Welcome to the first episode of DNA Dialogues! Today is February 29th, 2024. It is a leap year, so today marks an extra special rare disease day. Rare diseases can impact so many people - from patients themselves, to families, to broader communities. To celebrate rare disease day, we are going to dive into two recent articles from the Journal of Genetic Counseling that showcase the rare disease experience. The following interviews provide insight into the wider impact of rare disease, with a special focus on families. Segment 1: “Understanding type and quality of relationships between individuals with chromosome 18 syndromes and their siblings” Dr. Catherine Larson joins us in the first segment to talk about her recent article titled, “Understanding type and quality of relationships between individuals with chromosome 18 syndromes and their siblings”. Dr. Catherine Larson is a Child and Adolescent Psychiatrist and a sibling to Elizabeth, who has a Chromosome 18 deletion. After earning her undergraduate degree, she worked as a research assistant at the Chromosome 18 Research Center, where she began her research on Sibling relationships. Dr. Larson earned her Medical Degree from the University of Texas School of Medicine at San Antonio. She then went on to complete a General Psychiatry Residency Program, followed by a Fellowship in Child and Adolescent Psychiatry at The University of Texas at Austin, Dell Medical School. As a practicing psychiatrist, she opened her private practice in Austin, Texas, and she also returned to join the research team at the Chromosome 18 Research Center. Dr. Catherine Larson is double Board Certified by the American Board of Psychiatry and Neurology in General Psychiatry and Child and Adolescent Psychiatry. Dr. Larson earned her Medical Degree from the University of Texas School of Medicine at San Antonio. She then went on to complete a General Psychiatry Residency Program, followed by a Fellowship in Child and Adolescent Psychiatry at The University of Texas at Austin, Dell Medical School. In addition to her private practice, she is currently an Adjunct Assistant Professor at The University of Texas Health School of Medicine at San Antonio. Segment 2 “Families' experiences accessing care after genomic sequencing in the pediatric cancer context: ‘It's just been a big juggle'” Authors Sarah Scollon and Blake Vuocolo talk about their recent Journal of Genetic Counseling paper in the pediatric cancer space. Sarah Scollon is an Assistant Professor in the Department of Pediatrics at Baylor College of Medicine and certified genetic counselor for the Texas Children's Hospital Cancer Genetics and Genomics Program. She has served in dual clinical and research roles across the course of her career. Her research interests engage the overarching themes of (1) adaptation of genetic counseling practice to implement evolving technologies (2) optimization of patient-provider communication and education and (3) equity and inclusion for diverse populations in genetic medicine and research. Ms. Scollon has had a longstanding dedication to the care of children and families affected by cancer. She is an advocate for including genetic counseling into pediatric cancer care. She works to educate others on the role genetics and genomics can play in the care of patients with pediatric cancer and their families both from a clinical and psychosocial standpoint. Blake Vuocolo is a certified research genetic counselor in the Department of Molecular and Human Genetics at Baylor College of Medicine. She graduated from the Baylor College of Medicine Genetic Counseling Program in 2022, and her thesis work focused on access to follow-up hereditary cancer care in underserved pediatric populations through the KidsCanSeq study. Currently, her work focuses on exome and genome sequencing of medically underserved populations with undiagnosed diseases throughout Texas. Ms. Vuocolo is passionate about finding sustainable ways to improving genomic health equity worldwide. Her interests include exploring access barriers to receiving genomic care in different healthcare contexts and improving genetics education for non-genetics healthcare providers in under resourced regions of the country and beyond. MAUI is a beautiful destination for thousands of families who want to experience paradise and the "Aloha Spirit." My family and I (my wife, three kids, my oldest son's wife, and our granddaughter) made the trip to Maui during the first week of August. The first few days were as expected. Beautiful. Relaxing. On Tuesday August 8th, the day started with no power. What would happen from there would be something we as a family have never experienced and would change us forever. Today, I share that story.I took the time on the flight back to try to write down as much as I could about what had happened. On today's episode I'm talking to Eliza Fricker about PDA or pathological demand avoidance – and in fact I know many people interpret PDA as persistent desire for autonomy. I've done a few episodes on PDA before but never from a parent's perspective so after reading Eliza's book, The Family