We’re super excited to bring you this week’s episode with Senior Clinical Neuropsychologist, Simone Mangelsdorf-Collett (she/her). We chat with Simone about Functional Neurological Disorder (FND) – or, as Simone puts it “the most common disorder you’ve never heard of”. Simone specialises in FND, neurodegenerative syndromes, and movement disorders, and practices out of Yarra City Psychology in Richmond, Melbourne. She is experienced in both ends of patient care, including assessment and diagnosis and rehabilitation and intervention. If you’ve been wondering about FND, this episode is for you! We cover: What is Functional Neurological Disorder (FND), and how does FND differ from other neurological disorders (like epilepsy, stroke, Parkinson’s etc.) What does the term ‘functional’ mean, in this context? The historical context of FND and how our understanding of this condition has evolved over time. Some of the currently known mechanisms behind FND, including predictive coding theory. Some common vulnerabilities and triggers for FND. The overlap between FND, neurodivergence, and gender. Simone takes us through treatment options for FND and who would be involved in the treatment team. How Simone came to specialise in FND and her work in FND advocacy and education. Things mentioned: FND Australia Support Functional Neurological Disorder in Australia Functional Neurological Disorder Society (FNDS), for professionals. Here are some studies on the link between FND and Autism – Tamilson et al., 2024, Gonzalez-Herrero et al., 2024, Pun et al., 2020, Link between gender and FND – McLoughlin et al., 2023. We are excited to have Samantha Levy, PhD join the show again this week to discuss Functional Neurological Disorder (FND). Dr. Samantha Levy is a clinical psychologist specializing in youth with chronic pain disorders. She uses a biopsychosocial approach to address both physical and emotional struggles, working with children, families, and parents to help them align the mind and body for improved functioning. Dr. Levy also runs parent groups, writes a parenting blog for Creative Healing for Youth in Pain, and works as a psychologist for Whole Child LA. Functional neurological disorder (FND) is a condition in which often debilitating neurological symptoms appear without a clear medical cause, and this episode helps make some sense of FND, from the ways it manifests (paralysis, seizures, sensory disruptions) to the underlying emotional and psychological factors that ultimately shape it. We explore why FND tends to surface in individuals with a history of anxiety, depression, trauma, and/or other medical conditions and how social media and peers, particularly during the COVID-19 pandemic, have played a role in its seeming to be more common. In this episode, you will gain insight into how staying calm, validating emotions, and avoiding reinforcement of symptoms can help support recovery. Dr. Levy helps us understand that treatment for FND is not a one-size-fits-all approach. We discuss the importance of a multidisciplinary approach instead, including individual therapy to address emotional triggers, physical therapy to rebuild control over the body, and family therapy to create a stable support system. We also stress how schools and healthcare providers play a key role in recovery, and we explore strategies to ensure that the right team is in place. This episode of the podcast ultimately offers a message of hope – that FND can feel overwhelming, but with the right interventions, many children see significant improvement, and the experience can even lead to personal growth for the entire family!   Show Notes: [2:46] - Dr. Levy explains how FND causes real neurological symptoms without medical explanation and has historically been misdiagnosed. [4:49] - FND can involve paralysis, sensory loss, and/or seizures, resulting from the brain misinterpreting signals. [7:14] - FND often affects individuals with anxiety and can spread via social influence. [10:13] - Dr. Levy discusses how pandemic-related stress and social media exposure contributed to FND symptoms. [12:22] - Dr. Levy argues that identifying emotional, physical, and environmental triggers for FND episodes helps prevent and reduce symptoms. [15:14] - To avoid reinforcing FND symptoms, parents should minimize attention during non-epileptic episodes. [18:45] - Professional guidance is very important for parents who have children with FND. [19:19] - Encouraging open emotional expression helps children with FND take on triggers. [22:43] - Therapy for teens with FND tends to address anxiety, trauma, and coping strategies. [25:22] - Dr. Levy discusses how physical therapy, guided imagery, and emotional validation help teens with FND regain body confidence. [28:38] - Dr. Levy reflects on how a child's FND episodes decreased significantly after shifting family dynamics. [30:32] - Overprotectiveness can actually worsen FND symptoms, so teens should maintain safe, independent activities. [33:39] - Parents should seek support themselves because their stress can worsen their