I'm delighted to speak to Dr Caz Nahman and Dr Andrew Kirkland in this week's episode. We discuss the role of language and the environment when considering mental health in sport. Caz is a Consultant Child and Adolescent Psychiatrist. Caz has also completed additional training in sport psychiatry with the ISSP (International Society for Sport Psychiatry) and is currently Deputy Chair of the Royal College of Psychiatry Sports and Exercise Psychiatry Special Interest Group. Caz has experience working within a general child and adolescent setting as well as working in in-patient and community eating disorders services. She has previously been an elected member of the RCPsych Eating Disorders Faculty and in this role co-edited a book - New to Eating Disorders - designed as a training tool for staff new to working within this field. Caz's interests include compulsive overexercise; youth sport; eating disorders in athletes, autism spectrum disorders in athletes, life-style psychiatry and medical education. In addition, she is keen to improve standards of care for athletes with mental health problems. Andrew is a Lecturer in Sports Coaching at the University of Stirling. He is also a member of the General University Ethics Panel, a Chartered Scientist and a BASES Accredited Sport and Exercise Scientist (support & pedagogy). Andrew has worked as a sport physiologist and coach developer at British Cycling and supports others in developing performance environments. Andrew's research philosophy is driven by impact and translation of research into practice. Specifically, he uses Implementation and Behavioural Change Science to explore mental health in sport, 'merging' his practical experience with evidence-based practice with the aim of helping others to develop more effective sporting systems.

FROM THE AMERICAN ACADEMY OF CHILD & ADOLESCENT PSYCHIATRY: Suicides among young people continue to be a serious problem. Suicide is the second leading cause of death for children, adolescents, and young adults age 15-to-24-year-olds. The majority of children and adolescents who attempt suicide have a significant mental health disorder, usually depression. Among younger children, suicide attempts are often impulsive. They may be associated with feelings of sadness, confusion, anger, or problems with attention and hyperactivity. Among teenagers, suicide attempts may be associated with feelings of stress, self-doubt, pressure to succeed, financial uncertainty, disappointment, and loss. For some teens, suicide may appear to be a solution to their problems. Depression and suicidal feelings are treatable mental disorders. The child or adolescent needs to have his or her illness recognized and diagnosed, and appropriately treated with a comprehensive treatment plan. Thoughts about suicide and suicide attempts are often associated with depression. She is a Child and Adolescent Psychiatrist, as well as Training Director for the Child and Adolescent Psychiatry Fellowship at University Hospitals of Cleveland/Case Western Reserve in our home state ... Give me an O-H-I-O-!In episode 12, we did a broad overview of ADHD. We started with the hyperactive subtype. We ran out of time to delve into the the inattentive and combined subtype.Here, we review the definition and diagnosis of ADHD and then focus on the inattentive subtype, learning when and how these kids typically present to our attention. We discuss how gender may increase the likelihood of various subtypes and the importance of treatment of the whole person. We discuss the possibility of undiagnosed ADHD leading to other intrinsic and extrinsic behavior problems, thus reinforcing the importance of early diagnosis and treatment. So much of behavior intervention is recognizing our parental bias and instinct, cleaning our 'lense' when appropriate, and seeing our kids for who they are. Kids are not intrinsically bad or malicious. Beneath their sometimes troublesome and aggravating behavior is a child with a need and it is our job as parents and caregivers to uncover this root cause.

