plant foods aren't doing much for you other than taking up space in your stomach from things that could be worse. The real nutrition is from the animal foods. Simplemente se sigue el mismo parámetro de la dieta conocida como cetogénica o keto. Sin embargo, vale la pena entender por qué un corredor estaría interesado en este estricto plan alimenticio. Para comenzar, se busca que el cuerpo ocupe como energía cetonas en lugar de glucosa. Esto se logra manteniendo la ingesta de carbohidratos a un nivel muy bajo. Menos de 30 gramos por día es el nivel máximo permitido. La hipótesis de esta dieta keto para runners es que al haber mayor reservas de grasa comparadas a glucosa, es más inteligente basarse en grasa para correr que en glucosa. Miremos las reservas de energía en el cuerpo:   Bodegas de energía en el cuerpo. Esto se mide en kilocalorías (kcal). En total, tenemos alrededor de 115,000 kcal a nuestra disposición. Sin embargo, estas están presentes en diferentes formas: Grasa (71.8%) 80.000 en tejido adiposo 2.500 en triglicéridos musculares 75 en triglicéridos plasmáticos 7 ácidos grasos libres séricos Proteína (26%) 30.000 en Proteína muscular Carbohidratos (2.19%) 1700 en Glucógeno en músculos 500 en glucógeno en hígado 90 en glucosa en sangre ATP/CP (0.01%) = Trifosfato de adenosina y fosfato de creatina 5 en diferentes órganos Aunque una dieta keto para runners tiene ventajas, examinemos las desventajas que desglosamos en el episodio.   Desventajas: Emma de Sistemma.com.mx nos explico una a una estas desventajas: Mal aliento. Deshidratación. Aumento del colesterol. Caída de pelo y cansancio. Constipación y vómitos temporales. Puede producir piedras en los riñones, pancreatitis y deficiencias de vitaminas y minerales. Y Luis no hablo de algunas desventajas puntuales para los runners: El cuerpo quema carbohidratos y grasa de manera muy diferente. Louise Burke escribió en “Nutrición Deportiva: Mucho más que calorías†que la oxidación de grasa se inhibe en el cuerpo cuando la intensidad es más alta que el 75% de tu máxima capacidad aeróbica. Si sobrepasas esa intensidad, el cuerpo automáticamente cambia a oxidar carbohidratos, ya que la movilización de grasas, la sintetización de glucosa y la glucólisis toman más tiempo de lo que el cuerpo puede mantener un esfuerzo intenso. También, toma 20% mas de oxigeno liberar energía de grasas que de carbohidratos, lo que reduce tu economía. No representa mayor problema ya que tienes bastante energía de donde quemar. Otra desventaja, es la interrupción del entrenamiento. Inicialmente, no tendrás suficiente energía ni de cetonas ni carbohidratos para tu cerebro ni otras tareas del organismo. Por 6 meses lo único que estás mejorando es en la adaptación a grasa. Tiempos, distancias y  recuperación sufren. Percepción de cansancio aumenta. Pérdida de flexibilidad. El hecho que la dieta es tan estricta y comer más de 30 gramos de carbohidratos en un día te puede sacar de cetosis, te quita la habilidad de nutrirte en una carrera con lo que esté disponible. Por diferentes razones,  tu alimentación preparada no puede estar disponible en alguna oportunidad.   Ventajas. Los beneficios de salud de la dieta keto para runners son múltiples. Entre ellos: Baja de peso. Baja los niveles oxidativos del estrés. Reversa enfermedades cardiovasculares y síndrome metabólico. Ayuda con síndrome poliquístico debido a menos ingesta de insulina. Mejora la función cerebral. Retraso del envejecimiento. Ayuda con síndrome de intestino irritable. Disminución de inflamación general. Control de acidez estomacal. Favorece a personas con enfermedad de hígado graso. Menos migrañas. Estabilización de comportamientos bipolares y relacionados al autismo. Mayor facilidad para ayunar. Pacientes con Parkinsons, epilepsia y Alzheimer reportan mejoras. Esclerosis múltiple desapareció en un paciente. Aquí puedes ver el testimonio de la Doctora Terry Walhs. Disminución de acné. Ventajas para runners: Ahora, referente a las ventajas para runners, encontramos: En un estudio del 2015, investigador Jeff Volek de la universidad de Ohio, encontró que atletas que consumen 10% carbohidratos, 20 proteína y 70% grasas eran más de dos veces más eficientes quemando grasa que atletas de la elite que consumían una dieta rica en carbohidratos. Sin embargo, múltiples estudios no han encontrado ninguna ventaja a nivel de rendimiento entre carbohidratos y grasas. Muchos prefieren grasas ya que los hacen sentir más satisfechos y previenen los alto y bajos de azúcar asociados con carbohidratos simples. Corredores adaptados a grasa que compitan a un ritmo parejo, pueden hacerlo por muchas horas consumiendo menos calorías que atletas que se basan en carbohidratos. Por esta razón, resulta una ventaja para personas que sufren de problemas gastrointestinales. Esto sucede porque, durante la práctica de ejercicio por 9 a 24 horas o más, hacen que la digestión en el tracto se reduzca o elimine, creando gases, inflamación y náusea. Sin embargo, esta ventaja, es puesta en tela de juicio por Jason Koop autor de Training Essentials para ultrarunning, quien dice que el sistema gastrointestinal puede ser educado con una correcta planeación y entrenamiento. En sus palabras, cetosis, es una solución complicada a un problema relativamente simple.   ¿Cómo hacer la dieta keto para runners correctamente? Ahora, si después de evaluar las ventajas y desventajas sigues interesado, aquí ciertos recordatorios: Las únicas frutas permitidas son frutos rojos y limón. Algunas nueces son altas en carbs, como: nuez de la india, pistaches, almendras y piñón. Se debe de cuidar su consumo. Se permiten las verduras como: jitomate, espinaca, kale/col rizada, calabacita/zucchini, brócoli, pimiento, ejotes, espárragos, coliflor. Sin embargo, se debe de controlar la porción, Es recomendable una suplementación de vitaminas y minerales. También, llevar este plan de la mano con algún profesional de la salud. Alimentos que debes evitar: Pan, pasta, arroz, betabel/remolacha, papa, quinoa,camote/boniato. Legumbres como frijoles o lentejas, Frutas como  manzana plátano, mango. Controlar el consumo de zanahoria, En cuanto a runners se refiere, Luis nos habló de ciertos pasos que puedes empezar a tomar hoy para lograr una mejor transición cuando decidas irte 100% a la dieta keto para runners: Dieta baja en carbohidratos y alta en grasas saludables. No tienes que regirte por los estrictos 30 gramos pero si empezar a cortar los carbohidratos progresivamente. Entrenar en ayunas. Recuperarte con una dieta baja en carbohidratos. Entrenar doble jornada. Una para gastar todo el glucógeno. La segunda para enseñar al cuerpo a buscar energía de cetonas.   Ejemplo de dieta keto para runners. @Sistemma.Nutricion nos da un ejemplo de un menú en el día de una persona que decida escoger la dieta keto para runners: Desayuno: Huevos fritos en mantequilla con espinacas, aguacate/palta Colación / merienda / snack: Aceitunas o quesos maduros + fresas o frambuesas o nuez pecana o macadamia Almuerzo: Salmón sobre cama de calabacita rallada en forma de espagueti y aderezo de parmesano. Ensalada de espinacas con jitomate cherry con aderezo de aceite de oliva y vinagre. Colación / snack: Crema de coco con cacao en polvo y estevia. I talk to a lot of great people on this podcast but this may be the man who really knows this stuff at the deepest level and most qualified to talk about fat adaptation and ketogenic diets & therapies. He has been doing amazing work in this field for such a long time.   Dr. Jeff Volek is a professor, researcher, registered dietitian, and co-founder and Chief Science Officer of Virta Health. For the last two decades, Dr. Volek has performed cutting edge research on how humans adapt to diets restricted in carbohydrates with a dual focus on clinical and performance applications of nutritional ketosis. His scholarly work includes more than 300 peer-reviewed scientific manuscripts and five books, including a New York Times Best Seller, and he has provided more than 200 lectures at scientific and industry conferences around the world. He’s an all around great guy and someone I admire.   He very rarely does interviews so I was honored to spend some time with him. He is busy running his lab at OSU while also being a co-founder of a major health company I mention a lot called Virta. Before the agricultural revolution humans had very little access to carbohydrate Low carb or ketogenic diets are what our bodies were made to run on There’s probably only a small percentage of the population that is tolerant to these refined and abundant carbs in the modern food environment Carb tolerance also goes down with age What’s great is that basically everyone can adapt to a low carb diet and this makes sense from an evolutionary perspective We all have the ability to run on fat but eating a ton of carbs suppresses that   It’s helping to treat diabetes (reverse it or put it in remission), cancer, neurological diseases The preponderance of trials shows low carb diets are superior to low fat diets in a free living setting There was interest and studies in ketogenic diets for weight loss in the 50s and 60s but then we hit the “dark ages” of this research with the USDA guidelines and it was basically toxic to study low carb in the 70s, 80s, and 90s There’s been a huge amount of research done in the last 20 years that supports low carb diets There’s so much entrenched beliefs and momentum behind low fat paradigm it just takes a while to get past it - we’re doing really well actually The dietary guidelines don’t reflect the current body of science They dietary guidelines actually matter - affect what kids eat at school, military eat, what’s taught to dietitians I think we should have two dietary strategies offered as the