Struggling to get a good night's sleep, feeling completely exhausted and burnt out. Thinking that if she could just escape for a short amount of time everything would be ok. But even after a vacation, the feeling didn't go away. It wasn't until Rachel learned how to tune into her body using yoga & somatic practices that she was able to change the relationship she had with herself and her anxiety. Rachel has been a serial entrepreneur in the health and wellness space, with yoga and health coaching and even has her own podcast. She is also a life and business coach. She loves what she does. For her, coaching is amazing as she loves helping people change the way people see themselves and experience themself. She's grateful for being able to have clients and not work a traditional job because she'd miss out on such incredible experiences. Through her many business adventures she found that she hadn't truly figured out her passion through her own journey of trial and error. Although, she enjoyed the journey, it took years to figure out her purpose and she may not be done yet figuring it out. So her best advice is to stay consistent and provide tons of value ahead of time for others and you'll see rewards in your life. It doesn't matter if you haven't figured it all out yet, as long as you enjoy the journey and be open to what may come. The best advice she can give is learn how to manage your mind and stay in a positive mindset about each experience, and if you can't have a positive mindset, see if you can find someone who can help you develop one.

She is among the founding faculty and Associate Program Director of the Family Medicine residency program at Kaiser Permanente Santa Rosa, where she teaches both medical students and residents, and has been involved in the creation of an innovative 3 year curriculum designed to train family physicians for the future of medicine and community practice. She also serves as an assistant clinical professor in the UC San Francisco Department of Family and Community Medicine.  Dr. Friedman received her undergraduate degree from Harvard University with high honors in History of Science, and her medical degree from the Yale School of Medicine, where she also received a Masters in Health Science and completed published research on the electrophysiologic effects of relaxation techniques. Dr. Friedman has edited medical textbooks, written peer-reviewed journal articles, and been invited to speak locally and nationally on topics such as clinical nutrition, innovations in technology, preventive approaches to disaster medicine, and implicit bias and health disparities. One of her achievements to date as a musician-physician was co-authoring and performing in a full-length educational 80’s rock opera called Diabetes: The Musical. Rachel discusses the beautiful way women look after one another, the importance of being a hometown traveler, and her top tips for aspiring travel writers. For Rachel Friedman, being an entrepreneur is a lot like being on a trapeze without a net – and knowing you don't have a net. "But that's part of the exhilaration," she said. "The highs are really high, and the lows are really low." Friedman, the founder and CEO of Tenfold, knows a lot about the highs – she's successfully launched and scaled a business that recently made the Inc. 500 Companies List – and the business itself is only five years old. But she's no stranger to the lows, either. Friedman said that unfortunately, Tenfold, the local strategy and creative firm, is only part of 12% of businesses in the Fortune 500 that are owned by women. "That makes us proud, but also very concerned," she said. In this episode of the Women of Influence podcast, Friedman talks about the hurdles of being a woman business owner, gives the rundown on whether or not an MBA degree is worth the cost, and also who inspires her most – her restauranteur mother and jelly factory owner grandmother. "I think I learned from both my grandmother and my mother is that they were who they were," she said. "They weren't going to change who they were as a person, in whatever role they had – whether that role was boss or mom – and they did it in their own style."

Rachel Freedman, MD, MPH- Assistant Professor, Medicine, Harvard Medical School, Dana-Farber Cancer Institute presents a self-assessment question from an ASCO University course focusing on the treatment breast cancer.  AUDIO TRANSCRIPT Welcome to the self-evaluation episode of the ASCO University weekly podcast. My name is Rachel Friedman, and I'm a medical oncologist and clinical researcher in the Breast Oncology Center at Dana-Farber Cancer Institute. Today, we feature a self-evaluation question on the treatment of early stage breast cancer. We begin by reading the question stem. A 75-year-old woman with a past medical history of coronary artery disease and diabetes, with an ECOG performance status of 2, has been seen in the medical oncology office following a partial mastectomy for a 1 centimeter invasive breast cancer. The margins were clear and hormone receptors are reported as positive. The HER2/neu status is reported as negative. An Oncotype recurrence score is reported as 5. Which of the following is the best adjuvant therapy for this patient? Choice A, doxorubicin, cyclophosphamide, and weekly paclitaxel, followed by an aromatase inhibitor. B, docetaxel and cyclophosphamide for four cycles, followed by an aromatase inhibitor for five years with radiation therapy to the breast. C, radiation therapy to the breast and axilla, followed by an aromatase inhibitor. D, aromatase inhibitor. E, tamoxifen. The correct answer to this question is D, aromatase inhibitor. This patient has a poor performance status and high co-morbidity burden, with a higher likelihood of non-breast cancer deaths over breast cancer death, over time. The cancer is low risk, with a low likelihood for local and distant recurrence. Radiation and chemotherapy are unlikely to improve her outcome and will increase her toxicity risk. Further, we have multiple randomized trials to support safe omission of radiation and the setting of older age and stage 1 hormone receptor positive disease. Briefly, the benefit of adjuvant chemotherapy is anticipated to be negligible, and even harmful, in the setting of her low risk disease, advanced co-morbidity, poor functional status, and Oncotype score of 5. Chemotherapy should be avoided in this setting, making answers A and B incorrect. Although radiation should be considered and administered in most patients with invasive breast cancer, undergoing breast conservation, as mentioned earlier, we have prospective randomized controlled trial data, supporting omission of radiation in this clinical scenario, making option C also incorrect for this reason. Further, there is no indication for radiation to the axilla in a low risk breast cancer, such as the one described here, making option C incorrect for that reason as well. Option E, tamoxifen, is also a treatment option here, but may be less appealing in an older patient who lives a more sedentary lifestyle who may be at more risk for adverse events on tamoxifen, such as thrombosis, and where the use of aromatase inhibitors may be of a small incremental benefit over tamoxifen, with regard to risk for recurrences over time. The panel was moderated by Dr. Stephanie Ross of the Work and Labour Studies Program at York University and included the film's director Amie Williams, Wisconsin union activist Rachel Friedman, and local representatives Lisa Hammond, President of HWETL and Anthony Marco (me), Member of the HDLC Executive and OSSTF District 21.
