Martin A. Lee is an award winning journalist and author who as well as cannabis has written about fascism, media fairness, drug policy and the social history of LSD. Most people in the cannabis industry know Martin as the founder of Project CBD, a respected educational cannabis nonprofit that promotes and publicises research into the medical uses of CBD and other components of the cannabis plant.In this episode, Martin shares how back in 2008, as one of the few journalists attending the early International Cannabinoid Research Society symposiums, he was fascinated by the preclinical studies showing the therapeutic potential of CBD in a host of health conditions.However, on returning to California, he realised that only high THC cannabis was available to patients with no mention made of CBD.And so began his hunt for CBD-rich cannabis cultivars. His quest was eventually rewarded with the discovery of about six varietals, which Martin and a small group of other CBD enthusiasts began giving away to patients, dispensaries and growers in California. “The FDA and CBD have a tripolar relationship. Hemp CBD is not scheduled. CBD from cannabis is Schedule 1--dangerous. Epidolex is Schedule 5--safe.” In 10 years CBD (cannabidiol) has gone from being a relatively unknown substance to a ubiquitous health and wellness balm sold in gas stations, corner grocers' and thousands of websites. Because it is still unregulated in the US, manufacturers make all sorts of breathless claims about what it can do. It stops epileptic seizures! Eases your dog’s arthritis! Erases your frown lines! And while some of these claims may be true it’s no surprise that a CBD backlash has started. Perhaps you’ve read reports about the “toxicity” of CBD or its potential to “damage our livers in the same ways as alcohol and other drugs” or that it is dangerous to take with Tylenol. Or maybe you've read FDA iwarnings that it can cause suicidal thoughts or increase tendency toward depression, aggression or panic attacks. To set the record straight, I called Adrian Devitt Lee, Project CBD’s chief science writer and a PhD candidate in applied math at the University College in London. Adrian is the co-author of several articles in peer-reviewed journals (check out Is CBD Toxic to the Liver and CBD and Drug Interactions) and has been involved in cannabinoid research since 2011 at California Pacific Medical Center, Medicinal Genomics, and CannaCraft. Dr Max Alzamora, a physician of over ten years with a background in functional medicine and occupational health, has been prescribing medical cannabis since 2017.Dr Alzamora now runs CannaHope, a busy private practice specialising in treating patients with cannabinoid, is Medical Director at Cann Farm, Advisory Doctor at the NGO Gotas de Esperanza, and Board Member of the Society of Cannabis Clinicians Colombian chapter. I came across Dr Alzamora in a presentation he gave at a Society of Cannabis Clinicians' webinar in which he discussed the case of Glendy, a 16-year-old girl who had been having 10 seizures a day, despite all the pharmaceutical anti-seizure meds she was taking. After trying various CBD oils, Glendy found most relief from her seizures when Dr Alzamora prescribed her a cannabis oil containing CBDA (she now only has 10 seizures a year).At the time, I was researching an article for Project CBD about CBDA, so I took the opportunity to interview Dr Alzamora about Glendy's case history and his use of acidic cannabinoids, as well as his experience in general of treating patients with medical cannabis in Peru.It was a real pleasure to hear just what strides Peru and Latin America are making in integrating medical cannabis into their health systems, where doctors like Dr Alzamora have comparative freedom to think outside of the box when treating patients with cannabinoids. If you work in the cannabis business, you've probably heard of the endocannabinoid system, you might even talk about it in your marketing materials to explain why customers should buy your products. We were once carefree below-the-line film and TV workers: I did props and set dressing and Dave was an Assistant Director and occasional actor. Then we were thrust into family caregiving when my mom was suddenly diagnosed with Alzheimer's Disease. We learned so much from my mother's decade-long journey that I felt we had to share our best practices with other families in crisis. Cannabis Helps Dementia podcast was born out of my desire to shout from the rooftops how we and others have eased the suffering of loved ones living with dementia by using a medicinal herb where there is no other relief available. Patent # 6630507B1 “...cannabinoids are found to have particular application as neuroprotectants, for example in limiting neurological damage following ischemic insults, such as stroke and trauma, or