More and more people are, “doing their own research.”  Self-identified experts and influencers on podcasts (podcasts!) and social media endorse treatments that are potentially harmful and have little to no evidence of benefit, or have only been studied in animals.  An increasing number of federal leaders have a track record of endorsing such products. We and our guests have noticed that in our clinical practices, patients and caregivers seem to be asking for such treatments more frequently.  Ivermectin to treat cancer.  Stem cell treatments. Chelation therapy.  Daneila Lamas wrote about this issue in the New York Times this week -after we recorded - in her story, a family requested an herbal infusion for their dying mother via feeding tube. Our guests today, Adam Marks, Laura Taylor, & Jill Schneiderhan, have coined a term for such therapies, for Potentially Unsafe Low-evidence Treatments, or PULET. Rhymes with mullet (On the podcast we debate using the French pronunciation, though it sounds the same as the French word for chicken).  We discuss an article they wrote about PULET for the American Journal of Hospice and Palliative Medicine, including: What makes a PULET a PULET?  Key ingredients are both potentially unsafe and low evidence.  If it's low evidence but not unsafe, not generally an issue.  Think vitamins. If it's potentially unsafe, but has robust evidence, well that's most of the treatments we offer seriously ill patients!  Think chemo. What counts as potentially unsafe? They include what might be obvious, e.g. health risks, and less obvious, e.g. financial toxicity. What counts as low-evidence?  Animal studies? Theoretical only? Does PULET account for avoiding known effective treatments? Do elements of care that are often administered to seriously ill patients count? Yes. Think chemotherapy to imminently dying patients, or CPR. How does integrative medicine fit in with this?  Jill Schneiderhan, a family medicine and integrative medicine doc, helps us think through this. How ought clinicians respond?  Hint: If you're arguing over the scientific merits of a research study, you're probably not doing it right.  Instead, think VitalTalk, REMAP, and uncover and align with the emotion behind the request. Does the approach shift when it's a caregiver requesting PULET for an older relative who lost capacity? How about parents advocating for a child? “Getting people to care is the most important thing. I went all the way to the Maldives for research for my book How to Talk to a Science Denier because I wanted to see coral death. I wanted to see the Maldives. I wanted to see the country most under threat from climate change. One of my teachers was a 17 or 18-year-old kid who was the captain of a fishing boat. He said, "Oh, sir, outside the Maldives, no one cares." And that was when I realized that climate denial was not just about belief, it was about caring. He was right. Could you get people to care? How do you get people to care about what happens to the Maldives? They have to go there and meet people and/or know someone in order to care. “I had an absolutely wonderful high school history teacher, Dave Corkran. I dedicated On Disinformation to him. He taught me to think for myself and not to be afraid to express what I thought. And in college, Richard Adelstein, a very philosophical economist, who basically said, “Do not go to graduate school in economics, they won't let you do what you're interested in. You've got to go to philosophy graduate school.” So he was really my mentor in thinking that I could become a philosopher. Then there's my mom. She didn't go to college, but was extraordinarily intelligent and interested in all sorts of things. She was fascinated with Einstein and wanted to understand physics. When I was a little boy, she would wrap me up in a blanket on cold nights, and we would look at the stars. I was four years old, so I would ask, “What are the stars?” And she said, “They're suns. They're just very far away.” I also asked, “So all those stars in the sky, do they have planets like the Earth?” I still remember this to this day. She said, “Probably. We just haven't found them yet.” And this was 1967, so they hadn't found any yet. But when I gave her eulogy a few years ago, they had found 4,000 exoplanets, so she was right. What my mom was saying in 1967, that yes, there are other worlds out there, we just haven't found them yet, was so inspiring to me. She really was the one who made me become a philosopher. I try to channel the teaching she did in raising my own kids. The answer should never be “Because I said so.” It should be “What do you think? Let's have a conversation.” We never talked baby talk to our kids because my mom never talked baby talk to me. She treated me seriously as if my opinions mattered. My mom taking me seriously as a thinker from the age at which I could talk allowed me the confidence to go forward. It's to demoralize you into giving up on the idea of truth, to polarize us around factual issues, to get us to distrust people who don't believe the same lie. And even if somebody doesn't believe the lie, it can still make them cynical. I mean, we've all had friends who don't even watch the news anymore. There's a chilling quotation from Holocaust historian Hannah Arendt about how when you always lie to someone, the consequence is not necessarily that they believe the lie, but that they begin to lose their critical faculties, that they begin to give up on the idea of truth, and so they can't judge for themselves what's true and what's false anymore. Lee McIntyre is a philosopher and a scholar of science denial. He is a Research Fellow at the Center for Philosophy and History of Science at Boston University and a recent Lecturer in Ethics at Harvard Extension School. Lee is the author of many books for MIT Press, I will just mention three now.   