“Maybe getting a little clear on what this idea is so you can be even 5% or 10% more intentional about keeping those two things separate.” - Jill Farmer Contaminated time is a topic that comes up a lot in our conversations. It's a term that's been recently introduced in the field of stress management. Here, we discuss what contaminated time actually is and what someone can do about it. We first learned about the term from New York Times best-selling author and Pulitzer Prize winner, Brigid Schulte. Essentially, contaminated time refers to the idea that, whether we're at work or at home, our time can become “contaminated” by worries, thoughts, energy, and attention, which takes us away from the present moment.   Research has shown that those with less contaminated time are more productive. We discuss practical ways to better manage contaminated time that won't make you feel robotic. Much of dealing with contaminated time involves setting boundaries around things like checking and responding to emails. “It took me several years of going down that path to really realize that trauma healing is a journey, it's not a destination, and to have grace and compassion with myself. And when I was able to do that, I think my impact as a physician changed.” -Dr. Katie Cole, D.O. Burnout in health care is real and we would just like to let everyone know upfront that we will be discussing suicidal ideations in physicians, as well as trauma healing for physicians and health care professionals. We have Dr. Katie Cole who shares with Jill Farmer what inspired her to pursue a healthcare career. Her father was in the medical field and struggled with depression and mental illness, which then led her to study and practice psychiatry. She received a PTSD diagnosis due to childhood abuse and also lost her father to suicide, which led her down a path of deeper self-discovery. Dr. Cole & Jill discuss the stigma and shame around struggling with mental illness as a practitioner and how it impacts your approach to working in the field. Trauma healing is a journey, not a destination, and keeping that in mind helps you foster compassion for yourself and be a better caregiver. Dr. Cole believes that burnout in the medical community needs to be an international conversation and it's her mission to be a voice and an advocate for physicians and healthcare providers. Dr. Katie Cole is known as “The Healer's Healer.” She's an award-winning holistic psychiatrist, organizational well-being consultant, and change agent. Can I give myself permission to feel both the discomfort in feeling afraid to be vulnerable as well as the courage to be vulnerable?” - Dr. Manizeh Mirza-Gruber, MD, is a licensed, Board-certified Psychiatrist, Certified Yoga Teacher, Certified Mind-Body Medicine Skills Facilitator (CMBM)   Today's episode reveals the benefits of physician mindfulness with Dr. Manizeh Mirza-Gruber, MD, licensed board-certified psychiatrist. Master Certified Life Coach and cohost of of the podcast Jill Farmer welcomes Dr. Mirza-Gruber to have a conversation about why and how physicians should be encouraged to show vulnerability. As Dr. Mirza-Gruber shares in the episode, she has found that by being vulnerable with her patients, she is able to understand and help them better, and to ultimately find more joy in practicing medicine. By allowing yourself to be vulnerable and acknowledge your personal worries or doubts, Dr. Mirza-Gruber has found that physicians are able to better show compassion for themselves, so that they are then more suited to care for others. Today's episode works to normalize these physician experiences.    Manizeh Mirza-Gruber, MD, is a licensed, board-certified psychiatrist (ABPN), Certified Yoga Teacher (NACYT), and Certified Mind-Body Medicine Skills Facilitator (CMBM). She leads meditation groups, and is currently in a two-year Mindfulness Meditation Teacher Certification Program, Class of February 2023.    Manizeh was born and raised in Karachi, Pakistan and moved to Houston, Texas in 1988 for her post-graduate training in Psychiatry. She worked in community mental health for almost sixteen years providing psychiatric emergency services to a culturally diverse, underserved community.    In May 2017, she founded her own private practice, Manizeh Mirza-Gruber, M.D., PLLC - DBA Mindful in Practice™, to follow her lifelong passion of helping individuals become their true and authentic selves.   As an individual, parent, and psychiatrist, Manizeh follows a holistic approach to life. She believes in connecting mind, body, heart, and spirit for healing.    She has been facilitating Mind-Body Skills Groups and workshops since January 2018 to bring healing in the aftermath of Hurricane Harvey, and now during the Coronavirus pandemic.    Manizeh is happily married to her husband and has two adult children and a dog. She has a daily practice of running, yoga, and meditation. She has run several marathons, and since 2014 has been “fundracing” for a nonprofit organization, Back on My Feet. She is now one of their lead volunteers for the morning circle runs in the Houston Chapter. Dr. Zibin Guo is a professor in medical anthropology at the University of Tennessee Chattanooga and a Tai Chi Chuan Master. He specializes in Applied Medical Anthropology with a focus on applying traditional healing knowledge to develop intervention programs to promote physical and psychological well-being among vulnerable populations. In this episode, he shares about the benefits of Tai Chi for our busy physician audience. As he mentions in the podcast, you can experience the benefits of Tai Chi without dedicating a bunch of time or learning any difficult exercise regimens. He suggests learning two Tai Chi movements and integrating their practice into your life. He specializes in Applied Medical Anthropology with a focus on applying traditional healing knowledge to develop intervention programs to promote physical and psychological well-being among vulnerable populations.    