Des Lions de l'Atlas aux Éléphants de Côte d'Ivoire, en passant par les Aigles du Mali et les Guépards du Bénin, pourquoi la majorité des sélections nationales de foot en Afrique ont-elles pour emblème des animaux ? La logique a été respectée en ouverture de la Coupe d'Afrique des Nations de football : les Éléphants de Côte d'Ivoire ont battu 2 à 0 les Lycaons de Guinée-Bissau. Le lycaon, ce chien qui ressemble à une hyène, qui chasse en meute, ou en équipe, qui a d'ailleurs totalement disparu de la Guinée-Bissau (il n'en reste que quelques milliers dans toute l'Afrique), ne pouvait, sur le papier, ou la balance, faire le poids face à l'animal terrestre le plus lourd de la planète, l'éléphant.Six équipes de foot africaines sur dix ont pour emblème un animal, parce que l'Afrique abrite quelques-unes des espèces les plus charismatiques, à commencer par le lion. Trois pays se sont choisis comme emblème le roi des animaux, dans l'espoir d'être les dieux du stade : le Cameroun, et les Lions indomptables, le Maroc et les lions de l'Atlas, une sous-espèce disparue à l'état naturel, et le Sénégal, avec les Lions de la Teranga. « Par sa crinière, le lion impose la respectabilité. Il vous fait peur, souligne le journaliste Rémy N'Gono, consultant à Radio Foot Internationale. Quand les Sénégalais entrent dans un stade, ils agissent comme des lions : ils se jettent sur la proie. Mais que dire alors des Hirondelles du Burundi ? L'an dernier, le Bénin a officiellement changé de nom. Les Écureuils sont devenus les Guépards. Ce qui fait sourire Rémy N'Gono : « Dans la forêt, où les fauves sont là, l'écureuil est sur des branches en train de chercher des noix de palmistes. Mais qu'est-ce que l'écureuil peut gagner ? Du coup, [les Béninois] se sont dit : nous allons devenir des guépards. Mais vous le savez très bien : l'âne a beau changer de nom, il restera toujours un âne ! ». Malgré son nouveau surnom, le Bénin ne s'est pas qualifié pour cette Coupe d'Afrique des nations. À l'avant-dernière CAN, en 2019, les Lions de la Teranga se faisaient battre en finale par les Fennecs, les renards rusés du désert algérien. About our Guest:Dr. Yehya is a graduate of the University of California at Berkeley and the University of California at Los Angeles School of Medicine. After completing pediatrics training at Children's Hospital of Los Angeles, he completed his pediatric critical care fellowship at Children's Hospital of Philadelphia (CHOP), and joined the faculty after graduation in 2011. He is currently an Assistant Professor of Anesthesiology and Critical Care and Pediatrics at the Perelman School of Medicine at the University of Pennsylvania and an attending physician in the pediatric intensive care unit at CHOP.Dr. Yehya's research interests encompass all aspects of pediatric respiratory failure, with a particular emphasis on pediatric acute respiratory syndrome (ARDS) and mechanical ventilation. ARDS consists of sudden, severe flooding of the lungs in response to an inflammatory insult causing difficulty breathing, frequently requiring mechanical ventilation. Sepsis is a leading cause of ARDS in children. His long-term goal is better characterization of ARDS in children and to test therapies designed to improve outcomes. About our Guest:Dr. Yehya is a graduate of the University of California at Berkeley and the University of California at Los Angeles School of Medicine. After completing pediatrics training at Children's Hospital of Los Angeles, he completed his pediatric critical care fellowship at Children's Hospital of Philadelphia (CHOP), and joined the faculty after graduation in 2011. Daha önceki yazılarımda COVİD-19'un sonucu olarak özellikli yaş gruplarından “PEDİATRİK MIS-C” ve “PEDİATRİK ARDS”yi ele aldım. Bu yazımda da COVİD-19'un sebep olduğu pediatrik yaş gruplarındaki şok yönetimini ele alacağım. Şimdiden istifadeli okumalar. Pediatrik yaş gruplarında COVID-19'da kardiyak fonksiyon bozukluğu ya da ciddi sistemik inflamatuvar yanıt sendromu (SİRS) şoka neden olabilir. Şok açısından genel görünüm, bilinç değişikliği, deri rengi, periferik nabızlar, kapiller geri dolum zamanı (KDZ) (normal ≤ 2 sn), solunum sayısı, kan basıncı, idrar miktarı, serum laktat düzeyi değerlendirilmelidir.