We discussed the benefits of methods like 16:8 time-restricted eating and the 5:2 meal plan, and even compared IF to medications like metformin. Today, we're bringing that science into real life. We'll talk about how people fast for religious reasons, like during Ramadan, how athletes use IF to stay in shape, and how we can use IF as a tool in family medicine to support community health and A1c control.Intermittent Fasting in Religious PracticeRamadan just ended on 3/30/25, but this is a great time to talk about the broader role of fasting in religion and health. Many faiths incorporate fasting into spiritual practice and understanding this can help us better support our patients.Islam (Ramadan): Ramadan is a month where Muslims fast from dawn to sunset, focusing on spiritual reflection and self-control. No food or drink is consumed during daylight hours. Despite this, studies have shown that with good planning, fasting during Ramadan does not significantly impair physical performance or metabolic health.Key health tips for patients observing Ramadan:Hydrate well between iftar (sunset) and suhoor (pre-dawn).Break the fast with dates and water to gently replenish energy and electrolytes.Eat balanced meals with complex carbs, protein, and healthy fatsAvoid greasy, heavy foods right after fastingLight exercise (such as a walk) after iftar is beneficialReview medications with a healthcare provider, especially for those on insulin or sulfonylureas.For example: Metformin should be taken when you break your fast and then again before dawn. If its an extended-release metformin, take it at night. Metformin does not cause significant hypoglycemia and can be continued during Ramadan. Basal insulin is advised to be given at Iftar, and the dose should be reduced by 25-35% if the patient is not well managed. And regarding the fast-acting insulin, it requires a little more reading, so you can look it up and learn about it. Judaism: In Judaism, fasting is practiced on days like Yom Kippur and Tisha B'Av, typically lasting 25 hours without food or water. These fasts are spiritual and reflective, and patients with medical conditions may seek guidance on how to participate safely.Christianity: Many Christians fast during Lent, either by abstaining from certain foods or limiting meal frequency. Some practice partial-day fasts or water-only fasts for spiritual renewal.A branch of Christianity known as The Church of Jesus Christ of Latter-day Saintsoften observe a 24-hour fast on the first Sunday of each month, known as Fast Sunday, where they abstain from food and drink and donate the cost of meals to charity. This practice is both spiritual and communal.Cameron: Fasting for religious reasons, when done safely, can align with IF protocols and be culturally sensitive for diverse patients in family medicine.IF in Athletes and PerformanceIntermittent fasting is gaining popularity in the sports world. Athletes are using IF to improve body composition, increase fat oxidation, and enhance metabolic flexibility. A recent study, known as the DRIFT trial and published in Annals of Internal Medicine, found that fasting three non-consecutive days a week led to more weight loss than daily calorie restriction. Participants lost an average of 6.37 pounds more over 12 months.Why? Better adherence. People found the 3-day fasting schedule easier to stick to than counting calories every day.Benefits of IF for athletes:Encourages fat burning (via AMPK activation and GLUT4 upregulation, listen to ep. 179).Helps maintain lean muscle while reducing fat.No major drop in performance when meals and workouts are timed properly.What are some practical tips?Schedule workouts during or just before eating windows.Eat protein-rich meals post-workout.Avoid intense training during long fasts unless adapted.Stay hydrated, especially in hot environments or endurance sports.Broader Applications and Myths Around IFHormonal Effects of IF: In addition to improving insulin sensitivity, IF also affects hormones such as ghrelin (which stimulates hunger, remember it as growling) and leptin (which signals fullness). Over time, IF may help the body regulate appetite better and reduce cravings. IF can also decrease morning cortisol levels, the stress hormone. That's why it's important to monitor sleep, hydration, and stress levels when recommending IF.Circadian Rhythm Alignment: Emerging research shows that aligning eating times with natural light/dark cycles—eating during the day and fasting at night—can improve metabolic outcomes. This practice, known as early time-restricted eating (eTRE), has been shown to lower blood glucose, reduce insulin levels, and improve energy use. Patients who eat earlier in the day tend to have better results than those who eat late at