More than 10% of the U.S. population has received at least one dose of the COVID-19 vaccine, but injection rates are lower in black and brown communities, and many people express doubts about the shots made by Pfizer-BioNTech and Moderna. Vaccine hesitancy presents a barrier in the fight against coronavirus. A recent poll by Pew Research Center found that "Black Americans continue to stand out as less inclined to get vaccinated than other racial and ethnic groups: 42% would do so, compared with 63% of Hispanic and 61% of White adults."Our guest, Dr. Wrenetha Julion, professor and department chair at Rush University College of Nursing, discusses causes of vaccine hesitancy and how to encourage more people to take a high-effective vaccine against the virus. Vaccine hesitancy "is a significant problem," says Wrenetha. "It stems from a long history of mistrust and mistreatment of African-Americans and other groups in healthcare systems and research. Unfortunately, those things stick in people's minds." We examine the impact of the Tuskegee experiment and look at Wrenetha's suggestions for building vaccine trust in marginalized communities. Vaccine hesitancy and anti-vax sentiment have been around for as long as vaccines themselves have been available. Misinformation about vaccines has, for example, led to a decline in early childhood vaccination, resulting in the worldwide resurgence of the measles virus. In 2020, a vaccine appears to be the only viable path to ending the COVID-19 pandemic and returning to normal. But many distrust this new vaccine (or vaccines) and could refuse to get vaccinated.In this episode of Big Tech, Taylor Owen speaks with Heidi J. Larson, the author of Stuck: How Vaccine Rumors Start — and Why They Don’t Go Away and the director of the Vaccine Confidence Project. Larson’s work in vaccine hesitancy traces the root causes to a lack of trust in and anxieties about our institutions. That distrust is shared and amplified by the social media platforms we have available today. The COVID-19 vaccine is an opportunity to restore trust in governments, big pharma, scientists and the media, but only if it is handled correctly. Larson explains: “To me, it’s the ultimate litmus test at multiple trust levels. Because, as I say in the book, I don’t think we have a misinformation problem, as much as we have a relationship problem, and that the misinformation, in a sense, is kind of symptomatic of not trusting, in any of those domains.”

As the toll from the COVID-19 pandemic increased, polling suggests counterintuitively that resistance to a future vaccine has also risen. Anthropologist Heidi J. Larson identified several likely drivers of this, including political polarization, a focus on being ‘natural,’ the undercurrent of mistrusting the so-called elite. But in this Social Science Bites podcast, she tells interviewer Dave Edmonds that there’s another driver. Scientists themselves. Families who have some expertise in running their own affairs can come to resent “the elitism of science, the language of science, the ‘we know better’” which dismisses their experiences and more importantly, their questions. “A lot of parents feel very strongly feel that they have their own evidence of vaccine problems,” she notes, and the medical establishment has often not invested a lot into bringing the public along – even as the number of vaccines and the expectation of being vaccinated grows. “[The public is] saying, wait a minute – we want to have say in this, we want to be able to ask some questions … [W]hen they feel like the door in closed on the questions, that shuts down the conversation. I think in order to become unstuck, we need to have more dialog and be open.” Stuck is the name of Larson’s new book. Besides the obvious pun in the title, Larson explains Stuck: How Vaccine Rumors Start -- and Why They Don't Go Away also refers to being “stuck in the conversation, why the public health community has been losing some of the public enthusiasm for vaccines.” These are questions and concerns Larson routinely addresses in her role as director of the Vaccine Confidence Project, a World Health Organization (WHO) Centre of Excellence that addresses vaccine hesitancy, and as a professor of anthropology, risk and decision science at the London School of Hygiene & Tropical Medicine’s Department of Infectious Disease Epidemiology. The first organized opposition to compulsory vaccination arose in the United Kingdom in the late 1800s, she explains, as a reaction to mandatory smallpox vaccinations. “To this day that is one of the persistent themes that has fueled some of this resistance.” Nonetheless, as vaccinations remain one of the most remarkable health interventions available, the resistance that might be expected to erode in the face of a global health emergency hasn’t faded. “Strangely, in the context of the pandemic, the already amplified skepticism has taken another level of resistance, which is surprising to many of us. You’d think with such a serious disease and a pandemic it would be a time where people would say, ‘Wow, this is really an example of why we really need a vaccine.’” And the resistance, whether to a coronavirus vaccine or to vaccine in general, can be seen globally. In fact, Larson is seeing resistance groups linking up across borders – and an a most inopportune time. “I see the whole increase in the anti- and skepticism as being kind of a tipping point … We’ve always had all these other issues that have been challenging in getting enough people vaccinated, both from the supply of the vaccine and the access, logistics and all the rest. But this additional factor – we’ve stagnated in our global vaccination coverage and just can’t seem to get above a certain amount.” And those coverage levels in some cases fall below the thresholds needed for “herd immunity,” which in turn means we can expect more cases, whether or COVID, measles or even polio. Social media has helped skeptics get their messages disseminated, and Larson notes that the Wakefield autism scare arose the same year as the start of Google. “The sentiments are not new,” she says, “but the scale and intensity of them is.” In addition to her position in London, Larson is also a clinical professor in the Department of Global Health at the University of Washington and a guest professor at the University of Antwerp. She previously headed global immunization communication at UNICEF, chaired the Advocacy Task Force for the Gates Foundation-sponsored Gavi, the Vaccine Alliance, and served on the WHO Strategic Advisory Group of Experts Working Group on vaccine hesitancy.

