He has worked extensively with elite athletes, particularly in racket sports. Tendon issues like jumpers knee and Achilles tendinopathy are common in athletes. Eccentric training was a breakthrough in tendon rehabilitation. Heavy slow resistance training may be more effective than eccentric training. Tendons require regular loading to maintain health and prevent degradation. Aging begins around the age of 17, affecting tendon structure and function. Immobility can lead to rapid deterioration of tendon health. Tendon stiffness is crucial for performance and injury prevention. There is potential for improving tendon health even in older individuals. The Achilles tendon may require different loading than the patellar tendon. Cross-links in tendons can affect stiffness and brittleness. Exercise is crucial for maintaining tendon health and properties. Dietary choices can influence tendon integrity and health. Metabolic syndrome is linked to tendon issues and injuries. Advanced glycation end products accumulate in low turnover tissues like tendons. Tendon size can increase due to vascularization in tendinopathy. Inflammation may be present in early stages of tendon pain. Circadian rhythms could play a role in tendon recovery processes. Future research should focus on the metabolic aspects of tendon health. Today, Dr Emily talks about hope. And in a world where things can feel a little bit hopeless, this really couldn’t have come at a better time. In contrast to all of our understanding up until this point, hope is not an emotion. Expertly defined by Dr Emily, hope is a belief. And most significantly, it can be learned, and it can be cultivated. Psychological mic-drop. This episode delves into strategies for creating both a sense of collective and individual hope, and in a discovery which happens to be VERY on-brand for this podcast, Dr Emily talks to the significance of vulnerability and connection in living a hopeful life. We hope you enjoy this one as much as we did. If you would like to watch this full episode on YouTube, follow this link - https://bit.ly/41j0pT1
According to the Atlantic's Yoni Applebaum, America is STUCK - literally and otherwise. In his new book Stuck: How the Privileged and the Propertied Broke the Engine of American Opportunity. Appelbaum argues that America faces not just a housing crisis but a mobility crisis, with prohibitively expensive housing in prosperous areas preventing people from moving toward opportunity. Applebaum traces how zoning laws, initially driven by racism and classism, have created a system where Americans move less than ever before, despite more wanting to relocate. This decreased mobility has wide-ranging consequences for civic engagement, social cohesion, and economic dynamism. His solution: simplify building regulations, reform housing policy to facilitate mobility, and dramatically increase housing supply. Here are the 5 KEEN ON take-aways from our conversation with Appelbaum:* America faces a mobility crisis, not just a housing crisis: People can't afford to move to areas with economic opportunity, which has dramatically reduced the rate of Americans relocating (from 1 in 3 annually in the 19th century to 1 in 13 today).* Restrictive zoning laws have racist and classist origins: America's first zoning laws were designed to segregate Chinese residents in Modesto, and Berkeley's first single-family zoning aimed to keep out working-class people.* Decreased mobility has widespread negative effects: Beyond economics, reduced mobility damages civic engagement, social cohesion, and even contributes to political polarization and populism.* Tenements served a positive historical purpose: Despite reformers' criticism, tenements were vehicles for economic mobility that allowed cities to absorb immigrant waves - a capacity many cities have lost.* Applebaum offers three solutions: Simplify building regulations, redesign housing policy to facilitate movement rather than keeping people in place, and dramatically increase housing supply ("build baby build").Yoni Appelbaum is a deputy executive editor at The Atlantic and the author of Stuck: How the Privileged and the Propertied Broke the Engine of American Opportunity. Appelbaum is a social and cultural historian of the United States. Before joining The Atlantic, he was a lecturer on history and literature at Harvard University. He previously taught at Babson College and at Brandeis University, where he received his Ph.D. in American history. This is a public episode. In this episode of "The Psych Files," we delve into the complex and sensitive topic of sexual assault, focusing particularly on the phenomenon of tonic immobility, often referred to as "frozen fright." This involuntary response can leave victims unable to move or speak during an assault, a reaction that is not only common but also deeply misunderstood. Our journey begins with a fictional account from the Netflix series "A Nearly Normal Family," where a young woman experiences this paralyzing effect during an assault. This portrayal opens up a discussion about the real-life implications of tonic immobility, including the challenges it presents in legal contexts where victims' lack of physical resistance is often misinterpreted as consent. Throughout the episode, we explore the biological and psychological underpinnings of tonic immobility. Experts explain that this response is an evolutionary defense mechanism, akin to the "freeze" response observed in many animals when confronted by predators. It's a survival strategy, not a choice, triggered by extreme fear and a complex cascade of hormonal reactions in the brain. We also discuss the broader implications of misunderstanding tonic immobility, particularly the undue guilt and shame that can afflict survivors, complicating their recovery and the pursuit of justice. The episode highlights the importance of education and awareness, both for mental health professionals and the general public, to foster a more compassionate and informed response to survivors of sexual assault. This episode is not just an exploration of a psychological phenomenon but also a call to action to support survivors more effectively and to challenge societal misconceptions about sexual assault and victim behavior. #SexualAssaultAwareness #MentalHealthMatters #EndTheSilence: #SupportSurvivors #TonicImmobility #MentalHealthAwareness
In the second episode of season 2, I have a conversation that resonated very strongly with my own experience of pain and recovery. Samphire Hayes reached out to me about her story and I replied immediately. We share a story of chronic pain and of having to search for why our bodies were reacting that way. Our solutions were similar in terms of adopting a natural movement lifestyle and Samphire went even further to uncover her own story, by studying and adopting new practises she had never heard of before. Like myself, she has now chosen to use her learning and insight to help others. This conversation explores the experience of living with chronic pain and the journey towards building strength and finding hope. We talked about the link between chronic pain and trauma, as well as the impact of chronic pain on mental health. We cover the importance of self-advocacy, the power of small movements, breathwork and mindfulness and more, and the key moment when Samphire chose to ‘ditch the sofa' that had been her prison. Sedation and immobility impact more than patient outcomes in the ICU. Family members also suffer the isolation, burden, trauma, and loss from sedation and immobility in and after the ICU. Families are also at risk of developing post-ICU syndrome themselves. Nonetheless, they are left unprepared to care for their loved ones who are suffering the damage of sedation, delirium, and ICU-acquired weakness while struggling through their own personal trauma. Abstract: Brain states fluctuate between exploratory and consummatory phases of behavior. These state changes affect both internal computation and the organism's responses to sensory inputs. Understanding neuronal mechanisms supporting exploratory and consummatory states and their switching requires experimental control of behavioral shifts and collecting sufficient amounts of brain data. To achieve this goal, we developed the ThermoMaze, which exploits the animal's natural warmth-seeking homeostatic behavior. By decreasing the floor temperature and selectively heating unmarked areas, mice avoid the aversive state by exploring the maze and finding the warm spot. In its design, the ThermoMaze is analogous to the widely used water maze but without the inconvenience of a wet environment and, therefore, allows the collection of physiological data in many trials. We combined the ThermoMaze with electrophysiology recording, and report that spiking activity of hippocampal CA1 neurons during sharp-wave ripple events encode the position of the animal. Thus, place-specific firing is not confined to locomotion and associated theta oscillations but persist during waking immobility and sleep at the same location. The ThermoMaze will allow for detailed studies of brain correlates of immobility, preparatory-consummatory transitions and open new options for studying behavior-mediated temperature homeostasis. Abstract: Foxb1-expressing neurons are located in the dorsal premammillary nucleus (PMd) and further rostrally in the parvafox nucleus, a longitudinal cluster of neurons in the lateral hypothalamus of rodents. The descending projection of these Foxb1+ neurons end in the dorsolateral part of the periaqueductal gray (dlPAG). The functional role of the Foxb1+ neuronal subpopulation in the PMd and the parvafox nucleus remains elusive. In this study, the activity of the Foxb1+ neurons and of their terminal endings in the dlPAG was selectively altered by employing chemo- and optogenetic tools. Our results show that in whole-body barometric plethysmography, hM3Dq-mediated, global Foxb1+ neuron excitation activates respiration. Time-resolved optogenetic gain-of-function manipulation of the terminal endings of Foxb1+ neurons in the rostral third of the dlPAG leads to abrupt immobility and bradycardia. Chemogenetic activation of Foxb1+ cell bodies and ChR2-mediated excitation of their axonal endings in the dlPAG led to a phenotypical presentation congruent with a freezing-like situation during innate defensive behavior. In this interview, correspondent Polly Nash speaks to Aimee-Noel Mbiyozo, a senior researcher specialising in human migration at the institute of security studies in Pretoria. The pair discusses the concept of involuntary immobility, which occurs when people wish to migrate but are not able to do so. Mbiyozo recommends how Earth Refuge and lay people globally can shine a light on this overlooked issue and talks of the challenges of advocating for greater protection for climate refugees.