in the treatment of neurodegenerative diseases, such as Alzheimer's disease, Parkinson's disease and HIV dementia…” When we started to get serious about this research, we learned about Dr. Ethan Russo, who is a world renowned neurologist, psychopharmacology researcher, former Senior Medical Advisor to GW Pharmaceuticals (GW created Epidiolex and Sativex, cannabis based medications for epilepsy and MS symptoms) and he is a member of the Society of Cannabis Clinicians. Dr. Russo is a badass rockstar of medical cannabis research and among many other things, he wrote a review article in Frontiers in Integrative Neuroscience, “Cannabis Therapeutics and the Future of Neurology” that really got my attention. Dr. Russo gives the reader an introduction to cannabis medicine, then shows us the studies that have already been done that show how cannabis helps multiple neurological issues including epilepsy, brain tumors, traumatic brain injury, Parkinson's Disease and of course, Alzheimer's. As you can imagine, with a virus as new and novel as COVID-19 and a federal ban on cannabis, the research is far from conclusive. Professor Manuel Guzman is Professor of Molecular Biology at the Complutense University in Madrid where he has been a leading light in discovering the anticancer properties of cannabinoids such as THC and CBD. He led the team behind the first ever clinical trial for THC as an antitumoral agent in the rare type of brain cancer, glioblastoma. I recorded this interview for an article for Project CBD examining just how close we are to the approval of a cannabis-based anticancer drug, but managed to draw Manuel into a whole host of fascinating topics, on which he talked openly and frankly. A few months ago, I interviewed Dr Claude Cyr, a Canadian family doctor and author of a number of peer reviewed papers about the use of cannabis in palliative care for an article I was writing for Project CBD.Dr Cyr describes palliative care as the best 'port of entry' for integrating cannabis into medicine because it is one of the only disciplines where physicians actually have the time needed to spend with patients.He says: "Palliative care is a specific situation where we can actually put into question the core philosophy of medicine which is the evidence based paradigm. I think physicians need to stop obsessing over the evidence when their patients are dying and clearly telling them, ‘I’m really enjoying this, I’m getting huge benefits from this, I’m sleeping better, I’m eating better.’ But the physicians are nodding their heads and saying, ‘I hear you, but I can’t accept this because I’m still lacking evidence.’“But I think there is enough data out there to convince physicians that it’s safe for palliative care patients, and it’s predictable.” I particularly enjoyed discussing with Dr Cyr how the psychoactive nature of the cannabis plant can actually have a therapeutic place in end-of-life care, potentially helping a patient to accept their death. “When you look at the studies of psychedelics in depression and existential anxiety in cancer patients, some of these results have been dramatic,” says Dr Cyr. “Although cannabis isn’t a true psychedelic, there are some similar experiences that patients tell us about."At smaller doses patients experience a psycholytic effect, a lowering of the defences allowing people to explore other aspects of their psyche, and that’s when they start making connections between different aspects of their reality.”A great, informative interview to be enjoyed.Useful links:Doctors for Responsible Access. In the 1990s, scientists discovered endocannabinoids, which are natural cannabis-like molecules produced by the human body. They began to realize cannabis exerted its effects, in part, by mimicking our endocannabinoids. While trying to understand the effects of the cannabis plant, they discovered a complex cell-signaling system and named it the endocannabinoid system (more on that shortly). The word "endocannabinoid" stems from endogenous (a substance or process originating from within an organism, tissue, or cell) and cannabinoid (named for the plant, cannabis). The terms "endocannabinoid" and "endogenous cannabinoid" refer to the same thing and are used interchangeably. It is believed that the endocannabinoid system, present in all mammals (and many other animals), evolved in primitive animals over 600 million years ago. Humans and animals naturally synthesize endocannabinoids, which are chemical compounds that activate the same receptors as delta-9-tetrahydrocannabinol (THC), the best-known active component of marijuana (Cannabis sativa). Endocannabinoids help keep internal bodily functions running smoothly. Several endocannabinoids have been identified so far, but the two key players are anandamide (AEA) and 2-arachidonoylglyerol (2-AG). These