In 2005, Dr. Guo developed a form of wheelchair Tai Chi Chuan (TCC) program making a traditional healing/martial art accessible to people with ambulatory limitations. A key feature of this innovative program is that it integrates wheelchair motions with the flowing movements of TCC to transform the wheelchair from an assistive device to a tool of empowerment and artistic expression. Dr. Guo's program was debuted at the 2008 Paralympics.     Since 2016, funded by the United States Department of Veterans Affairs, Dr. Guo has been implementing the Adaptive Tai Chi for Veterans program across the country. “I admit that I’m a recovering people pleaser. In that recovery process, I thought, ‘something is wrong, I feel irritated by what this person just said to me. But I can’t seem to stop it because I’m here to serve. Therefore it’s okay, I’ll just tolerate what is happening.’ And it could be any number of examples, whether it was patients, colleagues, or even at home.  I wasn’t terribly good at setting boundaries. And it became clear that it was exhausting, and really not a very self honoring situation to be in. So I had to learn how to to do it. It is not a skill I was born with. Maxwell, you can find it here   “We’re kind of wired as humans to grow and to live our most meaningful, richest, flourishing life when we step out of that comfort zone, step out of the box and break the cycle of monotony in our lives by being willing to try new things.” -Master Certified Coach Jill Farmer   In today’s episode Coaches Gabriella Dennery MD and Jill Farmer talk about having the courage to try new things and why that is so important. So when was the last time you stepped out of your comfort zone and tried something new? Recently? Awesome! But if you’ve just been going through the motions lately, this episode is for you. Remember to give yourself grace through the process, let go of perfectionism and have fun!      Excerpts from the show: “Most of the time when people come to me with great fear and trepidation and all kinds of reasons why it's a terrible idea to try something new, it's because they're convinced that if they're not certain whatever it is will be a wild success or they're going to be perfect at it, they feel like something is wrong and therefore they shouldn't try it. So a lot of my encouragement and coaching around when somebody has this kind of spark or inspiration to want to try something new and then the other part of the brain that is saying, ‘Don't try something, you're not going to get this right, you're going to screw this up.’ They tend to want to default and listen to the voice of doubt or inner critic, the part of them that is saying, ‘You can't do it’ and pull back into that safe place. The problem with that is, of course, we're kind of wired as humans to grow and to live our most meaningful, richest, flourishing life when we step out of that comfort zone, step out of the box and break the cycle of monotony in our lives by being willing to try new things. So the price of admission for letting that caution voice always be the one telling you that it's uncertain, unsafe, better not try it, which is really strong in doctors and I love that a lot of doctors have a really strong voice of caution to try to keep everything safe for those that they're serving. We're not asking you to change that part of you that has prudence and caution. It's just to recognize that sometimes that can be a little over developed when it comes to things in your life that have lower stakes potentially that you could try and develop. And the price of admission for that is that you lose the reward of mastering new things, playing with new things, learning new things when you stay stuck in those old ways.” -Master Certified Coach Jill Farmer “I think you bring up an important point. How do you distinguish between the caution that you need to exercise at work to make sure that you mitigate risk for your patients and for your colleagues etc. etc. and what happens at home and outside of work? The ability to avail yourself of new ideas and new opportunities and a quality of life that may be different. You know, to expand your quality of life. It's easier to take that work philosophy home. So to be able to separate the two, I think it's about stepping out of the comfort zone. It's like, ‘Ok it's safe for me to try in-line skating now because I don't have to do that at work. I can do that at home. Yes, I may crash a few times and I'll need a few Band-Aids and I may look foolish. No, I won't master the first time on those skates, but I get to have a little fun and to learn something new that I've been curious about.’ It's kind of a mundane example, but a lot of times it's about knowing that work is work and play is play or outside of work is outside of work. It may not necessarily be play, it may be other things that you're interested in. So what do you think in terms of coaching and in terms of encouraging somebody to try something new, whether it's at work or in other situations? Is it fair to say that part of the encouragement is to say, ‘You're not necessarily going to be great at it right away?’ How do you approach that perfectionism, that need to get it right, that need to master everything right off the bat? And is it more of a need to impress somebody or to make sure it's done right as opposed to going into the situation with the curiosity of a child, with that growth/learning mentality which says there may be times when I'm going to suck and that's ok? What has been your experience with that and trying to push people out of that comfort zone?” Coach Gabriella Dennery MD “Well, what I like to tell people is there's just a lot of brain science behind the idea that when we have a growth mindset, which is what the work of the masters in positive psychology, which is really the science of success, positive psychology isn't about being a peppy person, it's how do we work with people who are on the well side of the psychological scale and help them thrive instead of just surviving. And one of the things we know about that science of success is that people who have a growth mindset, which means they look at a challenge or something new as an opportunity to learn, they are happier and more successful than people who look at challenges with a fixed mindset, which is what I call problem stalling instead of problem-solving. ‘Here's all the reasons that it's going to be hard and I'm not going to be great at it and I might make a mistake.’ So we know in that science of success also, that we have to set ourselves up to be allowed to make mistakes because mistakes are how we revamp. I'm not talking about mistakes that are going to harm patients, obviously. Since we're talking to physicians here, we understand that we're not encouraging that. But I find a lot of physicians are really uncomfortable with setting themselves up or making space for making mistakes anywhere and being willing to revamp and being willing to use those mistakes as data. I know you've heard me say it before, but I love what Richard Branson has said about the reason that he's so wildly successful. It's because he has a bigger capacity for failure and he kind of likes it when things go wrong because he feels like that's how he learns how to make things go right. I think that's what you were talking about. Making some space for mistakes to happen. Because that's data that can help whatever it is we're learning new and be more in line with what we want it to look like. And that's how we grow and actually thrive. When we have that setback, reconfigure, move forward motion, that's a really good place for humans to thrive.” -Master Certified Coach Jill Farmer “Absolutely. You're absolutely right about that. Because part of it and just speaking from experience being on wards and emergency rooms and CCU’s, it can potentially be a very judgemental environment and you don't want to make mistakes. Clearly for the sake of your patients and at the same time it's about how your colleague is judging you or how the chief is judging you or the chair is judging you. There's always a process of evaluation, of testing, of performance. So yes, getting out of your comfort zone in other arenas of your life or even in a career track or a different career choice can become difficult because of that fear of judging, that fear of being judged and being looked at in a certain way. You want to do the right thing, but what is the right thing? That is really still a very subjective thing. So to be able to allow yourself to just take a breath. To allow yourself to say, ‘You know what? I'm on a learning curve.’ For example, podcasting for me, although as much as I love it and enjoy it, it's still very new and I'm still on that learning curve. I've still got a lot to learn and I know I've made a lot of mistakes. At the same time it's learning how to enjoy that process. To fail forward instead of just seeing it as falling backwards and not trying again. So we get back on the horse. And yes, if it risks looking silly for a little while or sounding totally crazy for a while, that's ok, too. So it's being ok with making mistakes. I think that's the point that I think is a really really important point for why it is important to muster up the courage to try new things. You have to be ok to not be perfect and great with making mistakes and even relish in making mistakes so that you can learn and love it and laugh about it and say, ‘Wow, look at what it was six months ago compared to what it is today.’ And you enjoy the learning curve. You're able to just flourish in the learning curve itself. These are things that can contribute to our mental and physical health and help us to thrive. You need that creativity somewhere along the line and it's not just work related. I think if we could take work off the table for a minute, life is not just about that. There are other things going on. You need to recharge and I think one of our podcast guests said it wonderfully. Ysaye Barnwell talked about creativity and she said that sometimes you need to shut off one valve and open another one up and allow something else to come in. Just take a mental and physical break to recharge, to get the juices flowing, to have a different kind of community, a conversation. I took up African drumming when I was an attending physician within the first six months of starting my first job as an attending. I was in New York City and it was something I wanted to do, something I was curious about. I saw it on stage one day and there was a particular drum that I wanted to study. I said, ‘Well, I'm in New York City. This is the right place to be! Let's go for it.’ So I found a teacher. Within six months, I started a new job in July and by January, I was in drum classes. And to be around a different community of different people who didn't talk about patients and didn't talk about diagnoses and consultants. They were from different walks of life and they talked about anything and everything. And to be in a space where I was learning something completely different. So encouraging somebody by saying, ‘You know what? I know you think there's no time, and I think through the coaching work what's important is you realize that there is more time than we think there is.’ But even if you believe there is no time and there are other things that are more pressing or more important and a hobby is something you can just push aside… the hobbies are what you dig into even more when you're stressed. Why? Because it gives you that breathing room and that breathing space.” -Coach Gabriella Dennery MD  “Yeah, I love what you just said, I think it's really important. We talked about it in other conversations and other podcasts which I highly recommend folks tune into about procrastination and perfectionism. A lot of times people actually procrastinate doing their hobbies because they are so attached to the outcome. ‘Well I can't paint because I can't make a pretty picture out of it.’ This is an important time for us to really play with progress over perfect. It's the process not the outcome. The process is doing as you said, shutting off one valve and turning on another. The term that I use a lot of times with my clients is that we have to change the channel. As a kid growing up in the 70s before we got cable, I'm watching the Brady Bunch and all I've got is a bunch of white fuzz on the screen for some reason, you know whatever day there's not a good signal coming in. We had this trick where we would turn the channel and we’d watch a little Gilligan’s island for a while and then sure enough we’d come back and the channel was clear again. Our brains are a little like that too. We need to be shifting some of those neural pathways. We need to be reusing that glycogen in different ways and changing the channel is important. So one of the things that comes up in my physician clients who I'm encouraging to look for some ways to change the channel and to explore some hobbies and some interests and some passion, something that they're interested in outside of work. And they'll say, ‘Well I don't have any of those because I'm not good at it.’ So they're not a painter and they have no haute couture skills. They're not a drummer like you. I'm that way. I don't have any hobbies that are actually other high-end skills for me but that doesn't mean we shouldn't have ways to explore that. What do you say to people like that, Gabriella, who say, ‘Well I'm not good at anything else so how can I have a hobby?’” -Master Certified Coach Jill Farmer “Understand that it's not just about the activity itself, it's a benefit you get from it. So what are you going for? Are you going for perfectionism? Are you going for getting it right? So you don't do it because you're not going to be a world-class painter? I think my father sold three paintings in 86 years of life. It's not about that, it's about the pleasure of it. So where is your joy? Where is the pleasure? Where is that leisure time? And if you don't have a hobby, and I'm saying this to everybody, find one. Go for what you're curious about. And you can always drop it if it's something that you tried but it’s not interesting. So I'm curious about knitting, let me see how that works. Ok, that bores me. Let me find something else. Get curious. Go with where your curiosity leads. It doesn't have to be the next big thing. It doesn't have to be how you earn your living. It doesn't have to be your next business idea. It just has to be something that breaks the routine and helps you think and access your brain in different arenas. Where different pockets of your brain are activated you know. Get that creative right side brain going as opposed to always being about logical decisions. Stop being logical for a moment and be illogical. Have fun, go with your curiosity. Find stuff out. Ask people, ask your friends what they do. One thing I ask my clients is what hobbies did you have when you were growing up? What was fascinating to you then? What about your teen years or younger adult years before you went into that medical track? What were your personal side gigs that had nothing to do with money or performance but had everything to do with where you found your joy? That seven or eight-year-old kid knew what they were fascinated about, so if we have to go all the way back there then that's what I do until we find that thing. It may be something new, but just to tap into that curiosity again, that kid-like curiosity.” -Coach Gabriella Dennery MD “I love that so much. The hot track right here that we follow is what are you interested in and exploring it and also being willing to be more kid-like, like you said. As kids we would change our mind about stuff. Remember you'd be into something and then a couple years later you see that Aunt that you hadn't seen in a couple years and she’d ask if you were still into geodes and you're like, ‘No I'm not into geodes anymore.’ But that's ok. As a kid you thought it was ok and there was no crime in moving onto the next thing. And so I think to give ourselves permission as adults to play in that space is important. Another thing I realized around this subject over Covid times was I had kind of let my phone and my iPad become a bit of a hobby in a way that wasn't particularly satisfying to me. It was just a default mechanism more than a real place that was drawing me in out of curiosity. You know it's fine to play solitaire on your iPad, I thoroughly enjoy it and I'm not saying it's evil. It could be a really nice thing to do. It's just I let myself realize that was my default and there might be other things I wanted to do. So I've just come back to good old-fashioned reading mystery novels that I hadn't read for a long time. The other thing that I think is important that I know is something we're both passionate about, is looking for ways to be connected to other people. A sense of community is important to us. We know it's actually an antidote to burnout as well. So I love encouraging my physician clients to notice whatever they're interested in and are there places they can go be interested in that thing with other people? Whether it's sailing or needlepointing or biking or tae kwon do as you mentioned. Whatever it is. It's letting ourselves play. When we are hard workers and high achievers, which physicians are, we somehow think that play is for other people or for people that have more time on their hands, and we don't recognize that as creatures we were literally built to balance what we're doing in terms of effort with some play. 35: Perfectionism: Why 100% at Everything Doesn’t Add Up

