Former Engineering manager at BOAC and British Airways and Engineering Director at Cathay Pacific, Stewart takes us through the challenges of selecting and keeping aircraft in the air in both the Far East and in the UK. Though starting life as a BOAC apprentice, Stewart spent his early years on secondment to Kuwait Airways and Malaysia-Singapore Airline and he tells us about life living away from home often as the only engineer certified to maintain the airline's types. Back in the UK, Stewart spent most of the 1970s at Heathrow dealing with the challenges posed by the unions, engineers and the fleet of Boeing 747s and 707s. Steward finished his time as a full-time employee in Hong Kong as Cathy Pacific's Engineering Director, working with Boeing, Rolls-Royce and his engineers selecting and maintaining over 60 wide-bodied aircraft and Dragonair's fleet of narrow bodied aircraft. Not ready for the allotment, Stewart returned to the UK to take up consultancies and board positions with many aviation companies including Rolls-Royce and Airbus. Always a Committee man, Steward also discusses his term as President of the Royal Aeronautical Society, the Society of Licensed Aircraft Engineers and the International Federation of Airworthiness. During the interview, Stewart shares some amusing and fascinating tales of the people he met and the challenges he faced during his varied career. Phil Manzanera has had a remarkable life. Best known as the lead guitarist in Roxy Music, he's also worked as a writer, producer and/or performer with the likes of David Gilmour, Steve Winwood, Nico, Brian Eno and many more. He's just released a mammoth box set of his solo work which dates back to 1975's “Diamond Head” album. The box, entitled “50 Years Of Music”, comes at a time of reflection for Manzanera. He recently released a memoir, in which he detailed his childhood in Cuba at the time of Batista's overthrow in 1959, and his suspicion that his Dad, who ostensibly worked for BOAC airlines, was actually a spy. Manzanera lived in different parts of the Americas as a child and later went to boarding school at Dulwich College in London. Phil went on to explore his Latin American roots in his music, and another bizarre twist occurred in 2011 when Kanye West and Jay-Z sampled a guitar part from his 1978 “K-Scope” album for their track “No Church In The Wild”. Michael ‘Mike' Howlett's talks about his experience of flying successive generations of aircraft during his career as a pilot spanning over 70 years. Having learnt to fly on a Tiger Moth bi-plane, he flew first-generation jet fighters (the Meteor and Vampire), then the first generation long-haul, land-based piston-engine airliners in the form of the Boeing Stratocruiser. This was followed by flying the first of the long-range, swept-wing jets, the Boeing 707, and then the first of the ultra-large passenger aircraft, the Boeing 747 ‘Jumbo'. These aircraft not only ushered in different eras of air travel with a new level of passenger experience, they posed new challenges for their operators and aircrew. Mike provides fascinating insights into these changes. At just before 2 o'clock in the afternoon on Saturday, March 5, 1966, British Overseas Airways Corporation Flight 911, took off from Tokyo's Haneda International Airport. Bound for Hong Kong, the Boeing 707 broke apart in mid-air, just 15 minutes after departure. The crash left no survivors. The weather conditions were not an issue – at least, nothing obvious: there were no thunderstorms, no typhoons, there was no rain or snow. In fact, it was a sunny, clear, cloud-free day. It was so nice that the crew had requested a clearance so that they could stray from their intended route to Hong Kong, so that the captain could give the passengers a glorious view of the snow-capped and iconic Mt. Fuji. It was while viewing Mt. Fuji in this “sightseeing mode,” that Flight 911 met its demise.What happened to Flight 911? What caused the mid-air break-up? Was it a bomb? Did the plane strike the mountainside? Was it a mechanical failure of some kind? Was the plane unsafe? Had the crew made a mistake? Or, despite the sunny skies, was the weather to blame. In this episode, the Radar Contact Lost team will examine what happened. We'll look at the airplane, the pilot, the airline and Mt. Fuji itself; we'll also take a dive into the phenomenon known as Clear Air Turbulence - not only what it is, but how, why and where it occurs and why, on this day over Japan, BOAC Flight 911 was ripped apart in midair.

