Investigations play a critical role in the diagnosis, monitoring and screening for disease in general practice. And the use of such tests is increasing both in Australia and internationally. While some of this increase is appropriate, a growing body of evidence suggests that non-rational testing is a significant problem. This is costly and problematic for GPs but also poses significant harm to patients.Registrars, especially those new to general practice, are at particular risk of non-rational test ordering. Supervisors can play a key role is supporting this critical aspect of practice. A number of strategies have been described for the most effective and appropriate use of tests in the general practice setting.

One of the enduring challenges institutions face is how to assess the learning that an individual acquires when they are not enrolled at an institution, and whether and what to award as credit toward a degree in recognition for that learning. In this episode we revisit the concept of Prior Learning Assessment (PLA), and its outcome, Credit for Prior Learning (CPL), with two higher education professionals who saw the need for a better way to manage PLA so they built one. Key Takeaways:We need our students as much as our students need us. PLA is a way to build trust with our students and to affirm their learning and experiences outside of the academy. Registrars are uniquely positioned to bring transparency to the PLA process because we are the holders of so many pieces of the process. Understanding what's involved with PLA can also position registrars to advocate for it and educate others at the institution about it. PLA and the resulting CPL can lead to better student outcomes as they can move into classes that challenge them (rather than have to take lower-level classes of material they have already learned). Laboring Under Pressure Episode 4: Obstetric Emergency in South Africa with Dr. Meghan Hurley Contributors: Meghan Hurley MD, Travis Barlock MD, Jeffrey Olson MS3 Show Pearls Map of South Africa Referenced South Africa Geography Lesson There is a big disparity between Cape Town and its neighbor Khayelitsha. Cape Town is the legislative capital and economic hub of South Africa, known for its infrastructure, tourist attractions, and developed urban areas. Khayelitsha Township is a large informal settlement on the outskirts of Cape Town, with limited infrastructure and services compared to the city center. Many residents live in informal housing. This disparity is the lasting effect of how land was divided up and populations were moved around during Apartheid. Apartheid was a policy of segregation that lasted from 1948 to 1994. How does medical education work in South Africa? Medical education in South Africa typically follows a 6-year undergraduate program directly after high school Registrars our the equivalent of Resident in America. They are graduated doctors who work in hospitals under the supervision of senior doctors as they progress toward becoming specialists. Pearls from the case and the discussion afterward Whole blood from a draw can be used instead of urine on a POC pregnancy test. Wait a little bit longer before making a determination because blood is more viscous. Although the casettes are not approved for whole blood several studies have shown this to be efficacious. Free fluid in the abdomen and a pregnancy of unknown location is a rupture ectopic until proven otherwise. Appendicitis can present on the left side. Most commonly from an extra appendix, but can also result from situs inversus or mid-gut malrotation. This presentation can also be the result of an atypically large appendix. Fever is common in appendicitis (~40%) and becomes less common with older patients. Don't be falsely reassured by a normal hemoglobin in acute bleeding because patients bleed whole blood and the hemoglobin concentration is not affected. These patients should be resuscitated with whole blood. Give rhesus factor negative blood to female patients of childbearing age to prevent them from developing antibodies to the rhesus factor which can lead to Rh disease in future pregnancies. Rhogam can be given in cases of ruptured ectopic pregnancies to lower the risk of alloimmunization. Blood transfusions carry the risk of lung and heart injury from the extra volume. The treatment for this condition is to diurese the patient. In a wide-ranging discussion about grades and grading, Dr. Joshua Eyler discusses how grades are harming our learners and what we can do to help minimize that harm. We talk about the history of grades and how the current interval letter grade system came to prominence, the stress that grades cause students and the way that stress is exacerbating the mental health crisis. And we discuss alternative grading models under the umbrella of ungrading–along with clarifying what that term really means. Key Takeaways:Our current interval letter grade system wasn't developed because it was the best way to document student learning or engagement, it came into being because of the need for administrative efficiency. Given our role in the development of the transcript, registrars should take a lead role in helping think through What Comes Next after grades. Grades are excellent extrinsic motivators. Unfortunately extrinsic motivation is only good if the goal is compliance. If we want our students to engage in their learning, we need to shift the focus to intrinsic motivation by employing alternative assessment methods or “collaborative grading” models. We are in the midst of a significant period of grade reform. Registrars should leverage this opportunity to participate in and lead conversations at our institutions about alternative forms of assessment. Let's get started:Let's first jump onto the clean energy bandwagon. The Adani Group plans to invest between 25,000 and 27,000 crore rupees (or close to 3 billion dollars) in its first pumped-storage hydropower (PSH) facility. According to Mint's Anirudh Laskar, Adani Green will set up 5 gigawatts of PSH capacity over the next five years. This process involves moving water between two reservoirs at different elevations to generate electricity, essentially functioning like a giant battery that stores power during periods of low demand, and releasing it when needed. This technology is particularly essential for ensuring stable, round-the-clock power supply, unlike the intermittent nature of solar and wind energy. The planned facilities will be located across Maharashtra, Andhra Pradesh, Tamil Nadu, and Telangana.Diving into our second highlight of the day: In a world increasingly shaped by technology, smartphones may soon face obsolescence. The smartphone, often hailed as the Swiss Army Knife of the digital age, is now threatened by innovations like extended reality glasses, gesture-based interfaces, and brain-computer interfaces such as Elon Musk's Neuralink. Musk envisions a future where devices, such as the Neuralink, could render smartphones redundant by directly interfacing with the human brain to perform all the current functions of smartphones. And it isn't just