  “Giving yourself room to breathe, and at the same time that pressure to perform, perform, perform, was alleviated tremendously. So I think it's just finding those little hacks that make a big, big difference.” -Coach Gabriella Dennery MD   It’s understandable that you strive for perfection in your work life, but is it spilling over into your personal life too?   In today’s episode, Coaches Gabriella and Jill sit down to discuss perfectionism. You’ll hear why perfectionism can be so harmful, and different ways to combat this trait.    Let’s just go ahead and say it, perfection is unattainable, and trying to reach it can cause undue stress and anxiety. So let’s give ourselves a break and find the areas of our lives where 50-80% is enough, so that we can have the energy to put in more effort in the areas where lives depend on it.        Excerpts from the show:   “It’s stressful. The need to get that A+ all the time, get that letter of recommendation, get into a reputable residency program, pass the boards, etc. etc., get more letters of recommendation etc. Depending on your ambition and your aspirations and where you want to go, especially if you’re on an academic track, publishing, researching, etc. etc. It's a lot of pressure 24/7. And that pressure really starts early. It starts in undergrad when you're thinking about getting into med school and what you need to get the grades to get through the MCATs etc. etc. So there's not only that drive but it's accepted that somehow you have to be perfect and there's a lot of self judgment that goes around if you don't quite make the mark in your own estimation. It's not even in somebody else's. It's in your own estimation. After a while it becomes so automatic that it's something that becomes part of you.  I was reading an article from a psychologist who was talking about a university student saying that they worked really, really hard to get that A+, and they wanted to get that A+ and they got it. And then at the end, they said, ‘Well if it was really mine to have, I shouldn't have had to work so hard for it.’  Instead of saying, ‘Yeah I got my A+!’  So that need to be perfect prevents yourself from even celebrating the wins.” -Coach Gabriella Dennery MD     “I think it is so detrimental. Psychological research has shown that this is associated with depression, with suicidal ideations, and anxiety, this chasing chasing chasing. Chasing what exactly? So this becomes a given particularly in physicians. You're taking care of other people. You don't want to make mistakes. You want to make sure you get it right because somebody's life could be at risk. So it's trying to find that balance to say, ‘You know what, 80% is enough,’ and to get to that conclusion. To get to that ‘80% is good enough,’ that takes some work. Personally, it was years of being able to undo that habit. Of being able to say, ‘No, I don't have to be perfect, it has to be good.’ Because I'm not the only one involved in that person's care. I'm not the only one who has ideas and opinions and thoughts about that person's care. Or about how I even run my own life, because that spills over into personal life as well, (thinking,) ‘Everything has to be done right and I have to take care of everything.’ So to be able to say, ‘No, let me enlist people because I can’t take care of everything. I don't have all the ideas. Other people have other expertise that I can lean on.’ That was a big lesson for me personally as a physician.” -Coach Gabriella Dennery MD     “I had a beautiful experience of witnessing an insight for a physician several years ago, who was in her 50s and very burned out, and really struggling with that at that point in her career. Feeling like, ‘What have I done all this for? I'm so exhausted, I don't care like I used to,’ and all the things that show up in burnout. Through our conversations all of a sudden one day her eyes opened really wide and she actually started weeping and she said, ‘This is the first time in my whole life since I can remember, (since) when I was trying to get straight A's in sixth grade because I wanted to be a doctor someday, that I realized that I can want to do certain aspects of my life at a very high level of achievement, like taking care of the patient, like putting the extra in to make sure that I'm doing everything I can to take care of them. But I don't need to be at 100% in most other parts of my life.’  She said, ‘I know it sounds crazy but it just didn't dawn on me.’ It's like, ok, if I'm giving away all the crap that's loaded up in my garage, I don't need to find the perfect place to take every box, or if I need to feed my family, I don't need to have the perfect mix of macro nutrients. Because she started to realize that level of trying to pressurize herself with perfection in every other aspect was not allowing her to do the great job that she wanted to do in the one place where it mattered most.” -Master Certified Coach, Jill Farmer   “The thing I would say most importantly to everybody listening is, can you get more nuanced? Less of that distorted ‘all or nothing’ thinking. ‘Either I do it at 100 or I don't do it at all.’ Can you be more nuanced, and really be curious about your own life, and say, what are the areas where you're throwing in a lot more effort? If 51% is good enough then the amount of time you spend to get up to 100% is incrementally and exponentially a lot higher. “Begin to create a new script in our heads about ourselves, that looks more like how the person who knows, likes and believes in us the most talks about us.” -Master Certified Coach, Jill Farmer    In today’s episode, Coach Gabriella and Coach Jill answer a question from a listener about how to flip the script on imposter syndrome. Is imposter syndrome something that you have experienced or are experiencing? You’re not alone. Psychologists tell us that imposter syndrome is more common among high achieving people (source). That’s great, but what can you do to combat it? Tune in to find out!  Question: “Going all the way back to medical school, I feel like getting accepted was a fluke. I had friends who would have made excellent physicians, probably better than me, who didn't get in. I've been working in private practice as a board-certified physician for years but still sometimes when I'm working I imagine making a mistake and being outed as an imposter like, ‘She's a fake, the jig is up’. Sounds like classic imposter syndrome right? Until I started listening to your podcast, I never even questioned myself in this way of thinking but now I'm wondering is there something I can do to break this pattern of thinking?     