Chief Medical Officer at Northwestern Medicine Dr. Kevin Most joins the Steve Cochran Show to discuss the gradual shift towards non-contact football summer camps, the new at-home lymes disease test kit, and he eases your COVID-19 vaccine confusion.  Dr. Kevin Most's notes from the Steve Cochran Show: CTE - Chronic traumatic Encephalopathy- new study- not the number of concussions, more important is the cumulative impact to the brain. New study released last Tuesday showed that the chances of CTE are not only related to the number of head impacts but also to the cumulative impact of all the hits. This was a study that reviewed 34 previous studies that looked at the number of hits and the impact of those hits measured by helmet sensors This looked back over 20 years and studied the 631 players who had donated their brain to these studies that were overseen by Boston University This study looked at what aspects of head hits most commonly contributed most to CTE progression They reviewed number of hits to the head, number of years played, and force of the hits Study found that the best predictor was the cumulative force of the head hits over the course of their career, not the number of concussions (which are very under reported or identified) The study reviewed 631 brains, of those 451 or 71%, had  CTE The players who were estimated to have absorbed the greatest cumulative force had the most advanced CTE This reinforced that the concept of the impact of subconcussive  hits, rather than number of concussions was the driving force behind the diagnosis and cognitive decline. The data collected was from college players as the NFL has not released its helmet sensor data The study looks at the number of hits and the force of the hits and them tracks the cumulative data Different players get hit in different areas of the brain, lineman take hits to the front of their heads, where quarterbacks take hits to the back of their head most often. This is leading to research on different helmets for different positions Future studies will look at the location of the hits and the impact More data will come out as helmet sensors become universally used. Many high schools going to low contact practice and well controlled contact practice. Helmet technology has certainly advanced to safer helmets, however players are bigger and faster and technology cannot keep up with protection from the force generated. Helmets have sensors for the force of the hit and some actually have EEG monitors, both are used to be more objective when a player needs to be removed form a game. Covid Vaccines- Confused? You are not alone… Think back over the past 3 years- one shot or 2, age 16 or 18? Boosters for which strain? Who gets boosters, new booster this fall The Covid virus is first noted in a patient in  December of 2019 In January of 2020 the first case is found in Seattle, Washington, a few days later the first case was noted in Illinois The Covid Virus was genetically sequenced shortly after it was identified, this allowed for vaccine research and trials to begin The original vaccine was produced in record time using newer technology, and approved for individuals over the age of 18. Confusion/concern- made too fast Original Covid Vaccine was produced using the original strain of Covid ( SARS-VoV-2)- released December 11, 2020, Pfizer and Moderna This was a 2 shot series, released under an emergency authorization A month later J and J Covid vaccine gets released as a single shot for those over the age of 16, some skeptics about this, but shortages promoted it Confusion- why only one shot Shortly after that Pfizer vaccine is approved for children aged 12 and over, then follows up with an approval for 6 months to age 12. Confusion/ concern- enough research done on kids with new vaccine? Moderna vaccine gets approved for 6 months to age 11 Fast forward to August of 2022, Moderna and Pfizer get approval for new booster based on BA4 and BA5 strains, the predominant strains at that time. Confusion/Concern- will we need boosters for all variants Booster is approved as a single shot for those 18 and older to be given at least 2 months following the completion of the primary series. UK and Canada approve 2nd booster for those over age 65, XXXXX Date, US is silent. Confusion/concern- why are they doing a second booster and we are not US approves 2nd dose of Covid booster April, 2023, for those over 65 or immunocompromised. Booster is still BA4 BA5 based US no longer gives original Covid vaccine to those who have not been vaccinated, now move to “Booster” vaccine as the standard News out now, this fall there will be a new Covid Vaccine using the predominant strain at this time XBB This will be recommended for all this fall along with your annual flu vaccine. Confusion/Concern- will insurance cover the cost as it is not government stock Will we need another booster in the spring?  Confusion/Concern- we seem to need a booster 6-8 months after our last shot, will this happen again? Home test for Lyme disease One great thing that came out of covid was the advancement of testing in the home for the disease. We saw earlier diagnosis made with less hassle and cost Now we are seeing how that is being advanced Home test for Lyme Disease is in the research phase. Hopefully faster identification will lead to earlier treatment , thus preventing any long term consequences from the illness Currently we use blood tests looking for the bodies response to the infection, we look for antibodies to the bacteria. Unfortunately the body may take a couple weeks to make enough antibodies  to be at a level that we can identify them Now we have home tests where a patient can prick their finger and send a drop of blood and sent that to a lab, this still may not be positive for weeks and has a turnaround time of 3-5 days This delay in antibody detection allows the bacteria to spread throughout the body Test #1-Work is now being done with a simple urine test that will identify small portions of the bacteria within hours after the infection begins.  Virginia Tech Test #2- is even faster. It is a sticker that is applied to the skin immediately after the tick is removed, and if the tick was infected with the bacteria for Lyme disease, the sticker will change colors Both of these are a couple years away, however it is showing that we will be doing more in home testing and hopefully quicker diagnosis at a lower cost See omnystudio.com/listener for privacy information.

The Nigerian government has received 3.2 million doses of the Johnson and Johnson COVID-19 vaccines, from the Italian government in Abuja. The Executive Director of the National Primary Health Care Development Agency, Dr Faisal Shuaib, says Nigeria has fully vaccinated a little above 13.5 million people with the first dose of COVID-19 vaccine, representing 12.2 per cent of the eligible population in the country. He says over 23 million people who have received their first doses are yet to take their second jab, a situation the Nigerian government hopes to improve with the rollout of the Johnson and Johnson vaccine, which is a single shot.

