How do you know if all the changes you're making to manage your PCOS are actually working? How can you tell if your PCOS is actually improving? I got that question from a member of The Hormone Dietitian Community recently, and I thought you might want to hear my answer as well, so I dedicated this episode to answering how to know if PCOS symptoms are improving! PCOS is complex, and the symptoms can affect nearly every system in the body. And every case of PCOS is unique — different symptom presentation, different root causes driving those symptoms, different severities of root causes. Not to mention your unique needs, goals, preferences, lifestyle, time and budget to spend implementing changes, access to care, other medical conditions and medications, and more. It may seem like a lot, but the "perfect" PCOS plan for YOU has to be personalized. In other words, you can spend a lot of time trying different dietary approaches, different exercise regimens and routines, making sure you're getting enough sleep, finding a stress management approach that works for you, etc... but which changes are going to make the biggest impact on YOUR PCOS symptoms? And that's not even mentioning supplements... there are so many that can potentially be helpful in PCOS, but which is right for your situation? It's easy to waste a lot of time and money on approaches that don't work for you, so knowing how to evaluate if your PCOS plan is working or not — and relatively quickly — is important! In this week's solo episode, I'm walking you through how to know if your PCOS plan is working for YOU — in other words, how to know if PCOS symptoms are improving, and what timeline you can expect to see improvements happen on. This Week I Talk About: Common PCOS symptoms and the 4 root causes of ALL PCOS symptoms Why I don't consider diet and lifestyle to be root causes of PCOS My 4-pronged treatment approach for PCOS Why the order of my 6-step process impacts time to specific PCOS symptom resolution Which symptoms I typically see clear up first in my PCOS patients and students A realistic timeline for PCOS symptom improvement, and which symptoms take the longest to improve I also take a deeper look at 2 symptoms in particular — weight and infertility — because these are the two most common symptoms my clients and students want to improve What your next step should be in getting relief from PCOS symptoms Mentioned in this episode The PCOS Root Cause Roadmap – If you've been struggling with alll of the symptoms of PCOS in this episode and don't know where to start, The PCOS Root Cause Roadmap is my proven and proprietary 6-step signature method to PCOS success. I can help you identify and address the root causes of YOUR PCOS symptoms so that you can feel confident you're making the right nutrition, lifestyle, and supplement changes for your situation. The PCOS Root Cause Roadmap is based on the exact same methods and tools I use with my clients to get periods back, confirm ovulation, increase energy, decrease cravings, improve skin and hair, lose weight while eating MORE food, and get pregnant naturally. It contains everything you need for support including dietitian-designed suggested weekly meal plans and hundreds of PCOS-friendly recipes, 6 exclusive PCOS yoga videos developed to complement the modules of this program, and a private support community where you can join the over 700 women who've gone through the course. And you get lifetime access, so there's no rush in working through the program and you can always go back if things change. The PCOS Root Cause Roadmap is only open for enrollment 2x a year and the next round starts October 17, 2022! There will be an option to upgrade to VIP and add weekly group coaching and lab testing, but those spots will be extremely limited. Welcome back to The Down to Earth PCOS Nutrition Podcast! On today's episode I'm answering a question I hear commonly from women with PCOS: “Why am I always hungry?” More specifically, women with PCOS tend to struggle with: Not staying full after meals Constantly feeling the urge to snack Experiencing intense sugar and carb cravings Thinking about food often and struggling to feel satiety Hunger, cravings and increased appetite may seem related to willpower or self-control but in fact, there is a strong hormonal component behind these food-related sensations. What You'll Learn on this Podcast Episode: Why women with PCOS experience uncontrollable hunger. How Leptin, Cortisol and Insulin affect appetite and intense cravings. Why stronger willpower and self control are not going to help in most cases Some best practices to consider if you are constantly hungry. Many people with PCOS want to lose weight. And while it is absolutely your right to be comfortable in