Excerpts from the show:   “I don't think I've worked with a physician in ten years now in my practice as a coach that I haven't had somebody who either is experiencing or has experienced imposter syndrome. Both male and female physicians, surgeons, administrators… it's very common. It's just not always talked about. Everybody thinks they are the only one that is experiencing it. Lest we think that the reason we're experiencing imposter syndrome is because we are just not good enough, I think it's important for us to remember that Serena Williams has said, she too, experienced imposter syndrome. So if somebody as amazing as that, who we all look up to like that, experiences imposter syndrome, I think it reminds us that it happens to us as humans. And, it's not particularly helpful, right? To put it simply in psychological terms, imposter syndrome is feeling like a phony as though at any moment you're going to be found out to be a fraud, like you don't belong where you are, you only got there from dumb luck as was so articulately stated in the question. "The only way to stop feeling like an imposter is really to stop thinking like an imposter. I think that comes from being willing to find somebody that we really trust, whether that's a good friend, a partner, a family member, a coach, a therapist, or a colleague where we can sort of break the silence.” -Master Certified Coach, Jill Farmer   “It’s easy to default to that negativity bias because it's just kind of ingrained and programmed which means that getting back into the opposite, you actually have to train yourself. You have to practice it. You have to work at it very deliberately. So I love lists and I encourage my clients to write out lists. Name five things you accomplished today. Name five things you're grateful for today. It has to become an active practice, it can't just be, ok, it's not going to fall out of the sky and you're not going to undo the negative bias by simply wishing it away. You have to actively work at it to create new programming in your head, in your brain. “No, I'm not perfect and no, I can't do everything, but I’ve done pretty good and this is what I have accomplished and I feel good about that” as a way to again counteract imposter syndrome. It has to be an active, inside out process.” -Coach Gabriella Dennery MD   “Humility is one of twenty-one values in action that's defined by people in the realm of organizational psychology that looks at achievement and success. So it's valuable, but then a lot of times humility becomes a cover story. Where is it dialed up on the range? Is it a five, which is a healthy humility? Or is it dialed up at a ten, which then creates a situation where the dial of confidence gets dialed down to a zero. So, it's how do we blend those values in action in a way that allows us to be both humble and confident? Those two things can coexist and they coexist beautifully. It's important to note that psychologists tell us that imposter syndrome is more common in high achieving people. It's kind of ironic, but it's more common in people who tend to be extremely high achieving. Because they are often surrounded by other high achieving people and so then we get into the comparison thing and the patterns and ruts that we've already talked about.” -Master Certified Coach, Jill Farmer   “I used to say to my violin teacher that it’s good to be humble but it's a waste of time to be modest. Because modesty means that you're kind of giving yourself an inferiority complex. But humility, really by definition, is about standing in your own power and your own worth and yes it takes work and practice and process to get there. But that to me is the difference between, as you said, dialing it up to the point where you're diminishing yourself versus keeping it at a five, that good balanced range. So don’t be modest, be humble. (2020)     Excerpts from the show:   “It's common when you're reading articles about things like resilience and the ability to work under pressure, people keep talking about the Navy SEALs. And I was always like, ‘What is it with the Navy SEALs that seems to be so interesting to organizational psychologists and people that talk about performance?’ It's really interesting to research it just a little bit. So I'm going to share a little bit about what I learned and give you some practical, tactical, tangible ways that you can take some of the things that work well for the Navy SEALs into your practice and your life as well. For those of you that are not military experts, as I am not, a Navy SEAL is an elite special ops force that's within the Navy and it stands for sea, air and land. It's a special operations training that's very very hard to even be considered for, and for the thousands of recruits who actually meet these really astronomically challenging standards of physical and intellectual ability, you have to be really smart and in incredible physical shape, exceptional in your athletic ability. Only 250 people out of those thousand who get into training actually complete it because the training is really beyond grueling. So I'm not asking you to become superhuman or go through any superhuman training like what we see in the Navy SEALs, it's just to notice one of the things that has been shown as they've been researched time and time again is that they’re in highly stressful and pressurized situations but they seem to be able to handle it extremely well.” -Master Certified Coach, Jill Farmer    “So it's something that I've said for years and I think I even wrote about this in my book, There's Not Enough Time and Other Lies We Tell Ourselves : We can't control what happens in our life but we can control how we respond to it. What I discovered when I was researching this and writing my book about ten years ago, is that a lot of what I was doing in life was reacting, which was the amygdala kind of hijacking my brain and causing me to react without thinking. It was that split-second