Why some common sleep recommendations don't work (sleeping pills and prescriptions) - Dr. Jason Jones Elizabeth City NC, Chiropractor Over the years many patients come to our Chiropractic office at Elizabeth City NC and complain of insomnia even after taking sleeping pills and prescriptions. You might have experienced this and maybe forced yourself to sleep several times with pills but don’t seem to get a good result. Well, you’re not alone. According to a survey, about one-third of US adults with sleep problems complain that they had used pills and prescriptions but still don’t get a restful night. Besides, nearly 6 out of 10 people taking sleeping pills report side effects such as feeling confused, drowsy, or forgetful the next day. So, you might ask, “Why do some sleeping pills and prescriptions not work?” Well, firstly, you must understand that not all drugs sold over the counter or prescribed are safe and effective. Secondly, most sleeping pills and prescriptions are classified as “sedative hypnotics.”- meaning they only sedate you, but not helping you sleep adequately. All OTCs, for example, rely on older antihistamines- doxylamine or diphenhydramine, which causes drowsiness as a side effect. The oldest types are anti-anxiety drugs such as triazolam and temazepam, which makes you feel sedated by slowing the activity of your brain and CNS. The newest type of sleeping pills or prescriptions, including suvorexant and ramelteon, affects brain chemicals that are involved in the sleep-wake cycle. Why They Don’t Work Most of these drugs will only make you fall asleep faster or increase total sleep time by 20 to 30 minutes. But they actually don’t help you get adequate sleep. Studies have shown that drugs like ramelteon only makes people fall asleep 9 minutes faster than those taking a placebo. Also, participants who took suvorexant got only closed their eyes for 10 minutes more than those who took a placebo. Moreover, sleeping pills don’t seem to reduce your anxiety or clear your depression. They also don’t seem to even encourage the kind of inner and outer change needed for quality sleep. In a way, they only distract you from what you really need. The “Morning After” Effect Besides the short-term effect of sleeping pills and prescriptions, they also have side effects, causing people to feel dizzy, drowsy, confused, and unsteady the morning after. Some people with sleep problems have recorded hallucinations, sleepwalking, sleep driving, sleep eating, memory lapse, and odd behavior in the morning after taking sleeping pills and prescriptions. The bad part is that when long-term usage of these drugs can be habit-forming, and you are likely to suffer a rebound effect when you decide to stop them, and this worsens insomnia. More so, the risk of sleeping drugs gets heightened when you misuse them. For example, taking these drugs with other sedatives, alcohol, or prescription pain drugs can lead to slow breathing as well as a physical and mental impairment that may persist up to the next day. Besides, you take sleeping pills or prescriptions with 4 hours or less to sleep before starting an activity that requires your concentration, you can still have it circulating in your bloodstream, and this can make you less alert when you need to be active. So if what you’re looking for quality sleep and a regular circadian rhythm, you can follow some natural measures and possibly other techniques including some cognitive behavioral therapy for insomnia. These measures are safer and long-lasting.