child's FND symptoms. [35:45] - Dr. Levy points out that FND can be a wake-up call, prompting families to develop healthier ways to handle emotions. [38:00] - Dr. Levy reflects on how a girl's involuntary kicking symbolized suppressed anger, stressing a need for emotional expression. [41:27] - Various resources such as reactive programs and CHIP webinars help provide support for FND treatment. Feel confused about how to assess and treat someone with Functional Neurological Disorder (FND)? We got your back! In this episode host Erin Gallardo interviews FND experts Heather Salon, PT, DPT, NCS and Julie Maggio, PT, DPT, NCS on this range of neurological symptoms that includes functional movement disorders, seizures, and speech/communication issues. FND is characterized by abnormal brain connectivity and network dysfunction, often presenting with mixed symptoms. A key shift in recent years has been the recognition of FND as a "rule-in" diagnosis, rather than a diagnosis of exclusion. Learn what to expect during the subjective evaluation and how to approach it with best practice. Heather and Julie highlight the importance of identifying specific "rule-in" signs, such as Hoover sign, give-way weakness, and symptom variability, to confidently diagnose FND. They also emphasize the need for a comprehensive assessment, including evaluations of the motor, sensory, and autonomic systems. Also discussed are differential diagnoses like dystonia.   The conversation underscored the multidisciplinary nature of FND care, with physical therapists, occupational therapists, psychologists, and neurologists working collaboratively to provide optimal treatment. Sensory-based interventions, such as the use of Kinesio tape, abdominal binders, and visual feedback, were discussed as valuable tools in the PT's toolbox. Importantly, Heather and Julie stress the need to manage patient expectations and set realistic goals, focusing on improving function and reducing symptoms rather than aiming for complete symptom resolution. They also highlighted the importance of ongoing education and resources for clinicians interested in advancing their knowledge and skills in the FND field. However, this was not always the case… In this episode Nick talks us through the long process of initially being diagnosed with primary progressive multiple sclerosis, and the lifestyle changes that he implemented to manage his condition (one of these being the overcoming MS lifestyle). Nick then explains the adventure he eventually embarks on in the Himalayas, which inspires the creation of Mastering Mountains - a charitable trust helping others achieve their adventure dreams and connect with community. But, of course, as you have probably realised, his story doesn't end there. Several years later Nick realises he has been misdiagnosed, and suddenly finds himself navigating the world of living with FND… Follow Nick on Instagram: @nick_allen And check out Mastering Mountains: masteringmountains.org.nz Find That's So Chronic on IG and Tiktok: @thatssochronic @thatssochronic | @jessssbrien | #thatssochronic Free monthly newsletter: thatssochronic.substack.com Application form to share your story: https://forms.gle/csebLkwfwAjiLApK9 Disclaimer: Here at That's So Chronic we are sharing personal stories and are not advocating any type of treatment, therapy, procedure or intervention. In episode 181 of The Just Checking In Podcast we checked back in with Claudia Van-Nimwegen. Claudia is a mental health advocate, teacher and theatre artist. In Part 3 of her mental health journey we discuss an Eating Disorder (ED) which Claudia had previously been bottling up and suppressing for a long period of her life. In December 2022, the ED, the fibromyalgia (which Claudia had been diagnosed with and discussed in Part 2) and general stress combined into a dangerous cocktail and exploded, causing her to collapse at work. The left side of her body became immobile and she was admitted to hospital where she stayed for three weeks. During that time, she had to learn to deal with her ED, change her eating habits and tackle the voices in her head that were telling her to take her own life. These voices came in the form of an imaginary friend which Claudia said has been with her since childhood. Eventually, after lots of checks, delays and frustration, Claudia got some clarity on why she collapsed and was diagnosed with Functional Neurological Disorder (FND). She says this has explained so many things in her life and plays a part in every other diagnosis she has had. Despite that traumatic moment and all of these diagnoses, she has never let this define her. She was discharged after a month in hospital, is in recovery and back at work doing what she loves in teaching and drama. We finish by discussing how her faith got her through this traumatic event, how her church community supported her financially whilst she was off work and finding her voice. The Bookkeepers' Voice
The Bookkeeper Lost Her Voice | Amy speaks with Aphasia after being diagnosed with Functional Neurological Disorder (FND)