reaction which can be helpful, but what I wanted to learn how to do better was to respond. To not let my most primitive brain center that can only hear danger and alert signals make fight or flight based decisions for me, but to be able to pause just long enough to respond in a way that was going to create better outcomes, better results. Things are going to turn out better and that's something that the Navy SEALs seem to be really able to do in research that I found that was highlighted recently in a stress management special health report written by Harvard Medical School on enhancing your well-being by reducing stress and building resilience.” -Master Certified Coach, Jill Farmer     “Nassim Nicholas Taleb, coined the term anti-fragile and he even wrote a book about being antifragile (Antifragile: Things That Gain from Disorder). He defines fragility as the tendency to be damaged by volatility, uncertainty, the kind of things that often create that chronic stress response. He says an anti-fragile person realizes that stress is just the price we pay for being alive. Being antifragile helps us use our strengths to overcome challenges and become stronger in the process which is kind of the definition of resilience. So in this post-Covid year, we’re hearing a lot about resilience and it can feel really hard because we’re in what people like organizational psychologist Adam Grant are calling this languishing state where it's like, ‘Okay we survived it, not necessarily in full burnout mode where I'm paralyzed or not doing anything, but I'm not feeling a lot of juice, mojo or motivation;’ what is described in psychology as languishing. “That was really when it started for me, when I came out of the lab having done some exploration of Jon’s (Jon Kabat-Zinn) work and done some meditation. At that point if you can believe it, it was cassette tapes that Jon had been kind enough to give me. I came out of the lab a completely different person. I was actually having fun being a surgical resident. I was tired, you know I can’t say I didn’t have a bad day but I laughed with my colleagues. I had a great time. And I will share with you, Jill, that many many people, including my superiors and my residency director, noticed a huge change.” Antonia Stephen MD, Surgeon, Massachusetts General Hospital   In the last episode in this week’s series on meditation and mindfulness, Jill sits down with Dr Antonia Stephen to discuss how meditation and mindfulness can change your life. Start back at the beginning. When did you first become interested in this?” -Jill Farmer   “Like many things, I had exposure to the concept of meditation and mindfulness really as a kid. I grew up next door to Dr. Jon Kabat-Zinn who really is one of the modern fathers of mindfulness and who was instrumental in bringing mindfulness and meditation practices to western culture... I kind of knew it was there, I sort of knew what he did but not really. But I was aware of it and ...I would say that the moment that really grabbed me for the first time was when I was a teenager, and I happened to be a rower. I rowed in college. I was outside our house one day and Jon was leaving for the airport and I asked him where he was going and he said, ‘Well I’m going to work with the men’s U.S Olympic rowing team.’ Of course, that was fascinating to me because I had to assume that if the men’s Olympic rowing team was hiring Jon, paying him and flying him, I think at the time he was going out to California, that Jon was going to make them go faster and perform better. You know, make them win. That’s usually why Olympic rowing teams hire people, so that was interesting to me.”-Antonia Stephen MD   “For those of us who aren’t familiar as much with his work, can you give us just a little bit of a thumbnail about who he is and what he does?” -Jill Farmer    “Absolutely. He actually is a PhD. I think he was a graduate student or was working on his PhD at MIT and heard some lectures on mindfulness and meditation and was really fascinated with this. And what Jon did is look at this technique and he was interested in it and he started working initially with patients at UMASS Medical Center. Patients with medical issues such as hypertension and some mental health issues like anxiety and depression. He founded something called the MBSR clinic which was really the first of that kind in the United States, which is mindfulness-based stress reduction. He demonstrated that with these classes and with this center, which I recently read was actually started in a basement at UMASS, that these patients had improvements and were able to stop medications. He had subjective improvement and objective improvement. And as you mentioned, he actually published, studied and wrote about this and that was really one of the hooks that got western culture into his techniques.” -Antonia Stephen MD   “I think we spend very little time in that process examining why we’re doing things. What do we love about the idea of being a surgeon or a doctor? What do we love about the idea of going to medical school? If anybody had ever asked me those questions, I would have had absolutely no idea how to answer them. I think it was really my first few years of residency where they needed to be answered. If I’m going to be sleep deprived, if I’m going to work this hard, if I’m going to get up in the middle of the night, what’s my purpose? What’s my passion? I don’t think that I had ever had an opportunity to explore that. I think that can come naturally to some people depending on who they are in terms of their personality, in terms of what environment they’re in and who they spend time with and explore these things with. But for many, if not most of us, it’s all about the measuring stick getting to the next thing and I think that I found myself in a place where I really needed to examine that. So I tried some therapy, which I think is great, but as a surgical resident with the time commitment, the days and hours that therapists work and the expense was just not really feasible in terms of any sort of long-term plan. “You know, getting down to brass tax here, something isn’t going to help your work unless you put the effort in, unless you make a commitment and that’s hard. So one thing you have to realize is there is going to be days when you really don’t feel like sitting down and doing this but you’re going to try and make yourself do it anyways because you know it’s going to help you.”