On this week's episode of the Ready to Run Podcast, we have the pleasure of meeting with Lindsey Plass to discuss femoroacetabular impingement (otherwise known as FAI) and hip labrum tears in runners. Lindsey is a Chicago-based physical therapist, performance coach, author, and international speaker. Lindsey is a former college soccer player turned 10-time marathoner and triathlete. She completed an orthopedic physical therapy residency and manual therapy fellowship making her a board certified Orthopedic Clinical Specialist and Fellow of the American Academy of Orthopedic Manual therapists.  It was her own lived experience, nearly 10 years ago, with hip pain related to FAI and a labral tear that sparked her interest in helping others with hip issues getting back to doing the things they love.  She is a clinician and coach and owner of Plass Physical Therapy and Performance. She is currently faculty in the University of Chicago Medicine Orthopedic Physical Therapy Residency and University of Chicago Manual Physical Therapy Fellowship. In 2021, Lindsey was invited to be a part of the international Young Athlete's Hip Research Collaboration led by Dr. Henrik (Paul) Dijkstra who is the Director of Medical Education at Aspetar. Tonight I talk about the NHL stoppage, the CIHL playoff push and the Coronavirus. My guest graduated from the Medical school of the University of Hong Kong in 2007 and obtained his Fellowship in Orthopaedics & Traumatology in 2015. His postgraduate studies include a Masters of Science in Sports Medicine and Health Science (2013, CUHK) and a Masters in Public Health (2018, HKU). He’s the associate consultant in the Department of Orthopaedics and Traumatology of Tseung Kwan O Hospital, and is current the Surgeon in Charge in Sports Medicine and Arthroscopic Surgery. He is also currently Chief Medical Officer and member of the Board of Directors for the Hong Kong Lacrosse Association (HKLA). He served as the team doctor for all Hong Kong representative teams for lacrosse since 2013 and is one of the commissioned members of the Hong Kong Association of Sports Medicine and Sports Science. When I was in college, I did a paper on this new kind of knee surgery called Arthroscopic Surgery. It was so new that I had to use the research library in the hospital where my mom worked as a nurse. Of course, the advancement in caring for injured knees was amazing but one thing that came up again and again in the research papers was the absolute correlation between success and how many times the surgeon had done the procedure. You can be told how to swim, you can watch someone else swimming, you can write about swimming and even talk about swimming, but the only way to learn how to swim and to get better at it is to do it – and then to practice! So it is with being – and making – disciples! Today, we explore what this means!

Studies on the effect of partial- and full-thickness chondral damage of the hip on outcomes and the ability to achieve meaningful clinical outcomes are limited. Patients with grade IV chondral defects experienced worse functional outcomes, lower satisfaction, and increased pain when compared with both patients without chondral damage or grade I-III chondromalacia at 2-year follow-up. Several predictors were associated with achieving clinically significant function in patients undergoing hip arthroscopic surgery for FAIS.   Click here to read the article.

Finally a randomized controlled trial comparing physical therapy management to surgery for femoroacetabular impingement syndrome (FAIS)!!! And what were the findings? Well, not impressive on either end. Of course this generates more questions than answers but that's what good research does. Advancements in instrumentation and techniques have extended the scope of hip arthroscopic surgery to treat complex osseous deformities that were previously best addressed with an open approach. Global pincer-type femoroacetabular impingement is an example of an abnormality requiring osseous correction with a technically challenging access point. Hip arthroscopic surgery for the management of symptomatic labral tears in patients with combined overcoverage and coxa profunda is associated with improvements in patient outcomes and pain at a minimum 2-year follow-up. However, the degree of improvement is of lower magnitude compared with a matched cohort with normal coverage undergoing the arthroscopic management of symptomatic labral tears. While hips with lateral overcoverage combined with coxa profunda may have a smaller potential for improvement compared with hips with normal coverage, this type of osseous morphology is still repairable with arthroscopic treatment.   Click here to read the article.