In this episode of Real Talk with Kid Docs, we explore how to have the awkward and sometimes  difficult conversations with your children - you know, the ones about sex and drugs that we try to always push for another day. We are joined by Dr. Stan Brewer, D.O., a Child and Adolescent Psychiatrist to help guide us. We are reminded of the importance in developing an open and honest relationship with your kids to support these conversations and really all conversations at any age. A few tips: -Start earlier than you think.  You want them to hear it from you first. -Follow your kid's lead and engage in their curiosity to develop the conversation. For example, it is very natural for elementary age children to have questions about body parts. Use that to guide the conversation. -Use language and terms that are correct and appropriate. Stick with a level that fits their maturity level.It feels like the tween and teen years are new and unfamiliar territory. Remember, you have been parenting for a while now. Build on things you have already talked about in their younger years. You may be more prepared than you think!Where do I even start? Well, often, the first question kids ask that makes a parent squirm is, "Where do babies come from?" To celebrate rare disease day, we are going to dive into two recent articles from the Journal of Genetic Counseling that showcase the rare disease experience. The following interviews provide insight into the wider impact of rare disease, with a special focus on families.   Segment 1: “Understanding type and quality of relationships between individuals with chromosome 18 syndromes and their siblings”   Dr. Catherine Larson joins us in the first segment to talk about her recent article titled, “Understanding type and quality of relationships between individuals with chromosome 18 syndromes and their siblings”.    Dr. Catherine Larson is a Child and Adolescent Psychiatrist and a sibling to Elizabeth, who has a Chromosome 18 deletion. After earning her undergraduate degree, she worked as a research assistant at the Chromosome 18 Research Center, where she began her research on Sibling relationships. Dr. Larson earned her Medical Degree from the University of Texas School of Medicine at San Antonio. As a practicing psychiatrist, she opened her private practice in Austin, Texas, and she also returned to join the research team at the Chromosome 18 Research Center.   Dr. Catherine Larson is double Board Certified by the American Board of Psychiatry and Neurology in General Psychiatry and Child and Adolescent Psychiatry. Dr. Larson earned her Medical Degree from the University of Texas School of Medicine at San Antonio. In addition to her private practice, she is currently an Adjunct Assistant Professor at The University of Texas Health School of Medicine at San Antonio.  Segment 2 “Families' experiences accessing care after genomic sequencing in the pediatric cancer context: ‘It's just been a big juggle'”   Authors Sarah Scollon and Blake Vuocolo talk about their recent Journal of Genetic Counseling paper in the pediatric cancer space.    Sarah Scollon is an Assistant Professor in the Department of Pediatrics at Baylor College of Medicine and certified genetic counselor for the Texas Children's Hospital Cancer Genetics and Genomics Program. She has served in dual clinical and research roles across the course of her career. Her research interests engage the overarching themes of (1) adaptation of genetic counseling practice to implement evolving technologies (2) optimization of patient-provider communication and education and (3) equity and inclusion for diverse populations in genetic medicine and research. Ms. Scollon has had a longstanding dedication to the care of children and families affected by cancer. She is an advocate for including genetic counseling into pediatric cancer care. She works to educate others on the role genetics and genomics can play in the care of patients with pediatric cancer and their families both from a clinical and psychosocial standpoint.   Blake Vuocolo is a certified research genetic counselor in the Department of Molecular and Human Genetics at Baylor College of Medicine. She graduated from the Baylor College of Medicine Genetic Counseling Program in 2022, and her thesis work focused on access to follow-up hereditary cancer care in underserved pediatric populations through the KidsCanSeq study. Currently, her work focuses on exome and genome sequencing of medically underserved populations with undiagnosed diseases throughout Texas. Ms. Vuocolo is passionate about finding sustainable ways to improving genomic health equity worldwide. Miracle of Help is a 501(c)(3) charitable nonprofit using a community-led development model to help communities mobilize and design solutions that address their overall social and healthcare needs. Sierra Leone has the second highest infant mortality rate in the world. Inspired by firsthand accounts of the poverty and lack of medical services for pregnant women in eastern Sierra Leone, Miracle of Help (MOH) is helping the community build a Maternal Child Health Post (MCHP), for safe childbirth and pre- and post-natal care. The center is located in the village of Ngolahun, a village of slightly over 2,000 people, with no electricity or running water. Four smaller villages within walking distance (total population 2,000) will also receive services from the MCHP. During our monthly pop-up clinics our doctors have diagnosed some form of malnutrition in 80% of the children they see (see photos). As of the end of May 2022, we established two malnutrition clinics for severe and moderate acute malnutrition for children under the age