In this episode, I wanted to take a moment to share a poignant story about a patient I'll call James Doe. Over a year ago, James reached out, seeking help for a sharp tooth problem that was cutting his tongue. When I first visited his charming home, I was greeted by his wife, Elaine, and navigated through their cozy residence to find James seated in a recliner. Our initial appointment was filled with heartfelt conversations; James regaled me with stories about his five kids, grandchildren, and his adventurous past, including his service in the military and a long career that took him around the world. After treating his immediate dental issue, I advised James that he needed a comprehensive oral exam to address other dental concerns—advice he surprisingly heeded. Weeks later, I returned for a complete examination and treatment planning session. James and Elaine were faced with tough choices, such as whether to undergo transportation for oral surgery or to manage his condition at home. Elaine's emotional response emphasized the overwhelming nature of caregiving and the importance of support systems. Despite the challenges, James made pragmatic choices typical of many of my patients: opting for SDF placements and focusing on visible restorations to maintain his appearance. Unfortunately, James's health took a downturn, leading to a temporary stay at a care facility where dental hygiene was not prioritized. This exacerbated his dental issues, and upon his return home, we had to adjust our treatment plans. The story reached a touching and insightful conclusion during our last session. James, now bedridden and beset by rheumatoid arthritis, chose comfort over aesthetics for his dental care. He shared life lessons with me, reflecting on the time he wished he could reclaim to spend with his family. This sentiment resonated deeply, reinforcing my own philosophy of valuing time and cherishing moments with loved ones. House-call dentistry, as James' story illustrates, is not just about medical procedures—it's about connecting with patients on a personal level and making a profound impact on their lives. This fulfilling practice model is a unique and rewarding career choice that stands apart from traditional chair-side dentistry. For a big part of my life and even to this day, I have been a yes person; always willing to help people in whatever capacity they need me. However the energy behind my actions today is much more centered and genuine than it was before. Before it was more about pleasing others and wanting the world to have a favorable view of me. Now it's all about who I truly am. I had to reflect on myself in order to realign my beliefs and let go of some commitments that I had unconsciously made about why I was helping others. In short, I had to own myself and my actions in order to find purpose, meaning, balance and fulfillment. Today's walk is about owning who you are, and my walking partner understands this all too well. Liz Brunner has committed herself to reinvention and to something I call a re-deepening process allowing her to move closer to the center of who she is. I still have that commitment to service. It truly is a part of who I am, but the energy behind it now is so much more genuine. It's congruent and it's centered because the way that I serve is much more aligned with who I really am and not based on the expectations of others or my need of their approval.” – Luke (01:56) “A lot of people think vulnerability and authenticity is ‘oh you just wear your heart on your sleeve, and you share everything with everybody.' No! You share your stories with people who have earned the right to hear them. That's vulnerability. Authenticity is there is never a question about your actions and your behavior. They are in sync. That's being authentic. And I think it's really important that people, particularly when you are in this position of saying, maybe I should do something else, what am I going to do, how do I figure it out; you do have to be that authentic and that vulnerable with the people who have earned the right to hear your stories and people you trust. A Mosaic of CourageI remember September 11, 2001, like it was yesterday. I remember exactly where I was when I turned on the television to watch the morning news anchors on ABC7 in Manhattan. They were telling us that we were indeed under a terrorist attack and that we should get out of Manhattan if we could. Things were happening so rapidly that even as the news anchors issued those dire words, their instructions became obsolete as all traffic in and out of The City came to a grinding halt. Not knowing what to do, I joined the thousands of workers being evacuated from high rise office buildings all over the city and got swept up into the teeming masses of people flooding the streets of Midtown, Uptown and Downtown. The East Side and the West Side. From Battery Park to Washington Heights. My friend Cheryl Rogers later told me that she had walked from her office in Chelsea all the way to the Brooklyn Bridge and crossed over on foot to get home to her cat Cookie. In the days that followed, we New Yorkers, like the rest of the world remained inside, glued to the television. I watched the local News One a lot those first few days, and ABC7, because they were giving us the local, inside scoop. And we needed practical information – like where to go to get food and water. And where people could go to get information on the missing and the dead. On that day, Liz Brunner sat at the anchor desk at ABC-TV, WCVB NewsCenter 5 in Boston, MA - that city's top-rated newscast. Liz was the voice of reason, calm and a face that Bostonians all knew they could trust to tell them what was happening. The facts. 