    What does it take to keep wounded Soldiers alive when the next war means hours — not minutes — to the operating table? In this special collaboration between WarDocs and OP MED TV, recorded at the Defense Strategies Institute OP MED Symposium, COL Shaun Brown, MD, Commander of the U.S. Army Institute of Surgical Research, lays out the research agenda that will decide whether the wounded of the next conflict survive. COL Brown's path into Army medicine began with a rejection. Poor eyesight closed the door on the U.S. Naval Academy, so he pursued pre-med as a civilian undergraduate. When 9/11 happened, it solidified both his commitment to medicine and his decision to serve, and he took an Army HPSP scholarship in medical school. He trained in general surgery at William Beaumont Army Medical Center — a program with a long, quiet relationship with the special operations community — where attendings would vanish overnight for operational requirements. He chose colorectal surgery as a fellowship to add technical range he could use in civilian practice and on the battlefield, then joined Joint Special Operations Command at Fort Bragg after a year on staff in El Paso. Now commanding the Army's premier combat casualty care research enterprise, COL Brown is candid about what excites him and what worries him. He is most energized by the Organ Support and Automated Technology department, and he uses a widely shared video of a Ukrainian amputee evacuated by unmanned ground system to make his point: autonomous evacuation without autonomous medical support only moves the walking wounded. Ventilators that read changing physiology and adjust themselves, autonomous IV pumps, and en-route support are the missing half of the capability. His concern is combat wounds. In large-scale combat operations with prolonged evacuation timelines, Dr. Brown expects most damage control surgery to be done for sepsis rather than hemorrhage — the patients who cannot be evacuated become septic extremities. He also walks through the blood problem: whole blood is the standard, low-titer O supply will not be sufficient at scale, and the answer is shelf-stable components, including freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells. The conversation closes on people. Brown details how the Army sustains surgical readiness through a diversified platform of military treatment facilities, civilian partnerships, and untapped Veterans Affairs relationships; how he coordinates with the Reserve consultant to pair complementary skill sets on deploying teams; and why, quoting the Army War College, he still serves: you train for the known and you educate for the unknown. He recounts getting the recruiting call while loading a moving van in New Orleans, then completing assessment and selection before moving to Fort Bragg. (04:33-08:06) Commanding the ISR: Autonomous En-Route Care and the Combat Wound Gap COL Brown identifies the Organ Support and Automated Technology department as the work he is most excited about, using a Ukrainian unmanned-ground-system evacuation video to argue that autonomous platforms without autonomous medical support can only move the walking wounded. He then names his chief concern: combat wound research funding, and his expectation that in large-scale combat operations most damage control surgery will be done for sepsis rather than hemorrhage. (08:06-11:59) Blood, Plasma, and the Shelf-Stable Future of Resuscitation The discussion turns to the evolution from component therapy to 1:1:1 ratios to whole blood, and Dr. Brown's assessment that low-titer O will not be available in sufficient quantity for large-scale combat operations. He details work on freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells, noting that spray-drying is faster, cheaper, and uses equipment roughly the size of two ATMs — a major advantage for distributed manufacturing. (11:59-18:16) Burn Care, Surgical Readiness, and Partnerships Across Components COL Brown addresses the burn casualty problem in a future fight: forward Class VIII resuscitation supply, scaling the Advanced Burn Life Support course for deploying units, and partnerships with civilian burn centers, including placing Army burn surgeons in MILCIV sites. He then lays out the diversified surgical platform — military treatment facilities, community hospitals, and underused VA partnerships — and how he works with the Reserve consultant to pair complementary skill sets on deploying units. (18:16-22:22) Forward Surgical Capability Gaps and Why He Still Serves Asked what a forward surgical team still needs, COL Brown points to an off-the-shelf, infection-resistant biologic vascular conduit as a potential game changer over shunts — with the training investment that would require. He closes with his why, quoting the Army War College maxim that you train for the known and educate for the unknown, and asking who will be left to educate the next generation if experienced leaders walk away during the interwar period. Take Home Messages Autonomous evacuation