Chemotherapy, one of the mainstays of cancer treatment, can unfortunately act as a double-edged sword. While achieving the intended aim of killing cancerous cells, it also generates an accumulation of cell debris, which in turn, promotes tumour growth by stimulating inflammation in the tumour microenvironment. Dr Dipak Panigrahy and his colleagues from Harvard Medical School, USA, have conducted several studies in mice showing that targeting the tumour cell debris-mediated surge of proinflammatory and protumourigenic factors provides a strategy for enhancing the efficacy of chemotherapy.

Welcome to this first episode of season 3, and a great way to start with a conversation with Prof Dipak Kalra, formerly a Primary Care Practitioner in London, UK, academic at University College London, and Ghent University, and President of the Institute for Innovation through Health Data (i~HD). Dipak is internationally known and renowned with a reputation for cogent and clear thinking around health data, data interoperability and quality, and policy development, with i~HD being a leading European-based organisation dedicated to advancing the field. In this episode Dipak reverts back to his experiences in general practice in the 1980s, and the advent of electronic health records systems being introduced also to support use of data for research, albeit with underlying challenges around ethics and privacy governance, which of course still pervade to today. We discuss some of the challenges around ethics of data use, and privacy preserving aspects, as well as some of the more recent innovations, while focusing on the balance between risks of harms, versus benefits for patients themselves. Dipak outlines the vision and direction of i~HD, from its origin in the IMI EHR4CR project, today and in the future, particularly regarding collaboration and safer use of health data, data quality, best practice, and policy development in this domain at a European level. Ultimately, i~HD acts as a neutral catalyst in addressing development and innovation. Lastly, Dipak describes the vision he has for health data use, RWD/RWE and a 21st century frameworks to meet the explosive need for evidence, , avoiding 'bureaucratic processes' and ensuring enhanced efficiency in generating RWE.
Real-world data certainly plays a substantial role in understanding human health and disease. However, scientists find it increasingly difficult to extract research-specific data from massive unstructured and uncoded data sets. “What we are seeing now is a more proactive anticipatory mission to connect multiple repositories of health data and to enable a strongly de-identified, but fully anonymized, data set to be available for research questions,” Dipak Kalra, President of The European Institute for Innovation through Health Data, tells host Deborah Borfitz. In this episode, Kalra speaks about the rise of research data networks and the facilitation of more effective and affordable research. He provides details about the growing challenges facing research teams and the new regulations imposed by the European Commission. Dipak Patel had a first-class career spanning more than 20 years, including over a decade for Worcestershire before moving to NZ and playing 37 Tests and 75 ODIs for his adopted country. Perhaps best known for opening the bowling in the 1992 World Cup, Dipak repeated the dose in 1993 to help spin NZ to a rare Test victory against Australia. Since his playing days, Dipak stayed in the game as a coach, including stints with Central Districts, NZ U-19 and Papua New Guinea. We talk about coming through the grades predominantly as a batsman, England ambitions, the move to NZ, a tough introduction to international cricket against an incredible West Indies side, the NZ tour to Pakistan in 1990, cutting off Dean Jones and opening the bowling at the 1992 World Cup, plus plenty more. Episode 28 of Reporters Without Orders features our host Cherry Agarwal, along with Rohin Verma, two-time Ramnath Goenka award-winning Rahul Kotiyal, and Atul Dev of The Caravan.Atul begins by explaining the gist of his latest story -- a profile of Chief Justice of India Dipak Misra. “There are a whole bunch of charges against (Indian CJI Dipak Misra) in the impeachment motion that was eventually rejected by Venkaiah Naidu…I look at his entire career, I examine how these charges came to be, how these charges came to public light. Then I also look at his family history…There's an analysis of his High Court and Supreme Court judgments and coming up to right now, with everything that has transpired in the Supreme Court in the past year…”Cherry then asks Rahul why he chose to explore the story of communal violence in Uttarakhand. "It's been 18 years since Uttarakhand was created, and it was an area of Western UP which was never communally charged up. However, these increased in the past few years. Almost a dozen big incidents like that of shops being burnt and destroyed."“In the hilly districts of Western UP, no place has a Muslim population of higher than 3 per cent, but even there people have the fear that their sources are being taken over by a foreign other," explains Rahul. He then remarks on the Right-wing's tying of this mentality to the mass migration of Rohingya Muslims to the region. “If you look on-ground, you will hardly find any Rohingya over there. This narrative is spreading really fast on social media.”After Rahul remarks about the lack of analytical or critical local coverage of the event, Cherry comments, “It's up to the local media to take a side.”On the writing process (a 23-page story), Atul says, “You just have to sit down and get done with it.” He also reveals the role of the newsroom in the creation of the story. “After I give them the story, I think 7 or 8 people are actively involved for a whole month…”“In the end, everything that I had found, I was able to put down," adds Atul. I first interviewed Dipak shortly before the ANC conference of December 2017 that led to Cyril Ramaphosa being appointed as ANC Secretary-General and subsequently the President of South Africa, replacing Jacob Zuma. Since then the government has made various efforts to rehabilitate itself, and various state-owned enterprises blighted by corruption and mismanagement. Earlier this year Dipak was appointed as Special Advisor to Minister of Public Enterprises Pravin Gordhan to assist in rolling-back the impact of state capture.

Chintan Desai is a teacher and educator from Helena, Arkansas. He was born in San Luis Obispo, California. His parents, Dipak and Daksha, immigrated from Mumbai, India in search of a better life. With little money and few friends, they built a new life for themselves and their son. ?Their journey to America was not without great cost. They left behind a community of family, friends, and loved ones. But they understood that America was a place of opportunity, where their hard work would be rewarded. And they worked hard. The day after graduating from college, Chintan moved to Helena, Arkansas, where he taught fifth grade social studies. Jennie Lou Leeder, 7th generation Texan, is an educator, a businesswoman, a coach, and has served 10 years as County Chair for the Llano County Democrats (2007-2017). Jennie Lou's family moved to Llano when she was 9 months old. She is a graduate of Llano High School. She graduated Southwest Texas State University with a degree in Agriculture Education and minor in Physical Education. She also graduated Ohio University with a Master of Science in Athletic Administration. Growing up she was active in 4-H and received the Texas Gold Star Girl Award. She is very proud that her family was awarded the Texas Century Farm Award in 1992. During her tenure as Llano County Democratic Chair, Jennie Lou was a member of the Texas County Chairs Association and was on the Executive Board. In 2014 she was elected Senate District 24 Committeewoman where she served one term and also sat on the Rural Subcommittee, Urban County Subcommittee, and Legislative Committees. She was a national delegate to the DNC in 2008 and 2016.

Dipak Patel is a former MK commander, a former Director-General in the Department of Transport and now an investor and social entrepreneur who spends his time between Johannesburg and New York. In this episode Dipak talks about growing up in Durban, joining the armed struggle at the age of eighteen and finding himself as part of the negotiations process that enabled the transition to democracy at the age of twenty-seven. Dipak Srinivasan of the Applied Physics Laboratory at Johns Hopkins University played a pivotal role in the New Horizons mission whose probe just passed by Pluto, and since he was also Andy's roommate back in their college days he was kind enough to talk with Andy and Matt about his work on the project. Launched in 2006, New Horizons traveled over three billion miles to gather the most comprehensive data about Pluto that mankind has ever seen, including stunning imagery of mountains and frozen plains, and en route it got a boost by slingshotting around Jupiter, becoming the fastest-moving probe in history. Dipak also gives the inside scoop on other projects he's been involved in, including the MESSENGER Mercury orbiter and the upcoming Solar Probe Plus mission to scrape the surface of the Sun.