your own body, the misinformation about how losing weight with PCOS is the answer to all of your problems is rampant. Restrictive diets do more harm than good and research shows that weight cycling (aka, yo-yo dieting) is more harmful to health over the long run than never having gone on a diet at all. This week we're diving into why you might want to reconsider pursuing intentional weight loss with PCOS and what to focus on instead. So I brought in my friend and colleague Sam Abbott, who specializes in just this area. Meet Sam Abbott, a registered dietitian nutritionist who helps those with PCOS ditch diets, improve insulin resistance, and balance hormones without feeling guilty or stressed about food. She is passionate about empowering people with PCOS to find peace and balance with nutrition, hormones, body, and life. Sam is also the creator of the PCOS Food Freedom Program – a complete program for improving hormone balance without a side of diet culture or weight stigma. In this episode, I sit down with Sam to talk about intuitive eating, health at every size, and why weight cycling is dangerous. In this episode, we talk about Why Sam is passionate about working with people with PCOS Why it's important for nutrition practices to be accessible Why and how Sam uses intuitive eating and gentle nutrition principles in her program Why dieting and weight cycling can be dangerous, especially for people with PCOS What health at every size means to Sam and her practice Mentioned in this episode The PCOS Root Cause Roadmap - For the first time EVER I am hosting The PCOS Root Cause Roadmap LIVE! Usually, this program is a solo mission, but starting February 1st I'll be there to guide you through this journey. Together we'll be replacing failed diet plans with tried and true nutrition methods and strategies for people with PCOS. See you there! This week we're diving into a common problem for people with PCOS - weight loss. Many people struggle to lose weight without making their PCOS symptoms worse, and it's one of the most common reasons people want to work with a PCOS dietitian. With PCOS, it's not as easy as calories in/calories out or hopping on the latest diet trend. There are many factors that affect the ability to lose weight with PCOS and each patient has their own set of needs and goals, so it's impossible to offer blanket advice for how to lose weight with PCOS. And, depending on who you follow, you may even wonder if you're wrong to even WANT to lose weight. That's why I wanted to bring in a guest expert who has a whole program built around how EXACTLY to lose weight with PCOS. Meet Cory Ruth, a Registered Dietitian, Nutritionist, and women's health expert. Cory is the founder and principal of The Women's Dietitian and Instagram account @thewomensdietitian, a private practice and digital platform for women seeking nutrition support for hormone balance, PCOS, fertility, and weight management. She specializes in PCOS and nutrition therapy for infertility and assisted reproductive technology. In this episode, we talk about how women with PCOS can ACTUALLY lose weight — without making their PCOS symptoms worse. In this episode, we talk about Cory's experiences as a dietitian with PCOS WHY it's so hard for people with PCOS to lose weight Why keto diets and intermittent fasting don't work for people with PCOS How to have realistic expectations around weight as a woman with PCOS Which practical and sustainable lifestyle changes you can make to lose weight and improve your health Where to find Cory Ruth, MS, RDN: The Women's Dietitian Website: www.thewomensdietitian.com Instagram: @thewomensdietitian Facebook: The Women's Dietitian LinkedIn: The Women's Dietitian

Afterwards, they dig into the research and provide practical tips for how to approach your diet without needlessly restricting any particular macronutrient. Roses and Thorns: Sam's COVID lockdown fatigue, fatphobic phrases & gratitude for the complex crux of life Dylan's infinite struggle with consistent meditation & the frustration that comes with slow progress The Meat & Potatoes: Past Sam's search for the magic, secret diet hack: how she got into high carb no fat vegan, vegan keto, & raw vegan The need to "cut something out" in order to lose weight Past Dylan's search for the magic, secret diet hack: how he got into low carb, high carb, metabolic typing & organic everything Why they avoid nutrition advice from charlatans & diet gurus Why you shouldn't take nutrition advice from Gwyneth Paltrow The power of nebulous terms & false diagnoses in marketing Research Review of Kevin Hall's "Obesity Energetics: Bodyweight Regulation and the Effects of Diet Composition" A meta-analysis of 32 controlled feeding (metabolic ward) studies of 534 diverse