without autonomous care only moves the walking wounded: Unmanned ground and air systems can pull a casualty off the battlefield, but a platform alone does not sustain a patient who needs a ventilator, a pump, or a transfusion en route. The medical community must be in the ground-maneuver conversation early, because a small design change can turn a logistics platform into a casualty evacuation platform. Autonomous ventilators that read changing physiology and adjust themselves are the missing half of that capability. In the next war, sepsis may drive damage control surgery more than hemorrhage: Prolonged evacuation timelines change the casualty population that reaches a surgeon. Patients in uncontrolled hemorrhage far from a surgical team frequently do not survive the wait, while patients with survivable wounds that cannot be evacuated arrive septic days later. Combat wound research and combat wound solutions deserve renewed funding priority for large-scale combat operations. Shelf-stable blood components are the answer to a cold chain that will not hold: Warm whole blood remains the standard of care, but low-titer O will not be available in the quantities a large-scale conflict demands, and cold chain storage is a logistical vulnerability. Freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells are all in the research pipeline. Spray-drying offers a particular advantage: it is faster, cheaper, and the equipment footprint is small enough to support distributed manufacturing forward. Burn readiness is a supply problem, a training problem, and a partnership problem: Thermal weapons, lasers, fuel, and explosions could produce burn casualty volumes the system has not seen in decades. Resuscitation depends on adequate crystalloid and plasma forward, on teams trained to calculate burn surface area correctly, and on scaling the Advanced Burn Life Support course to deploying units. Long-term capacity also depends on formal relationships with civilian burn centers and on placing military burn surgeons inside those partnerships. Surgical readiness comes from a diversified platform, not from one hospital: Military treatment facility volume alone will not sustain a surgeon's skills, so readiness now depends on layering community hospital partnerships and Veterans Affairs relationships on top of the military caseload. Functional VA hospitals near large installations without strong academic affiliations are ripe for preferred referral partnerships. What surgeons need most is not trauma volume but complexity, which older patients with more complex medical conditions reliably provide. A Malawian immigrant who had been processed at the Musina repatriation centre and enroute to the Beitbridge border post in Musina, Limpopo, has died. According to information, the immigrant collapsed and died in the bus after complaining of ill health. Text: 2 Samuel chapters 6-9 Supplemental Text: The Fourth Thousand Years chapter 5 BYU Lecture 29: After battling Abner and the northern 10 tribes, King David unites all of Israel and moves the capital city from Hebron to Jerusalem. Peace in Israel, however, is short lived. The Philistines (Canaanites) rally their forces against Israel to attack King David – their Goliath killer. David inquires of the Lord what to do and is promised that the Philistines will be delivered into his hands. Sure enough, David's army soundly conquers the invading armies. Meanwhile, King Hiram of Lebanon (Phoenicians) sought a friendly alliance with Israel. He offers to build a fine palace for David, made from the famous cedars of Lebanon. He even provided the masons and carpenters to complete this beautiful, massive project. With his palace completed, David desires to build a beautiful house of the Lord. In anticipation of this, he sends for the Ark of the Covenant to be delivered to Jerusalem. Unfortunately, the holy process of moving the Ark was neglected and David was careless. Rather than instructing the Priests of Aaron to carry the Ark on their shoulders, as commanded by God several hundred years earlier, it was placed on a cart pulled by oxen. Enroute, the cart faltered and the Ark itself appeared to be in jeopardy of falling. Uzzah, sensing a tragedy, reached up to steady the Ark. This was in violation with God's command that no one but the Priests of Aaron may touch the sacred Ark. Immediately, Uzzah was struck dead. Realizing his carelessness, David called and authorized Priests of Aaron to carry the Ark on ther shoulders the remainder of the way to Jerusalem. An important lesson was learned that day; God will not be mocked. In humility, David wanted to build a holy temple to house the Ark, but through the prophet Nathan, the Lord forbad him. He did, however, give David the pattern for the temple when it would be built by his son Solomon. The Lord also promised David that he would be the ancestor of the great Messiah. David expands the might and strength of Israel until it borders the very lands as promised specifically by God to Abraham, Isaac, Jacob and Moses.