In The Scientific Attitude (2019), Lee argues that we can best understand what is distinctive about science not by focusing on its method or achievements, but on those instances of failure to live up to one of its most basic values —”the scientific attitude”—which is caring about evidence and being willing to change our theories on the basis of evidence. In his upcoming book On Disinformation (2023) [I mistakenly refer to this as "Truth Killers" in my intro in the episode], Lee draws on over twenty years of science denial scholarship and takes readers through the history of strategic denialism to show how we arrived at this precarious political moment and identifies the creators, amplifiers, and believers of disinformation. In this episode we will focus a lot on his book How To Talk To A Science Denier (2021). Early voting, voting by mail, and drop boxes for ballots has likely insured a permanent Democrat President. No longer do they have to campaign or provide compelling messages (see Joe Biden, John Fetterman, Katie Hobbs) in order to win elections. Rush Limbaugh predicted this and we revisit his wise words. GOP leaders McCarthy, McConnell, and McDaniel have offered no compelling reason to continue in leadership roles. All will consider any election "reform" as "looking in the rearview mirror" and work to help Democrats stay in power forever. Obamacare mandates mean you can't buy affordable insurance. Most people would like to be able to buy insurance that just pays for hospitalization or major illness, but that is not legal thanks to Obamacare (unless you are under 30 or have a "hardship"). Moderna vaccines have higher risk of myocarditis than Pfizer according to recent study released by the American College of Cardiology. Myocarditis may the culprit in "Sudden Adult Death Syndrome" but remember you can't talk about that because then you are a Science Denier! Is it time for Trump to ride off into the sunset? Michael Hiltzik is a Pulitzer Prize-winning business columnist for the Los Angeles Times. He has recently attracted a lot of (almost entirely negative) attention for a column and tweets arguing that mocking the deaths of anti-vaxxers is "necessary." Hiltzik joins to defend and explain his position to Current Affairs editor-in-chief Nathan J. Robinson, who believes that Hiltzik's stance is cruel and unhelpful. Michael argues that his argument is more nuanced than it is being characterized as. Nathan suggests that as the one who intentionally tweeted an offensive provocation, Michael is responsible for that characterization. Michael argues that dark humor is an important weapon, citing the Current Affairs article "The Necessity of Political Vulgarity." Nathan argues that while incivility can be tactically useful, directing it against people who have died of COVID does nothing to create more productive discourse on vaccines. But what about people in positions of power who use that power to spread damaging misinformation? Are they fair game? Should we "never speak ill of the dead"? With whom must we empathize? Are some people beyond empathy? Are we to show respect for the lives of those who do not respect the lives of others? These questions are hotly debated. Discussion of Michael's controversial column begins around 6:00. While there have been some published studies that were interpreted as suggesting GMOs may be harmful to health if ingested, other scientists have pointed out the errors in the methods or interpretation, and the general scientific consensus is that consuming GMOs has not been shown to be harmful to health. Documented reports of missed menstrual cycles after COVID vaccine administration are common enough that it is currently under investigation. However, typically menstruation returns after one missed cycle and there has yet to be any evidence that Covid vaccines affect fertility. Missed menstruation is also common amongst women who have had COVID. A recent study suggests that the benefits of taking a daily dose of aspirin outweighs the risk of bleeding problems only for people with specific risk factors, such as high blood pressure. Our Responsibility to Defend the Truth Science denialism has existed as long as science has existed. As a part of our social contract, we're responsible for challenging the spread of misinformation and understanding, especially when it comes to science. If we open ourselves up to these difficult conversations, we can offer up a path into more logical reasoning and avoid a culture where science and truth are rejected. Science Denialism is Dangerous All science denialism relies on a flawed blueprint of cherry-picking evidence, trusting conspiracy theories, trusting fake experts, and relying on illogical reasoning. The internet has given denialism a chance to be amplified, which is especially dangerous because it confuses people and muddies the line between fact and falsehood. Science denialism hurts us in so many ways, from killing our planet by ignoring climate change to taking lives because people don't trust vaccines and masks. Technique Rebuttal Content rebuttal is using facts to combat false claims. Technique rebuttal is challenging the logic of the argument. It may seem logical to defend the truth with the facts, but you can make more progress by talking about the core of people's beliefs. If someone has already made the choice to deny the facts, presenting them with even more facts will not be effective. Instead, build trust by making them feel heard, then point out inconsistencies in their reasoning and use facts judiciously. Nations are an example of useful delusions; they are an entirely human-made construct, yet they can present us with a unifying, shared identity.  There is a tipping point, however, at which our beliefs can become harmful to someone else. Shankar defines conspiracy theories as a particular type of self deception which can easily escalate into harmful behavior. Our conversation with Shankar touches on the Capitol Riots in Washington DC on January 6th 2021, and how conspiracy theories influenced the behavior of the rioters. “Conspiracy theories, I think are a sort of special case of self deception, which...can easily take over into things that are deeply harmful.” The challenge most of us face though, is how to talk to our friends and family who believe in conspiracy theories. We're not to blame for climate change. It's a part of the natural cycle. The earth is flat. The round Earth conspiracy is orchestrated by NASA and other government agencies. No one should get the coronavirus vaccine. Bill Gates wants to use it to implant microchips in people. This is, of course, all bunk. But how can we change the minds of people who believe it to be true? Lee McIntyre offered thoughts and suggestions to try and reach, communicate, and change the minds of science deniers in How to Talk to a Science Denier. And, he warned, it's best not to just dismiss them and ignore them. Why? Science denial can kill. From attending a flat earth conference to chats with anti-vaxxers, McIntire presented the five factors involved in science denial: cherry-picking evidence, belief in conspiracy theories, reliance on fake experts, logical errors, and setting impossible expectations on what science can achieve. Can we achieve understanding with the irrational? Not through belittling them or through anger, McIntyre said, but through calm, respectful rationality.
