This song came out in September 1976 as part of an album called "A Rumor in His Own Time." It's all about a sweet couple who have an amazing night together, and the singer wishes it would just go on forever—no goodbyes, just happiness. While Jeffrey's original version didn't get much attention, a big change happened when Clive Davis, the president of Arista Records, stumbled upon it just a few months later. He saw potential in it, and it was soon covered by a duo called Deardorff & Joseph in their first album with Arista.After Deardorff & Joseph went their separate ways, interestingly, the people managing them found new opportunities—you see, Marcia Day started managing Deardorff, while Susan Joseph took charge of Joseph.Fast forward to February 1978, and we have more covers of this enchanting tune! Maureen McGovern and the duo England Dan & John Ford Coley each released their versions. McGovern's cover didn't quite catch on, but England Dan & John Ford Coley hit the jackpot! We are dusting off our crystal balls today with three amazing guests who have all recently published an article on prognosis over the last couple months: Kara Bischoff, James Deardorff, and Elizabeth Lilley. To start us off we talk with Kara Bischoff about the article she just published in JAMA Network on a re-validation of the Palliative Performance Scale (PPS) in a modern day palliative care setting.  Why do this?  The PPS  is one of the most widely used prognostic tools for seriously ill patients, but the prognostic estimates given by the PPS are based on data that is well over a decade old. Next, we talk with James Deardorff about whether we can accurately predict nursing home level of care in community-dwelling older adults with dementia.  Spoiler alert, he published a study in JAMA IM on a prognostic index that does exactly that (which is also on eprognosis.org). Lastly, we invite Liz Lilley to talk about her paper in Annals of Surgery about prognostic allignment, including why as palliative care and geriatrics teams we need to take time to ensure that all disciplines and specialities are prognostically aligned before a family meeting.

In response, Spanish nativist factions zealously attacked them as unequal and unqualified, unleashing an intense political battle that lasted almost two decades. At stake was whether membership in the small colonial community and thus access to its most lucrative professions should depend on limpieza de sangre (blood purity) or values-based integration (Christian citizenship).  Max Deardoff's A Tale of Two Granadas: Custom, Community, and Citizenship in the Spanish Empire, 1568–1668 (Cambridge University Press, 2023) examines the vast, trans-Atlantic transformation of political ideas about subjecthood that ultimately allowed some colonial mestizos and indios ladinos (acculturated natives) to establish urban citizenship alongside Spaniards in colonial Santafé de Bogotá and Tunja. In a spirit of comparison, it illustrates how some of the descendants of Spain's last Muslims appealed to the same new conceptions of citizenship to avoid disenfranchisement in the face of growing prejudice. Daniel Deardorff (1952-2019) was a mythsinger, storyteller, ritualist, composer, andproducer. A survivorof paralytic polio and resultingparaplegia, and early teacher about Otherness, he toured extensively with renowned soft-rock band Seals and Crofts for ten years and produced award-winning albums for other artists. When post-polio sequelae required change, he became an independent scholar of myth teaching internationally with Robert Bly, Martin Shaw and others. The MythsingerLegacy Project http://mythsingerlegacy.org continues his work restoring myth to culture and community. Judith-Kate Friedman is a multi-media artist and serves as Steward of the Mythsinger Legacy Project. She and Daniel Deardorff were partners in life, love and creative collaboration for 14 years. Dr. Faith Fitzgerald once quipped that prognostic modeling is the “punctilious quantification of the amorphous.”  She has a point.  Prognosis is inherently uncertain.  As Alex Lee says on our podcast today, all prognostic models will be wrong (in some circumstances and for some patients); our job is to make prognostic models that are clinically useful.  As Sei Lee notes, the argument for developing prognostic models has won the day, and we increasingly use prognostic scores in clinical decision making.  What makes prognostic models for mortality different from models used for anticoagulation or risk of renal injury?  James Deardorff replies that there is something inherently different about predicting mortality.  Death is different.  For some reason clinicians who might be perfectly comfortable using an anticoagulation risk calculator might be skeptical of a mortality risk calculator (see this recent terrific JAMA IM study from Nancy Shoenborn on this issue).  And yet, the only thing that may be worse than a prognostic calculator is a clinician relying solely on their clinical intuition. Today our guests Alex Lee, James Deardorff, and Sei Lee, talk to us about the uses, limitations, and clinical use cases for prognostic models.  As a springboard for this conversation we discuss new prognostic models developed to predict (simultaneously) mortality, disability, and mobility impairment (Alex Lee first author, JAGS) and mortality for people with dementia residing in the community (James Deardorff first author, JAMA IM).   Both new models are now available and free to use on ePrognosis.   And Sei and Eric reminisce about slow dancing to “Forever Young” by Alphaville in their teenage years. My mom is an Asian woman in her 70s with osteoporosis.  She tried an oral bisphosphonate and had horrible esophagitis.  She said never again, though she eventually tried an IV bisphosphonate.  She had terrible flu-like symptoms.  She said never again.  But based on reports that symptoms are worse the first time, she tried the IV again the next year and fortunately experienced no symptoms.  (Story used with permission, thanks mom!).   I tell this story because these issues don't typically register as more than a nuisance for clinicians, who frequently don't understand why their older patient with osteoporosis is not taking a bisphosphonate.  But our patients are walking with their feet, and adherence to bisphosphonates for osteoporosis is poor and decreases with time. When we have a medication with up front harms and downstream benefits, it's critical that we consider the time to benefit, or how long it will take an individual to benefit from a test or treatment.  Think of the 10 years it takes to benefit from colon or breast cancer screening, which is designed to detect slow growing cancers.  For individuals with a life expectancy less than the time to benefit, the up front harms outweigh the downstream benefits.   In this context, we talked this week with James Deardorff and Sei Lee about their study of time to benefit of bisphosphonates for osteoporosis, published in JAMA IM. What they found somewhat surprised us: it's pretty short, about 1 year!   Which makes me feel better about urging my mom to get treatment.  Bisphosphonates are pretty darn effective, and act quickly. We also discuss discontinuing bispohsophantes, and if we can use the same logic we consider when starting them.   Toward the end we talk about the launch of ePrognosis' new Time to Benefit tool.  This tool provides a clinical recommendation for starting/stopping medications and cancer screening based on the prognosis of the patient in front of you.  See screenshot below.  You can either access it directly here.  I you use the Lee Schonberg prognostic index for community dwelling older adults you will be directed to the tool, and it will automatically place the life expectancy at the calculated prognostic estimate.   James and Sei requested a terrific song - Bad to the Bone - much better than the Hannah Montana Bone Dance song they threatened me with (maybe next time). But what kept a set of kingdoms that included Castile, Aragón, vast swaths of North and South America, Portugal, the Low Countries, Italian territories, and the Philippines from falling apart? Prof. Max Deardorff explores the legal underpinnings of this complicated system, including the early modern conception of the “republic,” the relationship between early modern vassals and the Crown, and the question of whether native subjects could ever hope to achieve enfranchisement in colonial cities founded by Spaniards. Deardorff highlights the importance of the Council of Trent, which conditioned a generation of Spanish Catholic reform and played a crucial role in defining early modern citizenship, and points out how royal strategies for integrating Moriscos (Andalusi converts from Islam and their descendants) into Christian society in recently-conquered Granada provided a blueprint for assimilating native subjects in the Americas.