night.Myths and Clarifications on IF:-“Fasting slows metabolism” In fact, short-term fasting may boost metabolism slightly due to increased norepinephrine. -“You can't exercise while fasting.” Many people can safely train during fasted states, especially for moderate cardio or strength training. -“Skipping breakfast is bad.” For some, skipping breakfast is a useful IF strategy—as long as total nutrition is maintained. You can break your fast at 2:00 pm, it does have to be at 7:00 AM.What to Eat When Breaking a FastBreaking a fast properly is just as important as fasting itself. Whether it's after a Ramadan fast or a 16-hour fast, the goal is to replenish energy gently and restore nutrients.Ideal foods to break a fast:Dates and water: provide quick energy, potassium, and fiberSoups: lentil or broth-based soups are gentle on digestionComplex carbs: whole grains like brown rice or oatsLean proteins: chicken, fish, eggs, legumesFruits and vegetables: hydrate and provide fiberHealthy fats: nuts, avocado, olive oilProbiotics: yogurt or kefir for gut supportBalanced meals with carbs, protein, and healthy fats help the body transition smoothly back to a fed state.Using IF in Family Medicine and Community HealthIntermittent fasting can be a practical, cost-effective strategy in family medicine. In areas with high rates of obesity and diabetes, like Kern County, IF offers a lifestyle-based tool to improve metabolic health, especially in underserved populations. IF is free!How IF can help in family medicine:Lower A1c levels: improves insulin sensitivity and glucose controlPromote weight loss: decreases insulin resistance and inflammationReduce medication dependence: fewer meds needed over time for some patientsEncourage patient engagement: flexible and easier to follow than strict calorie countingFit diverse lifestyles: aligns with religious and cultural practicesAddress food insecurity: structured eating windows can help patients stretch limited food resourcesHow to apply IF in clinic:Start the conversation by asking if the patient has heard of IFRecommend simple starting points: 12:12 or 14:10Emphasize hydration and nutrient-dense mealsMonitor labs and symptoms, especially in diabetic patientsAdjust medications to avoid hypoglycemiaProvide follow-up and patient education handouts if possibleWhat if a patient isn't ready to try fasting?For those not ready to commit to intermittent fasting, one effective alternative is walking after meals. A simple 10–20 minute walk post-meal can help stimulate GLUT4 receptors in skeletal muscle, promoting glucose uptake independent of insulin. This reduces the demand on pancreatic beta cells and may help improve blood sugar control over time. This strategy is particularly useful for patients with insulin resistance or early-stage type 2 diabetes.Conclusion: Intermittent fasting is not one-size-fits-all, but it can be a powerful tool for both individual and community health. From Ramadan to race day, IF has a place in family medicine when used thoughtfully. Encourage patients to work with their healthcare providers to find an approach that fits their lifestyle, medical needs, and personal values. Nobenega zdravila ni, s katerim bi vam čudežno skopnela teža. No, vsaj tako se je reklo do nedavnega. Potem ko je pred nekaj leti prišlo na trg kot nova sila v boju zoper sladkorno bolezen, se je vmes izkazalo, da je uspešno tudi pri hujšanju. Ozempic ni več le farmacevtski pojem, ampak predvsem popkulturni in finančni fenomen, ob čemer se v tokratni oddaji sprašujemo, ali ni navdušenje nad njim preuranjeno in kaj dejansko pomeni iskanje idealne teže z njim.Sogovorniki: endokrinologinja dr. Marija Pfeifer, dr. Lovro Žiberna, Katedra za biofarmacijo in farmakokinetiko, Fakulteta za farmacijo specialistka družinske medicine dr. Nena Kopčavar Guček, nekdanja uporabnica Ozempica Urška iz Ljubljane Študije, ki jih dodatno posredujemo v prebiranje (pripravila Vida Lestan): Ozempic in kontracepcija: Pojavljajo se primeri nepričakovanih nosečnosti pri ženskah, ki jemljejo zdravilo Ozempic sočasno z oralno kontracepcijo ali pa so bile predhodno diagnosticirane z neplodnostjo. Obstaja sicer več hipotez, zakaj bi lahko zdravila, ki pripadajo skupini agonistom GLP-1, večala plodnost, a tovrstne raziskave še niso na voljo. Mogoče je, da izguba telesne teže pri nekaterih ženskah vpliva na povrnitev redne ovulacije. Poleg tega lahko zdravilo tirzepatid, ki ima podoben mehanizem delovanja kot ozempic, ni pa semaglutid, manjša absorpcijo kontracepcije, kar zavira njeno delovanje. Zdravila, ki posnemajo delovanje GLP-1, med njimi tudi sloviti Ozempic, upočasnjujejo praznjenje hrane iz želodca v črevo, kjer pa se kontraceptivi absorbirajo v krvni obtok. Delovanje oralne kontracepcije pa je odvisno od njene