Pathophysiology/etiology Carotid artery disease is primarily driven by atherosclerotic plaque deposition.  Risk factors: hypertension, hyperlipidemia, diabetes, smoking, and advanced age. Nonatherosclerotic etiologies: fibromuscular dysplasia, carotid dissection, vasculitic disease, carotid webs, and trauma. When the endothelium is damaged, monocytes migrate to the site and differentiate into macrophages that take up oxidized LDL particles to become foam cells. Meanwhile, an inflammatory response occurs where activated platelets release thromboxane A2, platelet derived growth factor, and inflammatory cytokines that promote further platelet aggregation and vascular inflammation. Smooth muscle cells migrate and proliferate, forming the structural framework of the atheroma.  Within the lesion, necrotic debris and lipid accumulate, creating a vulnerable plaque. Plaque rupture exposes this material to the bloodstream, serving as a nidus for thrombus formation which can lead to ischemic events. Carotid bifurcation is particularly prone to plaque formation due to turbulent blood flow. Embolization of plaque from this area can result in TIA or ischemic stroke.  2. Presentation Patients are often asymptomatic and stenosis is incidentally found on imaging.  Symptomatic patients present with neurologic symptoms including unilateral motor and sensory loss, aphasia (difficulty finding words), dysarthria (difficulty speaking), amaurosis fugax (temporary monocular vision loss due to embolus to the ophthalmic artery), transient ischemic attacks Physical exam findings may be notable for auscultation of a carotid bruit. Patients may also have evidence of retinal artery embolization on fundoscopic examination (Hollenhorst plaque) or asymptomatic cerebral infarction.  3. Diagnosis USPTF recommends against screening for asymptomatic carotid artery stenosis.  In patients with no risk factors, SVS recommends against screening for asymptomatic carotid artery stenosis. However, they do recommend screening for asymptomatic clinically significant carotid bifurcation in certain groups of patients with multiple risk factors.  These risk factors include patients with clinically significant peripheral vascular disease, patients 65 and older with history of CAD, smoking, hypercholesterolemia, and patients prior to coronary artery bypass.  Relevant findings on physical exam or imaging findings may warrant screening, but screening is not recommended for the presence of neck bruit alone without other risk factors, as this finding has a low sensitivity and specificity for detecting clinically significant carotid artery stenosis.  Carotid duplex ultrasound: first-line imaging modality for both screening and initial evaluation of stenosis, noninvasive, low-cost CTA: rapid, high-resolution, three-dimensional imaging of vascular anatomy, risk of contrast and radiation exposure MRA: high-quality, three-dimensional imaging without radiation or contrast, expensive with longer acquisition time, can overestimate stenosis in severe disease DSA/angiography: gold standard, expensive, invasive, not generally recommended for routine diagnostic evaluation or screening 4. Classification Carotid artery stenosis is classified by degree of luminal narrowing. NASCET method: standard in current practice. Compares the minimal residual lumen at the point of greatest stenosis to the diameter of the normal distal internal carotid artery.  Classification of stenosis: Mild: 70 bpm, and ACT >250 seconds to optimize cerebral perfusion and minimize thrombotic risk. Clamp the carotid artery just proximal to the arterial sheath to establish active flow reversal.  Flow controller settings: Low setting High setting Flow-stop button: allows for temporary cessation of flow (used when we inject contrast).  Confirm flow reversal via two different ways:  The first way is to stop flow to the venous return sheath with the stopcock, clearing the line with hep saline injection, and then opening the stopcock and seeing the blood returning to the controller in a reverse fashion. The second way is to perform an angiogram with a small amount of contrast injection while holding the flow-stop button. Using the angio we want to make sure that contrast is flowing retrograde in the cervical ICA thereby confirming flow reversal.    