Philip Hogge first learned to fly in 1958 by completing a gliding course with the Air Cadets. He then obtained an RAF Flying Scholarship while still at school and learned to fly a Tiger Moth, gaining his Private Pilot's Licence. In 1962 he joined BOAC where he flew the Britannia 312 (the last propeller service across the Atlantic), then Brooklands built VC10s, Boeing 707s and eventually 747s. He was Flight Training Manager on 707s and then 747s, before becoming Chief Pilot on 747s, and finally GM Flight Operational Services in British Airways' flight operations department. Airline flying in the 1960s was still an era of glamour, adventure and excitement in what Phil fondly recalls as the Golden Age of Flying. He has written two volumes of short stories in which he recalls, in fictional form, the challenges and enjoyment he and his colleagues shared in those days.

In this special extended episode we explore the coordinated terrorist hijackings of four passenger airliners all bound for New York on the same day on September 6, 1970.  The events were a watershed moment and aviation would never be the same again. The four planes involved were: a TWA Boeing 707,  a SwissAir DC-8, a El Al Boeing 707, and a Pan Am Boeing 747 named Clipper Fortune.  A fifth plane, a BOAC Vickers VC 10, was hijacked three days afterward. Later in the program we are joined by Pan Am Captain Paul Lachapelle. A 30-year Pan Am veteran who  joined the airline in 1955 flying the 707 and later 747SP. He retired from United Airlines in 1993.The mission of the Pan Am Museum Foundation is “to educate, celebrate,and inspire present and future generations by preserving historical and diverse personal stories of Pan American World Airways.”Pan Am was a pioneer in air travel and still stands as one of the most iconic and innovative airlines in aviation history. That legacy lives on at the Pan Am Museum in Garden City, New York, where you can explore the rich history of the aircrafts and individuals at the heart of the company known as The World's Most Experienced Airline. The Board is responsible for the overall management of the system of airports under its control, including Los Angeles International Airport, LA/Ontario International Airport, and Van Nuys Airport. Ambassador Lawson also served as a Partner at the prestigious international law firm Skadden, Arps, Slate, Meagher & Flom LLP in their New York and Los Angeles offices until he retired in 2011.

This episode explores an accident at a time of Covid-19 – which may be too recent to have a direct effect on civil aviation safety and yet the causes appear to be directly linked to poor Cockpit Resource Management otherwise known as crew resource management. It has caused many an incident and accident, unfortunately. The Pakistan crash which took place in May in Karachi is also a warning about how airlines go about restarting their services after a lengthy shutdown. Flying is not like riding a bicycle. It has also led to immediate suspension of Pakistan International Airlines landing rights in the EU after shocking details emerged about systematic Airline Transport Pilot License exam cheating along with other cases of corruption. So the main point is an Airbus A320 crashed into heavily populated suburban area of Karachi in Pakistan on May 22nd 2020. Flight 8303 was a scheduled domestic flight from Allama Iqbal International Airport in Lahore to Jinnah International Airport in Karachi. The plane went down in a residential area near the Airport a few days after Pakistan lifted restrictions imposed over the coronavirus pandemic and resumed domestic flights ahead of the major Muslim holiday of Eid al-Fitr. Amazingly, two on board survived – both in Business class while at least one person on the ground died - a 13 year-old girl. As I said, Crew Resource Management failures appear to be behind this crash at least from the initial reports published in Pakistan. Crew Resource Management is also known as cockpit resource management. One of my instructors used to chatter to me during important phases of flight and I had to say “sorry Russell, I need to report our position” or reset instruments and he would smile in a knowing way. Cockpit resource management includes knowing when its time to shut up or shut you fellow pilot up and concentrate extra 100 percent on the job at hand. Landing an aircraft is one of those crucial moments. But when did Crew Resource Management start as a thing? The first person to talk about human interaction on the flight deck was a BOAC captain David Beaty who was a former Royal Air Force pilot. He wrote a book - The Human Factor in Aircraft Accidents in the late 1950s. It became part of the United Airlines pilot training handbook following the crash of a DC-8 in 1978 and eventually was recommended for all pilots by the National Transportation Safety Board. That was after a United Airlines Flight 173 crashed in Portland Oregon on December 28th 1978.