Experience of PDA, I knew I wanted to share her perspective with the Tilt community. During this episode, we talked about how demand avoidance is more extreme in a child with a PDA profile vs. the inflexibility and rigidity we might see in other neurodivergent kids, what Eliza has learned about herself parenting a child with PDA, and what her resistance was to the changes needed to her parenting style. My guest today is Eliza Fricker. Eliza has a 14-year old daughter who is autistic, PDA, and who struggled hugely with school. Eliza spent many years attempting to navigate the education system and advocate for her daughter, but after much frustration and despair, finally decided that leaving mainstream education was the only option. Eliza has published a book called Missing the Mark, in which, through her illustrations, she shows her process of trying to navigate a confusing, system which often fails to embrace neurodivergent children for who they are, focusing instead on the fixing of the child. She has also published The Family Experience of PDA, and a four-part podcast on her family's experiences, and now works to help families who are in a similar position. Today, Eliza shares how life has looked since her daughter left school, and how they embarked on the slow process of healing. Among other things, we chat about the enormous importance of being in the right environment, and how our own deschooling is key to accepting our children just as they are. In the second of this two-part series, Lighthouse staffer Jackie Jones rejoins the podcast to talk about two of our newest Retreat Family experiences: the one-day and weekend retreats. Jackie shares about the difference in the one-day retreat compared to the weeklong seaside retreats when it comes to relationships—how families attend these events in their community, along with other families and volunteers who also live nearby, allowing for friendships to form that can be carried on past the day of the event. She goes on to explain what the one-day event typically looks like, including many similarities to a weeklong treat, such as Common Ground (small group time for the parents), and Family Partners (volunteers who partner with the Retreat Families and play with the kids while the parents are at Common Ground). They also discuss the weekend retreats, an experience Lighthouse piloted in Colorado in 2021. Lighthouse will offer one in Colorado and in North Carolina this year, and applications are open now! These retreats, like the weeklong ones, are provided for free for the Retreat Families and have many of the same activities as well. The weekend retreats are particularly good for families who are unable to attend a weeklong retreat, whether because of treatment or parents' work. Jackie explains how a Lighthouse retreat is so much more than a vacation, and she shares what is required to apply to retreat, as well as the top three questions (and answers!) that she hears from families preparing to come on retreat.

Michele's oldest daughter's first word was “calculator”. Because she was highly verbal and seemed to need less intervention, Michele struggle to get a diagnosis for her. Her second son's presented quite differently, but Michele suspected that he too had autism. Knowing that early intervention was key, Michele was so disappointed to realize that it took until they were pre-teens to get help. She just wanted to understand why her children behaved the way that they do, and this sent her on a journey to get a Master's in Behavioral Therapy.Michele speaks directly to parents in her podcast Navigating the Spectrum. As a mom and an ABA therapist, she knows that an autism diagnosis can have an emotional pull on parents. For her though, the diagnosis was a relief since it means that she could get to work helping her children.In this episode, we have a great conversation about some of the negative perceptions about ABA therapy, and how she approaches those challenges in her practice. She points out that you can do speech all day when you're in the office, but that it only starts to click when the child also does speech at home with their family. One of the major reasons that she approaches therapy as a whole family event is that she knows exactly what it's like as an autism mom herself. Todd and Evan grew up in a small town in Western Massachusetts and were indoctrinated into the world of autism with the arrival of their youngest brother, Jay, who was diagnosed with autism at an early age. The intimacy that Jay promoted (and sometimes demanded) lead to the creation of a family communication style which emphasized togetherness through struggles and triumphs. Todd and Evan share many life lessons they learned from Jay which included how to be one true self, stay genuine in friendship, to separate with both sadness and compassion, and how an entire community can come to someone's aid. The intensity of a life shared with Jay's often unfiltered perspective lead to great moments of happiness as well as trying moments of helplessness and sadness. It is impossible to define Jay's autism as it is interwoven with his vibrant personality.
