From rising geopolitical tensions, to the worsening climate situation, and from the Gaza genocide to hot wars that threaten to spill into great power conflict, the world of the 2020s is one of concatenating crises. But is our moment an interregnum that will be followed by a new moment of relatively ordered hegemony? Juliano Fiori, director of the Alameda Institute, argues otherwise and makes the case that we have entered a post-order situation in which crisis is the new normal. In our conversation today, we talked about why Juliano believes the creation of a relatively stable new international order is unlikely and why China will probably not be able to assume a position of international leadership on the scale needed to create such an order. We also talked about Gramsci's concept of the Interregnum, and how disordering at the international level is mirrored and mutually constituted by the disordering of national polities and the world of work. And we also talked about the extent to which the pre-Donald Trump era really was a rules-based system given the extreme violence that the United States and its allies have been the authors of since WWII.

Instead, that our world is now one of sustained disorder. In his own writing, Juliano takes this one step further, arguing that the very notion of order as we've come to understand it is tied to the system of US hegemony that has dominated global politics since the end of the second world war.For Juliano this “order” is not only conceptual, but material. Sustained first by the unparalleled industrial base of American capitalism, and then by its transformation into the hub of global trade and finance - secured at every turn through military might. He argues that, in losing sight of this, progressives too often take this exception for granted, and with it the belief that its decline will organically precipitate the rise of a new stability - perhaps one governed by a more just or democratic set of institutions. But this is not a mistake we can afford to make. With the dominance of the dollar waning, the US grip on global capital is beginning to slip. And Trump's warmongerings, from Venezuela to Iran, now appear as the violent shocks of an empire in sharp decline.The materiality of what we once called “order” is coming to an end. So what, if anything, comes next? The continued rise of China? A patchwork of competing regional powers? In this AJNR Podcast Author Interview, Dr. Francis Deng and Dr. Amy Juliano discuss how CT can subtype Menière disease by assessing the vestibular aqueduct and adjacent bone. During normal childhood, the angular trajectory of the vestibular aqueduct progressively narrows; an angle greater than 120 degrees after age 12 indicates adult-persistent hypoplasia and thus the hypoplastic endolymphatic sac endotype rather than normal maturation. A retrolabyrinthine bone thickness of at least 1.2 mm serves as a practical surrogate for a mature vestibular aqueduct orientation, reliably excluding the hypoplastic endotype and identifying ears that do not belong to this high-risk subgroup. In contrast, in the degenerative endotype of Menière disease, retrolabyrinthine bone thickness is more variable and does not follow this simple threshold pattern, so CT features must be interpreted in a broader clinical and imaging context.
