Thursday, June 9, marks #NASHDay 2022, the 5th annual celebration of International NASH Day by the world's largest liver patient advocacy organization, the Global Liver Institute. With over 200 affiliated organizations and sponsors, many holding their own events, International NASH Day is not only a major day for education and communication, but also in a sense NASH's "New Year's Day," a moment to take stock of the last year and make resolutions for the future.To celebrate the occasion, Stephen Harrison joined Jörn Schattenberg, Louise Campbell and Roger Green to celebrate the event and comment on one crucial trend or event most signified what the last year has meant in the Fatty Liver community.Stephen went first, focusing on a pivotal change in Phase 3 trial design. Unti now, corporate sponsors have conducted one large Phase 3 trial with F2 and F3 NASH patients. The initial phase of the trial would include 1,000 - 1,200 patients with the goal of achieving sub-part H approval based on conditional endpoints. This trial would keep enrolling patients until there were enough patients in the trial for 4-7 years to prove an outcome-based endpoint and achieve full approval. In the new design, two trials transpire in parallel: the original trial with F2 and F3 patients and a trial with well-compensated cirrhotic patients focused solely on achieving an outcome. This design should prove easier to recruit, less expensive, and more likely to achieve outcome-based endpoints more quickly. Also, it allows the sponsor to evaluate "co-endpoints," as Stephen puts it, instead of needing to achieve dual endpoints. This way, the sponsor can succeed by achieving fibrosis improvement with only a stable NAS score or a decline in NAS score with no worsening of cirrhosis. All three panelists expressed admiration and appreciation for the benefits of this approach. Stephen noted how many people needed to collaborate for this to come to fruition and how much more collaboration is taking place in the community.Jörn went next, focusing on the extraordinary breadth in focus and content of pivotal conferences during the year. He mentioned the fireside chat with regulators at NASH-TAG and the broad discussions in Barcelona exploring how best to serve patients after we have a drug. Louise focused on systems thinking, mostly related to diagnostics, focused on how best to use non-invasive methods to screen, diagnose and stage patients. What she found striking is that these discussions are taking place before drugs are approved, which suggests the possibility that shorter after we have drugs to prescribe, we will have systems and protocols to get the drugs to the patients that need them most. Louise contrasted thought development in NASH to Hep C, where the drugs came along before we knew how to find the patients. Rounding out the group, Roger talks about the Law of Unintended Consequences and the Intercept Complete Response Letter from FDA. He recalled the initial thoughts that the failure to approve obeticholic acid would signal a perilous downturn in interest in Fatty Liver disease. Instead, he suggested, the passion of the community coupled with lack of funds led stakeholders to ask better questions and focus on more practical issues, which has led to the convergence we are seeing today.As the conversation wound down, Stephen and Jörn noted that providers will not tolerate "No" for an answer, recalling patients who suffered from HCC and died too young. In the end, Roger asked the group to make a bold predictions about what will happen next year, when the group gets together on the eve of International NASH Day once again to survey the past year. Unti Vineyards is a small, family-owned and operated winery specializing in Mediterranean style wines with vineyard personality. The winery, established in 1997, produces small lots of estate grown Barbera, Grenache, Montepulciano, Sangiovese, Syrah and Zinfandel. We also produce a dry Rose from Grenache and Mourvedre and a handful of white wines: Vermentino, Fiano, and Grenache Blanc. The winemaking involves using traditional methods which enhance and support an ability to make wines with a sense of place. We use artisan winemaking methods to enhance and support our ability to make wines with a sense of place, such as fermenting and aging 45 to 60 different small batches of wine each year, using whole clusters in our Rhone varietal ferments, using a variety of fermenting and aging vessels including stainless steel, concrete tanks, large French oak ovals and small French oak barrels. We have 60 acres of vineyards here in Dry Creek Valley, which is the sole source for all our wines. We approach our vineyards and winemaking as wine enthusiasts, particularly the wines from Italy, Southern France and Spain. California, and specifically Dry Creek Valley has a similar climate the above regions. This explains for why we have chosen to cultivate noble wine grapes, native to these Mediterranean regions contrary to many Dry Creek Wineries. One man's crazy is another one's sanity.