endocannabinoids are made from fat-like molecules within cell membranes, and are synthesized on-demand. This means that your body makes and uses them exactly when they’re needed, rather than packaged and stored for later use like many other biological molecules. AEA and THC are similar, but AEA doesn't get you high like THC does (CBD does not cause a "high"). AEA does have a calming effect, though. In fact, it gets its name from ananda, the Sanskrit word for bliss. What are cannabinoids? Cannabinoids are chemical compounds that trigger cannabinoid (and other) receptors in the body. More than 100 cannabinoids have been identified in Cannabis sativa plants (hemp and marijuana belong to this plant family). The two most commonly studied (and used) cannabinoids are cannabidiol (CBD) and tetrahydrocannabinol (THC). It is an established scientific fact that cannabinoids and other components of cannabis can modulate many physiological systems in the human brain and body. There is a massive (and rapidly growing!) body of evidence that supports the existence of the system and the therapeutic benefits of CBD. In the brain, cannabinoids and endocannabinoids work as neurotransmitters (chemical messengers that deliver information from one cell to the next.) Neurotransmitters all interact with a lot of different receptors and thus have a lot of different effects. Experts aren’t completely sure how CBD interacts with the ECS. But they do know that it doesn’t bind to CB1 or CB2 receptors the way THC does. Instead, many believe it works by preventing endocannabinoids from being broken down. This allows them to have more of an effect on your body. Others believe that CBD binds to a receptor that hasn’t been discovered yet. We will discuss receptors in more detail shortly. What is the Endocannabinoid System? The endocannabinoid system (ECS) is a biological system which plays many important roles in the human body. It is responsible for the physical and psychological effects of cannabis. Endocannabinoids and their receptors are found throughout the body. In each tissue, the cannabinoid system performs different tasks, but the goal is always the same: homeostasis, the maintenance of a stable internal environment despite fluctuations in the external environment. In the introduction to Getting High on the Endocannabinoid System, Dr. Bradley Alger eloquently described the ECS: The endogenous cannabinoid system—named for the plant that led to its discovery—is one of the most important physiologic systems involved in establishing and maintaining human health. Endocannabinoids and their receptors are found throughout the body: in the brain, organs, connective tissues, glands, and immune cells. With its complex actions in our immune system, nervous system, and virtually all of the body’s organs, the endocannabinoids are literally a bridge between body and mind. By understanding this system, we begin to see a mechanism that could connect brain activity and states of physical health and disease. The ECS exists and is active in your body even if you don’t use cannabis. Its functioning depends on many factors, including genetics, age, stress levels, diet, and overall level of health. Not only is the ECS a natural bodily system - it's a very important one. You may have heard a lot of claims about the medicinal properties of marijuana or about THC and CBD. Some of those claims may seem to be too good to be true, but research supports many of them. The reason for the far-reaching effects have to do with the wonders of the endocannabinoid system itself. As Dr. Dustin Sulak explains in The Endocannabinoid System, Cannabinoids promote homeostasis at every level of biological life, from the sub-cellular, to the organism, and perhaps to the community and beyond. Here's one example: autophagy, a process in which a cell sequesters part of its contents to be self-digested and recycled, is mediated by the cannabinoid system. While this process keeps normal cells alive, allowing them to maintain a balance between the synthesis, degradation, and subsequent recycling of cellular products, it has a deadly effect on malignant tumor cells, causing them to consume themselves in a programmed cellular suicide. The death of cancer cells, of course, promotes homeostasis and survival at the level of the entire organism. Endocannabinoids and cannabinoids are also found at the intersection of the body's various systems, allowing communication and coordination between different cell types. At the site of an injury, for example, cannabinoids can be found decreasing the release of activators and sensitizers from the injured tissue, stabilizing the nerve cell to prevent excessive firing, and calming nearby immune cells to prevent release of pro-inflammatory substances. Three different mechanisms of action on three different cell