The Bookkeepers' Voice

Play Episode Listen Later Jan 3, 2023 9:13

Having a voice is about so much more than talking, or even communicating. Finding your voice is about walking the difficult but rewarding path from being invisible, silenced and ashamed, to being heard, understood, and seen. When Amy started her bookkeeping business in 2015, she found clients difficult and at times abusive, which left her feeling isolated, stressed and questioning her own abilities. After connecting with other bookkeepers, Amy realised that many bookkeepers had the same struggles, experiences and often felt isolated in their business. Whether trying to communicate with a difficult client or set boundaries many felt undervalued and overwhelmed. When Amy started The Bookkeepers Voice Podcast, she wanted a way for colleagues in the Bookkeeping industry to feel less isolated, more connected and know they were not alone. Through sharing real experiences of what it's like running a small bookkeeping practice, and teaching of topics of interest, Amy makes you think, laugh and know you're not alone. In this episode, Amy tells her story of how she lost her Voice: literally. On 8 December 2022, she was heading to South Gippsland with her two sons and husband Will, to visit her dad for the weekend. But less than 30 mins into the trip, Amy felt unwell and became unable to speak normally (Aphasia). Will called an Ambulance and Amy was taken to hospital to be assessed for a stroke. Instead, she was Diagnosed with functional neurological disorder, a condition where there is a disruption between the brain and a function of the body. ironically, the disruption occurred between the one function that forms the basis in the foundation for the work Amy most loves to do - speaking, encouraging, telling stories, teaching and training, and giving Bookkeepers running businesses, a voice. Amy recorded this episode about a week after she came home from hospital, and although she had made a great deal of progress in recovering her voice, but the voice you will hear this episode isnot the one you are used to hearing. Since this episode was recorded, Amy's voice returned to normal, but has also had to further episodes causing a relapse of Aphasia. Amy is currently taking some time off work to recover, and to decide on a future direction that will take into consideration, her reduced work capacity. At this stage, the future of the Bookkeepers voice podcast, and The Savvy Bookkeeper is unknown. But we will post an update as soon as possible. Amy would like to express her gratitude to podcast listeners, and savvy members. You have given Amy a sense of purpose and joy in business. We ask that you would lift Amy and her young family up in prayer, and for healing and protection over Amy's voice, and for Savvy, a business Amy worked so hard to build. That it would not be wasted or destroyed.See omnystudio.com/listener for privacy information.

But we do the best we can. ”When writer and musician Eliza Hull was pregnant with her first child, like most parents-to-be she was a mix of excited and nervous. But as a person with a physical disability, there were added complexities. She wondered- Will the pregnancy be too hard? Will people judge me? Will I cope with the demands of parenting? More than 15 per cent of Australian households have a parent with a disability, yet their stories are rarely shared, their experiences almost never reflected in parenting literature. In We've Got This, twenty-five parents who identify as Deaf, disabled or chronically ill discuss the highs and lows of their parenting journeys and reveal that the greatest obstacles lie in other people's attitudes. “Disabilities bring colour to humanity”   Sandie Roberts had a childhood that focussed on her passions of dancing and gymnastics, however it was a difficult childhood, having been bullied at school and when she was 15 years old she fell down the stairs and her life changed forever.   As she got older she started struggling with her mental and physical health and was was diagnosed with Functional Neurological Disorder (FND). Even though Sandie had 3 children, the birth of the children was complicated and eventually she was told that she wouldn't be able to walk and is now a full time wheelchair user.   Sandie eventually started an instagram account sharing her reality and now inspires others all around the world with chronic disorders.   A story to truly give a new outlook on how we view disabilities.   Resources:         Support:

Mass sociogenic illness? Never heard of it until I read an article in the Atlantic Magazine last week (see below for link). It relates the concept of a functional Tourette's Syndrome-like issue that occurs when young people watch YouTube or TikTok videos of people with Tourette's and, in a way, begin to imitate them. The relevance: they define a Functional Neurological Disorder (FND) differently than Brain-based chiropractors do...but we work on these 'disconnected kids' by reconnecting them to their brains and nerve systems! https://www.theatlantic.com/ideas/archive/2022/02/social-media-illness-teen-girls/622916/
