“That's what metastatic breast cancer looks like now—patients can live an extended period of time. And sometimes I think we forget to cheer for stable disease. I tell patients they can live with weeds in their garden; they just can't let the weeds take over their garden. And today we don't have a cure. Highlights From This Episode “I think the most important and vital piece of having those conversations is making sure that we know—really know—that patient, because if we know them, that helps guide what they're needing right then, helps guide our verbiage, helps guide disciplines that we bring in.” TS 2:04 “What do they want to hear? I've been in practice a really, really long time, and I've had the entire spectrum. I've had patients say, ‘Tell me every single detail.' I mean, they want pictures. They want graphs. They want me to draw things. I've even had patients that want me to take markers and mark their body parts. … And then I've had patients that say, ‘I don't want to know anything. I trust my healthcare team. I'm going to proceed with treatment, but I really don't want to know anything.'” TS 3:40 “Remember to make things as simple as possible until we really know what the patient knows. We don't send our children to school and start them out in eighth grade; we send them to kindergarten for a reason. So we get basic information, and then we build on that. And I think we need to remember that when we're doing our patient education, whether it's regarding new chemotherapy or treatment plans or palliative care, we've got to remember to start simple. And maybe we build on that very quickly, or maybe it takes a bit more time. Number one—I actually think it helps with adherence because patients understand what we're asking of them and why we're asking that of them.” TS 12:00 “I think what's really kind of key to keep in mind is that patients are going to seek information. And so, we need to make sure that we're giving them really good, reliable, durable information because if we are not giving them good websites, if we're not giving them good written material, if we're not giving them good verbal information and education, they're going to contact ‘Dr. Google.' Dr. Google is good for a lot of things, but sometimes patients can go down a rabbit hole that's not appropriate or not accurate. That's not a good place for them to be.” TS 14:35 “If we find biomarkers that we call actionable, meaning that we find this mutation and we have a drug that blocks that mutation, that is what is going to guide and drive our treatment. Sometimes that can take a bit of time, right? And if we have a patient and they just find out they have metastatic disease, will they want treatment yesterday. And I understand that. … But frequently there is a very important period of waiting and allowing us to learn that enemy better by reviewing genomic testing, looking at that next-generation sequencing, looking at any positive biomarkers in breast cancer. They may have started out ER/PR positive. Are they still ER/PR positive?” TS 23:46 “I think when we're talking about goals of care, first of all, we need to make sure that the patient understands, when we're talking about metastatic disease today…, this is not a disease that we can cure, but hopefully it's a disease that we can manage for years to come. With that in mind, what's important to that patient? What is important to that patient in terms of life goals? What's important to that patient in terms of toxicities that they will allow and toxicities that they won't allow? TS 29:22 “If don't have a lot of medical knowledge, taking a pill seems less important than getting an IV. It seems like a bigger deal if I would miss getting my IV therapy. ‘Oh, whoops, I forgot to take a pill. Maybe it's not such a big, important piece of my treatment.' So education—when we set that patient down, helping them understand how this drug works, mechanism of action in a simple term, why it's important to take it as scheduled, why it's important to take it with food or without food, why it's important to take it consistently.” TS 34:41

Abstract: The loss of nuclear TDP-43 localization and its accumulation in cytoplasmic aggregates are hallmarks of neurodegeneration and major therapeutic targets. We recently demonstrated that TDP-43 binding to endogenous nuclear GU-rich RNAs sequesters TDP-43 in the nucleus and restricts its passive nuclear export. Here, we tested the feasibility of synthetic RNA oligonucleotide-mediated augmentation of TDP-43 nuclear localization. Using biochemical assays, we compared the ability of GU-rich oligonucleotides to engage in multivalent, RRM-dependent binding with TDP-43 and identified (GU)16 as a strong multivalent binder. When transfected into cells, unlike monovalent oligonucleotides that displaced TDP-43 from the nucleus, (GU)16 preserved steady-state TDP-43 nuclear localization and prevented transcriptional blockade-induced TDP-43 mislocalization. RNA pulldowns from (GU)16-transfected cells confirmed that (GU)16 induced high molecular weight RNP complexes, incorporating TDP-43 and possibly other GU-binding proteins. Transfected (GU)16 caused partial failure of TDP-43 cryptic exon repression, likely because the high-affinity oligonucleotides diverted TDP-43 from endogenous RNAs. The frequent juxtaposition of proteinaceous biomolecular condensates to cellular membranes raises the intriguing prospect that phase transitions