Carotid artery stenting, balloon angioplasty, and completion angiogram At this point, a standard carotid angioplasty and stenting procedure is performed. ENROUTE transcarotid Neuroprotection System device:  inner diameter of 8F and an outer diameter of 10F Has its own carotid artery stent system but is also compatible with all FDA-approved carotid stents.  Final angiogram is performed to confirm stent position, vessel patency, and absence of complications including vasospasm at the distal end of the stent and filling defects from protrusion of atheromatous material through the stent    Cessation of flow reversal and sheath removal Allow the flow reversal to run for a few minutes after the final balloon angioplasty to clear any debris.  Antegrade flow is restored by releasing the carotid clamp and closing the stopcocks on the neuroprotection system.  The patient is auto-transfused the blood from the flow line back to the venous system.  As the arterial access system is removed and the puncture site is closed with the U-stitch.  IV protamine is administered to reverse the heparin. Standard closure is performed at the incision site. Meanwhile, hemostasis is achieved after removal of the femoral vein sheath with brief manual compression.  Postop care/complications Postop care All patients after a TCAR should be monitored in the ICU setting for 24 hours, as an embolic stroke, hypotension with or without bradycardia, or hypertension can occur.  Should a TIA or stroke be observed, a carotid duplex scan and CT angiogram should be immediately obtained to assess the stent site and the presence of an embolic or thrombotic filling defect, dissection, or occlusion.  Dual antiplatelet therapy: continue for 45 days to 12 months Aspirin and statin therapy: continued indefinitely Surveillance duplex imaging: 4 weeks, 6 months, and 12 months, and annually thereafter. Postop complications Hematoma Stroke Myocardial infarction Cerebral hyperperfusion syndrome Sudden and excessive increase in cerebral blood flow to previously hypoperfused brain tissue is met with vasculature that cannot constrict appropriately from chronic vasodilation Leads to breakthrough hyperperfusion. This results in cerebral edema, intracerebral hemorrhage, and neurological symptoms.  Cranial nerve injury Hypoglossal nerve (CN XII) injury: ipsilateral tongue deviation. It is the most commonly injured cranial nerve.  Vagus nerve (CN X) injury: hoarseness and possible vocal cord paralysis.  Glossopharyngeal nerve (CN IX) injury: soft palate dysfunction.  Recurrent laryngeal nerve injury: voice hoarseness and inability to cough as it innervates all of the voice box muscles except for the cricothyroid muscle Marginal mandibular nerve injury: ipsilateral lip droop, injury is rare in TCAR.  Stent restenosis Pseudoaneurysm Access site infection

In addition to testing her limits on the trails, Tania is a coach and she has a podcast called the Ultra Happy Podcast, which she cohosts with her husband, Andy, and it follows her running journey.  Tania is also a world citizen: while she is from Mexico and now lives in Dubai, she has also lived in Canada and the US, in addition to living for several years in Scotland, which is where her husband is from. You'll hear about Tania's time in Scotland in this episode because it does figure strongly into her running story. As Tania says, she was born in Mexico, but she was built in Scotland, especially when it comes to trail running.  Tania came to running as an adult and initially hated it. But when she found the trails, she was hooked and now challenging ultra adventures are a driving force in her life. And, as if it weren't enough that Tania ran the 4 Deserts Grand Slam Plus in a 12-month period—which took place in Namibia, Mongolia, Jordan, Antarctica, and Chile—there is more. She did that with an even bigger goal in mind: the Cocodona 250, which she went after just weeks after the final stage race.  Tania's story is all about developing into the ultra-endurance athlete she is today, and especially how that's played out over the past few years. Tania provides excellent insight into what it takes to develop the mental, physical, and emotional skills to be able to take on these huge adventures.  Her strength, fearlessness, and patience, as well as her ability to focus on solutions, however uncomfortable they might be, instead of lamenting the tough situations she finds herself in—and her ability to always be so nice to herself, are all exemplary.