official guidelines instead of ending up high fat AND high carb like most americans artoday Dr. Volek believes low carb should be the preferred option for those with any sort of metabolic syndrome Personalized nutrition is the future but we’re just scratching the surface Mapping the genome and genetic testing aren't really panning out He’s looking at some biomarkers to help tell if people are mismanaging carbs What about eating more protein? It’s pretty straightforward to implement a ketogenic diet but it’s also pretty challenging in modern society His company Virta http://virtahealth.com is working on tools for this I’m doing something similar with SAPIEN http://sapien.org They use technology to efficiently scale the program to thousands and potentially millions of patients Dr. Sarah Hallberg ran the clinical trials and showed amazing efficacy of the program and keto diet in reversing type 2 diabetes Their blood sugar and hemoglobin A1c values were normalized which is pretty unheard of in the standard treatment of T2D They recommend eating fat to satiety but limit carbs and protein to achieve ketosis They recommend 1.2 to 2 grams of protein per pound of reference bodyweight Ketones have an anti-catabolic effect so they decrease protein breakdown - so no need to overeat protein Some people report less than ideal body composition on a ketogenic diet - may be because not getting enough minerals and electrolytes You can get a stress response as a result from this - sympathetic nervous system response and aldosterone causes potassium to be wasted as it tries to absorb more sodium If you have low sodium that has a negative effect on your protein status He has another book called The Art and Science of Low Carb Performance focused on athletics He had a great presentation at the conference I missed In the 60s and 70s Scandinavian researchers found out about the importance of muscle glycogen which led to the concept of carb loading A coach out of Florida invented Gatorade a bit later and then the Gatorade Sports Science Institute formed which really influenced our view sports and exercise science Since the 1960s it's been unanimous that all athletes should eat as much carbohydrate as possible before, during, and after exercise. It’s pretty insane. And there’s multi-billion dollar industries behind it to keep it this way He thinks athletes should be consuming as few carbs as possible while maintaining peak performance Consuming all these carbs are like absorbing punches to your body - you can get away with it for a while but the damage builds up It also prevents them from being able to burn their own fat He rewrote the textbooks and what was possible with VO2 max and fat oxidation in his FASTER study Ultra distance runners are winning races fat-adapted Burning basically 100% fat for 3 hours during tests They’re recovering better, not relying on tons of sugar and refined carbs packs, better GI function, and have greater enjoyment exercising He also studies military applications which overlap with these athletic advantages He says more importantly even are the cognitive benefits of the ketogenic diet They are also studying the effects of ketones on inflammatory pathways, an association with less oxidative stress, perhaps better immune functioning So far being in ketosis has been found to help people recover faster, cope with stress better, improved mental clarity and cognitive function Also helpful for traumatic brain injuries There’s so much more that needs to be studied and there’s so much promise for new applications of ketosis We need to support federal and private funding of research on ketogenic diets and therapies   Preorder the film here: http://indiegogo.com/projects/food-lies-post Film site: http://FoodLies.org YouTube: https://www.youtube.com/c/FoodLies Sapien Movement: http://SapienMovement.com   Follow along: http://twitter.com/FoodLiesOrg http://instagram.com/food.lies http://facebook.com/FoodLiesOrg   Theme music by https://kylewardmusic.com/

I used to drink 16 cans of Coca-Cola a day and eat big boxes of Little Debbie snack cakes a day. When McDonald's had that crazy Monopoly game, I would play that thing every year from the first year that they started doing it. And I would go five times a week during the workweek, even more sometimes, just to get those pieces. I could never get Boardwalk and it was just you know… that was my normal way of eating. What’s the best way to eat and the right way to exercise to ensure a healthy lifespan? Our guest today is Dr. Stephen Anton, a psychologist who has spent his career researching how lifestyle factors can influence not only obesity, but also cardiovascular disease and other metabolic conditions. Steve is an associate professor and the chief of the Clinical Research Division in the Department of Aging and Geriatric Research at the University of Florida. In today’s