I am reminded of Saint Augustine's report of an encounter between Alexander the Great and a rank Mediterranean pirate in which Alexander the Great asked the pirate why he terrorized the seas in pursuit of private gain, only for the pirate to turn the question around asking Alexander the Great why he terrorized the Mediterranean in pursuit of his own. When we are unable to discern between a single institution's naked assertion of power and its legitimate exercise of authority, we find ourselves in the midst of a legitimation crisis. Welcome to episode 64 of Activist #MMT. Today I talk with third-year MMT activist Hannah Judson. Hannah was introduced to MMT at a local community college in 2017. Her professor was BJ Unti who introduced both mainstream and MMT concepts and let the class decide for themselves which theory was more convincing. She later discovered that Unti was a student of Stephanie Kelton. Hannah received a decidedly non-MMT undergraduate degree in business from DuPaul University in Chicago. In September 2019, she impulsively flew out to Long Island, New York in order to attend the MMT conference at Stonybrook University. Despite knowing no one, she ended up being selected as a last-minute replacement to moderate a conference panel. She and her husband now share an apartment with MMTer Nathan Tankus in Queens, New York, and she has just begun an MMT-informed sociology PhD program at Stonybrook. Her hope is to further expand the interdisciplinary reach of the MMT project, which currently centers around law. Her primary interest, however, is the racial-wealth divide in the United States, and more broadly, stratification and inequality. (Stratification being the decisions and actions that cause and result in inquality.) Hannah describes how she got here from there, through a Zoom wedding in her home state of Washington, Zoom church in Chicago, and 11,000 miles of driving from Washington to New York to Washington to New York… in order to attend a Zoom PhD. In part two we drastically switch subjects to mental illness and anxiety, and how they are seen through and informed by MMT. Hannah and I both endured traumatic experiences in our childhoods which will remain with us for the rest of our lives. She discusses how she came to terms with this, how she manages it today, and how her Christianity influences her anxieties as well as her politics. Como las crisis o los temblores, se tratan de lapsus en los que aquello que parecía previsible se desarma. Pero, ¿Podemos pensar el deseo desde otro lugar? Ciertamente es en este elemento en donde se libra una batalla entre la libertad y el poder por ver quién termina construyendo a las personas. ¿Deseamos libremente a partir de los afectos en los que estamos inmersos? O ¿Deseamos aquello que nos dicen que nos falta. Que aún no tenemos? En este viaje por la fascinación, paramos a leer a Deleuze y su propuesta de una noción del deseo que desborda por todas partes.

Nuestra octava parada es la fascinación que nos produce el poder de esta frase, la aparición de la mujer en la filosofía y la ruptura de un equilibrio hecho a medida solo de los hombres.

Hubo un momento en el que la filosofía, que venía de reflexiones universales y abstractas, comenzó a mirar la historia. Empujada por la sospecha de que las cosas son lo que son por consecuencia y causa de un devenir, los filósofos se vistieron cada vez más seguidos de historiadores. En este camino, la figura de J.J. Rousseau se presenta como la más fascinante de todas. Mucho de lo cual tiene que ver con la sospecha de que la desigualdad no era natural. ¿Por qué unos tienen tanto y otros tan poco? ¿Cómo es el mundo en el que nacemos? ¿Qué significa reconocernos como supernumerarios? Desde la frontera de la modernidad, la filosofía de Descartes fue quizás la que más valoró este hermoso y molesto instrumento del pensamiento. Nuestro constante caminar por el mundo nos enfrenta a situaciones, personas y cosas que no conocíamos y que reclaman un lugar en el espacio que tenemos dedicado al conocimiento. Pero esto sólo puede ocurrir a costa de una operación fundamental, es decir, ponerle un nombre. ¿Qué significa ponerle nombre a algo? ¿Cuál es la diferencia entre nombrar y señalar? Hay algo que no pueda ser nombrado? Todas estas preguntas tienen un respuesta en la hermosas palabras de Nicolás de Cusa que nos invita a elevarnos desde nuestras palabras al problema del acto y la potencia. Nuestra cuarta parada es la fascinación que nos produce una metafísica de las posibilidades.
Un segundo semestre dedicado a la fascinación. Capítulo 3: ¿Iguales o diferentes? Desde lo profundo de nuestra historia, la pregunta sobre lo común o lo diferenciable nos acecha como una fantasma insoportable. Nos vemos parecidos, nos creemos diferentes, nos sabemos iguales, pero no dejamos de preguntarnos ¿Qué es el ser? ¿Qué significa ser persona? ¿Es la diferencia siempre un reflejo de la jerarquía entre los seres? Desde el borde de la Edad Media, Duns Escotto nos propone pensar el ser desde la Haecceitas, desde lo que hace de cada ser único e irrepetible. Nuestra tercera parada, entonces, es una estación ontológica, donde buscaremos en la univocidad encontrar como podemos ser iguales en la diferencia.

2 Corinthians 7:5-13 5 For when we came into Macedonia, we had no rest, but we were harassed at every turn—conflicts on the outside, fears within. 6 But God, who comforts the downcast, comforted us by the coming of Titus, 7 and not only by his coming but also by the comfort you had given him.