koncentracije – če je ta prenizka, lahko zavira njeno delovanje. Ameriško farmacevtsko podjetje Eli Lilly, ki je patentiralo zdravilo tirzepatid, bolj znano kot Mounjaro, priporoča uporabo drugačne oblike kontracepcije pred in med uporabo zdravila. Študija Federica Malla iz Univerze v Vigoju iz leta 2015, ki je sicer bila izvedena na živalskih modelih, je opazila povečano število potomcev pri miših, ki so bile tretirane z zdravili GLP-1, v primerjavi s tistimi, ki niso. Raziskovalci so opazili tudi povečane koncentracije luteinizirajočega hormona, ki je odgovoren za sprožanje ovulacije. Žal pa so raziskave na populaciji žensk nasploh zelo skromne. GLP-1 in odvisnost od alkohola: Zmanjšana potreba po uživanju alkohola pri bolnikih, zdravljenih s semaglutidom, kaže na potencialne terapevtske koristi za zdravljenje odvisnosti od alkohola. Ameriška retrospektivna kohortna študija elektronskih zdravstvenih kartonov 83 825 bolnikov z debelostjo kaže na od 50 do 56 odstotkov nižje tveganje za zlorabo alkohola v primerjavi z drugimi zdravili za zdravljenje debelosti. Točni mehanizmi delovanja še niso bili odkriti, vendar pa se receptorje GLP-1 povezuje z možganskim dopaminskim sistemom. Vključenost sproščanja dopamina pri uravnavanju uživanja hrane in alkohola bi lahko pojasnila, zakaj je semaglutid koristen pri zmanjšanju uživanja hrane in v živalskih laboratorijskih modelih za zmanjšanje uživanja alkohola in drugih drog. Čeprav ni podatkov o učinku semaglutida na absorpcijo alkohola, pa je mogoče, da poleg upočasnjenega želodčnega praznjenja upočasnjuje tudi absorpcijo alkohola. Ta upočasnitev pa zmanjša vpliv na dopaminski učinek, ki ga prinese uživanje alkohola. GLP-1 in nevrodegenerativne bolezni: Obetavne so tudi študije na področju nevrodegenerativnih bolezni, kot sta Alzheimerjeva in Parkinsonova bolezen pri bolnikih s sladkorno boleznijo. Hormon GLP-1 ima ključno vlogo pri povezovanju presnovnih in možganskih motenj. Študije na živalskih modelih so pokazale, da agonisti receptorja GLP-1 (GLP-1RA) obetajo tudi pri stanjih, kot so možganska ishemija ter Alzheimerjeva in Parkinsonova bolezen. Pri kognitivnih motnjah, povezanih z diabetesom in debelostjo, GLP-1RA izboljšujejo učenje in spomin ter zmanjšujejo nevrodegeneracijo. Pri modelih PD GLP-1RA izboljšujejo motorično aktivnost in varujejo dopaminergične nevrone, medtem ko pri modelih AD izboljšujejo nevropatološke značilnosti in kognitivne funkcije. Mehanizmi vključujejo zmanjšanje oksidativnega stresa, vnetja, apoptoze in modulacijo sinaptične plastike. Čeprav so te ugotovitve po večini osnovane na živalskih študijah, kažejo, da bi lahko bili GLP-1RA obetavna terapevtska možnost za kognitivni upad, povezan z diabetesom, in nevrodegenerativne bolezni pri ljudeh. Za potrditev teh učinkov pri ljudeh so nujne nadaljnje klinične študije. Članek iz leta 2024 s podobnimi ugotovitvami kot zgornji ponuja pregled nad ugotovitvami študij na živalskih modelih. Kljub obetavnim predkliničnim rezultatom pa so potrebne nadaljnje raziskave na tem področju. Primerjalna študija uspešnosti GLP-1, inhibitorjev DPP in sulfonilureaz za tveganje razvoja demence pri starejših bolnikih s sladkorno boleznijo tipa 2. Švedska raziskava je vključevala 88 381 udeležencev s sladkorno boleznijo tipa 2, starejših od 65 let. Raziskava, ki je trajala od leta 2010 do 2020, je pokazala, da je med omenjenimi zdravili, sicer primarno za zdravljenje sladkorne bolezni, GLP-1 najbolj znižal tveganje za razvoj demence. Potrebnih je več kliničnih študij.

The guys go over Jim Henson's early background, how he got into puppetry and his early successes appearing on American variety shows. Asif and Ali then discuss when they first encountered Henson's work. They then discuss Henson's lifelong collaboration with puppeteer and filmmaker Frank Oz. They then go over the successful and not so successful movies of Henson, the eventual sale of The Muppets to Disney and Henson's legacy. Jim Hensen died of toxic shock syndrome (TSS), so in the second half, Ali asks Asif about this diagnosis (36:19). Asif starts off by discussing the bacteria which cause TSS (Staph aureus and Group A strep) and how these bacteria produce toxins which cause TSS. He then talks about how common it is and the risk factors for developing TSS. Ali asks Asif about the relationship between hyper-absorbant tampons and TSS; Asif goes into detail about the history of this association and the increased incidence of this occurring in women using tampons in the late 1970's and early 1980's. J'en profite donc pour refaire le point et discuter des différentes études menées sur le sujet, des réalités de cette situation, et comme souvent, je vais déconstruire certains mythes entourant les accouchements à domicile. Saviez-vous qu'une étude de 