episode, we talk to Steve about his work in developing lifestyle interventions designed to modify people’s eating and exercise behaviors in an effort to improve their healthspan and lifespan. One of Steve’s best-known papers appeared in the Obesity Journal titled “Flipping the Metabolic Switch.” The study looked at intermittent fasting and suggested that the metabolic switch into ketosis represents an evolutionary conserved trigger point that has the potential to improve body composition in overweight individuals. Topics we cover in today’s interview include: The increasing prevalence of metabolic syndrome associated with aging. Why so many hospital health and wellness programs fail. How fasting and intermittent energy restriction promote autophagy. The relationship between muscle quality, body fat and health. How age-related loss of muscle function and mass leads to sarcopenia. For decades we’ve heard that diabetes prevention is simple—lose weight, eat less, and exercise more. But something is wrong with the conventional wisdom. Nearly 115 million people live with either diabetes or prediabetes in the United States, and that number is growing. It is time to reverse this trend. Today’s episode features an important interview with Dr. Jeff Volek, a researcher who has spent the past 20 years studying how humans adapt to carbohydrate-restricted diets.  His most recent work, which is one of the key topics of today’s interview, has focused on the science of ketones and ketogenic diets and their use as a therapeutic tool to manage insulin resistance. In 2014, Volek became a founder and the chief science officer of Virta Health, an online specialty medical clinic dedicated to reversing diabetes, a chronic disease that has become a worldwide epidemic. The company’s ambitious goal is to reverse type 2 diabetes in 100 million people by 2025. Earlier this year, The JMIR Diabetes Journal published a study coordinated by Volek and Virta that showed people with type 2 diabetes can be taught to sustain adequate carbohydrate restriction to achieve nutritional ketosis, thereby improving glycemic control, decreasing medication use, and allowing clinically relevant weight loss. These improvements happened after just 10 weeks on the program that Virta designed for people. In addition to his role at Virta, Volek is a registered dietitian and full professor in the department of human sciences at Ohio State University. He is a co-author of “The New Atkins for a New You,” which came out 2010 and spent 16 weeks on The New York Times best-seller list. The book is an updated, easier-to-use version of Dr. Robert Atkins’ original 1972 book, “Dr. Atkins’ Diet Revolution.” Volek has co-authored four other books, including “The Art and Science of Low Carbohydrate Living” and “The Art and Science of Low Carbohydrate Performance.” Both books are co-authored with and delve somewhat deeper than “The New Atkins” did into the science and application of low-carb diets. Volek received his bachelor’s degree in dietetics from Michigan State University in 1991. He went on to earn a master’s in exercise physiology and a PhD in kinesiology and nutrition from Pennsylvania State University. He has given more than 200 lectures about his research at scientific and industry conferences in a dozen countries. In addition to his five books, he also has published more than 300 peer-reviewed scientific papers. Although numerous studies have confirmed the validity and safety of low-carb and ketogenic diets, Volek and others who support carbohydrate restriction are often criticized for being so one-sided that their work comes across as more advocacy than science. But in “The Art and Science of Low Carbohydrate Living,” Volek writes: “What is the proper response when three decades of debate about carbohydrate restriction have been largely one-sided and driven more by cultural bias than science? Check it out!    Your Health and Wellness This Week: Strength Training & Self-Care On this week's episode of Open Sky Fitness, award-winning journalist,  certified strength and conditioning specialist, and author, Lou Schuler, spoke to Rob about his career and life experiences about getting fit and staying fit for life. Lou has had a career in the fitness journey for more than 20 years and has witnessed major changes and trends in the world of health. But before we get into the interview with Lou, Rob and Devon share some self-care topics. Learn what it means when you judge other people, how you can turn your focus back to you, and when is a good time to help other people in need. Lou Schuler's Fitness Advice On this episode, you'll learn what it was like for Lou to start lifting weights as a teenager with minimal sources available compared to the immense amount of information that you can find today on the Internet and in magazines. Today, As Lou reaches 60 years of age, he's realized