​1,2​ COVID-19 pozitif hastalarda şok bulguları varsa sekonder bakteriyel enfeksiyon olasılığı açısından uygun geniş spektrumlu antibiyotik başlanmalıdır. Antibiyotik öncesi kan kültürü alınmalı fakat antibiyotik tedavisi gecikmemelidir ​1–3​ Çocuklarda COVID-19 şok yönetimini şu şekilde sıralayabiliriz.​2​ 0-15 dk. Solunum sıkıntısı/yetmezliği, bilinç değişikliği, perfüzyon bozukluğunu tanı.Yüksek konsantrasyonda oksijen ver, monitörize et.PARDS varsa NİV/İMV düşün.İV/İO damar yolu sağlaBaşlangıç Resüsitasyon: Kristaloid solüsyon 20 ml/kg İV puşe, perfüzyon düzelene kadar devam et,40-60 ml/kg ilk 1 saatte (Akciğerde raller ve hepatomegali gelişmediği sürece), (çocuk yoğun bakım şartları yok ise 40 ml/kg)Hipoglisemi/Hipokalsemiyi düzelt.Antibiyotik başla ŞOK DÜZELMİYOR15-60 dk. Sıvıya Dirençli Şok: İV/İO inotrop başla. Tercihen;Adrenalin veya Noradrenalin düşün (0.05-0.3 ugr/kg/dk.)Adrenalin/Noradrenalin temin edilemiyor ise Dopamin başla.Olabiliyorsa ultrason kullanarak santral venöz kateter tak.Entübe değil ise entübasyon düşün (Sedasyon için İV/İO/İM atropin-ketamin) >60 dk. Katekolamin Dirençli Şok: Sıvı ve İnotrop/Vazopresöre yanıtsız hastada hidrokortizon başlanabilir.İleri monitörizasyon ve Doppler USG ile sıvı, inotrop, vazopresör ve vazodilatör tedavileri düzenle.Hedef: ScvO2 > % 70, CI: 3,5-5,5 L/dk./m Dekompanse şok bulguları devam ediyor ise: Santral venöz basınç (SVB) < 8 mmHg ise sıvı vermeyi düşün, ScvO2< % 70 ve laktat artışı devam ediyor ise dolaşım desteğine devam et, Adrenalin/Noradrenalin/Dopamin dozunu titre et.CI düşük ise Dobutamin/Milrinon/Levosimendan eklemeyi düşün.Kalp fonksiyonları normalse; Noradrenalin, Terlipressin, Adrenalin dozunu titre et. ŞOK DÜZELMİYOR *Israrlı katekolamin dirençli şok: Pnömotoraks, kalp tamponadı, İntraabdominal hipertansiyon (>12 mmHg) varlığını değerlendir, varsa düzelt*Refrakter Şok: VA ECMO Genel Yaklaşım Çocuk acil veya yoğun bakım şartları yeterli ise şoku olan çocuklarda ilk bir saatte 40-60 mL/kg İV (her dozda 10-20 mL/kg) sıvı bolusu verilmelidir. Her 10-20 ml/kg İV sıvı sonrası kalp hızı, kan basıncı, KDZ, karaciğer büyüklüğü, akciğer sesleri, idrar çıkışı değerlendirilmelidir. Çocuk yoğun bakım şartları olmayan yerlerde ve kan basıncı normalse sıvı bolusu verilmemelidir. Çocuk yoğun bakım şartları yok ve hipotansiyon varsa 40 mL/kg > 1 saatten fazla sürede İV sıvı verilmelidir. Sıvı olarak %0.09 NaCl, Ringer Laktat, Hartman, Isolayte-S solusyonu gibi kristaloidler kullanılmalıdır. Albumin rutin kullanılmamalıdır. Sentetik kolloidler ve jelatin kullanılmamalıdır. Yatak başında soğuk/sıcak şok ayırımı yapılmasına gerek yoktur.​2,3​ İleri hemodinamik monitörizasyon yöntemleri olarak kalp debisi (CO), kardiyak indeks (CI), sistemik vasküler direnç (SVR) ve ScvO2 ölçümü önerilmektedir. Hastaların laktat takibi önemlidir. Laktatın başlangıçtaki seviyesinden daha cok laktat düzeyindeki artış veya azalma hastanın iyiye ya da kötüye gidişinin göstergesidir.​1–3​ Takip önerileri Sıvıya yanıt vermeyen şok tablosunda inotrop kullanılmalıdır. Burada ilk ilaç olarak adrenalin veya noradrenalin kullanılabilir. Adrenalin ve/veya noradrenalin sağlanamıyorsa dopamin kullanılabilir. Adrenalin ve noradrenalin kullanımında klinisyenin tercihi, hastayla ilgili faktörler ve ilacın sağlanabilmesi belirleyicidir.

