Our guest today is Dr. Jason Fung, a Toronto-based nephrologist, and the best-selling author of “The Obesity Code,” “The Diabetes Code,” and “The Cancer Code.” Jason is best known for his success in combining a low-carb diet with intermittent fasting to help thousands of overweight patients reverse their type 2 diabetes, lose weight, and improve their metabolic health. Jason is the author of the blog “The Fasting Method” and the co-founder of the Intensive Dietary Management program, an initiative that provides low-carb dietary guidance and counseling on various fasting regimes. Jason is also the co-author with Jimmy Moore of “The Complete Guide to Fasting,” which looks at the history and culture of fasting and how it helps people improve their metabolic health. In today's episode, Ken is joined by Visiting IHMC research scientist Dr. Tommy Wood and together, they and Jason discuss: How in the beginning of his practice, Jason prescribed insulin for type 2 diabetes patients. How a series of landmark studies starting in 2008 changed Jason's mind about using glucose-lowering medication for type 2 diabetes. Jason's realization that type 2 diabetes is largely a dietary disease and therefore requires a dietary solution rather than a pharmaceutical one. The origins of Jason's Dietary Management program, which counsels overweight and obese patients to follow a low-carbohydrate, high-fat diet to reduce insulin. A critique of the “eat less, move more” strategy acclaimed by many obesity experts. Mark Mattson's research into the powerful impact of intermittent fasting on metabolic health and a recent paper that questioned the effectiveness of time-restricted eating when compared to daily calorie restriction. Recent research and evidence that fasting during chemotherapy may reduce the side effects of the treatment. Show notes: [00:02:25] Tommy opens the interview mentioning that Jason was born and raised in Toronto and asks what drew Jason to science as a kid. [00:03:43] Tommy mentions the irony that Jason has written several best-selling books, yet Jason was not fond of English or writing when he was in school. [00:04:53] Ken mentions that after graduating from high school, Jason stayed close to home and attended the University of Toronto, entering into medical school just after turning 19 to study internal medicine, eventually specializing in nephrology. Ken asks Jason what intrigued him about becoming a kidney specialist. [00:06:36] Tommy asks Jason what led him to go to UCLA after medical school for his specialty training in kidney disease at Cedars-Sinai Medical Center. [00:07:37] Tommy mentions that Jason has been practicing clinical nephrology in Toronto since 2001, and that in those early years of his practice, Jason saw patients with type 2 diabetes and prescribed medications to keep their blood glucose low. When that didn't work, he would prescribe insulin, which is the standard medical practice. Tommy asks what Jason observed during these early years of his practice. [00:09:28] Ken mentions that in 2008, two landmark studies were published, the ACCORD study and the ADVANCE study (a summary of ADVANCE). These studies were followed by two more studies, the ORIGIN and VADT studies, all four of which demonstrated that using blood glucose-lowering medication for type 2 diabetes didn't necessarily have the expected benefits. Ken asks Jason to talk about how these studies were eye-opening for him and confirmed his own experience in treating patients. [00:16:27] Tommy explains that in 1972 Robert Atkins published the “Dr. Atkins' Diet Revolution,” which shared his findings on the effectiveness of low-carb, high-fat dieting. Tommy goes on to mention that in the late 1990s, a string of Atkins-styled diet books surged in popularity, with most physicians being opposed due to the conventional wisdom that these high-fat diets would cause heart disease. Dr Jason Fung is one of the world's foremost experts on intermittent fasting. He practices in Canada as a kidney specialist and in 2012, he co-founded America's first intermittent fasting clinic. He's published multiple international bestselling books and he runs a global online coaching program called Intensive Dietary Management. Dr Fung also works tirelessly to communicate simple messages with the public on his YouTube channel.We begin this conversation by discussing how many unscheduled opportunities to eat there are in modern life. Our default should be not eating, with two or three meals planned in short windows over 24 hours. Instead, schools, workplaces and places of leisure are designed around opportunities to snack and for many of us, it's causing a host of problems.Jason goes on to talk about why he believes that calorie counting isn't the answer when it comes to getting your weight and health in check and why some foods are more fattening than others, even if they're equal in calories. Jason's approach is to encourage all of us to switch to what he calls ‘the right foods' – whole, unprocessed foods, low in sugar and refined carbohydrates, which will not stimulate large amounts of a hormone called insulin. We know what causes other diseases or infections. Cancer