A Tale of a Cat and a String"So what's 155 inches long and cost $5,000?" asks C."The piece of string we had surgically removed from the inside of our cat last week," replies Torya.And so begins the story of Jasper, affectionately known as Mr. Purrs, and his recent health scare. It all started when Jasper began vomiting for two straight days. Concerned for his well-being, we rushed him to the vet, where an X-ray revealed a linear object in his colon. Little did we know that this object would turn out to be a 155.5-inch long piece of white thread."He unthreaded the sewing machine and took it off himself," Torya explains. "A sewing machine that was somewhere where he shouldn't have been in the first place."The vet suggested keeping Jasper overnight to hydrate him and see if the thread would pass naturally. However, the next day brought more complications, including a fever and high calcium levels. It was clear that surgery was necessary to remove the foreign object from Jasper's insides.The Decision-Making ProcessThe vet presented us with two options: an ultrasound or surgery. Surprisingly, she recommended surgery as the first choice, acknowledging that the diagnostic test would likely lead to surgery anyway. This honest approach was refreshing, as it saved us from unnecessary expenses and allowed us to focus on Jasper's well-being.Less than two hours after the phone call, Jasper was undergoing surgery. The vet successfully removed the 155.5-inch thread from his insides, and we were relieved to hear that there were no lacerations or visible damage to his intestines.Jasper's Road to RecoveryJasper's recovery has been a rollercoaster of emotions. From his initial sassy attitude to his dramatic demands for attention, he has kept us on our toes. But despite his mischievous behavior, we can't help but be grateful that he's back to his feisty self."He's been a right menace today," Torya admits. "Yesterday, if I didn't pet him every 3 seconds like he demanded, he would knock things off my table, including full cans of soda into my power strip."But amidst the chaos, we can see that Jasper is healing. He's slowly returning to his normal routine, demanding pets and attention in his own unique way. And while he may still have a few more days of medication and a cone of shame to contend with, we have hope that he will make a full recovery.The Importance of Supportive FriendsIn times of crisis, the support of friends can make all the difference. We were fortunate to have the guidance and expertise of our vet, who not only provided excellent care for Jasper but also offered us honest advice and support throughout the process.But it wasn't just our vet who provided support. Graham Skelhorne-Gross, Jordan Nantais and Ashlie Nadler from our Emergency General Surgery Team for a discussion on cirrhotic patients.   Child-Pugh Score (https://www.mdcalc.com/calc/340/child-pugh-score-cirrhosis-mortality) ·      Bilirubin, albumin, INR, ascites, encephalopathy ·      Used to predict operative mortality based on cirrhosis severity ·      Mortality in EGS: - Child-Pugh A: 10% electively and 22% emergently - Child-Pugh B: 30% electively and 38% emergently - Child-Pugh C: 80% electively and up to 100% emergently Model for End Stage Liver Disease (MELD) (https://www.mdcalc.com/calc/10437/model-end-stage-liver-disease-meld?utm_source=site&utm_medium=link&utm_campaign=meld_12_and_older) ·      creatinine, bilirubin, INR, and sodium ·      MELD < 20 – 1% increase in mortality with each point increase ·      MELD > 20 – 2% increase in mortality with each point increase Pre-operative Planning ·      Identification of cirrhosis with physical examination, bloodwork and imaging ·      Involvement of other medical services (internal medicine, hepatology, ICU) as needed ·      Cirrhosis optimization, if possible ·      Abdominal wall mapping Unexpected Intraoperative Finding Communicate unexpected findings to the operative team and think of additional adjuncts you may need such as additional ports, topical hemostatic agents or energy devices. Think about why you are in the OR. If its an elective situation and can wait, consider bailing. If its emergent, you may have to do something more definitive. Exposure may be a challenge, you may have to alter your typical approach including where the assistant grabs and retracts. Extra hands are helpful. Bleeding can be a big deal. If possible, map out the abdominal wall ahead of time with cross-sectional imaging. Stay away from varices around the umbilicus or porta Ventral Hernia + Cirrhosis ·      Ideally, control ascites pre-operatively, if you can't consider leaving drains ·      Small (< 2cm) hernias close primarily ·      Larger (>2cm) hernias repair with mesh unless infected filed (controversial) ·      Minimally invasive repairs can be performed Benign Biliary Disease + Cirrhosis ·      Incidence of gallstones is 4-5 times higher in cirrhotic patients ·      Prophylactic laparoscopic cholecystectomy (LC) generally not done ·      LC generally considered acceptable in CP A or B but not C (exceptions: HD instability, gangrenous cholecystitis, hemorrhagic cholecystitis) ·      Cholecystostomy and ERCP are safe References:  Bleszynski, M. et. Happy 2023! For this podcast episode, I want to share with you an unexpected medical emergency I faced right before Christmas, which warranted emergent surgery. All turned out well, and on the grand scheme of things it brought to light an important health and wellness concept I want to share with you all.Getting perfectly honest with yourself, are you, right now, prepare to undergo any kind of emergency surgery, emergency procedure, or face an unexpected health crisis? It's very well known that if you are not in optimal medical health, any type of minor emergency that you face is going to be associated with unwanted complications. Those individuals that have chronic medical conditions, including diabetes, hypertension, high cholesterol, liver disease, and kidney disease are going to fare far worse than those without these chronic conditions. So with that said, what are you doing to prepare yourself for these possible scenarios? The vast majority of chronic illnesses can be medically managed (or better yet prevented) reducing the risk of these medical/surgical complications. With the New Year starting, this would be a great objective for all of us.
