In this podcast, Tiffany (Ayurvedic practitioner) and Vaidya. Jay Parla discusses an integral part of our daily routine, sleep. Just as food nourishes the body, sleep tends to nourish our mind; it rejuvenates and allows the entire body's systems to reset after a tiring day. From an Ayurvedic point of view, a "deep sleep" is a healthy disconnect of the mind from the body, avoiding any sensation in that period. Lack of it can cause mind-body misalignment and affect our performance and everyday function.A good night's sleep makes your body and mind refreshed and ready for the next day and brings an added enthusiasm in daily activities, making your day a lot more productive. On the other hand, a lack of it can cause mental stress, lack of concentration, irritability, and tiredness. Therefore, 7-8 hours of sleep is mandatory for a grown person to sustain a healthy lifestyle.In case of being unable to get this required amount of sleep, one can do meditation, an hour of which is equivalent to 2 hours of sleep. That brings us to different phases of sleep. According to Ayurveda, there are broadly 3 phases, namely Jagrita, Swapna, and Sushupti.Jagrita, or the awakened state, is the initial phase during which the person is still aware of their surroundings. They can hear what is happening in the background and feel if someone touches them. Swapna is the intermediate state where the mind is in a lucid state. And finally, one reaches a Sushupti state, which is what we earlier described as deep sleep. Jagrita and Sapna can be correlated to REM sleep, where the Sushupti is the non-REM sleep state.Since we're talking about sleep, we shouldn't forget about naps, often taken in the afternoon to 'regain' the energy lost during the day. Though one can take naps in summer for about 30- 45 minutes, it is advised not to take any otherwise. On the other hand, people above the age of 70 can take naps every day for an hour after their meal for about 30-45 minutes as this can help supplement their tissue needs. On the other hand, if they resume working immediately after lunch, their nutritional fluids might not get replenished, leading to deficiencies. It is the same case with someone who is sick or ill.Sleeping is part of everyone's routine and can be different for each individual. For example, people who work night shifts have a sleep schedule reversed to that of a person with a 9-5 job. A key to a healthy sleeping habit and also to a positive lifestyle is adhering to a sleep routine. In the case of people who work at night and face difficulty sleeping during the day can do a series of activities that might help them fall asleep. One is to massage their body with oil before taking a shower, and then take a nap. Another can be breathing exercises and massaging the face and structures around the eyes (24:13). There are certain herbs and ayurvedic supplements that might help if one has erratic sleep schedules. Still, it shouldn't become a habit, and people should instead focus on creating a healthier sleep schedule. Ayurveda suggests that one should eat light at night before sleeping, like rice and steamed vegetables or mild meat like fish or chicken. spicy and pungent acidic food should be avoided since they stimulate the mind and cause difficulty in sleeping.In conclusion, healthy sleep is necessary for any individual's well-being and shouldn't be neglected by calling themselves 'night owls' or 'insomniacs.'Never take your sleep for granted.
As per the diet is concerned one must stay away from diets that our making is he body produce more mucus for example taking a banana milkshake or eating cheese lasagna is not good for this time because they create more mucus which is detrimental to the lung expansion so- keep your lungs clean, keep them free from mucus. A great formula would be honey paste made from a quarter teaspoon of clove, black pepper, as both of them are excellent for keeping the sinuses clear of the mucus and the irritation, then licorice, which is great for the throat as it prevents throat irritation, lastly, turmeric, which is antiallergic. So you put together half a teaspoon of licorice, a quarter teaspoon of both clove and black pepper, and turmeric, and mix it with honey. This honey paste can be consumed by all the family members digging in with a spoon 2 or 3 times a day will help the mucus membrane be free of any kind of irritation.Recipe:1/4 TSP of Clove1/4 TSP of Turmeric1/4 TSP of Black Pepper1/2 TSP of Licorice1 TBSP Raw Honey Or Coconut Nectar or Molasses as a vegan alternativeMix WellDosage:1 TSP 2 or 3 times a day