is the second cause of death in America, yet we don't know what it is. The way we think about cancer has changed significantly throughout the years. There is an enormous shift in how we see the disease, affecting how we treat the disease. There is a recognition that cancer is actually an obesity-related disease. In 2003, there was convincing evidence about the correlation between cancer and obesity. In fact, it's a massive part of cancer. Diet turns out to be 30-35% related to cancer causation. Breast cancer and colorectal cancer are the most prominent obesity-related cancers. “Diseases of metabolism are also diseases of growth.” -Dr. Jason Fung [16:15] Every Cancer Develops From Your Own Cells The number of mutations that are required to get cancer far outstrips the rate of mutation. Why does every cancer look exactly the same? People will have identical cancers that develop independently of each other. Cancer is an evolutionary disease. You can take the tools of evolutionary biology and apply them to cancer instead. All the different mutations are not random because they all end up in the same place. It's not a random chance mutation; it's a guided mutation. Cancer is not part of your body; it's foreign. The immune system will attack cancer cells. [25:00] A Seven-Day Water Fast Could Shrink Tumors The research on water fasts is preliminary. A water fast will maximize autophagy. It will put your cells into maintenance mode. When you fast, your insulin will lower as well as your mTOR. Cancer is a tough thing to study because you cannot use preventative treatments once it gets past a certain point. If you want to study prevention, then you need lots of people. [29:00] The Correlation Between Glucose and Cancer Cancer runs on glucose much better than they run on fat. If a cell is trying to grow, then it needs fuel. Some cancer cells can run on protein and fat. Glucose is the most readily available fuel, and it makes the cell run efficiently. If you starve the cell of glucose, then it can change. Before you develop cancer, nutrition can be enough to prevent it. [38:35] How Bad Fats Could Increase Our Chances of Cancer Bad fats could possibly increase our chances of cancer. However, the data isn't there yet. Anything that increases inflammation is going to create a risk of cancer. Cancer develops from inflammation and chronic damage. Anything that causes chronic damage to the body puts pressure on the body to be in survival mode. [42:50] The Benefits of Metformin If you have type 2 diabetes, then metformin may be better for cancer prevention. Metformin will turn down excessive growth. If you are in growth mode all the time, it's bad for longevity. You want your metabolic rate to be lower by eating less. [45:30] Getting Too Much Autophagy Too much autophagy is bad and too little autophagy is bad. You want to be somewhere in the middle. You want to maintain a middle ground of autophagy for the most part. When you eat too much, you'll turn off autophagy. [47:40] How Fasting Can Improve Chemotherapy Chemotherapy will kill cancer cells. Fasting will turn down normal cell growth. Therefore, they will be less susceptible to chemotherapy. Cancer cells cannot turn themselves down as easily. Fasting is safe during chemotherapy. Plus, it is shown that you can handle the chemotherapy better when fasting. AND MUCH MORE! In this episode, you'll discover: [00:17] How Dr. Fung got into the health space, particularly nephrology As a Math and Science person, Dr. Fung initially thought he would pursue the field of engineering. He then decided that being a physician was more fitted to his style. After years in the university, he figured that he was also invested in doing research. As a physician, he can help people and do some research at the same time. In choosing a specialty, Dr. Fung felt that Nephrology suited his personality better. Nephrology is a complex specialty which is sort of a thinking-person speciality unlike cardiology which is more of an action specialty (i.e surgery) [4:13] Diabetes, Obesity and Intermittent Fasting The most common and important cause of kidney disease is Type II Diabetes which is closely related to weight gain and obesity. Treating kidney diseases with medications is the least effective way to treat people. You have to look at the root cause of the disease in order to be successful in treating it. Which is why, for Dr. Fung, the real question was “what causes weight gain?” Nutrition is not considered part of the medical specialty which explains why doctors depend on medications to treat certain illnesses. Second question: What are you gonna do about it? Obesity was not existent during the 1970s. Now, 70% of Americans are obese. Intermittent Fasting: People suffering with Type II Diabetes for 20+ years immediately felt better by intermittent fasting. Fasting is less fun that eating pizza and doughnuts. Fasting is tough work. It is important to give people undergoing intermittent fasting the support and education they need. [12:30] The Calorie Debacle Coca-Cola created a group called the Global Energy Balance Network to keep people focused on calories. They wanted people to think