Respiratory syncytial virus (RSV) is very common and can cause disease ranging from mild upper respiratory tract infections to bronchiolitis, pneumonia or even death. In the UK, intensive care wards in the winter are extremely busy and 20,000 to 30,000 children are hospitalised with RSV each year. This is a disease that causes a huge burden in infants both in the UK and globally. Dr Simon Drysdale, a consultant in paediatric infectious disease, joins our hosts Dr Emma Lim and Dr Chisto Tsilifis to discuss the maternal RSV vaccine, which has been rolled out across the UK, plus advances in research and other recently licensed vaccines. RCPCH had a significant role in ensuring the RSV immunisation programme was introduced in the UK following publication of the JCVI's recommendation to develop one. In March 2024, we published an open letter signed by over 2,000 members urging the UK Government to implement a full RSV immunisation programme for both infants and adults across the UK. Our letter generated media coverage and UK parliamentary attention with MPs and Peers supporting our calls. As a result of our campaign, we welcomed announcements from all four UK governments confirming the rollout of new RSV programmes for infant protection from August 2024 in Scotland, and September for Northern Ireland, Wales and England. Dr Simon Drysdale is Consultant Paediatric Infectious Diseases Specialist at Oxford Children's Hospital and a Senior Lecturer in Vaccinology at the University of Oxford.  This podcast is a collaboration between the Royal College of Paediatrics and Child Health and Medisense. And a Nova Scotia-based research facility has been on the forward facing innovation through the work of the Canadian Center for Vaccinology. Doctor Scott Halperin is the director of the lab that's been making contributions to the research since long before COVID-19. And recently, they received a $1 million grant from GSK (GlaxoSmithKline) to further the work of the staff of 100 working in the Halifax area. Dr. Halperin tells us why the work is important and how it will make a difference. Sarah Gilbert is the Said Professor of Vaccinology at the University of Oxford. She works on vaccines for many different emerging pathogens, including influenza, Nipah, MERS, Lassa, Crimean-Congo haemorrhagic fever, and in 2020, she initiated the SARS-CoV-2 vaccine project. Working with Oxford colleagues she is able to take novel vaccines from design to clinical development, with a particular interest in the rapid transfer of vaccines into manufacturing and first in human trials. She is the Oxford Project Leader for ChAdOx1 nCoV-19, a vaccine against the novel coronavirus, SARS-CoV-2 which is now in use in many countries around the world.  Paul Allan Offit is an American pediatrician specializing in infectious diseases, vaccines, immunology, and virology. He is the co-inventor of a rotavirus vaccine. Offit is the Maurice R. Hilleman Professor of Vaccinology, professor of pediatrics at the Perelman School of Medicine at the University of Pennsylvania, former chief of the Division of Infectious Diseases (1992–2014), and the director of the Vaccine Education Center at The Children's Hospital of Philadelphia. Dr. Stanley Plotkin, MD ( https://en.wikipedia.org/wiki/Stanley_Plotkin ), is an American physician, scientist, and scholar, who in the 1960s, while working at Wistar Institute in Philadelphia, played a pivotal role in discovery of a vaccine against rubella virus (which is now used worldwide as a key component of the MMR vaccine), and has worked extensively on the development and application of a wide range of other vaccines including those for polio, rabies, varicella, rotavirus and cytomegalovirus ( https://www.epiv.eu/ ). Dr. Plotkin graduated from New York University in 1952 and obtained a medical degree at Downstate Medical Center in Brooklyn. He was a resident in pediatrics at the Children's Hospital of Philadelphia and at the Hospital for Sick Children in London. In 1957, Dr. Plotkin served in the Epidemic Intelligence Service of the Centers for Disease Control (CDC) of the U.S. Public Health Service for three years and then served as a member of Wistar's active research faculty from 1960 to 1991. Today, in addition to his emeritus appointment at Wistar, Dr. Plotkin is emeritus professor of Pediatrics at the University of Pennsylvania ( https://www.med.upenn.edu/apps/faculty/index.php/g275/p1554 ), and works as a consultant advising vaccine manufacturers, biotechnology firms, non-profits and governments. Nearly two years after Covid-19 was first detected in Wuhan, the world is still very much in a pandemic. The toughest phase may behind us, but we are still faced with the possibility of new variants of concern, waning immunity and vaccine evasion. As first-generation vaccines start to show their weaknesses, next-generation vaccines are becoming more and more important. While hundreds of vaccines are in the pipeline, there are a few frontrunners and other interesting candidates that might be meaningful additions to our Covid vaccine armamentarium. Experts discuss the current status of a number of protein-based vaccines including candidates from Novavax, Vaxine, Clover, GSK/Sanofi and others. DNA vaccines are also still an attraction with the first authorization for Zydus Cadila in India. Other mRNA vaccines might offer meaningful differentiation, while vaccines targeting the S and N protein might also have greater importance when it comes to tackling future variants. While nasal and oral vaccines seem appealing for the prevention of infection and transmission, they also have some inevitable hurdles to overcome and are still a long way off materializing. Experts discuss future manufacturing hurdles that will require greater global collaboration to achieve future vaccine production and distribution goals. As the world tries to take a hold over the delta variant, we're all watching and waiting to see if the virus is going to evolve into something new. There are fears that natural selection could potentially yield a new variant that will completely escape all of our current vaccines. While these scenarios are hypothetical, experts discuss the need to be vigilant and prepared without unnecessary panic. Experts unpack the virus' evolution so far into delta and how that has impacted our goals for herd immunity. They talk about the possibilty for delta to change into a variant more transmissible or virulent, and how potentially leaky vaccines might impact or enhance the virus' evolutionary path. In August, Joe Rogan highlighted a paper on his podcast "The Joe Rogan Experience," which raised public concern that our current vaccines are potentially encouraging new potent variants to emerge. And it seems everyone, has been trying to get this interview but I do my bet to Lead with Love and be nonconfrontational and respectful which has led to me be able to actually speak cordially with Paul on many occasions. Offit, MD is the Director of the Vaccine Education Center at the Children's Hospital of Philadelphia as well as the Maurice R. Hilleman Professor of Vaccinology and a Professor of Pediatrics at the Perelman School of Medicine at the University of Pennsylvania. He is a recipient of many awards including the J. Edmund Bradley Prize for Excellence in Pediatrics from the University of Maryland Medical School, the Young Investigator Award in Vaccine Development from the Infectious Disease Society of America, and a Research Career Development Award from the National Institutes of Health. Dr. Offit has published more than 160 papers in medical and scientific journals in the areas of rotavirus-specific immune responses and vaccine safety. He is also the co-inventor of the rotavirus vaccine, RotaTeq, recommended for universal use in infants by the CDC; for this achievement Dr. Offit received the Luigi Mastroianni and William Osler Awards from the University of Pennsylvania School of Medicine, the Charles Mérieux Award from the National Foundation for Infectious Diseases; and was honored by Bill and Melinda Gates during the launch of their Foundation's Living Proof Project for global health. I recently saw an Open letter to Vermont legislators several years ago who were considering vaccine legislation from an immunologist named Dr Tetyana Obukhanych. The letter was resurrected and published on the website called Health Impact News. Basically her goal in the letter is to say that its unwarranted the parents should be forced to get their children vaccinated before going to school.
