Pilot and Maintenance Failures Create Dramatic Turboprop Incident – Episode 195

Pilot and maintenance failures combined to create a dramatic turboprop incident that became a criminal case. John Goglia and Greg Feith explore the incident with a Piaggio aircraft that lost an elevator on the first flight of the day and continued with operations. The air taxi flight crew landed and flew to next airport without performing a preflight inspection. John and Greg talk about the indications the pilots could have detected to recognize problems even if they could not visually inspect the elevators.John cites the NTSB investigation of the 2000 crash of an Emery Worldwide Airlines DC-8 as an example of how a pilot may be able to feel the difference in the control response when a flight control surface fails. The investigation found that maintenance done in response to mandatory AD 28 days before this incident had not been done properly. The nuts holding the elevators in pace were not torqued properly. The plane flew 128 hours with loose hardware.Adding to this incident, the head of maintenance of the charter company operating the plane took steps to hide evidence from the NTSB and FAA. A criminal investigation led to a fine that put the company out of business and a conviction for the head of maintenance.Related documents are available at the Flight Safety Detectives website.Don't miss what's to come from the Flight Safety Detectives - subscribe to the Flight Safety Detectives YouTube channel, listen at your favorite podcast service and visit the Flight Safety Detectives website.Music: “Inspirational Sports” license ASLC-22B89B29-052322DDB8

Hamilton-based aeronautical engineers have developed a new multi-million dollar aircraft to help fight the impacts of climate change.  The new 750 XL SuperPac, which can take off without a sealed runway, has been designed to assist in controlling wildfires and offer humanitarian aid.  NZAero CEO Stephen Burrows says the plane has taken seven years to develop at a cost of $10 million  He told Mike Hosking that they're one of only nine turboprop manufacturers in the entire world.  Burrows said their aircraft is lighter, more agile, climbs better, and its tricycle undercarriage makes it easier to land and manuever on the ground than its competitors.   LISTEN ABOVE See omnystudio.com/listener for privacy information.

In addition, Christiaan has become well-known for his aerial photography, which has been reproduced widely in print and online. Dan: This episode is brought to you by Weather Guard Lightning Tech at weather guard. We support design engineers and make lightning protection easy. You're listening to the Struck podcast. I'm Dan Blewett Allen Hall: I'm Allen hall. Dan: And here on struck, we talk about everything. Aviation, aerospace engineering, and lightning protection. All right, welcome back. This is struck episode 18, Allen. What's going on? Allen Hall: Hey, it's been a great week, Dan, pretty busy things. Just starting to pick up. Looks like air. Aviation's getting a little, a little more busy. Flights are ticking up a little bit. So it's Dan: gonna be, yeah, you got another trip this week. Don't you? Allen Hall: I want, so this is my second trip outside of Massachusetts. Uh, we're a little curious to see if the. Any changes have occurred in the, uh, airport experience and on the aircraft, it seems like the Southwest airlines was what were travel on. It is still blocking out the middle seats. And there was a report by MIT I saw yesterday or day before that was talking about how that reduced. Um, exposure to COVID by like roughly 50%, which, which was interesting. But the last time we traveled through the airports, airports seem busy. They were really busy, but the parking lots were empty. It doesn't make a lot of sense, but that's what it was. Um, so yeah, it can be a busy week. Dan: Well, I heard, yeah, I heard Delta's gonna keep, uh, their middle seats blocked off until September, but then again, other. Uh, companies hadn't really followed suit. And of course, with American airlines, they didn't even seem to enforce, uh, one of our United States senators wearing a mask, the whole flight, apparently. So Allen Hall: it's not Dan: still spotty regulation across the industry. Allen Hall: Well, it's not regulation, right? Uh, the FAA can't oppose company Dan: policy, company policies, Allen Hall: right. And the companies can remove you from an aircraft for whatever reason they want it's denial of service. That's what it is. So, yeah. Uh, when we traveled, uh, on the, on the last flight, it was, I think there was one or to people who weren't wearing a mask, but you notice what happens is it kind of get shuttled to the back corner of the airplane. What seems to be part of the approach that at least Southwest was taking at the time. And I've heard that from other airlines that they try to take people who decided not to wear a mask for whatever that could be medical conditions, that dissolve variety of reasons why that would occur. Dan: Oh. So they know ahead of time. So it was just thinking to myself, I'm like, well, how would they know if they're not on the plane yet? Yeah. But I guess some people. Allen Hall: Some people with breathing problems, you know, it's, it's, it's a big deal. They have a mask on it. Doesn't make it easier to breathe. Dan: Sure. And this is true. This is true. So in today's show, we're going to cover, I've got a good, good smattering of topics. We're going to cover Jaunt Air Mobility's aircraft which uses ROSA technology, which is a slow rotor, uh, just really interesting, uh,

MIDAS Aviation produced another great piece of analysis for our community where they reveal several very interesting facts and figures about the aircraft types used in Europe. In our discussion with Becca we particularly looked closer on the share of turboprops and their value for the European regional markets which is many times underestimated. Team of MIDAS Aviation is joining us in Valencia next week for the AviaDev Europe 2019 and Becca will be speaking at the route development workshop we prepared for the tourism boards so she also gave us a little sneak preview about what she will be sharing at the workshop.

Se hacen llamar Larsen C & The Millenials. Cerramos programa con el nuevo trabajo del pianista Kenny Werner, que presenta "The Space", en piano solo. Toda la información y derechos: http://www.elclubdejazz.com

On April 25, 2013, during a routine flight of a U.S. Customs and Border Protection (CBP) DHC-8 Turboprop aircraft in Puerto Rico, the crew spotted an unusual object. They caught the object on their thermal imaging system as it flew in from the ocean, over Rafael Hernandez Airport, then back out into the ocean. The crew did not know what it was they caught on video. Eventually, a UFO field investigator for the Mutual UFO Network (MUFON) discovered the existence of the video from a friend, who then allowed her and other trusted individuals to examine the video. She brought the video to Florida MUFON State Director Morgan Beall, who helped her put together a team that included Robert Powell, MUFON's Director of Research. This team of individuals, all of whom have scientific backgrounds and/or work in technology, then spent over a year investigating the video to determine what the object might have been. They ruled out planes, balloons, drones, and birds, and, in the end, have not been able to discover its origins. In this interview, we talk to Beall and Powell about the group's investigation, the details of their findings, including the analysis of radar data they received via a Freedom of Information Act Request, and the feedback they have received regarding their report. To find out more about their investigation, visit: ExploreSCU.org.
