It also highlights the need for a robust veteran services commission to ensure ongoing advocacy and support for those who have served. Dr Andrew Khoo is the Director of Medical Services at Toowong Private Hospital and is the Deputy Chair of the College's Military, Veterans' and Emergency Services Personnel Mental Health Network. He is also the Chair of the Open Arms National Advisory Committee and a member of the DVA Mental Health Expert Advisory Group. A/Prof Jon Lane, FRANZCP, MBBS (Hons), PhD, has been in the Army since 1989 and is also an Afghanistan veteran. He is a clinician in private practice, as well as holding several different roles. These include being the inaugural Chief Psychiatrist for the DVA; the Psychiatry Lead for the University of Tasmania School of Medicine; and has adjunct appointments and research projects with the Queensland Centre for Mental Health Research and Gallipoli Medical Research Foundation. He is also the current Chair of the College's Military, Veterans' and Emergency Services Personnel Mental Health Network. Dr Peggy Brown AO is currently the Chief Medical Officer at Medilinks and holds several other key leadership positions. These include Chair of Mental Health Australia, Board Director at Wellways Australia, Chair of the RANZCP Community Collaboration Committee, Member of the Clinical Governance Advisory Committee for the Australian Commission on Safety and Quality in Health Care, and Patron of the BPD Foundation. She has also served as a Commissioner for the Royal Commission into Defence and Veteran Suicide, Senior Clinical Advisor at the Australian Commission on Safety and Quality in Health Care, and Chief Executive Officer of the National Mental Health Commission. She has also held the positions of Director-General of ACT Health and Chief Psychiatrist/Director of Mental Health in Queensland, the Australian Capital Territory and the Northern Territory. The third World Health Organization Global Patient Safety Challenge ‘Medication without harm', emphasises the need for improved infrastructure through better reporting practices and cross-organisational learning from adverse events and near misses. Medication is implicated in one-fifth of serious palliative care patient safety incidents, with approximately 25% of these incidents involving continuous subcutaneous infusions. Inadequate analysis of continuous subcutaneous infusions as safety-critical, risk-prone interventions dependent on complex structural and human factor issues is a lost opportunity for learning.     What this paper adds? Continuous subcutaneous infusion incidents occur across all settings including the home, hospices and hospitals and particularly after the transfer of patients between settings with harm present in nearly three-quarters of reports. Multiple points of system failure were identified in continuous subcutaneous infusion incident reports including monitoring and supply (405, 31%), administration (383, 29%) and prescribing (268, 20%); recurring contributory factors included discontinuity of care within and between care settings, inadequate time, inadequate staffing and unfamiliarity with protocols. Narrative descriptions of psychological and social harm, alongside physical harm risk, are not being adequately recognised or responded to through existing approaches to measure harm in palliative care, hindering learning in practice   Implications for practice, theory, or policy The structural changes needed to minimise harm and maximise safety in palliative care are likely to be replicated in other parts of the world where patient safety reporting practices are less well established, for example, shifting from focussing on lack of experience and competency at an individual practitioner-level to addressing deficits in working environments and infrastructures for care provision. When patients move between care locations, more attention should be given to the timeliness and effective transfer of medication management (e.g. if someone is discharged from hospital to a care home that rarely uses continuous subcutaneous infusions for palliative care, this needs to be preceded by refreshing staff skills and ensuring they can access further community support if needed). Join Ben for his discussion with attorneys Taylor Asen and Trevor Savage about their recent $2.4 million verdict in Bangor, Maine in a case involving a surgical mal positioning that caused an arm injury and CRPS.  Taylor and Trevor explain how they worked closely with the client to get her to the right specialist who could diagnose her medical condition.  They talk about the challenges of presenting a CRPS injury at trial.  They discuss how they resisted the defense effort to stipulate to liability and were able to present compelling evidence of corporate indifference and system failure in the lack of safeguards or systems to make sure that the surgery was done safely, and the lack of follow through, investigation, or changes that came out of this incident.  They discuss how they responded to the defense's 11th hour surprise surveillance video, which the judge allowed into evidence over objection.  They used the surveillance to further polarize the case and show the lie in the hospital's claim to be taking responsibility for its actions.  They discuss the use of data analysis to recognize the true value of the case and to formulate the amount asked for in closing. About Taylor Asenwww.gideonasen.com Taylor specializes in medical malpractice cases, trucking accident cases, and other complex personal injury cases. Daniela Cambone speaks with Jim Bianco, founder and president of Bianco Research, to discuss the recent rise in Treasury yields, its impact on the economy, the state of the banking system, and the real estate market. Bianco explains that the rise in Treasury yields indicates a rejection of previous lows, setting up a base for higher yields. "It means that the economy is doing OK," he continues. However, he points out that inflation becomes a concern as the Fed may not cut rates until the summer. Additionally, Bianco states that systematic risk within the U.S. banking sector cannot be ruled out, as many smaller banks engaged in commercial real estate lending have been negatively impacted by the discounted value of office buildings. “What is an office worth today when people only go there three days a week? And the answer is we don't quite know, but we do know it's not what it was worth in 2019.” He also explains the reasons behind the current real estate recession and suggests strategies for protecting wealth in this uncertain environment.
