2027 will be the 250th Anniversary of the Battle of Saratoga and the surrender of General John Burgoyne's forces to those of General Horatio Gates. After a series of military defeats over the winter of 1776–1777, British military leaders developed a bold plan to gain control of the Hudson River and divide New England from the rest of the colonies. Three armies would converge on Albany: one under Lieutenant General John Burgoyne moving south from Quebec, one under General William Howe moving north from New York City, and a third under Lieutenant Colonel Barrimore St. Leger cutting east from Lake Ontario along the Mohawk River Fort Stanwix lay directly on the path of St. Leger's force, making it a key defensive position for the Continental Army. By delaying St. Leger's troops and forcing a retreat, the garrison's stand at Fort Stanwix contributed to Burgoyne's surrender at the Battles of Saratoga a month later, a major turning point in the course of the war. To look at this battle, we are joined by today’s guest William Kidder, author of Defending Fort Stanwix: A Story of the New York Frontier in the American Revolution. He offers an account of life in and around the fort in the months leading up to the siege, detailing the lives of soldiers and their families, civilians, and the Haudenosaunee peoples with a focus on both the mundane aspects of military life and the courageous actions that earned distinction. We discuss the stories of local men and women, both white and Indian, who helped with the fort's defense before, during, and after the siege and showcases an overlooked story of bravery and cooperation on New York's frontier during the American Revolution.See omnystudio.com/listener for privacy information.

In the past five years, the use of immunotherapeutic agents for advanced cancers has emerged as a promising alternative to tyrosine kinase inhibitors and chemotherapy, making it an exciting time to be practicing oncology. In this episode, Dr. Tyler Sandow interviews oncology experts about the landscape of advanced hepatocellular carcinoma (HCC) and the current state of immunotherapy treatments. He is joined by medical oncologists Dr. Jonathan Mizrah, Dr. Lingling Du, and Dr. Adam Burgoyne, as well as interventional oncologist Dr. Zachary Berman. Physicians, nurses, nurse practitioners, and physician assistants can follow this link to earn CME / CE credits for completing an accredited learning activity related to this discussion: https://www.cmeuniversity.com/course/take/125737 --- This podcast is supported by an educational grant from: AstraZeneca https://www.astrazeneca.com/our-therapy-areas/oncology.html With additional support from: Boston Scientific https://www.bostonscientific.com/en-US/medical-specialties/interventional-radiology/interventional-oncology.html --- SYNPOSIS Drs. Burgoyne and Mizrahi provide a primer on immunotherapy and explain how they communicate the principles of this treatment to their patients. Dr. Du discusses the Imbrave clinical trial and how recent studies have shown improved overall survival when immunotherapeutic agents are used, especially when multiple agents targeting various pathways are employed. When choosing between different regimens, the doctors consider factors such as the patient's underlying liver function, symptom burden, and prior treatments. Importantly, the doctors also discuss contraindications to immunotherapy, including a history of organ transplant, autoimmune disease, and poor performance status—all of which put patients at high risk for deterioration with this treatment. The treatment of patients with poor liver function remains controversial, as underlying cirrhosis may prevent the recovery of liver function. Dr. Berman outlines recent clinical trials studying the effects of transarterial chemoembolization (TACE) combined with immunotherapy. Finally, the doctors discuss the future of HCC treatment and the benefits of continued innovation in both interventional and medical oncology. --- TIMESTAMPS 00:00 - Introduction to Immunotherapy 04:32 - Notable Clinical Trials 13:39 - HCC Etiology and Immunotherapy Outcomes 18:43 - Contraindications for Immunotherapy 23:05 - Adverse Effects from Treatment 25:14 - Combination Therapy 36:22 - Considerations for Immunotherapy Dosing 40:26 - The Future of HCC Treatment --- RESOURCES Atezolizumab plus Bevacizumab in Unresectable Hepatocellular Carcinoma, IMbrave150 Trial (Finn et al, 2020): https://pubmed.ncbi.nlm.nih.gov/32402160/ Tremelimumab plus Durvalumab in Unresectable Hepatocellular Carcinoma, HIMALAYA Trial (Abou-Alfa et al, 2022): https://evidence.nejm.org/doi/full/10.1056/EVIDoa2100070 Nivolumab versus sorafenib in advanced hepatocellular carcinoma (CheckMate 459): a randomised, multicentre, open-label, phase 3 trial (Yau, 2022): https://www.thelancet.com/journals/lanonc/article/PIIS1470-2045(21)00604-5/abstract Nivolumab (NIVO) plus ipilimumab (IPI) vs lenvatinib (LEN) or sorafenib (SOR) as first-line treatment for unresectable hepatocellular carcinoma (uHCC): First results from CheckMate 9DW (Galle, 2024): https://ascopubs.org/doi/10.1200/JCO.2024.42.17_suppl.LBA4008 Randomized Phase 3 LEAP-012 Study: Transarterial Chemoembolization With or Without Lenvatinib Plus Pembrolizumab for Intermediate-Stage Hepatocellular Carcinoma Not Amenable to Curative Treatment (Llovet, 2022): https://pubmed.ncbi.nlm.nih.gov/35119481/ Find this episode on BackTable.com for more resources.

