As a Neonatal Nurse Practitioner for 30 years, Tara Sundem was a frontline worker in the growing opiate epidemic, caring for the newborns suffering through opiate withdrawal.  Her passion to transform the healthcare experience for them and their families led her to found Hushabye Nursery, a nonprofit dedicated to caring for opiate-exposed newborns in their first weeks of life. The 12-bed inpatient nursery opened its doors and its arms to these babies and their mothers in November 2020. Hushabye Nursery has grown from an idea to “save the babies” in 2016, to a unique model of care for this underserved population that focuses on the family unit, both prenatal and postnatal. The nursery has 12 rooms for inpatient detox customized for infants born with Neonatal Abstinence Syndrome (NAS), where their family members can stay, and provides intensive outpatient services for all family members as part of Hushabye's continuous care. For our 51st podcast episode, I discussed a contensious topic in the neonatal world, Gastroesophageal Reflux (GER) and Gastroesophageal Reflux Disease (GERD). Although it is a common occurrence amongst all infants, the lack of updated, clear, and consistent diagnostic and management recommendations remains, especially for the NICU population. Due to my personal and professional experiences, it is difficult for me to have one solid stance on the topic. As a former NICU nurse, I saw so many infants experience reflux. As a NICU parent, my son William, struggled with reflux once we brought him home and yes, at that time, it was managed with medication. And finally, now as a Neonatal Nurse Practitioner, I am more knowledgeable about what the evidence does and does not show regarding reflux and I understand its complexity. So I can fully appreciate how nurses advocate for their patients. I also completely acknowledge the internal battle parents endure as they helplessly watch their baby grapple with reflux and its associated symptoms, and I also embrace what the research has shown and why providers do not hastily start infants on reflux medications. Jen was a bedside RN for 10 years at a Level 4 NICU that specializes ECMO, surgeries, cardiac and genetics patients. She then obtained my Doctor of Nursing Practice and continue to practice in that same Level 4 NICU, as well as a Level 3 NICU and a Level 2 NICU at a local community hospital. She specializes in cardiac patients but also love attending deliveries and taking care of feeder grower babies at the community hospital. For this podcast episode, I was very honored to be joined by Ms. Mary Coughlin. Mary is a Neonatal Nurse Practitioner, but also a published author of several pieces of work. She is well-known in the neonatal world for her development of the core measures for developmentally supportive care, but most recently with her published work and creation of the trauma-informed professional certification program as well as additional programs, workshops and masterclasses through Caring Essentials Collaborative. I was so honored to be able to sit down and discuss Mary's work experience that led her to the transformational work she is doing today. Just listening to Mary is transformational! As NICU clinicians, Mary reminds us of the importance of our role and why it is so crucial for us to embrace our vulnerability, take our armor down and truly connect with each other on a human level. Trauma-informed care is not about doing more as a clinician, it is about being more of your loving, compassionate self so you can make a difference by fully showing up for your patient and their family. We also discussed the trauma that is so common for NICU parents throughout their NICU journey and in the years to follow. So many NICU parents feel alone and isolated in the process, yet it is common to so many. We discussed the word trauma and why it is often downplayed or not spoken openly about it NICUs like it should be. And most importantly, we share the importance of working through and moving through your trauma, which will be difficult but is essential for the well-being of the entire family unit.Trauma-informed care is a concept that has been applied to several areas of medicine and patient care, but its application to the NICU is still emerging. Mary learned about trauma-informed care and knew that she had to introduce it into the NICU world. Not only does trauma-informed care in the NICU directly affect the baby, but it also addresses the family as well as the clinicians who are caring for the infant and family. Mary explains why the personal journeys we all go through and each of our stories play into an intricate web that involves each and every one of us. It is through this that we can all more intimately connect and heal one other. As an ethicist, Sandra provides health system navigation and  mediation of conflict around morally charged issues. Sandra's primary focus and research efforts are directed toward end of life care planning and building hospital infrastructure to support early integration of Palliative Care. Sandra is a Master Trainer in the Serious Illness Care Program, a program aimed to enhance understanding of patients wishes, values and goals of care as they near end of life. In 2021 Sandra founded a health care consultation service, Quality Life Planning, that provides serious illness and end of life care planning, health advocacy, system navigation and public education on relevant health care issues.  