participants Calories & protein were matched between all subjects On average, the Higher Carb Lower Fat diets were shown to be better Why this result is statistically significant, but practically meaningless Fat Oxidation: why this term is extremely misunderstood & totally misused in marketing The power of mechanistic overreaching: EPOC/"afterburn effect", intermittent fasting, insulin The power of understanding energy balance, thermogenesis Why getting attached to a specific nutritional or dietary ideology leads us astray from biology & science How should you be eating? Sam's experience with a low/no fat diet: hormones, low satiety Sam's experience with a low carb diet: low energy for training, PCOS Why we don't seem to do well with extremes How to factor in satiety Meeting your fat intake minimum (1g per kg of BW) Meeting your protein intake minimum (1g per lb of BW) Why partitioning/budgeting your macros properly matters most during a fat-loss phase Research will never tell us what *everyone* should do The importance of trial and error Fat-loss never comes down to cutting out a macronutrient The intersection of toxic diet culture Why we all deserve the ability to feed ourselves with ease & confidence How it always comes back to no extremes, just the unsexy combination of balance & moderation The power of habit Why we all need a manageable level of cognitive restraint If weight loss were easy - our species wouldn't exist Looking for coaching? “You can eat perfectly, and if you don't get those circadian rhythms in line, that leptin, ghrelin, that hunger-full satiety center will disregulate. If it does and you develop leptin resistance where the central nervous system and leptin don't talk, no matter what you try to do, your system will be dysregulated, your weight will be a problem, your hormones will be a problem, your fertility will be an issue.” – Dr. Dian Ginsberg There is a reason you have PCOS. Our ancestors developed it as a survival mechanism. Now those mechanisms are at odds with modern lifestyle conditions. Dr. Dian Ginsberg is a specialist interested in helping women with PCOS get to the root of our symptoms and naturally bringing our bodies into balance and health. As she says, PCOS doesn’t define you. “It's just something that's developed. And by unwrapping what's happened, by following some of these tips, you can reverse what you have.” Listen in (or read the transcript) as we discuss: - Getting to the root of your PCOS - Why the birth control pill, Metformin and spironolactone aren't really fixing PCOS - What we can control to help our genes work better - Light and circadian rhythm - Insulin resistance vs. gut issues
On today's episode, I focus on clarifying two important aspects of healthy eating I often receive numerous questions about: meal timing and snacking. When it comes to eating, timing does matter. Studies have shown that rather than eating bigger meals later in the day, as most families in the U.S. do, it's actually healthier to frontload your calories and eat larger meals earlier. Eating your biggest meal later in the day isn't conducive to healthy eating because these calories often get converted into fat and are stored, leading to an increased risk of high cholesterol, diabetes, and other metabolic problems, especially for women with PCOS. Therefore, I recommend eating a lighter meal at night, something that I know isn't conducive for every lifestyle. Because of this, I encourage listeners to flip-flop lunch and dinner 2-3 times a week in order to begin practicing better meal timing in an easier way. The second half of today's episode then focuses on snacking. Eating 1-2 snacks a day can help with appetite control and can prevent against overeating. However, if you are someone who uses snacking to help manage hunger, I recommend being strategic about your snacking in 3 specific ways. These include: Making sure your snacks are pre-portioned Making sure your snacks are pre-planned so that you know what time of day you're going to be eating which snack Making sure your snack always contains a protein By following these guidelines, you'll be able to begin building better eating habits concerning meal timing and snacking, just in time for the new year. Begin 2020 by getting healthier and healing your body, a journey I hope to continue with you throughout the year. So please, make sure to subscribe and leave a review, and let's start this new decade off together! What You'll Learn in This Podcast Episode: Meal suggestions for smaller dinners, bigger breakfasts, and healthy snacking How managing meal timing contributes to healthier eating habits The importance of managing insulin for women with PCOS Why eating three healthy meals a day is better than eating several small meals