that 100 calories of broccoli is equivalent to 100 calories of Coca-Cola which is completely wrong. If you think that if you can eat cookies, drink coke and exercise then, you'll be healthy, you are wrong. But, Coca-Cola wants you to think that this is right. Hormones control hunger, telling our body when to eat and when to stop. Different calorie sources elicit different hormonal responses. Diet and exercise are totally different things. Diet is what you eat. It affects the liver mostly. Exercise is mostly about skeletal muscles. Calorie is not a physiologic term. There's no calorie-receptors in our body. We think that excessive intake of calories causes obesity. We think that this is a caloric imbalance. Too many ‘calories in' compared to too few ‘calories out' leads to weight gain. This Calorie Balance model of obesity has been drilled into us since childhood. Read more about the Calorie Debacle here. [18:40] Intermittent Fasting as a reasonable alternative to treat patients Ketogenic diet and intermittent fasting are not as famous as low-carb diet. Jason Fung received a lot of backlash when he started to introduce intermittent fasting to his patients. Because of IF, patients were getting better. At the end of the day, you can't ignore that. Other doctors started to notice the success of IF. Through his book, The Obesity Code: Unlocking the Secrets of Weight Loss, Dr. Fung explains how to use intermittent fasting to break the cycle of insulin resistance and reach a healthy weight—for good. [25:55] Why the common treatments for type 2 diabetes seem to make things worse Type II Diabetes is a reversible disease. The standard treatment of Type II diabetes focused on getting the sugar down by medications. Doctors should've focused on the patient's diet to help them lose weight because if you lose weight, your diabetes gets better. Jason Fung forever changed the way we think about obesity and he wanted to do the same for type 2 diabetes through his book The Diabetes Code: Prevent and Reverse Type 2 Diabetes Naturally He explains why conventional treatments that rely on insulin or other blood-glucose-lowering drugs can actually exacerbate the problem, leading to significant weight gain and even heart disease. People cannot just make a decision to burn more fat calories, or to feel less hungry. These are physiological issues and must be addressed via physiology. [31:49] Dr. Fung's Intensive Dietary Management (IDM) program now called The Fasting Method The IDM program helps people reach their health goals through intermittent fasting and simple-to-follow time-restricted eating strategies. It's a structured digital lifestyle intervention developed by Megan Ramos and Dr. Jason Fung that provides you with all the education and support you need to succeed. At least several thousands of people have gone through the Intensive Dietary Management program and the number keeps adding up. For 2019 years, people have already been talking about fasting and they don't talk about it as a terrible thing. Fung clarifies that he didn't invent intermittent fasting. He just started talking about it because it seems logical to him. [37:14] Guinness World Record on the Longest Recorded Fast In the 1971 edition of The Guinness Book of Records, Barbieri's 382-day fast was recognized as the longest recorded. The most important thing is that people feel well throughout their entire fasting. For 365 days a year, 3 meals a day, you are taking a total of 1000 meals per year. Dr. Evelyne Bourdua-Roy is a family medicine doctor, who graduated from Université de Montréal in 2015, and from Université Laval in 2012. Dr. Bourdua-Roy has a Master's Degree in French Linguistics from Université de Montréal and a Bachelor's Degree in Translation (French-English-Spanish) from York University. She is also board-certified in Obesity Medicine by the American Board of Obesity Medicine (ABOM). She is training with Dr. Georgia Ede on the ketogenic diet and mental health, and on hormone replacement therapy with Dr. Neal Rouzier. She has co-authored two best-selling books in Quebec (in French): Perdre du poids en mangeant du gras and Perdre du poids en mangeant du gras tome II (Pratico Pratiques Editions). We know what causes other diseases or infections. Cancer is the second cause of death in America, yet we don’t know what it is. The way we think about cancer has changed significantly throughout the years. There is an enormous shift in how we see the disease, affecting how we treat the disease. There is a recognition that cancer is actually an obesity-related disease. In 2003, there was convincing evidence about the correlation between cancer and obesity. In fact, it’s a massive part of cancer. Diet turns out to be 30-35% related to cancer causation. Breast cancer and colorectal cancer are the most prominent obesity-related cancers. “Diseases of metabolism are also diseases of growth.” -Dr. Jason Fung [16:15] Every Cancer Develops From Your Own Cells The number of mutations that are required to get cancer far outstrips the rate of mutation. Why does every cancer look exactly the same? People will have identical cancers that develop independently