Musk. Tech and internet analysts have long speculated about a future where wearables and embedded chips could supplant the functions of smartphones. What challenges could this future pose, and which technologies could dominate this transformative tech landscape? Mint's Leslie D'Monte explores these questions in today's Mint primer.Now, we will shift focus to regulatory developments: Indian authorities are looking to expand their scrutiny beyond LinkedIn and Samsung to include more local branches of multinational corporations. In fact, six unlisted Indian units of MNCs are now under the lens of the Registrars of Companies. Officials are meticulously reviewing disclosures and shareholding information of these companies, as reported by Mint's Gireesh Chandra Prasad. Earlier this month, the RoC in Uttar Pradesh found that two Samsung subsidiaries had failed to adequately disclose Samsung Electronics' executive chairman Lee Jae-Yong as a “significant beneficial owner." Last month, LinkedIn's Indian subsidiary was penalised 27 lakh rupees for failing to comply with SBO reporting standards, involving several top executives, including Microsoft's CEO. A source informed Gireesh that India's business landscape is expanding rapidly, with approximately 150,000 new entities registering annually. This growth underscores the need for stringent regulatory oversight to maintain order and prevent future complications.Moving on to our fourth story of the day. In a rapidly growing economy like India, the challenge of generating sufficient jobs to meet demand is formidable. Despite significant economic growth, many educated young Indians find themselves underemployed, leading to widespread frustration and discontent among the youth. This employment crisis has also had political implications, as evidenced in the recent election outcomes, which saw the ruling BJP lose its absolute majority, resulting in a coalition government. Compounding these challenges is the widening economic disparity with the wealthiest 1% of Indians controlling 40% of the nation's wealth, while the bottom 50% owning just 6.4%. Despite these challenges, experts said the government's significant investments in capital expenditure has been pivotal in driving India's rapid economic growth.As Prime Minister Modi begins his third term, there is increasing focus on recalibrating economic policies to ensure inclusive growth across all segments of society. Mint's senior editor N Madhavan examines the need for Modi 3.0 to craft a new economic blueprint. Wrapping up today's episode with some positive news for consumers: In a meeting held in New Delhi on Saturday, the GST Council, chaired by Union Finance Minister Nirmala Sitharaman, announced significant reductions in tax rates on essential items, as well as measures to simplify business operations. The tax rate on everyday items such as milk cans and solar cookers has been reduced from 18% to a more consumer-friendly 12%. Besides, students will also benefit from GST exemptions on hostel stays exceeding 90 days, provided the monthly fee is below 20,000 rupees. Furthermore, in a move aimed at easing costs for daily commuters, GST has been waived on platform tickets and other railway-related services. E-commerce sellers will also see a reduction in the tax collected at source from 1% to 0.5%, which is expected to free up working capital for thousands of small and mid-sized businesses. The Council also proposed legislative changes to provide relief to businesses, including waivers on penalties and interest for tax demands from the early years of GST implementation, provided these are settled if settled by March 2025. This week Emma speaks to Dr Sylvia Kama-Kieghe, a GP from Sheffield who is vice chair of the RCGP South Yorkshire and North Trent Faculty and a chair of the RCGP Northern Faculties International Medical Graduate steering group.They talk about the challenges that international medical graduates - or IMG - GPs face and what the NHS and general practice can do to support them.Around half of the doctors who enter GP training each year are now IMGs, which means they received their initial medical qualification outside the UK.Sylvia herself qualified as a doctor in Nigeria. In this episode she talks about her own move to the UK in 2004, and some of the difficulties IMG doctors face in the UK, including racism, long-standing problems related to visas and the challenges of practising medicine in a completely different country and culture.She also explains why the RCGP Northern Faculties IMG steering group was set up and what it is hoping to achieve, and offers some practical advice on how practices can best support IMG doctors. GP registrars and supervisors need to be aware of the high community prevalence of abuse and violence during their consultations. As members of this community, practitioners may themselves be vulnerable in the context of their own lived experience. It is important for supervisors to demonstrate trauma-informed medical education while supporting registrars in trauma-informed care, with a whole-of-practice approach to trauma closing the loop on the scaffolding of both trainee and patient.
• Guidelines, standards, and resources to address Remote Auditing•The purpose of auditing includes verifying the conformance of an organization's processes and management system to defined requirements. It depends on the type of audit and the objective, the stated criteria, which can vary. The standard/s which an audit may be conducted could be an organization's own internal procedures or work instructions, a management systems standard such as ISO 9001, AS9100, or International Automotive Task Force (IATF) 16949; ISO 22000, “Food Safety Management Systems (FSMS) customer-specified requirements; or government regulations such as FAA/Nadcap, NRC, etc...• Remote auditing has been a hot topic the last year, given the circumstances surrounding the COVID-19 pandemic over the last year. However, remote auditing has been around for over a decade. Its popularity now is being spurred by advances in technology and globalization. There has been a considerable increase in multi-site companies with operations scattered across the globe and more companies engaging in international supply chains that require auditing.• Regardless, proper planning is key for contingency and understanding the kind of risks to achieve audit objectives based upon the scope/criteria, and the most suitable and available technology, as well as the auditor and auditee's complete understanding of the (ICT) Information and Communications Technology, platform/s to be used.• Companies, Registrars, and Accreditation Organizations are now and must continue to reinvent and adapt to the “new normal” regarding “Remote Auditing” and figure out ways to achieve a balance in assuring Quality Management System conformance versus not auditing at all and maintaining the rigor and respect of a QMS and/or Accreditation program as we move forward.• ISO 19011-Annex A.1-option and A.16 for remote and virtual auditing and the ISO/IEC 17021 has recognized remote auditing since 2011.