Sandra also continues to practice nursing as a hospice nurse. Sandra studied Mindfulness Based Stressed Reduction and MBSR Intensive through Mindfulness Hamilton. Our NICU Journal called Our NICU Roadmap has officially been published! My vision for this journal stemmed from the lack of a comprehensive NICU journal available when our son was in the NICU. I wanted to develop a journal that not only was a great resource for parents, but also served as a place for you as parents to document your baby's progress each day while in the NICU. As a Neonatal Nurse Practitioner, I had the medical knowledge and experience on my side during our son's NICU journey, so I knew the questions to ask our son's NICU care team, but I felt there was not an appropriate place to document it all. Our NICU Roadmap equips and guides NICU parents with resources, information and questions to ask so you can stay actively involved in your baby's daily care. In today's podcast we spotlight my favorite part of the journal, the daily log. As any NICU parent knows, there are new changes that occur daily in the NICU. Personally, I wanted to document all of those changes, not just as a keepsake, but because I wanted to keep track of all of our son's daily weight gains or losses, lab results, ventilator changes, feedings, who cared for him each day, how I felt, the plan of care plus so much more!I created the daily log so you as parents not only had a place to document your baby's daily updates, but also to help guide you with questions we believe you should be asking the NICU care team each day! Jill Larocque speaks about the Edmonton experience. Jill Larocque has worked in the Neonatal Intensive Care Unit for almost 20 years. And how did some of those same skills create the opportunity to start an entrepreneurial business, first started as a side business? Marcia Grondahl candidly shares not only the most valuable skills she developed and learned as a nurse, then nurse practitioner, for over 20 years, but how to handle life's curves. While working in the medical field, she transitioned from being a single mom with a 14-year old daughter to marrying Chris, a widow, taking on his three young children who had recently lost their mom. She shares how difficult that transition was and how important her faith was in the process. The standout skills that connected her career transitions were teaching and training. You will hear how those abilities were extremely helpful as she started working for GE Healthcare in Nursing Informatics and why she chose to make that particular career change. You will also hear about the most valuable ingredient that helped her in managing teams across the country. Plus, how fast she could pack her bags for travel! Marcia talks about the difficulty in transitioning from working in a hospital to working more remotely for GE Healthcare at home, also traveling a great deal for the company. She shares some key points that helped her in establishing a home office and some of the boundaries she put in place. Marcia was making good money at GE, but really wanted to start helping people with their health and wellness. The company she chose to do this with was DoTERRA Oils. DoTERRA is a multi-level marketing company and she knew she'd need to develop a downline, but the upline was also important. (Find out why!) Deborah Discenza is a leading patient advocate for babies born prematurely. Combining careers in technology and publishing, Ms. Discenza has initiated several different outlets to raise awareness and education around preemies, including Preemie Magazine, PreemieWorld, and her book The Preemie Parent's Survival Guide to the NICU which she co-authored with little man Director/Producer Nicole Conn. Her work focuses on providing information to parents in simple terms to enhance health literacy, critical when parents are dealing with the very challenging environment of a neonatal intensive care unit. For her work in this area, Ms. Discenza has been widely recognized and is a founding member of many organizatons as well as a regular author to several publications. She has received awards for her work and continues to be very active helping to promote better awareness for parents and others in the health field.Suzanne Staebler has been a leading voice in the field of neonatal nursing for almost 25 years. Currently, she maintains her practice the Pediatrix Medical Group of Tennessee, in Nashville and has a healthcare/health policy consulting business.  Previous positions included APRN leadership positions and faculty appointments within neonatal nurse practitioner programs.  Suzanne also is active on multiple different national organizations including acting as the Governing Council Chair of the National Association of Neonatal Nurse Practitioners and sits on the board of directors for the National Association of Neonatal Nurses. Suzanne graduated from Baylor University before obtaining her nursing degrees from the University of Texas Health Science Center in San Antonio and Texas Christian University. She is a board-certified Neonatal Nurse Practitioner and has authored articles on the subject of the health policy implications of regionalization.

Listen NowWhile the vast majority (70%) of people turning 65 will need long term care services for an average of three years, only 7 million Americans own a long term care insurance policy.  Medicare does not provide for long term care and Medicaid only covers long term care costs for those with very limited financial means.   The CLASS Act, a provision within the ACA, would have created a voluntary and public long term care insurance policy for employees but the ACA provision, as written, was unworkable such that the Obama adminstration abandoned its efforts to implement the CLASS Act in late 2011.  (The Congress offically repealed the provision in early 2013.)     During this 25-minute interview Ms. Garner discusses the need for long term care insurance (both for the elderly and younger disabled), the origins of the CLASS Act and why the provision was ultimately unsuccessful and the continuing need for related reforms to both entitlement programs, i.e., Medicare and Medicaid, and the long term care insurance market.Ms. Connie Garner is currently the Executive Vice President for Public Policy at United Cerebral Palsy.  Previously, Ms. Garner worked at Foley Hoag where she served as Policy Director in the Government Strategies Practice Group and as Executive Director of Advance CLASS, Inc., a position she still holds.  For 17 years prior she was Policy Director, Disability and Special Populations, to the U.S. Senate Committee on Health, Education, Labor and Pensions Committee.  In that role, she was the lead Democratic Committee architect for the CLASS Act, the major long-term care legislation that was a part of the ACA.  Ms. Garner also served in the U.S. Department of Education.  She received her B.S. in Nursing from the University of Pennsylvania, a M.S. in Nursing form George Mason, and an Ed.S. in Special Education from George Washington.  She is certified as a Pediatric and Neonatal Nurse Practitioner and is the mother of seven children.For a primer on long term care insurance see this DHHS Assistant Secretary for Planning and Evaluation (ASPE) research brief: http://aspe.hhs.gov/daltcp/reports/2012/ltcinsRB.shtml.  This is a public episode.