that his focus on strength training and fitness has shifted compared to when he was younger. Lou currently works out about 3 days a week and focuses on core stability training and mobility training. By focusing on more isometric exercises, Lou has been able to focus on improving his balance while still staying strong, stabilizing the muscles, and avoiding joint pain. Growing Stronger Over Time Whether you're just beginning to strength train or you're returning to it after a break, it's important to start with the basics and progress over time. Lou's advice is that you shouldn't feel that you need to speed up your workout to lift heavy weights or take on a Cross Fit class. Don't do anything intense until you've mastered the basic movements. Back to the Basics of Strength Training Even if you've been lifting for years, going back to basics with a program that includes completing 2 sets of 12 repetitions can be fatiguing because your body has changed over time and it's not used to doing this exercise.     Staying Strong for Life When we're young, and especially if we're involved in sports, we can feel unstoppable. It's a great sensation to feel like you're unbreakable and don't have any limits when it comes to pushing your body's limits. On the other hand, not matter what our age is, we all need to take care of our body's health for life Athletes try to lift more than they can really handle and that's where injury comes into play. As we age, just like Lou, different types of strength training and balancing exercises come into play. At some point in our lives, we begin to prioritize exercises that help us to stabilize our body and condition the muscle to help reduce any type of future injury. If an injury should occur, we want to make sure that our body is strong enough that we can recover quickly.   Staying Healthy With Age   "There is no turning back the clock. I'll never get back the body that I used to have when I was 30, 40, or even 50. There's just no way." - Lou Schuler   A lot of people may believe that they can have this same body forever by taking a special pill, but we have to realize that our bodies will age and we cannot workout at the gym in the same matter that we did when we were younger. As we get older, it can be discouraging to think about everything that our bodies used to be able to do when we were younger that we cannot do now. However, the best thing about gaining experience with fitness and strength training over time is that we know our bodies better than we did before. Now we can suspect when an injury might come up, we know when to slow down, and we recognize when we've reached our limits. We just have to treat our bodies well over time and take care of it the best we can through fitness and nutrition.    About Lou Schuler   Lou Schuler is an award-winning journalist, a certified strength and conditioning specialist, a contributing editor to Men’s Health magazine (you can find most of my articles here), and author or coauthor of many popular books about diet and strength training. At school Vicky was a nationally ranked swimmer before switching to athletics and taking up the 1500 metres. She didn’t start competing in triathlon until her second year at Loughborough University in 2005 when she was approached by British Triathlon. Vicky was selected for the London Olympics in 2012, however due to an unfortunate bike crash placed 26th, but since then she has become a figure head in the world of women’s triathlon. The 2014 Commonwealth Games were a positive turning point as she secured her first major individual medal with a Bronze and bettered that with a Gold in the Mixed Team Relay alongside the Brownlee brothers. Vicky has had a sensational 2015 season proving she is one of the world’s best. She has notched up two individual wins on the ITU circuit, one in Cape Town and one recently in Edmonton. Along with these impressive wins she has consistently achieved podium finishes throughout the season with a crucial 3rd at the Grand Final in Chicago, which has secured her place at the Rio Olympics. AIR DATE: September 12, 2013 at 7PM ETFEATURED EXPERT:  & FEATURED TOPIC: "Cyclical Ketogenic Diets For Health And Performance" At the recent 2013  in Atlanta, Georgia last month, Jimmy Moore was the moderator of a fantastic panel of experts addressing the topic "Ketogenic Diets & Exercise Performance" featuring a star-studded line-up of Paleo fitness studs that included Robb Wolf, Mark Sisson, Ben Greenfield and Jamie Scott. The general consensus by most of these top names regarding the future use of the ketogenic approach for exercise and general health is the idea of regularly cycling in and out of ketosis to experience the maximum benefits. This is something that our expert guests this week know just a thing or two about. They are an exercise scientist, nutrition expert, and the author of The Carb-Nite