Welcome to the third installment of the Updates in Music Audiology workshop recap from the NHCA 2025 conference. This week we will help Sensaphonics celebrate their 40 year anniversary by featuring the voices of founder Michael Santucci and hearing clinic director Shannon Switzer. Together, they shared a brief history of in ear monitors, common fit and use issues, and recommendations for their safe and effective use. Their presentations have been edited slightly for the audio-only format. Michael Santucci is a Doctor of Audiology and president of Sensaphonics, Inc., Chicago, IL. In 1985 he founded Sensaphonics which designs, develops and manufactures custom in‐ear products for the music industry as well as a variety of other hearing conservation applications. For over 40 years he has provided Audiological consultation and customized hearing protection strategies to both Chicago area musicians and many of today's most prolific music performers from all genres of music. Today, Dr. Santucci continues his hearing conservation efforts, spearheading the development of innovative new products and technologies that have 3 patents for the 3DME IEM system and the dB Check Pro sound level analyzer. Both products are NAMM tech award finalists. Dr. Santucci is the Chair of the Music Induced Hearing Loss Task Force for the National Hearing Conservation Association. Dr. Santucci is also a member of the Audio Engineering Society (AES), where he is Chair of the Technical Committee on Hearing and Hearing Loss Prevention. In 2015 through this year, Dr. Santucci was invited by the World Health Organization to participate as an expert consultant as part of the WHO's Listen Safe Initiative in Geneva SZ and is a core member and coordinator for MLS. Sensaphonics is the first winner of the Safe in Sound award presented by the National Institute of Occupational Safety and Health (NIOSH) and the NHCA for achievements in technology, was the first winner of the prestigious Larry Mauldin award for pioneering new ideas for hearing loss prevention and was twice presented the Governor's Award and once presented a Fellowship Award from the Audio Engineering Association and has been awarded Honors of the Academy with the 2023 Samuel Lybarger award from the American Academy of Audiology. Shannon Switzer, Au.D. is the Director of the Sensaphonics Musicians' Hearing Clinic in Chicago and is a featured audiologist in the MusiCares x Tuned teleaudiology partnership. As a lifelong clarinetist herself, she became fascinated with hearing conservation for musicians while earning her Doctorate of Audiology at Northwestern University. She primarily provides direct patient care to over 1000 music industry professionals annually through a variety of channels; the Sensaphonics clinic, mobile visits at music venues in Chicago, onsite clinical residencies with orchestras around the country, and teleaudiology visits with musicians around the world. Dr. Switzer is passionate about promoting hearing health through public education. She lectures at conferences, gives interviews to various music media, and trains audiologists in music audiology best practices. She has recently joined the WHO's Make Listening Safe committee for safe listening in music venues and organizes high-profile musicians to promote World Hearing Day on their social media. She is an avid concert-goer and can't help but chat about hearing protection in the crowd, even when she's “off the clock”.

State Representative Nick Santucci is serving his first term in the Ohio House of Representatives. He also serves as the Senior Consultant for Workforce and Community Engagement at VAZA Consulting. Prior to joining VAZA Consulting, Santucci was the Director of Government Affairs and Workforce Development at the Educational Service Center of Eastern Ohio. He also served as Manager and later Director of Education and Workforce Development at the Youngstown/Warren Regional Chamber. He then moved into the economic development department and was promoted to Director of Government and Public Affairs. Karen Santucci is a veteran court reporter who is now the Director of the Court Reporting Program at Plaza College in Forest Hills, New York. Court reporters play an essential and required role in judicial proceedings, plus they do captioning work when requested in relation to the Americans with Disabilities Act (ADA.) It's a cool way to get into the courtroom without being a lawyer or judge, or it allows you to learn about all sorts of things while doing captioning! According to the Bureau of Labor Statistics, “Court reporters have a critical role in legal proceedings, which require an exact record of what occurred. These workers are responsible for producing a complete, accurate, and secure transcript of depositions, trials, and other legal proceedings." In regards to captioning work, this is closed captioning on televisions, and also with college students, business meetings, or anyone that requests to have a court reporter to take down the meeting, lesson, or event on the steno machine.Karen Santucci began her court reporting career right after high school. After graduating Stenotype Institute in Manhattan , she started working at the Queens County Grand Jury where she eventually became the Assistant Supervisor of the Grand Jury Reporters. After many years, she left to raise her 3 children. In 2001, she returned to the field as a court reporting instructor and realized her true calling was teaching and encouraging people to enter this great profession. In this episode of BackTable Urology, Dr. Esther Han (USMD Hospital) and Dr. Richard Santucci (Crane Center) discuss genital gender affirming surgery techniques and postoperative management. --- SHOW NOTES First, Dr. Santucci discusses his career pivot from academic trauma reconstruction to private practice gender affirming reconstruction. After 17 years at Detroit Medical Center as director of trauma reconstruction fellowship, he left his position to learn gender-affirming surgery at the Crane Center in Austin, Texas. He was drawn to gender affirming surgery because of the dynamic techniques and novel research in the field. Next, the doctors review of vocabulary for transgender patients. Dr. Crane notes that vocabulary is always changing and advises doctors to not assume a patient's gender or surgical preferences based on appearance. Additionally, Dr. Han explains why using the word “normal” over medically correct terms can confuse and cause discomfort in patients. Next, Dr. Santucci summarizes the gender affirming surgeries he performs. He explains trans women surgeries, such as full vaginoplasty and vulvoplasty. In trans men, he performs metoidioplasty and phalloplasty. However, Dr. Santucci emphasizes that patients are given the freedom to customize their own surgeries by picking which anatomical parts they would like to keep, change, or remove. Complications, such as urethral strictures and infections may occur in all these surgeries. Additionally, he emphasizes the need for addressing fertility with trans men, as some of them would like to keep their ovaries for egg harvesting. Then, the doctors discuss the recent change in World Professional Association for Transgender Health (WPATH guidelines), which now only require one letter from a therapist instead of two in order to receive genital surgery; however, insurance companies may require more letters. The doctors also contemplate the presence of post-operative regret, since gender affirming surgery is difficult to reverse. Dr. Santucci explains that regret is often tied to postoperative complications. Then, he delves deeper into the management of phalloplasty complications, such as postoperative incontinence, postoperative infections, and erosions. He also emphasizes the importance of prostate cancer screening for trans women. Although it is rare, the prostate cancer they develop is testosterone independent, making it more difficult to treat and more aggressive. Finally, Dr. Santucci shares what a typical week in his life looks like and reflects on how multidisciplinary collaboration in the OR has made his operations more efficient and effective.

Most of our systems that involve children are designed on the compliance model — that's true in parenting and educating. We've leaned on compliance in our educational system since the 1800s, and it's been part of our parenting culture for centuries. And yet, neuroscience has now taught us that there's a monumentally better way. Through research we know that our neurobiology is a driving force in behavior — particularly our autonomic nervous system and our brain. We know that there are sensitivities and differences in our neurodivergent kids' neurobiology too, making it crucial that we adopt this brain-based lens when responding to and managing behavior.