Merhabalar acilci.net ailesi. Uzun bir birlikteliğin kısa başlangıçları olmalı. Ben de kısa bir giriş ile sizlerle uzun bir yolculuğa çıkmayı düşünüyorum. Bu bölümde farklı klinik tablolar ile prezente olabilen kritik COVID-19 çocuk hastalarda ARDS takip ve tedavisini güncel literatür bilgileri ışığında derlemeyi planladım. Giriş COVID-19 erken döneminde pediatrik vakaların daha az olduğu algısı mevcut iken ilerleyen dönemde özellikle varyantların artmasıyla literatüre sunulan vaka serilerindeki artışla birlikte çocukların da erişkinler kadar SARS-CoV2 ile enfekte olabildikleri tespit edilmiştir.​1​ New York şehrindeki 9 çocuk yoğun bakım ünitesine yatan 70 çocuk hastayı değerlendiren bir başka çalışmada hastaların %17’sinde ciddi sepsis, %30’unde pediatrik ARDS (PARDS), %20’sinde inotrop ihtiyacı, %12,9’unde akut renal hasar gelişmiş ve 2 hastada mortalite bildirilmiştir. ​2​ Önce İngiltere ve İtalya sonrasında pek çok ülkede 2020 yılının sonlarına doğru çocuklarda COVID-19 ilişkili PARDS geliştiği bildirilmiştir. COVID-19 ilişkili septik şok, akut akciğer hasarı, invazif olmayan ve invazif mekanik ventilasyon gereksinimi, ekstrakorporeal solunum ve / veya dolaşım desteği ihtiyacı, akut renal hasar, MIS-C(Multisistem İnflamatuar Sendrom) , Kawasaki hastalığı gibi kritik çocuk hastaların çocuk yoğun bakım ünitesinde takip ve tedavisi gerekmektedir. COVID-19 ile ilişkili PARDS COVID19’a bağlı PARDS yönetiminde Pediatric Acute Lung Injury Consensus Conference (PALICC) önerileri geçerlidir. Hastalarda solunum yetmezliğinin boyutuna göre değişik düzeylerde oksijen ihtiyacı olmaktadır. New York’dan bildirilen çok merkezli bir çalışmada çocuk yoğun bakıma kabul edilen hastaların %70’inde oksijen desteğine gerek duyulmuş, bunların %20’sine NİMV, %28,6’sına İMV uygulandığı bildirilmiştir. Oksijen tercihen rezervuarlı oksijen maskesi ile uygulanmalıdır. Eğer rezervuarlı oksijen maskesi yetersiz kalırsa seçilmiş hipoksemik solunum yetmezliği olgularında yüksek akımlı nazal oksijen tedavisi (HFOV) ve MİMV desteği uygulanabilir. Ancak bu noninvaziv yöntemler uygulanırken diğer hastalarda olduğu gibi normal prosedür uygulanmalı gecikmiş entübasyondan kaçınılmalıdır. Tablo 1.
