Carfentanil is a synthetic opioid that is typically used as a tranquilizing agent for elephants and other large mammals, according to the Drug Enforcement Administration. When administered to humans, the drug is 10,000 times more potent than morphine and 100 times more potent than fentanyl, according to the DEA. The Russian and Chinese governments have been profoundly involved in the global opioid trade, as has the United States government. Chinese and Russian actors have been known to mass-produce carfentanil, a highly potent synthetic opioid, not only as a drug and cutting agent, but as a chemical weapon. Carfentanil and other fentanyl derivatives are the most common agents in what have been termed "deaths of despair" in the poorer, neglected regions of the United States and Russia. The drug trade has always been wrapped up in political intrigue, and the role of the United States, specifically the CIA, is no exception. As Peter Dale Scott has written about, the so-called War on Drugs has mostly been an excuse for intelligence agencies and cartels, often in alliance, to enrich themselves at the expense of other people. Iran-Contra was only one small piece of the gigantic puzzle, and even then, it goes deeper than assumed. For instance, the CIA in the '50s and '60s was deeply tied with the business of psychedelics manufacturing and distribution, in part because the recreational use of such enabled them to gather a large recruitment base for experiments involving combining drugs, electronics, and various forms of torture and abuse in order to determine if the individual mind, and society at large, could be controlled. Maybe it was these experiments that fed into worldwide delusions about electronic harassment, or what is often called 電波 in Japanese... This week on Legally Speaking with Michael Mulligan:Duress is a defence, sometimes referred to as an excuse, for the commission of a criminal offence. The rationale for the defence is the idea of moral involuntariness. These are the requirements for the defence:There must be an explicit or implicit threat of present or future death or bodily harm. The threat can be directed at the accused or a third party. The accused must reasonably believe that the threat will be carried out. There is no safe avenue of escape. This element is evaluated on a modified objective standard. A close temporal connection between the threat and the harm threatened. Proportionality between the harm threatened and the harm inflicted by the accused. The harm caused by the accused must be equal to or no greater than the harm threatened. This is also evaluated on a modified objective standard. The accused is not a party to a conspiracy or association whereby the accused is subject to compulsion and actually knew that threats and coercion to commit an offence were a possible result of this criminal activity, conspiracy, or association. Once the accused person demonstrates that there is an “air of reality” with respect to each element of the defence, the Crown would need to prove that the defence does not apply. In the case discussed, the accused, and his brother, were on the same unit in jail as a man known as Big Newf. Big Newf demanded that the accused smuggle drugs into the jail. If the accused didn’t do this Big Newf, who had a reputation for violence, would harm the accused, or his brother. Big Newf arranged for a surety to help the accused get bail and then the person acting as a surety had the accused swallow and insert drug packages into his rectum. The accused was then required to turn himself into the police to get readmitted to jail. The accused did not think he had any safe avenue of escape because Big Newf, and his associates, had access to his brother who was still in jail. The trial judge, and the Ontario Court of Appeal, both concluded that the Crown had not proven that the defence of duress did not apply.Also on the show, the BC Court of Appeal upholds an 11-year jail sentence, for a man with no previous record, who plead guilty to trafficking in carfentanil. The man had been selling drugs online and shipping the drugs via Canada Post. Online advertising for the drugs included statements such as “one of the premium Fentanyl vendors in Western Canada.”, “carfentanil … [w]hen used responsibly … is proven to be very safe”, and “we have the best stealth period”.Finally, on the show, a BC Court of Appeal decision increasing the wrongful dismissal award in favour of an articling student who was fired by her principal is discussed. The court described the lawyer’s conduct as “high-handed, malicious, arbitrary or highly reprehensible misconduct that departs to a marked degree from ordinarily standards of decent behaviour.” The fired student was awarded $118,934 in general damages, $25,000 in punitive damages, $50,000 in aggravated damages, and $10 for breaching an articling agreement. Canada’s hidden crisis:  Opioid toxicity deaths were falling across the country prior to the COVID-19 pandemic. But then came lockdown in March 2020. Experts say the record-breaking 2020 death toll is linked to the COVID-19 