Abstract: Sleep disruptions are among the most commonly-reported symptoms across neurodevelopmental disorders (NDDs), but mechanisms linking brain development to normal sleep are largely unknown. From a Drosophila screen of human NDD-associated risk genes, we identified the chromatin remodeler Imitation SWItch/SNF (ISWI) to be required for adult fly sleep. Loss of ISWI also results in disrupted circadian rhythms, memory, and social behavior, but ISWI acts in different cells and during distinct developmental times to affect each of these adult behaviors. Specifically, ISWI expression in type I neuroblasts is required for both adult sleep and formation of a learning-associated brain region. Expression in flies of the human ISWI homologs SMARCA1 and SMARCA5 differentially rescue adult phenotypes. We propose that sleep deficits are a primary phenotype of early developmental origin in NDDs, and point towards chromatin remodeling machinery as critical for sleep circuit formation. In this episode I sat down with one of my brothers closest friends, Zach Deardorff. Zach is a survivor of child sexual abuse at the hands of his own brother.  1 in 6 boys are sexually abused or assaulted, and unfortunately Zach falls into that statistic. He talks about how he was trapped in a life of abuse with no one to count on and protect him including the relationship with his toxic father. Zach had to basically raise himself and figure out life on his own at a young age which has not been an easy road. Zach never reported his brother, and only in recent years he told his dad about the abuse. Zach talks about how he tried to numb himself by using substances, having toxic relationships and always being in chaos. Zach has been through hell and back, but he has worked hard to be where he is now. Even though he still struggles at times, Zach now has a beautiful wife, a house, has a relationship with God and has cut ties with his toxic relationship with his family for his own mental health. It was important to have Zach on the podcast to help break the stigma on male sexual abuse/assault. It can happen to anyone and it is nothing to be ashamed of. Abuse does not discriminate. Im so proud of where Zach is today and I'm proud of him for turning his life around and breaking the cycle. The acclaimed Ghanaian photographer Nii Obodai is renowned for his distinctive black and white hazy silhouettes of distant landscapes and his stark compassionate portraits. As a youngster he spent time in England and now he’s back as artist-in-residence in the East Riding of Yorkshire, where he’s working on a new exhibition on environmental themes. Nii is spending time in the marshes and woodland of the nearby nature reserve, photographing wildlife and the people who tend the landscape. But he’s not using the latest digital camera to take his pictures – instead he’s interweaving the techniques of the past by learning to photograph with an old-fashioned large format Deardorff camera, shooting in black and white and making prints. The work is based on techniques used by one of the founding fathers of British photography, William Henry Fox Talbot. Felicity Finch joins Nii as he explores the marshlands and prepares to take his pictures which will be displayed in the Beverley Art Gallery. She follows him through the process – which can take hours – of waiting for the birds to come into shot, pressing the shutter and reloading the films. She also watches as he takes portraits of the workers at the reservoir. Later in the seaside town of Scarborough we hear how Nii develops the films in the total darkness of an ad hoc darkroom and uses the traditional method of salt printing to create images in a variety of monochrome tones of rich earthy browns. It’s a learning process for Nii and while most of the images turn out the way he wants, others are less successful. We hear how he begins to make his final selections and how he is influenced by the potency of memory and landscape.

These competencies seem particularly relevant in our current political climate. My guest is Darla Deardorff. She has spent the past decade thinking about intercultural competencies. What are? Can scholars agree on a common framework? And is it possible to measure them? For Darla, intercultural competencies are as vital as math and science for education. Darla Deardorff is the Executive Director of the Association of International Education Administrators at Duke University. Her latest co-edited book, Intercultural Competence in Higher Education: International Approaches, assessment, application, was published by Routledge in June.
