We've talked a lot about comprehensive dementia care on the GeriPal podcast but while the evidence is clear that these programs work, the uptake has been limited largely because there hasn't been a strong financial case for it. Don't get me wrong, the evidence points to cost savings, but as Chris Callahan and Kathleen Unroe pointed out in a JAGS editorial in 2020 “in comprehensive dementia care models, savings may accrue to Medicare, but the expenses accrue to a fluid and unstable network of local service providers, patients, and their families.” The good news is that the financial case for comprehensive dementia care is changing thanks to a new Center for Medicare and Medicaid Innovation (CMMI) alternative payment model (APM) called Guiding an Improved Dementia Experience (GUIDE) Model.  This model will give participating programs a per-member-per-month payment to offer care management, care coordination, and other services such as caregiver training, disease education, and respite. On today's podcast we talk with Malaz Boustani of Indiana University, as well as Diane Ty, the senior director of the Milken Institute Center for the Future of Aging, about the GUIDE model.  We'll cover why the GUIDE model is important (hint - it all comes down to funding), how it was developed, what components will it include, and how it will be evaluated. Vjeko and I discuss the riveting, recently concluded FIDE Grand Swiss. India's ascension continued as GM Vidit Gujirathi and IM R. Vaishali's had impressive victories in the Open and Women's Sections respectively. With more spots in the 2024 FIDE Candidates now settled, we also look ahead to what is shaping up to be quite an entertaining fight for the right to challenge for the World Championship titles. Lastly, Vjeko followed this event quite closely, so he recommends several interviews and games from an action-packed tournament for us to check out.  Following Vjeko, I catch up with Chris Callahan (38:00) about what is new with LiChess.org since our last chat in 2021. Andy Chamberlin is also with the U of VT, farmers with their post-harvest processes to increase efficiencies and profitability while maintaining safe food handling processes. Though it's common to associate low tunnels with winter production, we talk about the strategies that make low tunnels effective in the hotter part of the season, most notably with shade cloth, which has the advantage that it can be watered through, unlike floating row cover and many of the other commonly used coverings. Football is a waiting game. Fans all over America watch their teams anxiously for months with one common desire, a chance to win. Today's guest, Patriots fan, Chris Callahan, knows this waiting game quite well and it may have even prepared him for the most worthwhile wait of his life – the journey to his son, Brady. In this episode, Chris dives into their dual diagnosis of endometriosis and male-factor infertility, and the obstacles he and his wife faced on their path to parenthood. While they did have fertility coverage through Chris' work, two rounds of IVF were still a financial burden. Chris, impacted mentally, physically, and financially, decided to share his story on social media in hopes to encourage others to know they're not alone in this journey. In his post, he highlights the importance of employers choosing to invest in the lives of their employees through family building benefits. The responses took him a bit by surprise, and he soon found a whole community that was standing with him in solidarity and sharing stories of their own.  Elizabeth Keen and her husband Al have been farming at Indian Line Farm in Great Barrington, MA for over 25 years. After taking a sabbatical in 2016, they had the realization that after 10-15 years “just running the farm” it was time that some things needed to change to make the farm more sustainable in their next phase of life. The upgrades to the farm included the purchase of a delivery van, a 30'x96' hoop house, and replacing the 6-wire high tensile electric fence with an 8' fence to keep the deer out. Additionally, Elizabeth decided it was time to make some improvements to the post-harvest area on the farm.  This podcast gives a tour of the barn upgrades and the conversation is mostly led by my colleague Chris Callahan of the University of Vermont Extension. Over the weekend, the United Nations Climate Change Conference, or COP 27, went into overtime as nations came to an historic agreement to establish a loss and damage fund. This fund is meant to give resources to countries who have experienced the worst effects of climate change. Some like to think of it as climate reparations. There are a lot of factors that might have created the momentum for this historic agreement to go through after many years. An interesting one is that it's becoming more and more difficult for big emitters like the United States to deny their role in contributing to climate change, particularly as new scientific studies have been pivotal in creating a pretty unimpeachable basis for climate responsibility. But, just because science can verify certain realities does not mean that it's a straight path forward for climate justice. To get a sense of what factors are coming together to achieve climate justice, Lawfare associate editor Hyemin Han merges the legal and policy perspective with the science perspective in a conversation with Karen Sokol, a professor at the Loyola University New Orleans College of Law and a fellow at the Princeton School of Public and International Affairs, and Chris Callahan, a PhD candidate at Dartmouth College who co-produced a scientific study that informed negotiations on loss and damage at COP 27.Support this show http://supporter.acast.com/lawfare. The importance of cybersecurity continues to increase, both as more functions become connected to the internet and as adversaries exploit cyberattacks as a weapon in future conflicts. From the SolarWinds Orion hack to the breach of a Florida water treatment plant, recent attacks have underscored the critical function cybersecurity plays to national security interests. ERDC is playing a key role in the efforts to strengthen the nation's cybersecurity. It protects the Defense Research and Engineering Network, safeguarding the intellectual property of defense labs across the United States and the