a day Helpful Links: Questions, or interested in becoming a client? Eating your biggest meal later in the day isn’t conducive to healthy eating because these calories often get converted into fat and are stored, leading to an increased risk of high cholesterol, diabetes, and other metabolic problems, especially for women with PCOS. Therefore, I recommend eating a lighter meal at night, something that I know isn’t conducive for every lifestyle. Because of this, I encourage listeners to flip-flop lunch and dinner 2-3 times a week in order to begin practicing better meal timing in an easier way. The second half of today’s episode then focuses on snacking. Eating 1-2 snacks a day can help with appetite control and can prevent against overeating. However, if you are someone who uses snacking to help manage hunger, I recommend being strategic about your snacking in 3 specific ways. These include: Making sure your snacks are pre-portioned Making sure your snacks are pre-planned so that you know what time of day you’re going to be eating which snack Making sure your snack always contains a protein By following these guidelines, you’ll be able to begin building better eating habits concerning meal timing and snacking, just in time for the new year. Begin 2020 by getting healthier and healing your body, a journey I hope to continue with you throughout the year. So please, make sure to subscribe and leave a review, and let’s start this new decade off together! What You’ll Learn in This Podcast Episode: Meal suggestions for smaller dinners, bigger breakfasts, and healthy snacking How managing meal timing contributes to healthier eating habits The importance of managing insulin for women with PCOS Why eating three healthy meals a day is better than eating several small meals a day Helpful Links: Questions, or interested in becoming a client? Many of us think that once puberty passes, acne will be a thing of the past. Unfortunately, acne does not discriminate, and it can affect people of all ages. Today, I continue my conversation with Brie Wieselman about how PCOS can cause adult acne in women. This is the second part of my interview with Brie. You'll want to check out Part 1 if you haven't already before listening this episode. Brie Wieselman is a digestive health and hormone specialist who works with women all over the world through her online practice. She has helped thousands of people understand their health so they are empowered to live better. Polycystic ovary syndrome (PCOS) is an often misunderstood cause of adult acne in women. Last time, Brie and I discussed PCOS, its symptoms and how to tell if you have it. Today, we're diving in deeper to talk about the best ways to manage PCOS using a whole range of tools and strategies. Brie shares her advice on how to manage this condition so you can have healthy skin and feel better all around. Have you used any of Brie's strategies to manage PCOS? Leave a comment! In this episode What to do once you receive a diagnosis of PCOS The importance of monitoring your blood sugar in order to understand your hormonal health Why managing your diet and activity levels is integral to managing your hormonal health Some of the best therapeutic nutrients to consider to manage insulin sensitivity and PCOS including inositol, D-Pinitol, curcumin, chromium, and, in certain cases, berberine The most important food group to avoid if you have PCOS Why a healthy liver and microbiome is crucially important to healthy hormonal health Quotes “You're definitely going to be better off doing more balanced meals where you're focusing on getting a whole bunch of vegetables, ideally two cups of vegetables per meal and your plate is mostly vegetables and then maybe a palm-sized serving of protein of any kind and a tablespoon or two of really healthy fat.” [25:06] “When a woman walks into my practice and has PCOS I want to get her hormones in balance and cycling and regulated as quickly as possible. Now that sounds very straightforward and easy. Just get the hormones to have a good feedback loop and cycle. But the thing is that this is a whole body multi-system issue, so that's highly dependent on a couple of other really important body systems, namely our liver and detox system.” [37:19] Links Find Brie Wieselman online Take Brie's Healthy Hormones Quiz here! Are you worried about getting breast cancer? If you are looking for more information about breast cancer prevention and support during recovery, then you’ve come to the right podcast. I interview Carol Lourie who talks not only about breast cancer prevention and recovery but also about all things PCOS, blood sugar balancing, and emotional wellbeing. Visit https://holisticwellness.ca/podcast for the complete show notes of every podcast