Bikram Vaidya grew up in Kathmandu, the capital of Nepal, and ran a pub there for years before coming to the U.S. to get a culinary arts degree at Western Culinary Institute/Le Cordon Bleu in Portland, Oregon. Vaidya later went on to teach at Le Cordon Bleu and was a founding member and lead instructor at the Oregon Culinary Institute. For the last 15 years he has been dedicated to cataloguing the cuisines of his homeland. Vaidya has trekked across Nepal and spent time staying with families to learn their recipes and the cultural traditions behind their ingredients. One of the most important aspects of walking the spiritual path - which is really just a path of freedom through inner refinement - is finding a good teacher. A good teacher enables us to stay connected to our intention by reminding us, again and again, should we forget that intention…whilst getting lost in the wild jungle of the untrained monkey mind. We are all together in this grand classroom, for our soul to play out the karmas that it needs to play out, to learn what it needs to learn on its way home, to the place beyond form, and beyond time. On the heels of yet another holiday season, we close out 2024, and interestingly, this year we are doing it on a New Moon. What better time to take stock of our lives, as we enter this new moon and new year. What better time to realize the abundant nature of the Universe, the many gifts we have been given, and how many of those gifts are immaterial, and actually, in the form of teachers, healers, and friends. People we have learned from, people who have been our guiding light in times of darkness. Today's guest, is one such beloved teacher, healer, and friend to many. We could introduce Ramkumar as a Vaidya, a Vedic Scholar, and one of the founding doctors of Vaidyagrama, an Ayurvedic Healing Village in Coimbatore, India that has helped thousands of people heal from around the world through the ancient practice of Panchakarma. We could make a list of his keynote speeches at various international conferences, share personal testimonies of how his satsangs and teachings have impacted me and fellow students, patients, colleagues, and friends of his. But, none of these descriptions seem to encapsulate him. Because this would be describing someone who identifies with their ego, and Ramkumar simply does not. What he is, or who he is, is an actual embodiment of Ayurveda; a living, breathing, flowing, channel of the Vedas, and we are so thankful that today, we get to share him with all of you. May you sit with this conversation, particularly his words, with an open mind and soft heart, and may the doors of curiosity continue to open you to places within yourself that have the answers that you are looking for. In the 20th installment of the Inside JABA Series, I'm joined by Drs. Brennan Armshaw, Manish Vaidya, and Sachen Mehta. As always, I'm also joined by JABA Editor in Chief, Dr. John Borerro. In this episode we discuss a fantastic paper that is out in the summer issue of JABA titled, “Surface electromyography-based biofeedback can facilitate recovery from total knee arthroplasty.” In brief, this study that examined a novel treatment approach for rehabilitating patients following knee replacement surgery. And guess what, they used simple behavioral techniques to radically improve physical therapy outcomes! Orthopedics is something that's been on my mind lately as I am recovering from a broken metacarpal bone in my hand. It's probably something of interest to others who have either had orthopedic problems themselves, or who have taken care of hose who have. However, even if you're not interested in orthopedics, I encourage you to really dig into this episode because it's just a great case study of applying our science to a novel problem that affects millions of people . In this JCO Precision Oncology Article Insights episode, Miki Horiguchi provides a summary on “Clinical Trial Diversity: A Bend in the Arc Towards Justice”, by Tannenbaum, et al published on September 19, 2023 in JCO Precision Oncology. The editorial discusses the need for inclusion of under-represented groups in clinical trials. TRANSCRIPT Hello and welcome to JCO Precision Oncology Article Insights. I'm your host Miki Horiguchi, an ASCO Journals Editorial Fellow. Today, I will be providing a summary of the article titled “Clinical Trial Diversity: A Bend in the Arc Toward Justice” by Drs. Susan Tannenbaum and Jennifer Miller. This editorial accompanies the article “Representativeness of Patients Enrolled in the Lung Cancer Master Protocol (Lung-MAP)” by Dr. Vaidya and colleagues. In the previous episode of this series, our Social Media Editor, Dr. Rafeh Naqash, interviewed Dr. Mary Redman, a Senior Author of the Lung-MAP article. Dr. Redman shared the background behind the Lung-MAP development, some highlights from her paper, and her career trajectory as a biostatistician. I strongly recommend listening to the interview if you haven't done so. To begin I'll provide a brief summary of the Lung-MAP information before discussing the editorial. The Lung Cancer Master Protocol or Lung-MAP is a biomarker-driven master protocol that evaluates multiple molecularly targeted therapies for patients with advanced non-small cell lung cancers under a single trial infrastructure. Since Lung-MAP began enrolling patients in 2014, it has addressed the challenges of implementing precision medicine at oncology clinics and assuring equitable patient access to molecularly targeted therapies. In addition to meeting an unmet need in terms of treatment, Lung-MAP meets an unmet need in terms of accessibility to precision oncology clinical trials for all types of patients who get lung cancer in the United States. Specifically, Lung-MAP utilizes a public-private partnership that includes the National Cancer Institute's National Clinical Trials Network, the SWOG Cancer Research Network, Friends of Cancer Research, the Foundation for the National Institutes of