-Antonia Stephen MD, Surgeon, Massachusetts General Hospital    Tune in this week as we dive into all things dealing with meditation and mindfulness with Dr. Antonia Stephen MD.    In our first episode on meditation and mindfulness we learned how it can make you a better physician. That sounds great, so now let’s delve into how to fit meditation and mindfulness into a busy schedule. It sounds really good and all that but really to see the benefits and make it happen you absolutely need to start a regular practice. It’s hard when you’re really busy and it’s hard when you have a million other things that you feel like you should be doing. So I would say that the number one thing, which we touched on a little bit in our prior podcast, is to start small. Start real small. What if you challenged yourself to do five minutes once a week? It sounds so absurdly small that it’s kind of almost silly not to do it. But I think you know setting a really small goal and then having somewhat of a plan to keep you on task with it. A small journal or a little reminder on your phone. I would say the second thing, which is a relatively new thing, meditation and mindfulness has been around for a while but having an app on your phone and having a guided meditation where you fire up the app and it’s ten minutes or it’s five minutes, it’s whatever you know it is and that way it’s sort of all set up for you. Then you don’t have to worry about setting your own time or worry about keeping yourself on task because somebody is there sort of helping you do that. So I would say that those are two of the the things that I think are most helpful for busy people when they’re starting this type of practice. Number one, start really small and have an accountability system and download an app on your phone.” Dr. Antonia Stephen     “Can you give us the simplest explanation of the difference between meditation and mindfulness and how those two things are either different or similar to each other?”- Jill Farmer   “Yes, that’s a great question. Mindfulness is really the ability to be present in the moment of being here. Not in the runaway train mind. Not in the past thinking about what happened yesterday and what you would have said differently. Not thinking about your plans for the future. Not thinking about all the things you have to get done. Not thinking how angry you are at a colleague or a family member. It’s actually just being, and sometimes being present is feeling angry or whatever it is but you’re here, you’re in the moment. And when you’re that, you can really be anywhere but caught up in your thoughts. So you can feel your toes, you can feel your breath, you can look at the person in front of you and really look at them. You can hear the sounds coming from outside. That’s really being mindful.”- Dr. Antonia Stephen   “To be mindful, that’s the practice and that’s the meditation practice. The way that it was put to me, and this was actually directly from Jon Kabat-Zinn, is that you know obviously being present and being focused is a great thing. We all want to do that. I think that’s great but just calling upon yourself to do that when the stakes are high and when the pressure’s on and when you’re tired or sleep deprived or stressed or feeling threatened in any way, it’s too strong, you’re not going to be able to do it. By sitting and practicing meditation so that meditation is the practice so that you can be mindful that you are actually forming new neural pathways in your brain that are going to allow you to be mindful throughout your day and your life. Another way I’ve heard it described, which I absolutely love, is that our brain has ruts. We fall into those ruts because they’ve been there for many many years and reformed over time and to try to climb out of those ruts can be very difficult and even demoralizing and discouraging. So you have to form new ruts and those are the new neural pathways that you’re forming when you’re practicing meditation for those ten or twenty minutes or even that one minute and then those new ruts are there for when the time gets tough.”- Dr. Antonia Stephen   DocWorking believes the time has come to prioritize the health and wellness of physicians. “That reminds me of Richard Branson, the billionaire entrepreneur. People have asked him often, what is his key to success and one of the things he says is, I’m just not as afraid of failure as everybody else because I learn something so important every time something goes different than what I planned it to be. He doesn’t even say something goes wrong or ‘I failed.’ And so that reminds me so much of listening to your stories of being able to say ‘Oh yeah, I thought it would go that way, but look how it’s gone differently.’ I mean it’s no surprise to me that you became a scientist and that you have solved so many problems for so many people because you look at each problem as possibility, not as a measure of your self worth immediately in the moment whether it goes exactly as planned. Importance of Setting Boundaries

“I admit that I’m a recovering people pleaser. In that recovery process, I thought, ‘something is wrong, I feel irritated by what this person just said to me. But I can’t seem to stop it because I’m here to serve. Therefore it’s okay, I’ll just tolerate what is happening.’ And it could be any number of examples, whether it was patients, colleagues, or even at home.  I wasn’t terribly good at setting boundaries. And it became clear that it was exhausting, and really not a very self honoring situation to be in. So I had to learn how to to do it. It is not a skill I was born with.