12 years later, on April 15, 2013, she would assume the seat at the anchor desk again, as this time terror visited the city of Boston directly with the Boston Marathon Bombing, and the chase for the bombers that ensued in the following days. When she talks about both events, her journalistic resolve melts as her eyes soften. The humanity of the person shines through. It was that tangible humanity that drew viewers to her during her 28-year career in front of the camera. Specifically for men, masturbating 1-3 times a week (especially if not having sex to completion with a partner) has been shown to improve erectile function, maintain healthy sperm production, and it may even help lower the risks of prostate cancer. Can it be harmful? Just like anything, if it is done to excess, of course it can be harmful to our well being. For example: * If you are a man and have too strong of a grip while masturbating, it can decrease sensation sensitivity, especially with a partner. So it is recommended to adjust your grip when you spend some time alone, to allow your member to feel more sensations. * You could also cause your penis to swell if you whack it too often, so if that starts happening you might want to give it a bit of a break. * If you use toys and don't clean them properly you could earn yourself a lovely bacterial infection, and same goes with hands! So if you have a vulva, make sure that anything you plan to stick or allow in to your vagina has recently been sanitized, with a high quality toy cleaner, or a plain old thorough washing with soap. * Masturbation addiction is real and can create a lasting harm. Chasing that dopamine and endorphin high has led to people losing their jobs, negatively affecting their relationships (both intimate & platonic), and dropping the ball on life's assorted responsibilities. If you have found yourself canceling plans to stay home and have a wank, if you find yourself unable to refrain from rubbing one out in a public or generally frowned upon environment, if you aren't able to enjoy sexual intimacy with your partner because you're comparing it to your solo flights, you may be addicted to masturbating. If I'm in a relationship, is it cheating on my partner? Of course it depends on your relationship agreements, some consider it cheating for their partners to experience sexual pleasure with anyone other than themselves. However, we are of the mind that that is generally shortsighted, selfish, and comes from a place of deep insecurity.  We are all born with our own sex life, dormant in our genes and ready to bloom in adolescence.  We would've had our own sexual identity regardless of the whether we met our current partner.  I'm gonna say it as bluntly as I can:  They Don't Own You.  If you have an agreement to exclusively enjoy partnered sex with them it's fine, but your body is still your own.  You decide whether restricting your self-pleasure is part of your relationship agreement, but I can't think of any logical reason why it would be considered cheating. However, there are times when you might already have an agreement in place with your partner, especially if you are attempting to impregnate your female wife. If you are both wanting to make a baby and you spend your precious semen into a hankie instead of her fertile womb, she very well could have a valid reason to be upset. Or if you made plans to enjoy some intimate time together but you forgot and wanked it just a few minutes earlier, that will definitely cause some disappointment. As always, it is crucial to maintain open & consistent communication with your lover in order to avoid (as much as possible) roadblocks and upsets such as these. 4. How do I talk to my partner about it? * Carefully.  There's a lot of reasons you may hesitate, there's all kind of risks: fear of judgement or conflict, lack of a sex-positive understanding, indoctrinated shame, religious reasons, embarrassment, believing myths about self-pleasure… it can be intimidating. Show empathy & respect. * Casually.  It's such a charged topic, better not to be too dramatic about it. When is it not a good idea? * This should be pretty simple, but any place or time where you could be causing harm to yourself or others.  * Masturbation is a private act and the only time others should see you do it is if you both (or all) consent to it. * If your responsibilities, social life, or general life balance starts to suffer, you may need to ease back, and/or seek professional help. There is no shame for needing help, we all need a helping hand sometimes.  6. What does “God” say about it? * As you probably know, most of the worlds religions either outright prohibit male masturbation, or strongly discourage. However, the roots of those prohibitions are all about “wasting” semen because the primary directive is to procreate (farms take a lot of hands to work and the infant mortality rate used to be brutally high).