Health, Foundation Medicine, pharmaceutical companies, and lung cancer advocacy organizations. There are thousands of sites around the country that can offer Lung-MAP. In the Lung-MAP article, the authors sought to examine whether Lung-MAP improves access to precision oncology clinical trials compared to conventional standalone trials. To this end, the authors compared accrual patterns by sociodemographic characteristics between Lung-MAP and a set of ten clinical trials for advanced non-small cell lung cancer conducted by the SWOG Cancer Research Network. The authors found that patients enrolled in Lung-MAP were more likely to be older, from rural or socioeconomically deprived areas, and with Medicaid or no insurance compared with conventional clinical trials. However, female patients and patients of Asian race or Hispanic ethnicity were underrepresented. The authors emphasized in their conclusion that further research examining participation barriers for underrepresented groups in precision oncology clinical trials is warranted. In the associated editorial with the Lung-MAP article, Drs. Tannenbaum and Miller discuss some efforts to include diverse populations in clinical trials such as those of Lung-MAP and the continuing challenges we are facing. The editorial begins with a striking example of an industry-funded trial, where pharmaceutical companies submitted a new drug for US Food and Drug Administration approval to treat patients with non-small cell lung cancer in the United States. However, the pivotal trial was conducted wholly in China, enrolling significantly younger patients than those with that type of cancer in the United States. There were no Black nor Hispanic-identifying patients and far more men than women in the trial. The product was not approved by the US FDA. Although the US FDA has approved many other products where the trials were unrepresentative , this example suggests that in order to capture elements in future patient populations in the United States, study samples need to include patients from under-represented groups as well. The authors also introduced a recent study result that showed racially and ethnically under-represented patients, and their clinicians, are more likely to trust and use new medical products when the trials have enrolled a diverse population. This fact suggests that clinical trials that include a higher proportion of women and older adults, as well as patients from racially and ethnically under-represented groups, help to gain acceptance for the drug even after its approval. The authors then introduced some efforts toward enhancing clinical trial diversity, including the National Institutes of Health Revitalization Act of 1993, the Institute of Medicine's 2003 report, the US FDA's 2020 guidance, and the innovative public-private collaboration of Lung-Map. While several studies suggested that clinical trials funded by the National Institute of Health have improved enrollment of patients from under-represented groups, industry-funded trials have still fallen short of these goals. Since industry-funded trials play a crucial role in developing novel drugs, industry must be held accountable for clinical trial diversity and make greater efforts to improve the situation. The authors introduced additional guidance toward this end. They include the US FDA's 2022 new draft guidance to industry, recommending that sponsors of clinical trials submit a Race and Ethnicity Diversity Plan. Another is the Food and Drug Omnibus Reform Act of 2022 where the US FDA can require sponsors to have plans that include diverse populations in their clinical trials. These efforts are pivotal steps in the direction of making biomedical research more accessible and inclusive and lead to promoting health equity across the country. In this informal talk held as a live session for the few fortunate participants who got a chance to sit at the feet of the master, Vaidya elaborates on the ayurvedic approach to Pain Management. He begins with a description of the fundamental millenia-old texts that form the foundation of his Shaka Vansya Ayurveda lineage: Charak Sanhita, Susrut Samhita, and Vagbhat Ashtang Hridaya. Later, he discusses pain management techniques and recounts experiences of real clinical cases. In this special presentation, made as a tribute to Vaidya RK Mishra, her distinguished disciple, formulation tester, and patient consultation partner of 20 years, Dr. Marianne Teitelbaum, of whom Vaidya himself once said "She has the light of the rishis, and before I live this body, I want to teach her everything I know" summarizes Vaidya's inconmensurable contribution to the science of Ayurveda. Vaidya Mishra, was the heir of a millenia-old lineage of ayurvedic healers whose origins are recorded in the Puranas (ancient Sanskrit Vedic texts). They practiced the purest and most elevated form of ayurveda and had the highest understanding of the human physiology; an understanding that went beyond body organs and functions and holistically took into account the connection, communion, and harmonious balance of the physical body as the housing of the soul, put in motion by the flow of Prana and kept in harmony with the natural laws of the universe, as the true means of attaining HEALTH and BLISS.