of each other. Cancer is an evolutionary disease. You can take the tools of evolutionary biology and apply them to cancer instead. All the different mutations are not random because they all end up in the same place. It’s not a random chance mutation; it’s a guided mutation. Cancer is not part of your body; it’s foreign. The immune system will attack cancer cells. [25:00] A Seven-Day Water Fast Could Shrink Tumors The research on water fasts is preliminary. A water fast will maximize autophagy. It will put your cells into maintenance mode. When you fast, your insulin will lower as well as your mTOR. Cancer is a tough thing to study because you cannot use preventative treatments once it gets past a certain point. If you want to study prevention, then you need lots of people. [29:00] The Correlation Between Glucose and Cancer Cancer runs on glucose much better than they run on fat. If a cell is trying to grow, then it needs fuel. Some cancer cells can run on protein and fat. Glucose is the most readily available fuel, and it makes the cell run efficiently. If you starve the cell of glucose, then it can change. Before you develop cancer, nutrition can be enough to prevent it. [38:35] How Bad Fats Could Increase Our Chances of Cancer Bad fats could possibly increase our chances of cancer. However, the data isn’t there yet. Anything that increases inflammation is going to create a risk of cancer. Cancer develops from inflammation and chronic damage. Anything that causes chronic damage to the body puts pressure on the body to be in survival mode. [42:50] The Benefits of Metformin If you have type 2 diabetes, then metformin may be better for cancer prevention. Metformin will turn down excessive growth. If you are in growth mode all the time, it’s bad for longevity. You want your metabolic rate to be lower by eating less. [45:30] Getting Too Much Autophagy Too much autophagy is bad and too little autophagy is bad. You want to be somewhere in the middle. You want to maintain a middle ground of autophagy for the most part. When you eat too much, you’ll turn off autophagy. [47:40] How Fasting Can Improve Chemotherapy Chemotherapy will kill cancer cells. Fasting will turn down normal cell growth. Therefore, they will be less susceptible to chemotherapy. Cancer cells cannot turn themselves down as easily. Fasting is safe during chemotherapy. Plus, it is shown that you can handle the chemotherapy better when fasting. AND MUCH MORE! He then decided that being a physician was more fitted to his style. After years in the university, he figured that he was also invested in doing research. As a physician, he can help people and do some research at the same time. In choosing a specialty, Dr. Fung felt that Nephrology suited his personality better. Nephrology is a complex specialty which is sort of a thinking-person speciality unlike cardiology which is more of an action specialty (i.e surgery) [4:13] Diabetes, Obesity and Intermittent Fasting The most common and important cause of kidney disease is Type II Diabetes which is closely related to weight gain and obesity. Treating kidney diseases with medications is the least effective way to treat people. You have to look at the root cause of the disease in order to be successful in treating it. Which is why, for Dr. Fung, the real question was “what causes weight gain?” Nutrition is not considered part of the medical specialty which explains why doctors depend on medications to treat certain illnesses. Second question: What are you gonna do about it? Obesity was not existent during the 1970s. Now, 70% of Americans are obese. Intermittent Fasting: People suffering with Type II Diabetes for 20+ years immediately felt better by intermittent fasting. Fasting is less fun that eating pizza and doughnuts. Fasting is tough work. It is important to give people undergoing intermittent fasting the support and education they need. [12:30] The Calorie Debacle Coca-Cola created a group called the Global Energy Balance Network to keep people focused on calories. They wanted people to think that 100 calories of broccoli is equivalent to 100 calories of Coca-Cola which is completely wrong. If you think that if you can eat cookies, drink coke and exercise then, you’ll be healthy, you are wrong. But, Coca-Cola wants you to think that this is right. Hormones control hunger, telling our body when to eat and when to stop. Different calorie sources elicit different hormonal responses. Diet and exercise are totally different things. Diet is what you eat. It affects the liver mostly. Exercise is mostly about skeletal muscles. Calorie is not a physiologic term. There’s no calorie-receptors in our body. We think that excessive intake of calories causes obesity. We think that this is a caloric imbalance. Too many ‘calories in’ compared to too few ‘calories out’ leads to weight gain. This Calorie Balance model of obesity has been drilled into us since childhood. Read more about the Calorie Debacle here. [18:40] Intermittent Fasting as a reasonable alternative to treat patients Ketogenic diet and intermittent fasting are not as famous as low-carb diet. Jason Fung received a lot of backlash when he