in proteins and lipids could be co-regulated. Here we investigate this possibility in the ribonucleoprotein (RNP) granule-ANXA11-lysosome ensemble, where ANXA11 tethers RNP granule condensates to lysosomal membranes to enable their co-trafficking. We show that changes to the protein phase state within this system, driven by the low complexity ANXA11 N-terminus, induce a coupled phase state change in the lipids of the underlying membrane. We identify the ANXA11 interacting proteins ALG2 and CALC as potent regulators of ANXA11-based phase coupling and demonstrate their influence on the nanomechanical properties of the ANXA11-lysosome ensemble and its capacity to engage RNP granules. The phenomenon of protein-lipid phase coupling we observe within this system offers an important template to understand the numerous other examples across the cell whereby biomolecular condensates closely juxtapose cell membranes. Abstract: Altered RNA metabolism is a common pathogenic mechanism linked to familial and sporadic Amyotrophic lateral sclerosis (ALS). ALS is characterized by mislocalization and aggregation of TDP-43, an RNA-binding protein (RBP) with multiple roles in post-transcriptional RNA processing. Recent studies have identified genetic interactions between TDP-43 and Ataxin-2, a polyglutamine (polyQ) RBP in which intermediate length polyQ expansions confer increased ALS risk. Here, we used live-cell confocal imaging, photobleaching and translation reporter assays to study the localization, transport dynamics and mRNA regulatory functions of TDP-43/Ataxin-2 in rodent primary cortical neurons. We show that Ataxin-2 polyQ expansions aberrantly sequester TDP-43 within ribonucleoprotein (RNP) condensates, and disrupt both its motility along the axon and liquid-like properties. Our data suggest that Ataxin-2 governs motility and translation of neuronal RNP condensates and that Ataxin-2 polyQ expansions fundamentally perturb spatial localization of mRNA and suppress local translation. Overall, these results indicate Ataxin-2 polyQ expansions have detrimental effects on stability, localization, and translation of transcripts critical for axonal and cytoskeletal integrity, particularly important for motor neurons. Abstract: The midbody (MB) is a transient structure at the spindle midzone that is required for cytokinesis, the terminal stage of cell division. Long ignored as a vestigial remnant of cytokinesis, we now know MBs are released post-abscission as extracellular vesicles called MB remnants (MBRs) and can modulate cell proliferation, fate decisions, tissue polarity, neuronal architecture, and tumorigenic behavior. Here, we demonstrate that the MB matrix,the structurally amorphous MB core of unknown composition,is the site of ribonucleoprotein assembly and is enriched in mRNAs that encode proteins involved in cell fate, oncogenesis, and pluripotency, that we are calling the MB granule. Using a quantitative transcriptomic approach, we identified a population of mRNAs enriched in mitotic MBs and confirmed their presence in signaling MBR vesicles released by abscission. The MB granule is unique in that it is translationally active, contains both small and large ribosomal subunits, and has both membrane-less and membrane-bound states. Both MBs and post-abscission MBRs are sites of spatiotemporally regulated translation, which is initiated when nascent daughter cells re-enter G1 and continues after extracellular release. We demonstrate that the MB is the assembly site of an RNP granule. MKLP1 and ARC are necessary for the localization and translation of RNA in the MB dark zone, whereas ESCRT-III was necessary to maintain translation levels in the MB. Our data suggest a model in which the MB functions as a novel RNA-based organelle with a uniquely complex life cycle. We present a model in which the assembly and transfer of RNP complexes are central to post-mitotic MBR function and suggest the MBR serves as a novel mode of RNA-based intercellular communication with a defined biogenesis that is coupled to abscission, and inherently links cell division status with signaling capacity. To our knowledge, this is the first example of an autonomous extracellular vesicle with active translation activity. Copy rights belong to original authors. Last month, Cordae tweeted about how after a four-hour road trip it dawned on him that his debut album The Lost Boy is better than the follow-up From A Birds Eye View. He closed the post with “My bad y'all. Ima do better next time. I got sumn to prove.” Today (August 11) he released his first steps toward redemption in the two new singles “Unacceptable” and “So With That.” A known talented lyricist, the 24-year-old flexed his ability to craft two unique listening experiences. “Unacceptable,” produced by ATL Jacob and Hendrix Smoke, is earnest ear candy for modern-day music listeners with a mid-tempo trap feel. “So With That” sounds like he took a road trip to the past, with a backpack rap-type beat laced with high-pitched vocal samples. The accompanying video finds the “RNP” rapper riding in the car with a woman before later sitting on a couch rapping while she yells at him. Surely Cordae hopes that the fan reception to these records is the inverse of her ferocity as he works toward his next project. These records come over almost seven months after From A Birds Eye View, which featured Lil Wayne, Lil Durk, HER, Gunna, and Stevie Wonder.