pandemic. They say the global crisis has worsened the illicit drug market, pushed individuals to use in isolation and decreased access to life-saving services. Fentanyl is described as being 100 times more potent than morphine. Carfentanil, the most powerful analogue, is upwards of 10,000 times stronger. The purity of already impure street drugs deteriorated when COVID-19 fractured supply chains. Experts say higher concentrations of fentanyl and fentanyl analogues made their way to the streets. In this episode, Stephen and I discuss many topics, from safe consumption sites to medical heroin to Dilaudid dispensing machines on the streets. Yet, the underlying theme of this podcast episode is the value of having empathy and forming relationships with people who struggle with addiction. People who inject heroin are real people who deserve a chance to recover, no matter how long it takes for them to be ready finally. British Columbia, Canada, takes harm reduction to a new level with supervised consumption sites and many other programs to help people who use drugs on the streets to stay safe and help them along the path towards recovery. These programs serve as a positive example for what other countries, such as the United States, that do not allow these advanced forms of harm reduction, could do to save many more lives from the deadly effects of opioids and other street drugs.Stephen Finlay is the manager of addiction services in the Surrey region for Lookout Housing and Health Society. Lookout is a non-profit organization that provides homeless shelters, supportive housing, subsidized housing, outreach, harm reduction, and related services throughout southern British Columbia, including Vancouver. Surrey is one of the largest cities in the Greater Vancouver area and has experienced problems arising from homelessness and drug use for many decades. As an addiction services manager, Stephen oversees an Intensive Case Management team, a supervised drug smoking site, and SafePoint, which is the second federally sanctioned supervised injection site in Canada. SafePoint opened in June 2017. Since it opened, there have been over 190,000 visits to SafePoint. The staff has reversed almost 1,500 overdoses, with no fatalities. Before joining Lookout, Stephen served as executive director of ARA Mental Health Advocacy, a small non-profit that provided advocacy services to people living with mental illness and poverty. He also has 29 years of experience in marketing, product development, and regulatory affairs at TELUS, Canada's second-largest telecommunications carrier, and four years of experience in marketing research at Procter and Gamble. Stephen served as a civilian volunteer for Law Enforcement Against Prohibition from 2006 to 2015. On top of an already staggering alcohol problem, the Substance Abuse and Mental Health Services Administration says that 1 in 10 Americans has a drug problem, with drug overdoses alone killing about 72,000 people in the United States just last year. I’m dedicating this episode of our show towards bringing more awareness of the dangers of drug and alcohol abuse and its impact on our society as a whole so that we can work together to change addiction in America. My first guest, Kay White, is a spiritual life coach, certified yoga instructor, and the executive director at Villa Kali Ma, which is a recovery program exclusively for women. We do not have an overdose crisis. We have a poisoned drug supply. Fentanyl and its analogs are killing tens of thousands of Americans every year. Alarmingly and with ominous future implications, fentanyl-laced cocaine fatalities in Ohio are now outstripping fentanyl-laced herion fatalities. This trend began in 2016 but the Ohio Bureau of Criminal Investigation (BCI), who operates the three state crime labs that first learned about it, never made the data public. In this episode we speak with Dennis Cauchon, founder of Harm Reduction Ohio, who just this summer used public records laws to obtain the data from BCI and publish it, warning people that this poisoning crisis had now spread far beyond just the heroin market. Who is adding fentanyl and carfentanil to the cocaine supply? How many lives has Cauchon saved by spreading this information to the public? And why did BCI sit on the data for three years and not tell anyone? According to Cauchon, they simply didn't realize what they were looking at. Disconnected from the drug culture, they just assumed that mixing fentanyl with cocaine was a new drug trend. They had no idea that cocaine users were unaware that their cocaine contained fentanyl, and that they didn't want their drugs contaminated with it. Cauchon was able to educate some of the folks in BCI. "After initially being reluctant to provide the information," he say, "BCI was remarkably helpful, which showed a real concern about drug overdose deaths, not just helping police convict people on drug charges." This is the state of prohibition. This is drug war culture. In this episode we also speak to Donna May, a mother who lost her daughter to heroin after her doctor cut her off from the