Solution as well as  named  and a board-certified family physician from Gilbert, Arizona named . These two knowledgeable men were here in Episode 45 of "Ask The Low-Carb Experts" addressing the issue "Cyclical Ketogenic Diets For Health And Performance." TENDERGRASS FARMS GRASS-FED/PASTURED MEATS:NOTICE OF DISCLOSURE: THE WHITE CHOCOLATE RASPBERRY QUEST BAR:NOTICE OF DISCLOSURE:  Here are some of the questions we address in this episode: PEGGY ASKS: I see no reason why I should "cycle" in and out of ketosis. That seems a bit ridiculous to me, actually, when my performance is so fabulous remaining in ketosis virtually all the time. It's been truly miraculous for me. What added benefits will I get from going out of ketosis on occasion rather than simply staying in a constant state of ketosis for my exercise performance? ALAN ASKS: Dr. Peter Attia on his “Eating Academy” web site (http://eatingacademy.com/nutrition/ketosis-advantaged-or-misunderstood-state-part-i) says that research shows "someone in nutritional ketosis – even if eating zero carbohydrates – still has about 50-70% of a normal glycogen level, as demonstrated by muscle biopsies in such subjects." I have been on a ketogenic diet myself since May 2013. And as a recreational athlete, I like to do long endurance runs of 13+ miles at a time. Sometimes my runs are great, but other times the performance is poor and I bonk near the end. It feels like I have depleted my muscle glycogen stores at this point. What strategies would you recommend that I do to boost the muscle glycogen prior to going on a long run while on a ketogenic diet? ADAM ASKS: Is there such a thing as bulking up on a ketogenic diet? If you can’t bulk up in the traditional sense, then can you at least build muscle or see body recomp changes using a ketogenic approach? JERIS ASKS: I do a lot of Bikram Yoga and I've been on a low-carb, high fat ketogenic diet for several months. I noticed at first I was getting that ammonia smell and bonking near the end of the 90 minutes. I tried loading up on different carbs and experimented with this on several occasions. A bunch a fruit right before, sweet potatoes the night before--nothing worked. Finally, I heard Ben Greenfield talking about MCT oil and I was already putting that in my coffee in the morning. So I tried putting it in my water for the yoga and amazingly I could do the whole class with no carbs and without bonking or getting the ammonia smell. Maybe I just wasn't getting enough calories of any kind or I was eating too much protein. My question is this: Is it okay to use MCT oil daily in order to stay in ketosis? I guess cycling in and out would mean back-loading with a sweet potato or some other "safe starch" once or twice a week. I'm not doing that right now, I'm just eating a bunch of veggies and a little bit of berries for carbs, so I THINK I'm staying in ketosis. If you stay in ketosis, then won't you become fat adapted more quickly? DENNIS ASKS: If someone is engaging in a 24-hour intermittent fast a couple of times a week, then should they break the fast with starches? LINDA ASKS: I am a 50-year old female runner who has been running for nearly three decades. I’ve always eaten carbs and low-fat protein 5-6 meals a day up until a year and a half ago when I started to lower the carbs and up the fat. I am at a healthy weight, but found that adding the fat made me feel so much better. I’ve been eating mostly Paleo with a little bit of dairy. I’ve really enjoyed listening to the ketogenic success stories of athletes like Ben Greenfield, Vinnie Tortorich, and of course Jeff Volek and Dr. Steve Phinney. My question is how is this working for the ladies? Even all the male podcasters have female co-hosts that seem to whisper in the background that they need more carbs, especially when they race or train at high intensity. I will admit, when I posted to Ben about how many carbs to eat when training for a half marathon, he said at least 100g on high volume days and scale back on rest days. I know eating for leanness and eating for performance are two different goals, but I would like to hear more about low-carb performance for women, especially at you get older. DAMON ASKS: If someone ate a cyclical ketogenic diet but never got blood ketone levels to a significant level, then it seems to me that they would be in no man’s land where they have no ketones for energy but are also carbohydrate depleted. How can someone with this issue of low ketones and depleted carbohydrate stores figure out how to increase ketones to improve their health and performance? LAWRENCE ASKS: Keifer says that staying on a ketogenic diet for too long can lower your testosterone levels. But low-carb researchers like Dr. Steve Phinney and Dr. Jeff Volek never mention this as a long-term problem for people in a constant state of nutritional ketosis. On what basis does Keifer