When healthcare systems say they don't have the resources to support proper practices, is that true? How much do deep sedation and immobility cost our ICU teams, hospitals, and healthcare systems? How can we use the research to support our plight for safe staffing ratios and evidence-based practices? Let's dive deep into the money talk. www.daytonicuconsulting.com --- Support this podcast: https://anchor.fm/restoringlife/support
Why does your loved one need physical therapy ASAP? Mobility is a life-saving intervention. There are very few exceptions in which mobility is not safe for ICU patients. We will discuss the lethal risks of Immobility and muscular atrophy and how mobility (even and especially on the ventilator) can help your loved survive and thrive. Let's talk about the impact of illness or hospitalization on one's health. I see it happen firsthand as a therapist working in the hospital. People lose their ability to walk and complete self-care with independence within a very short period of time, making it harder for them to go home without additional support. Consider this: How at risk are you? How about your partner, the one you care for? We are moving less and getting out into nature less as we further develop our society. Too many hours spent in front of various screens is just 1 example and cause. RESEARCH article:https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2759840 The objective of therapists like myself is to discharge the patient to a SAFE environment with adequate support. Over the course of 1-2 weeks, I see patients move from a short-term rehab plan to a home plan when they are able to cooperate in strength, endurance, and self-care skill-building interventions with OT and PT. When they can't participate (due to medical instability) or they refuse to participate, we see the loss of potential for home discharge The same thing can happen at home with an illness or during home recovery from a hospital discharge, so do everything you can to keep your LO moving with the intention. Look at what can happen to the body with immobility: https://www.frontiersin.org/articles/10.3389/fnut.2021.633987/fullAre you seeing this happen with your loved one, either at home or at the hospital, and feeling helpless? Here is what you can do. keep them moving and doing as much as they can for themselves so they are always at a starting point of advantage. Look at what your loved one can do easily. Any mobility or activity that you see them do like: sitting up at the side of the bed or without the support of the couch/chair. Standing up without losing balance, walking, any daily task they do well - have them do that MORE or for longer periods of time than they are currentlyThe HOW: Start a simple movement and exercise program - do it at least once a day https://www.caregiverbydesign.com/seatedexercises (FREE DOWNLOAD VIDEO TO USE)Use the right support items to maximize safe mobility and self-care: walker, canes, adaptive tools, shower chairs, grab bars, toilet seats/ grab bars, etc CONSISTENTLYGet out of the house, stay social, keep the mind engaged and this includes problem-solving through basic daily tasks. Let's get you and your partner living the life you actually want by sorting out what your vision for living is, what the boundaries are and how to build strength, mobility, and independence for you both. https://www.caregiverbydesign.com/coaching Find out more at the link. There are some phrases that set a therapist's teeth on edge. For Dr. McCoy, these are "That's just the way I am", "That's just the way I was raised.." and "It's my parents' fault because..." In this episode, she discusses what such sentiments really mean to the therapist and to the prognosis of the client. She also suggests ways to begin to grow out of psychological immobility to experience the joy of taking responsibility for one's own life.