started to introduce intermittent fasting to his patients. Because of IF, patients were getting better. At the end of the day, you can’t ignore that. Other doctors started to notice the success of IF. Through his book, The Obesity Code: Unlocking the Secrets of Weight Loss, Dr. Fung explains how to use intermittent fasting to break the cycle of insulin resistance and reach a healthy weight—for good. [25:55] Why the common treatments for type 2 diabetes seem to make things worse Type II Diabetes is a reversible disease. The standard treatment of Type II diabetes focused on getting the sugar down by medications. Doctors should’ve focused on the patient’s diet to help them lose weight because if you lose weight, your diabetes gets better. Jason Fung forever changed the way we think about obesity and he wanted to do the same for type 2 diabetes through his book The Diabetes Code: Prevent and Reverse Type 2 Diabetes Naturally He explains why conventional treatments that rely on insulin or other blood-glucose-lowering drugs can actually exacerbate the problem, leading to significant weight gain and even heart disease. People cannot just make a decision to burn more fat calories, or to feel less hungry. These are physiological issues and must be addressed via physiology. [31:49] Dr. Fung’s Intensive Dietary Management (IDM) program The IDM program helps people reach their health goals through intermittent fasting and simple-to-follow time-restricted eating strategies. It’s a structured digital lifestyle intervention developed by Megan Ramos and Dr. Jason Fung that provides you with all the education and support you need to succeed. At least several thousands of people have gone through the Intensive Dietary Management program and the number keeps adding up. On a recent episode of Fasting Talk, I heard Megan says it is detrimental to your health to not get enough calories in on your feeding days. She noted that patients who only eat 500-800 calories a day are the hardest ones to help because their metabolism has been slowed down for so long. But then in last week’s episode from Low Carb Breckenridge with Dr. Fung answering questions, he said it’s okay to eat one meal a day when you intermittent fast and that he doesn’t really care about how many calories you get in that meal. There’s no way I could get over 1,000 calories in one meal. So if you are doing this six days a week then aren’t you getting to that place where Megan says it is detrimental with the low calorie intake? Please help me with this. Leota 4. Megan Ramos gives us a very good understanding of what intermittent fasting is, the different protocols she and Dr. Jason Fung recommend, and how massive amounts of weight can be lost by implementing it. This is the most comprehensive information we have had on the subject on this podcast, including some very useful tips on how to use the different protocols. What is the basic dietary approach when helping patients? They actually start their patients with basic intermittent fasting, for about 7 to 14 days, based on bloodwork and medical history, and the patient’s comfort level. For the dietary approach, they start weaning out carbohydrates. As patients finish food items in their pantry, they’re requested to buy different options. More fatty foods. Too much protein can turn in to glucose in the body, and that’s what we don’t want. What are the different types of intermittent fasting protocols available? In their office the offer the following: 24 hour fasting protocol- 3 times a week. Recommended for patients that are trying to be preventative, or shed the last 20 pounds. 36 hour fasting protocol- Day 1: breakfast, lunch and dinner; Day 2 fast; Day 3: breakfast, lunch and dinner. 42 hours fasting protocol- Day 1: lunch and dinner; Day 2: one day fast; Day 3: lunch and dinner. This is used for significant weight loss and for those who already have diabetes. How to apply intermittent fasting as a woman? It will impact our hormones as females. But there is nothing differently that they do from their male patient, including for menopausal, pre-menopausal and post-menopausal women. For those in menopause it will be more difficult to lose weight, and must adhere to the guidelines more stringently. No desserts. Women do lose weight a bit slower than men, but hormones do tend to adjust within 6 weeks to match the rate at which men lose weight. What is the positive impact of intermittent fasting for women? It regulates hormones. You can regulate periods on this protocol, and makes them shorter and easier. Less water retention Less cravings. Suppresses appetite Increases bone growth production, which can help with osteoporosis. Helps recycle all proteins that have gone bad, preventing Alzheimer’s, dementia and cancer. Recommendations for heavy exercisers? Start with 24 hour intermittent fasting. It is recommended exercisers hydrate with water and salt 1 hour before exercise, or bone broth, so that muscles are hydrated. Don’t start exercising until they’ve been in a fasted state for 20 to 22 hours, and break the fast afterwards. She goes over some of the types of foods one should be eating while during fasting.