2021 a montré que 35% des Françaises auraient aimé accoucher à domicile ? Pourtant, les Accouchements À Domicile (AAD) ne représentent que 0,2% des naissances en France. Cela s'explique en partie par le manque de soutien des autorités de santé envers les sages-femmes qui accompagnent les naissances à domicile, ce qui fait qu'il y a trop peu de sages-femmes prêtes à le faire. Chronic pain is a silent pandemic affecting somewhere between 8 and 28 million people in the UK alone. Recent headlines (https://www.bbc.co.uk/news/business-65596283) reveal it's contributing to 2.5 million Brits leaving work. But more alarming still, is our response.The UK prescribes more opioid pain killers per capita than any other country in the world. This is in spite of NHS guidelines warning against its use for chronic pain. Malnutrition is a business issue. The link between malnutrition and poverty might be an obvious one. The links between malnutrition and business are perhaps less so. During this conversation we are going to deep dive into both, whilst looking at the intersections between nutrition, climate change, conflict and communicable disease. As the world braces for some of the most extreme food shortages seen in generations – we are going to talk hunger and malnutrition and why everyone, everywhere should be sitting up and engaging on this deeply sad topic. During this podcast hear from two of our world's experts on childhood development and nutrition. Maniza Ntekim and Dr. Alok Ranjan. Maniza leads the Early Childhood Development East and Southern Africa initiative at the Conrad N. Hilton Foundation. Previously she's served as a regional advisor for UNICEF's Eastern and Southern Africa Regional Office, overseeing the early childhood development programmes. Alok is the Director of Programmes and Investments for The Power of Nutrition. He is a medical doctor with a post-graduate degree in Community Medicine. Prior to The Power of Nutrition, he led the Nutrition program at the Bill & Melinda Gates Foundation for India, Bangladesh, Indonesia and South Asia. “We need to work across multiple levels of deprivation. Working in silos will not give us long term sustainable impact. ” Explains Alok. Together they warn about the world sleep walking into the global nutrition crisis and that every 60 seconds a child is becoming severely malnourished and almost 50 million people are hungry. They are determined that business needs to step up – that malnutrition is a business issue, without healthy people, businesses cannot succeed. Maniza expounds: “This is a not just a charity issue, it's a matter of business survival. Businesses need skilled human capital to grow, to be competitive and to be productive. The basis of a good business is human capital. We know that human capital is developed in the early stages of life; 80% per cent of the human brain is developed by the age of three. If a significant proportion of the population within which you hire is poorly nourished and not had the chance to be developmentally on track that's critical for your business.” Maniza and Alok are deeply practical and pragmatic in how we can collectively address malnutrition and hunger and what businesses in particular can do about it. Together they make the business case for investing in good food – from healthy workforces to higher cognitive performances. Neurologic researchers from Britain's University of Birmingham studied 605 adults aged 44 to 57 years who were followed up for as long as 13 years with cognitive testing. The analysis also showed that this tendency toward dementia was more prominent in men than in women. High Cholesterol and eating more protein [20:56] Kimberly says: Hi Love your show. I have been listening since the beginning. Please never stop recording. In the last couple years (since covid hit) I have lost 20 lbs. I have a muscular body type, I am 54 years young , 5'4", weigh 137lbs (post menopausal) workout 5 days a week with crossfit, 1 day Yoga, and walk the dog 2 miles a day. I attribute the weight loss with eating more protein. I now eat almost 120 gms a day where before I probably only got 50-70 if that (did not track) . My protein sources are usually chicken , smoked salmon, tuna, turkey, once a week red meat. I do add pure protein bars (20gms) towards the end of the day when protein levels aren't there yet. I went to the doctor and got bloodwork done and my cholesterol went up. It went from the total 226 to -244, LDL-101 to 131, to HDL-105 to 101, ratio-2.1 to 2.4, Triglycerides-61 to 62 from Sept of 2020 to Sept 2021. ugh I know I should probably ignore it because my ratio seems great and I feel great. For some reason its bugging the crap out of me, how can I lose weight and eat more protein and have the numbers go