prescription opiates that relieved her symptoms of anxiety. We hear from Dr. Torsten Passie, who helped spearhead heroin maintenance programs in Germany. And we talk to Dr. Gabor Maté, best-selling author and addiction treatment specialist from Vancouver, Canada, who explains why drug users are one of the most scapegoated groups in our culture. This episode describes the problem as well as the solution. And the solution lies not just in changing drug laws, but also our own attitudes and false beliefs about drugs and the people who use them. Narcan and Synthetic Opioids: vive la résistance? Probably not. Read this absolutely brilliant piece from The Tox & The Hound here. (They did all the hard work and we stole their sources.) Opioid "resistance" to naloxone is most likely not a thing, per se. The reported effect from synthetic and novel opioids are unlikely to be due to the agent's binding affinity for receptors within the brain, but rather from an ability to rapidly permeate the blood brain barrier much faster than "traditional" opioids such as heroin. Even in cases where a synthetic opioid agent was identified, the vast majority of cases did not need more than 4mg of naloxone to achieve reversal. Synthetic opioids don't bind any more "tightly" to receptors than naloxone. Synthetic opioids will usually cross the blood brain barrier faster than traditionally encountered agents. Most available evidence shows that synthetic opioid toxicity does not require significantly more naloxone to achieve clinically significant effect. Ergo, the traditional serial naloxone dosing algorithm does not need much modification. 0.04mg -> 0.4mg -> 2mg -> 4mg -> 8mg -> 10mg Not all that "overdoses" is an opioid. Consider all other causes of altered mental status or coma. Namely: hypothermia, hypoxia, and hypercarbia. Acidosis may potentiate the effect of opioids, highlighting the demand for timely and effective ventilation. Polypharmacy or adulteration is increasingly common. Consider intoxication by additional agents. Anchoring bias is a dangerous phenomenon: don't get burned! The Nose Knows. Or does it? Intranasal (IN) naloxone is popular among many EMS agencies as well as law enforcement, fire departments, and bystanders. IN naloxone has been shown to be effective in several randomized controlled trials for successful reversal of opioid intoxication. However... There are important pitfalls to be cognizant of when choosing this option for delivering naloxone.  Intranasal naloxone has poor bioavailability when compared to IV or IM dosing, so higher doses may be required to achieve clinical effect. This is further potentiated by the maximum volume able to be absorbed by the nasal mucosa (around 0.5mL). Patients administered intranasal naloxone may have a variable or delayed response in achieving reversal. Protect Ya Neck Standard isolation precautions are adequate protection against the overwhelming majority of overdose scenes. In the rare instance where respiratory or splash exposure is a concern, a properly fitted N95 mask and goggles will be sufficient. To date, there has yet to be a laboratory confirmed case where a first responder or emergency healthcare provider has suffered a clinically significant opioid intoxication (bradypnea, hypoxia ) as the result of an occupational exposure to fentanyl or its analogues. The sweeping criminal justice bill that Gov. Charlie Baker signed earlier this month received widespread, but not universal, praise as a welcome turn away from the tough-on-crime policies of the 1980s and ‘90s. One notable dissenting voice was that of Setti Warren, the former Newton mayor who is now one of three Democrats vying to challenge Baker in the November election. Warren said there was lots that he liked in the bill, which pulled back sanctions in all sorts of areas, including eliminating or ratcheting back several mandatory minimum drugs sentences. But he denounced the inclusion in the bill of new mandatory minimum sentences related to the synthetic opioids Fentanyl and Carfentanil and said he would have vetoed the legislation because of them. Sen. Will Brownsberger, the lead Senate author of the bill, called Warren's position “uninformed,” and said it prompted him to endorse Jay Gonzalez, one of Warren's rivals in the Democratic primary for governor. Warren and Brownsberger sat down together to discuss their differences in this week's Codcast. The crux of the debate came down to finding the right balance between sticking with one's principles and the need for compromise to move things forward.

Author: Don Stader, M.D. and Rachael Duncan, PharmD BCPS BCCCP Educational Pearls Carfentanil is an opioid elephant tranquilizer that can be used recreationally.  It is 1000 times stronger than fentanyl, and looks like a white powder. Healthcare workers must be cautious when carfentanil overdose is suspected, as the drug can be absorbed through caregivers’ skin if it is present on the patient’s clothes. Overdose may require large amounts of naloxone, and a drip may be started at a rate equivalent to the bolus dose that the patient responded to.