episode. Topics Discussed in this Episode: The link between hormonal conditions or imbalances Why PCOS often goes undiagnosed The top-down vs the bottom-up approach to PCOS How to get a Hashimoto’s diagnosis if you have it Some of the important testings involved in PCOS Why blood sugar management is important for our overall hormonal health What are some indications of having blood sugar problems Why you need to detoxify your body and your home How acupuncture can support hormonal health The Eclectic Triphasic Medical System (ETMS) What are the essential things to eat and things to avoid for breast cancer prevention and/or recovery The steps to take to dive deeper into our emotional wellbeing Key Takeaways: If you don’t handle the emotions that come with chronic disease, you don’t quite recover your health. PCOS is a syndrome, which means there are multiple different contributing factors to its symptoms. Having PCOS doesn’t necessarily mean that you are going to have fertility problems or failed multiple miscarriages, but it can often lead to that if the underlying metabolic dysregulation is not properly dealt with. And you don’t have to have cysts to have PCOS. Your hormonal health is part of your overall health and it’s a reflection of how well you take care of yourself on a daily basis. Diabetes has been known to be called the ‘silent killer’. That is because severe symptoms of blood sugar are often ‘asymptomatic’. The healthier your body is when you go into IVF, the better the results will be. In Chinese medicine, if your organs and your chi or the freeways of your energy are imbalanced, then your hormones will be imbalanced. If you have very large, dense breasts, even if you are doing regular self-breast exams, it can be very difficult to pick up tumors. Action Steps: If you have thyroid issues, switch to a gluten-free low inflammatory diet to help recover your thyroid health. Make sure that you have certain nutrients in your diet or take supplements to really support thyroid health. If you’re looking to get an IVF, work with someone for three months prior to get your body in better shape and have a more positive response to IVF. If you’re going to do chemo and radiation, make sure that it’s the right chemo for you, that your healthy cells are being protected, and that the side effects are lessened. If you have a family history of breast cancer, find an oncologist or a breast specialist who really understands that and who can put you into a diagnostic mammogram category with maybe a breast MRI every eight months or once a year. Once you get a diagnosis, it’s always best to go and get a second opinion. Don’t just take the first thing that is said to you but do a little bit of research. For breast cancer care/prevention/recovery, avoid saturated fats, fried food, fast food, sugary drinks, standard meat, sugary over-the-counter boxed cereals, and alcohol. Carol said: “Unfortunately, in our society, we’re not taught about how to stay balanced as we’re little… This should be something that’s part of standard education -- how to have a balanced life and how to stay in touch with our inner being and how to take care of ourselves.” “It takes a lot of support to get through a health crisis, whether it’s PCOS, infertility, failed IVFs, multiple IVFs or breast cancer... Find a woman or a doctor who really will support your highest self and not diminish or dismiss your concerns. And don’t give up." Thanks for listening! The birth control pill is the standard prescription when a woman is diagnosed with Polycystic Ovary Syndrome, typically without outlining the risks posed specifically for us. For many, it manages to mask PCOS symptoms. For others, the side effects are immediately intolerable. For all, symptoms resume when we must stop taking it. Worse, many struggle with Post Birth Control Pill Syndrome. So, what can we do? Dr. Jolene Brighten and I discuss this and much more in this podcast. Listen in or read the transcript. - What is Post Pill Syndrome and what we can do about it? - Testing to have done while on the pill - Often undisclosed risks of the pill including: thyroid issues, adrenals, inflammation, autoimmune diseases, leaky gut, food sensitivities, anxiety, blood clots, yeast infections, and heightened risk of suicide - Contraceptive alternatives for women with PCOS - Why an orgasm a week is good for your health

Listen as we also discuss: - Critical importance of sleep quality - Tips on how to effectively advocate for yourself or your daughter - Pros and cons of diagnosing adolescents - What she says to adolescents who may have PCOS - Why related diagnoses (thyroid, sleep apnea, etc.) are often missed in women with PCOS - The counsel she offers her own patients to improve their quality of life