up? Should I be concerned? Thanks you both rock. 3. I came across a supplement that uses a combination of Tributyrin and Vitamin D, and supposedly promotes a healthy gut, aids in healing leaky gut, helps reduce bloating, improves digestion and vitamin absorption, boots immune system etc. etc. etc. What they say is that Tributyrin has been used in Spain for at least 3 years but is just starting to come to the US. There are some studies on mice but not a ton in humans yet. I'm just wondering if you've done research on Butyrate/Tributyrin and whether or not you'd suggest experimenting with something like this. I am mostly paleo for about a decade, but my belly pooches out a ton (granted, I've had 2 kids and I'm working on core function), I haven't had consistent bowel movements in months, and while my stress levels are somewhat high because I'm an entrepreneur, I get 7 hours of sleep a night and do my best to keep myself regulated. I've tried probiotics, drink kombucha, try to get resistant starches (hate fermented foods unfortunately) and am pretty well versed in diet/health (thanks in large part to you!). I'm interested in trying out this supplement but very wary of putting anything in my body that is unknown and I can't find much info from people I trust (like you) on this particular topic. So I guess my question is twofold - what do you know about Butyrate/Tributyrin and also, how to decide what is safe to experiment with? Can you point us to any reliable strength, aerobic, and flexibility benchmarks for men and women over 50? Ideally, the fitness assessment could be done at home without special equipment. It could be useful for participants to develop greater awareness of their relative fitness. Some seniors feel great about their occasional walks in comparison to their physically-deteriorated, sedentary peers. But, outside of my health coaching sessions, the doctor will plainly state that just “walking isn't cutting it.” For brain health, it could me most therapeutic for them to engage in complex, cognitively challenging physical activities, such as dance, martial arts, or sports (at an appropriate level), and check to see what supplemental exercise is needed to meet cardio, muscular, and flexibility targets. The fitness assessments I found from the Mayo Clinic https://www.mayoclinic.org/healthy-lifestyle/fitness/in-depth/fitness/art-20046433 seem questionable in that both men and women at age 65 have the exact same target of 10 classic pushups. Does the upper body strength differential between the two sexes really disappear by age 65? I couldn't find an especially good resource for ages 50-90. For instance, the Fullerton Functional test/Senior Fitness test has arm curls as the best assessment of upper body strength. The presidents challenge at health.gov has assessments for students but not elders. If people want to be physically functional and free from chronic illness at a ripe old age, shouldn't they have be able to look for what needs attention at age 55, 60, 65, etc? I've been training jiu jitsu (nogi) for about 6 years and absolutely love it… except for the fact that every morning after training I experience uncontrollable itching on my body, predominately my legs, but sometimes all over. Karen, a first-time mother, talks about the how she came to choose where to give birth. As a Vet with a husband who is an anaesthetist, they were hot on their research and took a very measured and evidence based approach to making their decision. Karen talks us through the research she did, both clinical and anecdotal through conversations with friends and how she did a full circle from wanting an elective c-section to deciding on a home birth. She talks about the benefits of having home visits from the same midwife from the home birthing team once their decision had been made and all of the other preparation, she was doing to for her upcoming birth. A paper published in the Lancet Digital Health found that less than 1% of clinical AI studies reviewed were of sufficient quality to have confidence in their results. This is a huge problem. Dr Xiao Liu and Professor Alastair Denniston are part of the team who published that study — and to tackle this this problem — they have today published the first international standards for the reporting of clinical AI trials. These are called SPIRIT-AI and CONSORT-AI and have today been published in Nature Medicine, the Lancet Digital Health and the BMJ. The goal of these guidelines is to improve the quality of clinical AI studies and increase their transparency. We talk about the general quality problem in clinical AI research, we talk about their guidelines and some of the recommendations within them — and also about what the future of clinical AI validation looks like.