types for a single purpose: minimize the pain and damage caused by the injury. The presence and critical functions of the ECS across many systems of the body, including the nervous and immune systems, explains why such a wide variety of ailments and disease states are responsive to cannabis-based interventions - including the use of CBD. The three main components of the ECS are: Endocannabinoids, small molecules that activate cannabinoid receptors Cannabinoid receptors, which are located in the nervous system and throughout the body - endocannabinoids and cannabinoids bond with them Enzymes that help break down endocannabinoids and cannabinoids after they are used These functions all contribute to homeostasis, which refers to the stability of your internal environment. For example, if an outside force, such as pain from an injury or a fever, throws off your body’s homeostasis, your ECS kicks in to help your body return to its ideal operation. What are cannabinoid receptors? Cannabinoid receptors sit on the surface of cells and “listen” to conditions outside the cell. They transmit information about changing conditions to the inside of the cell, jump-starting the appropriate cellular response. We have (at least) two types of cannabinoid receptors: CB1 - in the central nervous system (brain and nerves of the spinal cord) CB2 - in the peripheral nervous system (nerves in your extremities), the digestive system, and specialized cells in the immune system Cannabinoid receptors are believed to be among the most plentiful in our central nervous system, and some researchers hypothesize that we could have a third, undiscovered one, as well. CB1 and CB2 are expressed in mammals, birds, reptiles, and fish. Endocannabinoids can bind to either receptor. The effects that result depend on where the receptor is located and which endocannabinoid it binds to. For example, endocannabinoids might target CB1 receptors in a spinal nerve to relieve pain. Others might bind to a CB2 receptor in your immune cells to signal that your body’s experiencing inflammation, a common sign of autoimmune disorders. Initially, research suggested endocannabinoid receptors were only present in the brain and nerves, but scientists later found that the receptors are present throughout the body, including our skin, immune cells, bone, fat tissue, liver, pancreas, skeletal muscle, heart, blood vessels, kidney, and gastrointestinal tract. Through those receptors, the ECS helps regulate a lot of important functions, such as: appetite and digestion metabolism immune function chronic pain inflammation mood sleep reproduction/fertility motor control temperature regulation memory and learning cardiovascular functioning muscle formation bone health liver function pleasure/reward stress skin and nerve function [activecampaign form=1] What role do enzymes play? When endocannabinoids have finished their job and brought things into balance within the body, certain enzymes come along to break them down and prevent them from going too far and upsetting the balance in the opposite direction. The two main enzymes that are responsible for breaking down endocannabinoids once they’ve carried out their functions are: fatty acid amide hydrolase (FAAH), which breaks down AEA monoacylglycerol acid lipase (MAGL), which breaks down 2-AG These enzymes ensure that endocannabinoids get used when they’re needed, but not for longer than necessary. This distinguishes endocannabinoids from many other molecular signals in the body, such as hormones or classical neurotransmitters, which can persist for many seconds or minutes, or get packaged and stored for later use. CBD can affect overall levels of endocannabinoids in the brain, referred to as “endocannabinoid tone.” CBD inhibits the FAAH enzyme, which as explained above, breaks down anandamide. This means CBD can increase anandamide levels by preventing FAAH from breaking it down. Inhibiting the FAAH enzyme has been shown to be a useful strategy for treating anxiety disorders - some of CBD’s anti-anxiety properties may come from its ability to inhibit this enzyme, in other words. What is Clinical Endocannabinoid Deficiency? When the ECS becomes dysregulated, this is called clinical endocannabinoid deficiency. Experts believe this dysfunction is likely implicated in nearly all disease conditions. According to U.S. National Institutes of Health scientists Pal Pacher and George Kunos, “Modulating endocannabinoid system activity may have therapeutic potential in almost all diseases affecting humans, including obesity/metabolic syndrome, diabetes and diabetic complications, neurodegenerative, inflammatory, cardiovascular, liver, gastrointestinal, skin diseases, pain, psychiatric disorders, cachexia, cancer, chemotherapy-induced nausea and vomiting among many others.” Project CBD elaborates: When the endocannabinoid system doesn’t function properly, our health suffers. By modulating the endocannabinoid system and improving endocannabinoid tone, CBD and THC can slow, or in some cases stop, the progression of various diseases. Dr. Ethan Russo, a board-certified neurologist and psychopharmacology researcher, believes that many chronic diseases may be caused by clinical endocannabinoid deficiency. In an interview with Project CBD, Dr. Russo explained what a deficiency of endocannabinoid function might look like: If you don’t have enough endocannabinoids you have pain where there shouldn’t be pain. You would be sick, meaning nauseated. You would have a lowered seizure threshold. And just a whole litany of other problems. It occurred to me that a number of very common diseases seem to fit a pattern that would be consistent with an endocannabinoid deficiency, specially these are migraine, irritable bowel syndrome, and fibromyalgia. They have some things in common. They’re all hyper-algesic syndromes, meaning that there’s seems to be pain out of proportion to what should be going on, in other words you can look at the tissues they look okay, but there’s biochemically something that’s driving the pain. In the last two years interest in CBD outpaced THC and has yet to peak. The world is awash in CBD, but because the market is unregulated, it’s difficult to know what you’re buying. I couldn’t find any evidence verifying that, but Google did reveal this surprising trend:CBD vs THCIn the last two years interest in CBD outpaced THC and has yet to peak. The world is awash in CBD, but because the market is unregulated, it’s difficult to know what you’re buying. Confusion abounds. And for good reasons. Recent reports show that labels on the bottle sometimes bear no relationship to the oil or tincture inside. Some CBD oils have high levels of THC (the standard amount is just .3%, which is too low to cause any sort of high); other brands contain no CBD whatsoever. Too many claim to have a certain number of mgs of CBD but when tested, are shown to be wildly inaccurate. Other issues to beware: Is your CBD free of contaminants? Much of the CBD from China or Eastern Europe was grown on soil that may be contaminated by industrial waste. If a product hasn’t been tested by an independent, 3rd party lab, you’ll never know for sure. And if it has been tested, where can you see the results? Transparent companies post results on their websites. The terms on the label are also perplexing: Here’s a quick guide to an isolate versus a full spectrum or broad spectrum oil. CBD Isolate is pure CBD extracted from hemp with no other cannabinoids. It is dried and crystalized, and many contend it is less effective than Full Spectrum CBD, which is extracted from hemp with all of the accompanying cannabinoids, including that tiny amount of THC. Broad Spectrum is CBD is Full Spectrum, but with all of the THC removed. Finally, there’s the price -- a sore spot for anyone on a low or fixed income. It’s not uncommon to find a 1000mg bottle of CBD costing $60. At 25mg twice a day that comes to over $1000 a year, which is sky high for a product produced from a plant that grows wild almost everywhere in the world. The best way I can contribute to stopping shoddy business practices is through education. That’s why this episode features Martin Lee, Director of Project CBD, the California-based nonprofit that has been disseminating research about this cannabinoid long before it was a Google trend. Martin tackles the challenges of navigating the unregulated market to find good, reliable medicine. Our second interview is with Dr. Junella Chin (featured several times in previous episodes on our podcast), who has treated over 10,000 patients with CBD in the last 15 years. Martin A Lee is an activist, journalist, and author who has published a number of works, including Smoke Signals: A Social History of Marijuana – Medical, Recreational, and Scientific. He is Co-Founder & Director of Project CBD, a non-profit organization he started with Fred Gardner in 2010. Lee co-authored his first book, Acid Dreams: The CIA, LSD and the Sixties Rebellion, in 1985. It explored the use of LSD during the 1960's, including mind control experiments conducted by the CIA. Lee went on to co-found Fairness & Accuracy in Reporting (FAIR) in 1986 in order to eliminate corporate and establishment media bias. He served as the first Editor of the organizations publication Extra! and later became its Publisher. Lee co-authored his second book, Unreliable Sources: A Guide to Detecting Bias in News Media, in 1990 as an expansion on his work with FAIR. Despite his work in non-fiction literature, Lee may be most recognized for his as an investigative journalist. He was awarded the Pope Foundation Award for Investigative Journalism in 1994.