This Podcast Is Episode Number 0210 And It Will Be About Contractor Bookkeeping Basics Explained Contractor Bookkeeping Basics and Why Today’s technology says “Do Everything On Your Phone” Use an app to create an estimate, or better yet just send the estimate in the body of the email. That works when the customer’s reply is “Do It”. Great you start the job, finish the job, never send the customer an invoice; Why – They Don’t Need One – they know how much to pay from your original email. and you Included short explanatory paragraphs about each item of work you did. You move on to the next job. Weeks go by. If the customer is expecting a “formal invoice” and do not plan to pay until they receive one. More time passes; you are waiting to “GET PAID” and the customer is waiting “To Pay You”. Some customers will take the extra effort to call, leave a message, text, or email you asking for the invoice so they can pay. MANY customers will say nothing until The Contractor sends the invoice they are NOT Paying and NOT doing any follow up. Months go by, and still, no check arrives. Many contractors think they have someone who hasn’t paid but are unsure of who the customer was. Often times the contractor feels too much time has passed and doesn’t try to go back and bill old customers. What came in the mail was all of the supplier invoices for the job. All of the subcontractor invoices, and YES, the payroll for the job has been paid. The person who did NOT get paid was The Contractor. Get The Money, Collect Ongoing Payments, Get Job Deposits – Use Customers Funds for Material. DO NOT BE THE BANKER FOR YOUR CUSTOMER! Contractors are not bankers! First Step No matter what app you use. Make sure it has a “mini” invoice program for emailing customer an invoice. You can then go back and have a paper trail that you asked the customer for money. Second Step Track your time Use T-Sheets to keep track of the jobs you were on. (This is good for Contractors who do not have employees) What If you have to lien the job? One of the questions are when was the last day on the job? The answer could be the difference between having any lien rights and collecting the money or getting nothing but attorney or lien service fees. Use a Lien Service for Notice to the Owners on larger jobs. Double check with your supplier. Does the supplier send out Notice to the Owner? Even if your Lien Rights are gone – Maybe your Supplier’s Lien is Valid and Collectable. Third Step Accept Credit Cards. More customers have room on their credit card than “Cash In The Bank.” Getting Money Now offsets the fees charged by your Merchant Service Provider. Many services that offer invoice programs (Estimating Software, Dispatch Software, Invoice Software) have an add-on feature for Merchant Services. Customer follows the link, puts in their credit card number or bank routing and account number for easy payment. Again, worth the fees to GET PAID NOW! Behind the scenes, every Contractor should do additional Bookkeeping. Some invoice programs will track the payments received, but that is the extent of the bookkeeping. The Contractor’s Bookkeeping needs go beyond sending an email to the customer with the price of the job and taking the check to the bank. Customer Invoice Tracking knows Who Owes You Money AND How Much A proper Construction Contractor Bookkeeping System is setup to create a Customer Invoice. Receives Payment. Seeing the balance if anything is owed. Accounts Receivable Aging Report is one of the reports banks look at before giving you a business loan. Bankers want to know What is your volume (sales) What have you collected (customer payments) and How Much Do Customers Still Owe You and finally How Long Does It Take You To Collect. In some industries 30-60 days is common. I have heard stories about larger retail stores who paid promptly after the first 6 months. It was surviving the first 6 months that was hard for smaller suppliers and Tenant Improvement Contractors. Accounts Payable is keeping track of the bills owed to a supplier or subcontractor. A proper Construction Contractor Bookkeeping System uses the Accounts Payable feature to track the unpaid bills. Again, the Banker wants to know how long it takes to pay your bills. The Bank wants to get paid and unlike The Contractor who is embarrassed to call an old customer; The Bank is very willing to Call Early and Often for any outstanding payments due. Another reason to keep A proper Construction Contractor Bookkeeping System is so you don't Over Pay a supplier or subcontractor, cell phone, the internet, liability insurance, vehicle insurance and more. The bills keep coming and depending on your payment schedule you may receive the Past Due Reminder Notice before the Original Bill has arrived. Some utility companies have a history of what appears to be randomly sending Original Bill Some insurance companies if you do not immediately pay the invoice when it comes then the payment will be late and subject to a late charge. Based on where the Out of State payment needs to be mailed to it may be impossible to mail (by regular mail) and be received on time. Hence – Online Payments Accepted. The benefit to The Contractor is proof the payment was made with a time and date stamp (Print) and usually an auto-generated email as backup confirmation that the payment was made. Overpaying Sub Contractors can easily happen if checks are written on the job site. When using Hand Written checks be sure to use “Carbonless Checks” to have a written record of payments made. The payments than can be added to the Bookkeeping System without waiting for them to clear. Bankers like the reports from a Formal Contractor Bookkeeping System (QuickBooks for Example) It is an easy to understand report. Bankers can usually tell if the numbers are off because there is missing transactions, no enough income, too many expenses. Contractor’s who do their own bookkeeping and are sometimes “Just Lost” Depending on the banker they use. Whether or not you get the loan or line of credit, you are after depends heavily on the way your Financial Reports are put together. The Net Result is that the Rates and Fees charged maybe higher and Amount Loaned maybe lower than expected! Another Group That Likes Formal Bookkeeping System is the Local, State, and Federal Governments (IRS) City Governments can require their revenue reports and payment due Monthly, Quarterly, Annually. In Washington State; The Department of Revenue can make every contractor report and pay their Sales Tax Liability Monthly. For Companies with employees, Washington State Payroll taxes are due Quarterly. The Internal Revenue Service want the 941 Quarterly Payroll Reports filed Quarterly. If tax liability for the Quarter is over $2500.00, then the payments are due Monthly. Larger companies have to pay their payroll tax liability at the time of each payroll. Reminder Estimated Tax Payments are due Quarterly.
