"There is a frenzy of trying to use technology to re-establish the healing human connection in the doctor-patient interaction. These efforts range from advanced transcription of voice-to-record, scribes who do the data recording during a patient encounter, and so on. The IT department at NYU Grossman Medical School, where I teach, worked with the dominant and much-loathed electronic medical record to create a patient-focused dashboard for each doctor. The dashboard both improved quality of outcomes and reduced costs. These attempts are useful, but they don’t address the root cause of this inhuman encounter: billing, the link of patient data to cash. In each of these tech improvements, the doctor is still forced by the money-people to massage the bill for the highest private insurance payments. There is a war across the screen — and like all wars, it’s about money. On one side, doctors are forced by the accountants to bill the most. On the other side, insurance people are striving to pay the least. No 'program' or 'bot' could do this job. It’s trapping doctors in the epidemic called burnout, moral injury, or just plain abuse. Forcing us to lie, to provide maximum payment. So long Hippocratic Oath. I think the solution is obvious. Why are VA hospitals’ electronic medical record systems widely preferred to any of the other ones out there? Because they are not-for-profit billing machines. All across the U.S., each VA diagnosis and treatment has approximately the same reimbursement. No war across the screen. No Hippocratic hypocrisy." Samuel Shem is a psychiatrist. He is a Professor in Medical Humanities at NYU School of Medicine, and Visiting Artist at the American Academy in Rome. His novels include The House of God, Fine, Mount Misery, In the Heart of the Universe, and The Spirit of the Place (which won two national "Best Literary Novel" awards in 2008 and 2009). He is coauthor with his wife, Janet Surrey, of The Buddha's Wife, and the award-winning Off-Broadway play Bill W. and Dr. Bob, about the founding of Alcoholics Anonymous, and also We Have to Talk: Healing Dialogues Between Men and Women. Dear Friends & Colleagues,Our guest this week is Samuel Shem.  Many of you will immediately recognize the pseudonym for the author of the classic piece of medical fiction - The House of God. I grew up in healthcare reading his book over and over again. For those of you who are unfamiliar with that piece of fiction, I would urge you to read it. It is a brutally honest, true-to-life, incredibly humorous depiction of the unjust abuses of medical training in one of the most prestigious and prototypical internship/residency programs in our country. It is also one of the most empathetic gifts anyone has ever offered up to those of us who endured medical training.Now, nearly four decades after the publication of that classic novel, Dr. Shem has published another book, Man’s 4th Best Hospital.  Whereas the House of God chronicled the trauma inflicted on doctors (and nurses) in training, this book provides some unique perspective on the dehumanizing aspects of healthcare delivery for patients and providers. Shem is currently Professor of Medicine in Medical Humanities at the NYU School of Medicine.  He is a novelist, playwright, and, for three decades, a member of the Harvard Medical School faculty. His novels include The House of God, Mount Misery, and Fine. He and his wife, Janet Surrey, co-authored the award-winning Off-Broadway play Bill W. and Dr. Bob, the story of the founding of Alcoholics Anonymous.  Dr. Shem is a graduate of Harvard College and Harvard Medical School; and earned a PhD as a Rhodes Scholar at Oxford. In this interview, you’ll hear:Shem’s understanding of writing as a way to empathetically resist the dehumanization of healthcare delivery.The unique way in which Shem distills and articulates the fundamental problems facing doctors and patients in healthcare today.The “big resist” and the solutions Shem shares to counter the root cause problems facing healthcare delivery today.Some of Shem’s opinions will, no doubt, be controversial, and some may consider his solutions extreme. I found Dr. Shem’s analysis to be thought provoking and disarming in its honesty and simplicity.  And while many of his observations are not earth shattering, he finds a way to say them in new and insightful ways. Whether you agree with him or not, what’s clear is that Shem is coming from a place of tremendous respect and empathy for patients and providers. Shem’s novel and his commentary are incredibly topical and timely.  Many, if not most, would agree that our healthcare delivery system is inadequate, poorly aligned to the healthcare needs of our populations, financially unsustainable, and in need of significant disruption. What I found most compelling about his book and this interview is Dr. Shem’s unrelenting hopefulness. He deeply believes that as long as we are connected with one another in just pursuits; as long as we can all have a voice in honest, open dialogue; as long as we are able to speak truth to power; as long as we can be critical thinkers and creators, and as long as we maintain a mutual empathy and a respectful sense of humor, we can change the world for the better. What his recent critics have misunderstood about Shem is that he is a human rights activist as much as he is an artist. He is not, in my opinion, writing for the purpose of winning awards or critical acclaim, although that may occur. He is here to actively resist the dehumanizing forces in healthcare, to catalyze positive change, and to make the world different and better.      We recently honored the memory of Martin Luther King Jr who said, “Our lives begin to end the day we become silent about things that matter.”  Sam Shem, in the tradition of MLK, refuses to be silent. EPA Podcast Series: Giving First!- An Interview with Baruch Kim, MD -Networking -Bestdocz -Eastern Pain Association Dr. Baruch Kim obtained his undergraduate degree in Psychology at Stony Brook University with a simple desire to study the "human mind." At Stony Brook University, he participated in conducting a research study in human memory. He has extensive experiences in teaching his peers and students including a self-developed course in Rapid Reading, Optimization of Human Memory, Mid Mapping, as well as his online Chemistry Series and many others.    Dr. Kim finished his medical school in New York, completed his internship in Michigan. He then moved back to New York to join his residency program at the Rusk Rehabilitation of NYU Langone Health, where he has been serving as an administrative chief resident. Dr. Kim will soon begin his journey in pain medicine at the ACGME-Accredited Pain Management Fellowship Program at NYU.    Dr. Kim has featured in the newsletter at NYU on multiple occasions with his institutionwide projects in patient safety. Has contributed to improving the interpretation service at NYU Tisch, NYU Langone Orthopedic Hospital and NYU Brooklyn Hospital for all physicians. He also made official institutional guides (tutorial animations) on how to report patient safety events at three hospital sites, and how to perform the IPASS (handoff). He has also created tutorial animations on High Reliability Organization for all house staff at NYU with his sincere dedication to supporting "the culture change" in patient safety.   At the beginning of his PGY 4 year, he was nominated by NYU residency program and the Association of Academic Physiatrists as an ACGME Review Committee member of his field. Dr. Kim has numerous leadership positions on a regional, and national level. In this conversation, Shem tells me about his motivation for the House of God and reveals that there will be a sequel in 2019 on the current state of medicine.

Ray Noble talks to Samuel Shem, the acclaimed author of The House of God. The House of God, published in 1978, has in many ways become a cult book in the medical profession. It follows a group of medical interns at Beth Israel Hospital over the course of a year in the early 1970s, focusing on the psychological harm and dehumanization caused by their residency training.  For this dialogue we took the opportunity to catch up with Samuel Shem when he visited Oxford last year.  We hope to have a more extensive interview with Samuel Shem in the near future.
