He completed his residency in Anatomic and Clinical Pathology at The Johns Hopkins Medical Institutions (JHMI), and his fellowship in dermatopathology, also at JHMI. Dr. Hosler is very involved in education, teaching at the fellowship, residency, and medical school levels. He is the Director of the ACGME-accredited Dermatopathology Fellowship Program at ProPath. He actively speaks, writes, and directs courses on dermatopathology. He is the editor and co-author of two dermatopathology textbooks - Molecular Diagnostics in Dermatology: practical applications of molecular testing for the diagnosis and management of the dermatology patient (Springer) and Diagnostic dermatopathology: a guide to ancillary tests beyond the H&E (JP Medical Publishers). He is also the author of the melanocytic chapter in Weedon's Skin Pathology (4th, 5th, and 6th Eds, Elsevier) and many peer-reviewed journal articles and reviews.

Discordance between Apolipoprotein B (ApoB) and low-density lipoprotein cholesterol (LDL-C) is similar, except rather than counting just LDL particles, ApoB is a measure of the numbers of lipoproteins that have an ApoB attached. Discordance between ApoB and LDL-C can lead to either an underestimate or overestimate of ASCVD risk. And therefore there may be important implications for someone who does have discordance. Additionally, it is such cases that suggest that a measurement of ApoB may provide additional information beyond traditional lipid measures in assessing a person's cardiovascular risk. Recent work from researchers at Liverpool John Moores University has investigated whether discordance is associated with certain dietary patterns. In this episode, one of the researchers involved, Dr. Ian Davies, is on the podcast to discuss this work in addition to wider questions in the diet-lipids-CVD field that remain to be answered.

Comparing parent and provider priorities in discussions of early detection and intervention for infants with and at risk of cerebral palsyRachel Byrne, Andrea Duncan, Tracy Pickar, Stephanie Burkhardt, Roslyn N Boyd, Mary Lauren Neel , Nathalie L Maitre AbstractBackground: Although literature suggests that parents need support when their child is diagnosed with cerebral palsy (CP), it is unclear to what extent providers implement these supports in practice and what parental perspectives surround provider early diagnosis and management of CP. Therefore, we aimed to characterize and compare experiences of providers and parents of children with CP with regards to early detection and intervention.Method: Seventeen parents participated in day-long world-café style workshops focused on categories extracted from the International Classification of Function framework and recent systematic reviews of early detection for CP. Thirty regional providers (generalists, specialists, and therapists) caring for infants with CP completed surveys with scaled score and open-ended questions. Quantitative and qualitative data were independently assessed by two reviewers to identify prominent themes.Results: All parents (100%) stated early diagnosis or high risk for CP classification was beneficial compared with only 50% of providers who often gave early CP diagnoses before 12 months. Top parent priorities were honesty and positively phrased messages. Providers most often addressed cognition, primary care need, motor, and feeding issues (80%, 62%, 54%, 54% frequently/sometimes). Matching priorities for discussion were neuroimaging timing/risk/benefit, cognition, primary care, motor, and feeding/nutrition. Discordance occurred for participation, parent well-being, pain and vision, with parents wanting more education and resources.Conclusions: Receiving early diagnoses or high-risk for CP classification is a parent priority. Alignment between parents and providers exists for International Classification of Function domains of body functions/structures and activity, but less for those of environment, personal, and participation.