On this episode of Small Town Life, host Dave Warner interviews author William L. Kidder, who just completed the book 'Defending Fort Stanwix.' It's a story of the New York Frontier during the American Revolution. The book focuses on the successful defense of Fort Stanwix, located in what is today Rome, New York. Fort Stanwix played an important role in the 1777 Saratoga campaign, which resulted in the capture of British forces led by General John Burgoyne. This campaign is widely considered to be a major turning point in the American Revolution. Burgoyne's surrender raised Patriot morale and contributed to the formation of the alliance with France, which contributed greatly to victory in the War for Independence. In the epilogue, Kidder sketches the postwar lives of some of the fort's prominent defenders, outlines the fort's decommissioning and fall into ruin, and describes its late twentieth-century reconstruction and designation as a national monument. As Kidder says in the interview, you can pick up copies of the book at the Fort Plain Free Library, or you can pre-order it on Amazon. --- Support this podcast: https://podcasters.spotify.com/pod/show/studio25/support

WRITE STUFF IS BACK! After a hiatus this year, we are back to bring you some new great reads and authors! Tonight we kick off the conversation as we chat with author Avellina Balestri about any and everything. From writing, to faith, to weird TV shows as she talks about her books and we showcase her newest release called: All Ye That Pass: Gone for a Soldier, her American Revolution novel. If you don a scarlet coat at the price of your conscience, the color will only remind you of the wound in your own soul! If you sign away the faith of your fathers, all lesser goods will be forfeited too!” Young Edmund Southworth could not have foreseen the path his life would take upon befriending Gentleman Johnny Burgoyne, British military veteran and parliamentarian. As Catholic recusants from the north of England, Edmund's once noble family has struggled to survive for centuries in the shadow of the Anglican ascendancy. But General Burgoyne offers him the chance to put past humiliations behind him by joining the Church of England and donning a scarlet coat as an officer of King George III. Although his conscience is uneasy, Edmund embarks upon Burgoyne's March to subdue the American revolutionaries by splitting the colonies in two. He finds unexpected love in the arms of Abby Vanderkamp, a supporter of American independence, who will do anything to free her father from a British prison hulk or, failing that, strike a blow for the hard-pressed rebel cause. As the British advance through the New York wilderness devolves into increasing brutality and instability, Edmund will have his already divided loyalties tested to the breaking point under the influence of this hostile land which presents him with new challenges and opportunities alike. The British Army in North America conducted two campaigns in 1777. John Burgoyne led one army south from Canada to seize control of the Lake Champlain-Hudson River corridor resulting in the battle of Saratoga. Rather than assist Burgoyne's campaign, William Howe led his army from New York City on the Philadelphia campaign. Although Howe captured Philadelphia, the events of 1777 led to the French Alliance and ultimately American victory in the American Revolution. pcntv.com/donate pcntv.com/membership-signup pcntv.com

This podcast presents a fascinating conversation on the latest ways artificial intelligence can impact decision making. Offer: Magnesium is integral for 600+ biochemical processes in the human body. The common misconception is that consuming more magnesium will automatically improve health and well-being. The truth is that there are various forms of magnesium, each of which is essential for a variety of physiological processes. Most people are inadequate in all forms of magnesium, while even those considered "healthy" typically only ingest 1 or 2 kinds. Consuming all 7 of magnesium's primary forms is the key to accessing all its health benefits. That's why we packed 7 forms of 450mg of elemental magnesium into each serving of Wild Mag Complex. Plug in, listen, and hear: Ways this applied-sciences firm uses biologists, chemists, and physicists to deploy deep learning techniques. Some application examples, such as utilizing artificial intelligence in healthcare by developing prediction tools for a global ambulance company, and Methods they use for financial services, from actuarial to forecasting models and doubling down on quantum-driven hedge funds. Charlie Burgoyne, Valkyrie's CEO, educates listeners on the phenomenal potential of artificial intelligence in healthcare, financial services, government applications, and even the entertainment and transportation space. He tells listeners to think of his firm as a modern-day Bell Labs, basically a research institute turned industrial team. Valkyrie employs biologists, chemists, and physicists to work on different verticals, deploying algorithms and machine learning techniques to dramatically improve operational efficiencies and customer retention. He adds that while they do a degree of consulting, they're predominately scientists solving industrial problems. He shares some of these challenges that AI easily meets, such as helping the largest ambulance company in the world develop COVID modeling predication tools and resource allocation plans. For example, they can predict what counties are likely to have an outbreak of disease and why, and inform them how to best provide and allocate PPE accordingly. He adds that their most impactful projects live in the defense sector, identifying different types of behavior that help complex data systems adjust new data and develop supporting algorithms. But a nice vacation is also on their list of services, and they've recently helped a large cruise line identify which systems are creating failures, what can be optimized, and how they can develop operational capabilities to keep the ocean clean and still expand their operation. He finishes with an interesting discussion of their approach to financial services and where they go against the grain by embracing quantum-driven hedge fund models. Listen in to learn more about how Valkyrie deploys AI for a better future. For information about the company, see their website at valkyrie.ai. No British General of the Revolutionary War has been written about more than John Burgoyne. That's because of his surrender of his army at Saratoga, New York in 1777, widely seen as the turning point in the Revolutionary War. He is considered a reckless lout, and there's plenty in his life story to support this characterization. He gambled heavily and possibly had to flee England as a young man to escape his debtors. His father-in-law eventually paid Burgoyne's debts and got him another commission in the army, just in time for the 7 Years War. There he served admirably and became a war hero. But 300 years after his birth, the many lives of Burgyone -- dashing cavalry colonel of the Seven Years War, satirical London playwright, reformer Member of Parliament, gambler in the clubs on St James's Street – have been forgotten.Today's guest is Norman Poser, author of From the Battlefield to the Stage: The Many Lives of General John Burgyone. We look not only at the Saratoga campaign, but also elements of Burgoyne's eventful life that have never been adequately explored. He was a socialite, welcome in London's fashionable drawing rooms, a high-stakes gambler in its elite clubs, and a playwright whose social comedies were successfully performed on the London stage. Moreover, as a member of Parliament for thirty years, Burgoyne supported the rule of law, fought the corruption of the East India Company – he was a sworn enemy of Clive of India whom he denounced with all his might – and advocated religious tolerance.