of five. Our staff was trained by clinicians from Project Peanut Butter & Partners in Health, and both clinics have been certified by the Sierra Leone government. Since the inception of the clinics, twenty-five children have been rescued from malnutrition and their mothers given jobs selling soap to support them. Currently, twenty-seven children are enrolled in the two clinics. The mothers learn how to make Bennimix (provided by MOH), a blend of nutritive food substances to feed their children. Miracle of Help's fundraising efforts to establish a safe birthing center / women's health clinic in Sierra Leone were recognized by Ambassador Dr. Francis Kaikai at the Consulate of Sierra Leone in NY city, December 3, 2018. In this episode of the Brawn Body Health and Fitness Podcast - Dan is joined by Dr. Arman Taghizadeh to discuss Mental Health Considerations for Performance, Injury, and Illness in Athletes. Arman Taghizadeh, M.D. also known as “Dr. T”, is a Johns Hopkins trained Board-Certified Adult, Child and Adolescent Psychiatrist also specializing in Sports Psychiatry. He is the founder of Mindset Training Institute® (MTI), host of the “The Mindset Experience®” podcast and a 2023 TEDx speaker. Dr. T grew up in the greater Baltimore area and attended the Gilman School where he was a wrestling All-American. He graduated Magna Cum Laude from James Madison University where he was awarded a varsity letter every year on the NCAA Division I wrestling team. He received his M.D. from the University of Maryland School of Medicine in 2004, as a member of the prestigious Combined Accelerated Program in Psychiatry (CAPP). He completed both his Adult Residency and Child and Adolescent Fellowship at Johns Hopkins Hospital where he was elected Chief Resident from 2008-2009.   Dr. T maintains an independent practice in Baltimore and is a faculty instructor at Johns Hopkins Hospital in the Division of Child and Adolescent Psychiatry. He is the official psychiatric consultant to Gilman School in Baltimore, a member of the #SameHere Psych Alliance and serves on the advisory board of Synergy Dryland.  He is also the medical director for “The Hidden Opponent,” a top mental health non-profit recognized as a standout resource for athletes by Kobe Bryant. Baltimore Magazine has selected him as “Top Doctor” every year since 2016.  The Baltimore Business Journal and Living Classrooms Foundation have recognized him as a “Rising Star.” He is a member of the International Society for Sports Psychiatry, a Distinguished Fellow of the American Academy of Child and Adolescent Psychiatry (AACAP) and a Distinguished Fellow of the American Psychiatric Association (APA). If you find yourself full of excuses not to commit, you're not alone – it's a widespread occurrence. Dr. Mark grew up in the Philadelphia suburbs, received his undergraduate degree from Wake Forest University, and attended both medical school and psychiatry residency at Temple University School of Medicine (now Lewis Katz School of Medicine at Temple University). He served as Chief Resident in his third year of residency at Temple.  He completed Child and Adolescent fellowship at Thomas Jefferson University, where he served as Chief Fellow.   During his fellowship, Dr. Mark was awarded a research grant.  He has several publications in peer-reviewed journals and has presented his research at the annual meetings of both the American Psychiatric Association (APA) and the American Academy of Child and Adolescent Psychiatry (AACAP).  In 2013, his Adherence in Pediatric Psychopharmacology-A Multi-dimensional Perspective and Approach was a docent selection of the American Academy of Child and Adolescent Psychiatry. Dr. Mark has taught hundreds of medical students and residents, and most recently was the lead Child and Adolescent Psychiatrist in a non-profit public outreach campaign to Philadelphia schools through Michael's Giving HAND (Micahel P. Donatucci Foundation, Inc ), teaching school administration, teachers, counselors, and parents about how to identify and address Depression, Anxiety, Substance abuse, and Bullying.   When he's not working, Dr. Mark enjoys spending time with his family, walking around Longwood Gardens, checking out live music, and being outdoors with his Labradoodle.   Prioritize Your Fitness The number one excuse we hear is that people don't have time to exercise. But making the time to take on healthy behaviors can actually make you more productive in all other areas of your life.  It really comes down to having well-defined goals, setting your priorities, and sticking to them. I find this especially helpful if you feel you've failed in the past – you simply didn't have these goals as your priorities then, which is okay. I understand that it can feel overwhelming to try to start a fitness routine when you already have so much going on. Letting the overwhelming feeling win is easy, but by setting out your top three priorities daily, you can start getting a grip on it all.   Put Yourself First As women physicians, we're used to putting everyone else before ourselves. This is counterproductive! By putting yourself first, you fill your cup, giving you so much more energy, space, and opportunity to serve the other people and priorities in your life.  One of the other big excuses for fitness is procrastination. We tend to procrastinate on what isn't a priority for us. What if you did make your health and fitness a priority? Imagine what could change. I want you to invest in yourself. I want you to put yourself first. I want you to prioritize your health, wellness, fitness, and happiness. You are so worth it. Do your decisions align with your top priorities?