world. ERDC also performs independent assessments of cybersecurity controls, conducts research to discover innovative cyber-protection strategies, and performs adversarial emulation to proactively identify and mediate vulnerabilities and security weaknesses for the Army, DOD and federal agencies. Chris Callahan (of Indiana University) and Lee Jennings (University of Oklahoma) have some righteous anger. Why do we have comprehensive cancer care centers and not comprehensive dementia care centers? We have a body of evidence dating back 30 years to support people with dementia and their caregivers with Comprehensive Dementia Care. Lee Jennings added to this robust body of work with a study published in the Journal of the American Geriatrics Society demonstrating that a comprehensive dementia care program based out of UCLA reduced ED visits, hospital length of stay, increased hospice enrollment, and delayed time to admission to long term care. As Chris Callahan notes in his accompanying editorial in JAGS, a fundamental problem with our healthcare system is that savings from Comprehensive Dementia Care accrues not to the dementia care program, but to the hospital and Medicare (ED visits, hospital length of stay) or Medicaid (long term care admission). Our fragmented healthcare system lacks the coordinated big picture financial incentives to make this happen for our patients. One more major point - Chris Callahan emphasizes in the podcast that we as clinicians need to stop saying, "there's nothing that we can do for dementia; nothing works." That's simply not true. While we lack Comprehensive Dementia Care programs in nearly every region of the US, we have tremendous community resources for people with dementia and their caregivers. Specific engagement is with food producers, processors, and distributors to improve efficiency, quality, safety and cost control through integration of technology, systems integration and process controls Hans Estrin is a Produce Safety Specialist for UVM Extension. He co-develop UVM’s Practical Produce Safety Program in 2010, and since 2014, has developed and coordinate the Community Accreditation for Produce Safety (CAPS) with the Vermont Vegetable and Berry Growers Association. As an On Farm Readiness Review (OFRR) lead for Extension, Hans provides on-farm technical support to help Vermont growers comply with FSMA and meet market expectations. Hans is an AFDO certified PSR trainer in produce safety, and regularly co-leads Produce Safety Alliance (PSA) Grower training courses. Hans also coordinates UVM Extension’s Post Harvest Team, focused on a team approach to integrating efficiency, profitability and food safety on Vermont Produce Farms. Hans further supports the development of local produce markets and businesses; He co-founded the Windham Farm and Food Network (now Food Connects), helped launch VT Dinners and Manadnock Menus, and has supported the start-up other Vermont Food Hubs. In his former career, Hans taught high school science for decades, around the globe, and worked on several produce farms.

Well, we're back. It didn't quite take us a month to get back to podcasting after the Cotton Bowl, thank goodness. Truthfully, schedule conflicts ended up being the issue more than the "hangover" from the Cotton Bowl. But we're back now, and we're ready to bring some joy back to your ear holes. Cotton Bowl Aftermath Before you turn off the podcast, were NOT doing a full recap of the game. We're not going to be Debbie Downers. We're actually talking about some of our favorite moments. You shouldn't be afraid to listen to this part. If the loss still stings, take a listen. We're surprisingly upbeat about what happened in the game. Kendal Briles is going to be a great OC and fantastic play caller. K.D. Cannon had an otherworldly game against a fantastic defensive back. Jay Lee throwing a fantastic WR pass for a touchdown. Bryce Petty having an absolute gem of a game and setting a Cotton Bowl Record. There was plenty of really fun stuff that happened in that game, despite the outcome, and it's worth celebrating. This is out of order, but it's so good that it has to go last in this paragraph, and two words will sum it up. LaQuan McGowan. That is all. Plus, the aftermath hasn't produced the narratives about how far off we are that we expected after that loss. That's very good news, and should be cause for optimism. There hasn't been a Preseason poll released where Baylor's not at least Top 7 at this point. While most consider the "way-too-early" preseason polls as meaningless (and for what to expect from a team, they generally are), they do tell you how your program is viewed, and Baylor is rising in the estimation of most folks. That's very exciting. Cris Dishman Hire & Coaching Changes We're also talking about the hire of Cris Dishman as the Safeties Coach who will replace Brian Norwood. We're all very excited about the hire... He's got NFL chops, which is something new for our defensive staff,  and we're all looking forward to seeing what he can do. If you haven't been paying attention the past few days, it appears that they're already putting him out on the recruiting trail and is making an immediate impact in that arena. It's very exciting to see. We also talk a bit about Kendal Briles' move to OC and play caller, and the fact that we were so wrong about trick plays in the Cotton Bowl. We're happy to be wrong there, because that was fun. Aside: Thank God that Chris Callahan is still alive. Recruiting Updates We're in the home stretch of the 2015 Recruiting Class Season with National Signing Day, so we're looking into what's left to pursue, plus we're looking at some of recruits that have committed to Baylor in the past couple of weeks. Buckle up, folks, it's going to be a very wild ride. Specifically, we're spending some time on Eric Ogor, his ability, and why it appears that he's been lightly recruited. If he adapts to the collegiate level, the Bears could have a real gem there. And he's got one of the best names in recent memory. Yeah, we go back into the 3-star vs. 4/5-star debate. There's more talk here, and Pras asks the most speculative question you've ever heard on the podcast. Zomg Basketball We sorta trip over ourselves into some basketball talk. I will say this. We. Love. Scott Drew. There's more basketball coming on the podcast! Very exciting!