Autant d'images qui, d'Adam et Ève au Christ, jalonnent l'Ancien et le Nouveau Testament.   "Notre innocence est notre identité la plus profonde"   La honte et la culpabilité Au début du livre de la Genèse, l’homme et la femme se trouvent libres et sans honte devant Dieu qui les a créés à son image. Il a suffit de la parole d’un serpent pour que ce couple tombe dans la honte et la culpabilité. Yes, both content and context are important aspects of your podcast. But it’s the tone of your show that acts as the delivery-device that translates the sounds (and sights) to thoughts, ideas, and feelings inside your listeners’ brains. It’s arguable that the theme music you’ve chosen for your show has the biggest role in setting the overall tone. Hard-hitting, over-driven guitars wheedling away in the background at the start of the episode sets a particular tone. A mellow piano sets the opposite tone. And a quirky, plucky ukulele means something else entirely. The same goes for transition music used inside your episodes. That little bit of music, even if it only plays for a couple of seconds, is part of the tone of your show. Does it fit?  The words used in the “canned” bits of your episodes impact the tone. As does the length of those bits. How those words are voiced is huge. The cadence of the words spoken also sets the tone in a similar way. A pro-announcer will lend a certain amount of… well, professionalism to your show. But only if that’s what you’re going for. Each choice you make impacts the tone of your show. Savvy podcasters will spend a significant amount of time adjusting EQ settings of all the voices and elements that go into a show. It’s a fair amount of work, but it has huge implications on the overall tone of an episode. There's more to a podcast than the sounds the listeners hear. All of the not-audio parts of your show also set the overall tone of your show.  Yes, I mean the show-level artwork and the episode-level artwork, title, and summary. But I also mean the artwork for your website, newsletter, or social shares.  Discordance isn’t always a bad thing, though it usually is. Think about an orchestra or even a three-piece band playing with one player out of tune. The notes played might be technically correct, but it’s going to sound unpleasant. And you’re going to stop listening.  Your listeners might have that same disconnect when your punchy theme music plays under a soft-spoken episode intro. Or they’ll see the garish colors of your attention-grabbing audiogram and then be taken aback when they a heartfelt testimonial of a survivor of abuse. That’s bad too. My fear is that you might read this and assume I want everything in your podcast to be clean, pristine, and polished. I assure you I do not. Some shows are purposely designed to be almost toneless. Others are intentionally crafted with changing tones from episode to episode. And if that's the intention you put into your podcast, fine. Lean into that.  It's all about intent and intentionality. Maybe you don't need a tune-up of your show. Here is the latest of our recordings from The British Society for Phenomenology’s 2018 Annual Conference ‘The Theory and Practice of Phenomenology’. Arthur Rose is from Durham University, and his paper is titled ‘Reorienting Breathlessness: A Case against Symptom Discordance’.   Abstract: “In Phenomenology of Illness, Havi Carel identifies a ‘Janus-faced duality’ to breathlessness: ‘it is so real and overwhelming to the person experiencing it and yet so invisible to those around her’ (Carel 2016: 109). The discrepancy between external appearance and internal reality replicates a further discrepancy, known as symptom discordance, between the person’s perception of breathlessness (‘subjective experience’) and the oxygen saturation of their blood (‘objective measurement’). Not only is the breathless person challenged by another’s failure to recognize their experience, their own interoceptive ability may not match what they feel to what medicalised testing shows. Carel usefully relates this mismatch to Merleau-Ponty’s distinction between the objective and the lived body. The affordances of the objective body come to define new limits on the bodily habits that determine the lived body’s engagement with the world. To emphasize the reshaping of world that faces the person with breathlessness, Carel uses geographical metaphors or articulations that demonstrate both forms of discordance. But, Carel does not devote much attention to extremes of symptom discordance: either where a person has objectively low levels of oxygen saturation but does not complain of breathlessness or, conversely, where a person does complain of breathlessness but has no measurable oxygen deficiency. This paper seeks to draw on Carel’s account of breathlessness to consider how it responds to the problems symptom discordance presents for ‘external’ verification, by either casual or medical perception. By shifting the terms of the account, from concordance-discordance to orientation-disorientation, it reframes a continued reliance on the subjective-objective distinction as a matter of cognitive direction. Drawing on Sara Ahmed’s engagement with Merleau-Ponty’s account of orientation, it argues that the experience of breathlessness can be usefully thought of as a series of orientations: to the world, the sensations of breathlessness and their scientific measurement.”   The British Society for Phenomenology’s Annual Conference took place at the University of Kent, in Canterbury, UK during July, 2018. It gathered together philosophers, literary scholars, phenomenologists, and practitioners exploring phenomenological theory and its practical application. It covered a broad range of areas and issues including the arts, ethics, medical humanities, mental health, education, technology, feminism, politics and political governance, with contributions throwing a new light on both traditional phenomenological thinkers and the themes associated with classical phenomenology. Gratian was a contemporary of Peter Lombard. He wrote The Decretum or The Discordance of Discordant Canons. Explore how Canon Law developed into case law and was used by the Church for control. Canon Law was regarded as a supplement to the Bible. The Reformers argued against the Canon Law as equally authoritative to Scripture. Gratian set up a system of courts and procedures and succeeded in establishing the Papacy as an appellate court. Luther, after the Reformation, burned the Canon Law books.
