Cardiogenic shock accounts for up to 5% of acute heart failure presentations and around 14–16% of patients reported in cardiac intensive care datasets. It complicates up to 15% of all myocardial infarctions and is the leading cause of death post-infarction. Using pharmacological agents alone may increase left ventricular afterload and myocardial oxygen demand, resulting in complications. Thus, mechanical circulatory support (MCS) devices have emerged as important therapeutic options. As evidence remains uncertain, MCS selection depends on clinician preference and local availability.An updated systematic review and meta-analysis of high-quality RCTs and propensity score-matched studies (PSMs) was performed to compare the outcomes of MCS devices with no MCS and each other and investigate which MCS is the most effective in reducing mortality.To learn more about the findings of this study listen to this podcast.

Not necessarily get to the you know, I say research is always about trying to get to the truth, and we're far from it and our piece, hopefully, will get us a little closer to it, but it clearly isn't going to be it. But I do think, you know, I think we all agree that we're kind of at a point where we're stuck, you know, and stroke rehab, there doesn't seem to be a lot of movement forward clinically, you know, we've gotten really good at standardized care and even that's starting to be eroded, you know what I mean, in some jurisdictions? And I think it's time for us to really think about what do we pivot to next? Right? What do we add onto that to make it better, get people excited about stroke rehab again and sort of end that erosion of funding that is occurring in some centers and encourage more funding in other centers? Compares multiple treatments against each other with: direct evidence and can estimate indirect evidence (can make estimates for comparative effectiveness of two interventions which might not be compared in the literature) The best interventions according to the Network Meta-Analysis are.... Modified Constraint Induced Movement Therapy. Now it's time to start using the treatments that work. Adjunct treatments, standardized treatments, the future and improved care Wondering how to help therapists use the research on interventions that work to maximize recovery 85% of interventions that work are adjunct treatments we don't use very often – we need to take advantage of them, especially the cheap ones. Andrea Cipriani is NIHR Research Professor at the Department of Psychiatry, University of Oxford and Honorary Consultant Psychiatrist at the NHS Foundation Trust in Oxford. His main interest in psychiatry is evidence-based mental health and his research focuses on the evaluation of treatments in psychiatry, mainly major depression, bipolar disorder and schizophrenia. Dr Cipriani is author of 267 peer reviewed scientific publications (Scopus), mainly systematic reviews, meta-analyses and randomised controlled trials in psychopharmacology, however in the past few years he has also been investigating relevant issues in epidemiological psychiatry and public health, like patterns of drug consumption, risk of serious adverse events (most of all suicide and deliberate self harm) and implementation of treatment guidelines. Being interested in the methodology of evidence synthesis, Dr Cipriani has now a specific focus on network meta-analysis and individual patient data meta-analysis, trying to assess the validity, breadth, structure and interpretation of these statistical approaches to better inform the mental healthcare decision-making process. He is currently Editor in Chief of Evidence-Based Mental Health (ebmh.bmj.com) and also on the Editorial Board of the Lancet Psychiatry, the Australian and New Zealand Journal of Psychiatry and the Cochrane Collaboration for Depression, Anxiety and Neurosis