make this claim? JASON ASKS: Is a cyclical carb strategy REALLY necessary for everyone or is it more dependent on the specific kind of activity you are engaging in? I stay in a constant state of ketosis and run 20 hour/week as well as hit the gym twice weekly. I never feel low on energy and actually feel great pretty much all the time. In fact I do NOT like the feeling I get when I am kicked out of ketosis. In light of this, is there some unknown performance reason why I should be cycling in carbohydrates into my diet? And if I do decide to add in some carbohydrates, how do you determine the correct quantity so you don’t prevent keto-adaptation from taking place? MINDY ASKS: I’m a 47-year old female who weighs ~175 pounds and I want to lose body fat. I’ve been on a Paleo/low-carb plan for about 3 years and I swing kettle bells a few times a week for exercise. I am currently attempting to do my own version of Kiefer’s CarbNite plan with a very low-carb, higher fat program for six days followed by a smaller carby snack, carby dinner, and ice cream on day 7. I’ve had blowout CarbNites before and then end up spending the next 6 days just losing what I gained from that event which is why I'm scaling back on them. Also, I initially did your high protein protocol and started gaining weight. I have found that much lower protein and higher fat just works and feels better for me. What, if anything, would you advise I change about the way I’m implementing a cyclical ketogenic diet? DAMON ASKS: What is the maximum amount of time that you suggest being low-carb before beginning to cycle in and out of ketosis? Other than diabetes, are there any other conditions that may prevent someone from getting into ketogenesis if they are eating low-carb, high-fat? MARK ASKS: What is the relationship between ketogenic cycling frequency, the amount of workload during exercise, and lowering body fat percentage if your goal is continual fat loss? GREG ASKS: Does Kiefer or Dr. Patel promote using tools like blood glucometers or blood ketone meters along with the carb back loading protocol? I am particularly interested in the context of someone like myself who is around 15% body fat while doing heavy lifts like back squats, deadlifts, bench presses, and military presses a few times per week. Is there a particularly good way someone could use these meters to optimize and individualize things like carbohydrate intake post workout? PAUL ASKS: On The CarbNite Solution, is it possible to kick start things by lowering your protein intake down to the bare minimum needs perhaps around 30-40g a day? If so, is this something you’d do for 6 of the seven days or just a few of them? DEBBY ASKS: I'm currently using the CarbNite Solution protocol and an avid baker using coconut oil and coconut milk predominantly in my baking. I live a primal/Paleo lifestyle and limit my carbs to rice, sweet potatoes, and other real food starchy carb sources. I have heard that combining MCT oil with carbs, like sweet potatoes, raises insulin levels. If this is true, then would using coconut oil with other clean carbs such as the ones I listed above do the same? And if so, is there a limit to how much coconut oil to consume with our carbohydrate on a CarbNite plan? JULIE ASKS: I am a 34-year old female currently following the CarbNite Solution program. I had gestational diabetes with both of my kids 2 and 3 years ago. At my heaviest weight I was 260 pounds and my fasting blood sugar level was about 104. I gradually reduced carbs and finally discover CarbNite Solution that helped me get down to 185 pounds with an 84 fasting blood sugar in the morning and 5.4 A1c level. However, my blood sugar sometimes goes up to 97-104 on other mornings. Is this something to be concerned with? Should I consume less carbs on my CarbNite until my readings return to normal the next morning? Or am I just overthinking this? JOS ASKS: I'm a 34-year old female and just started weight training about almost a year ago. I am 5’5” and weigh 110-112 pounds with about 19% body fat. My goal is to increase muscles while minimizing fat gain. My current workout program mainly focuses on basic lifts such as a glute bridge, squats, deadlift, push and pull. I've been on CarbNite Solution for almost a year and have lost a good amount fat and gained a bit more muscle. I used to be that skinny fat Asian girl. Recently I've just started to increase my training intensity by going a bit heavier in weights. I heard that once you increase the workout intensity that we need to increase our carb re-feeds during the week, as opposed only having it once a week. What do you consider “working out with intensity” and why can't we just increase the amount of carbs in our once a week carb re-feed CarbNite instead of eating more carbs during the week? Or do you think I should start switching to your other protocol, Carb Back Loading?