In this episode we’ll look at in-air breakups of aeroplanes – caused by poor flying, poor design, or poor maintenance and bad weather. In some cases all four of these together. However as with all things aviation, every accident leads to an equal and opposite reaction .. to misquote the great Sir Isaac Newton. That reaction luckily for us, is called Aviation safety standards. The terrible truth is that people die and then safety improves. So let’s start with the 32 year-old Charles Rolls. He was one half of the great Rolls-Royce engine company but his end was rather unfortunate. Probably the most famous of all in-air break ups involved the notorious de Havilland Comet. It took three catastrophic failures all within a year before the airliner was grounded. Launched by BOAC in 1952, the Comet was the world’s first jet airliner and was an attractive plane too. Aviation buffs swooned over its swept back look, the modern jet liner was born and it could fly right across the Atlantic without a stop. However, it had a serious flaw. The windows and doors. One of the most incredible in-air failures ended with almost everyone surviving. In April 1988, part of the fuselage of an Aloha 737 flying from Hilo to Honolulu shredded at 24,000ft. A flight attendant was swept overboard – everyone else survived. Imagine sitting in the open air with nothing between them and the ocean except for a safety belt. That may be so, but it took a 1991 accident to kick start a proper global culture of aviation safety. The mid-air break up of the Continental Express Flight 2574 – an Embraer 120 Brasilia, was a scheduled domestic passenger airline flight operated by Britt Airways from Laredo International Airport in Laredo, Texas, to Houston Intercontinental Airport or IAH in Houston, Texas. A break-up of a plane over Peru deserves special mention at this point. As you’ll hear in this series, there are many examples of a single person surviving a plane crash. And this is one of them. Today we hear about the extraordinary story of Juliane Koepcke. She was 17 years old and sitting in the window seat next to her mother on board a Lansa Aircraft flight 508 from Lima in Peru to Pucallpa in the middle of the Amazon Rain Forest. There’s another I have to mention and it involved something known as Clean Air Turbulence which led to an in-air breakup of a commercial airliner. In the case of BOAC flight 911 callsign Speedbird 911, clean air turbulence produced an estimated 7.5Gs that caused the Boeing to disintegrate over Mount Fuji in Japan on 5th March 1966. Clean Air turbulence will be covered in a future podcast, but needless to say there’s no warning. All 113 passengers and 11 crew perished.

Captain of one of the first transatlantic flying-boats, J. C. Kelly-Rogers takes his audience through the challenges and romance of setting up and flying the transatlantic passenger route. Starting with the agreements between the UK, Canada, Ireland and the USA to set up schemes in the late 1930s, he gives insights on how he and others helped Imperial Airways and then BOAC to overcome the challenges of establishing the first scheduled service, including surveying the route and establishing the best methods to carry-out flight-refuelling, before talking of his experiences of operating the service during the Second World War. The lecture was originally entitled “Atlantic Flying Twenty-Five Years Ago” and was the 5th Short Brothers Memorial Lecture to the presented to the Belfast Branch of the Royal Aeronautical on 1 December 1964. Following the mixed success of the Brabazon programme of civil aircraft development immediately after the Second World War, from the early 1950s, the Conservative government sought to encourage "private venture" civil projects. This policy depended on orders from the nationalised airlines: in return they expected to shape specifications - with a direct affect on two key airliners. By the end of the 1950s, both the VC10 and the Trident were experiencing commercial problems at a time when the government wanted to drive home its merger policy. This necessitated the return of direct government support in the form of repayable launch aid. But too late to save either aircraft from failure in the world market - or BOAC from bankruptcy.

In 1959 Tomoko Takekawa’s life was full of promise, having moved to Tokyo from her parents home in Kobe had given her a whole new level of independence and freedom. That year she was also hired as a stewardess for the British airline BOAC. It seemed the whole world was opening up for her, unfortunately this is where Tomoko’s story ends and the story of this week’s case begins. On March 10th, 1959 Tomoko’s lifeless body was found strangled and lying face up on the banks of the Zenpukuji River. Though police scoured the Stewardess’ social life and airline connections no one was ever charged with her murder. The case remains unsolved to this day. After an early career at Lloyd’s of London, he went on to learn about the importance of customer service, working for BOAC, the forerunner of British Airways, something further reinforced during his time as a broker consultant at Scottish Equitable. Derek went on to set up his own successful IFA business which he ran for 18 years before selling it in late 2006. Derek spotted an altruistic opportunity to provide a better service to directly authorised IFAs and in 2007 Panacea was born. One of the world's first adviser communities started in a coffee shop near Lloyds of London. They shared ideas, talked about how to write more profitable business and looked for new opportunities. Derek started Panacea Adviser which he describes as the modern online equivalent of that first financial coffee shop.