Glad you asked!  If a pregnant person gets COVID-19, they are more likely to get severely ill compared to non-pregnant people. Risks to pregnant people can include hospitalization, intensive care, the use of special equipment for breathing, or in the worst cases, death.  Pregnant people who have COVID-19 also have a higher risk of stillbirth, pre-eclampsia and preterm birth. COVID-19 has also been associated with newborns being admitted into intensive care units. Some reasons for this may be: In unvaccinated people, COVID-19 can invade and destroy the placenta. This can lead to a lack of oxygen to the fetus. COVID-19 has led to increased stress, mental health problems and substance use, which can affect maternal health. Q2: How likely is stillbirth in people with COVID-19? Stillbirths due to COVID-19 are uncommon, but they do happen. Overall, there are about 24,000 stillbirths in the United States each year. But a recent study by the Centers for Disease Control and Prevention (CDC) found that: Pregnant people who had COVID-19 when they delivered their babies were almost twice as likely to have a stillbirth as someone who did not have COVID-19. The severity of COVID-19 symptoms was the key factor in their heightened risk of stillbirth. Unvaccinated pregnant people were the most severely ill.  The Oregon Center for Health Statistics (CHS) does not have stillbirth rates due to COVID-19 because this information is not collected on the stillbirth certificate in any way that could indicate that COVID-19 was the cause of a stillbirth.  Q3: In the United States, the number of people who died during pregnancy or shortly after giving birth increased during the pandemic. Why? At the beginning of the pandemic, the number of people who died with pregnancy related issues increased sharply. This was partly due to pandemic-related disruptions. For example: Hospitals hadn't figured out how to deliver obstetric care safely.  Health systems were not set up yet to manage telehealth, and many doctors stopped seeing patients in person.  Hospitals were crowded and emergency rooms were filled with COVID-19 patients. There were also barriers like having kids home from school that didn't allow for parents to get away for medical appointments. Q4: The CDC recently reported that Black people are dying from childbirth at greater rates than white people. Why is this happening? Vaccination rates among pregnant people are lower in general. Fewer than one-third of pregnant people were vaccinated before or during their pregnancy. But the lowest rates have been among pregnant Black people, at 15 percent.  In Oregon, Black people have suffered disproportionately from the pandemic.  While the percentage of people who tested positive with COVID-19 who died is higher among the White community, the percentage of people with COVID-19 who were hospitalized is higher in the Black community. And the overall case rate per 100,000 among the Black community is nearly double that of the white community. But, Black people experienced higher rates of maternal mortality long before COVID-19. Reasons for this include: Structural health inequities that have complex root causes:   Multiple factors contribute to these disparities, such as variation in quality healthcare, underlying chronic conditions, structural racism, and implicit bias. Social determinants of health have historically prevented many people from racial and ethnic minority groups from having fair opportunities for economic, physical, and emotional health. The leading causes of pregnancy-related deaths are cardiovascular conditions, and infections. Research has found that cardiomyopathy, a disease of the heart muscle; blood clots to the lung; and hypertensive disorders of pregnancy contribute to a higher proportion of pregnancy-related deaths among Black people than among white people. Some experts find that pregnant people's complaints are often dismissed, and that is probably much more significant for Black and brown people. In the United States, one-third of the pregnant people and new mothers who died in 2020 were Black, though Black people only make up 13% of the total population.  In Oregon, Black people make up only 2.2% of the total population of the state.  According to the Oregon Center for Health Statistics (CHS), there were 19 maternal deaths in 2020, two (10.5%)  of those were of Black birthing parents. Q5: Is it true that if you get vaccinated during pregnancy, this can protect your child during pregnancy and after the baby is born? In general, mothers pass antibodies to their babies during pregnancy. This protects them against diseases in the early months of life when they have no antibodies of their own. Recent reports from the CDC have shown that people who received the COVID-19 mRNA vaccines during pregnancy similarly can transmit antibodies to their fetuses, which may help protect them from COVID-19 after they are born. The antibodies have been found in umbilical cord blood. At 6 months old, 57% of infants born to vaccinated people had detectable antibodies against COVID-19. mRNA COVID-19 vaccination during pregnancy can help protect babies younger than 6 months old from hospitalization due to COVID-19. The CDC's report noted 84% of babies hospitalized with COVID-19 were born to people who were not vaccinated during pregnancy.  The CDC also reports that women who breastfeed and have been vaccinated with an mRNA vaccine have antibodies in their breast milk, which can help to protect their babies. The COVID-19 vaccines are not considered a risk for people who are breastfeeding, nor for their nursing babies because the vaccines do not contain a live virus or alter people's DNA. Q6: What are the long-term effects on the baby when a person gets a COVID-19 vaccine during pregnancy? Scientific studies to date have shown no safety concerns for babies born to people who were vaccinated against COVID-19 during pregnancy. Based on how these vaccines work in the body, experts believe they are unlikely to pose a risk for long-term health effects.  Q7: Is there a risk for miscarriage or infertility from the COVID-19 vaccine? NO! The COVID-19 vaccines have undergone rigorous safety testing. Researchers have found both the Pfizer and Moderna vaccines produced robust immune responses in pregnant people and did not damage the placenta.  There is currently no evidence that any vaccines, including COVID-19 vaccines, cause fertility problems in people.  None of the three FDA-approved vaccines change people's DNA or interact with the organs necessary for reproduction.  People who are thinking about having children can get the vaccine as an effective way to prevent infection and complications caused by COVID-19 during pregnancy.  It is not necessary to avoid getting pregnant after getting vaccinated against COVID-19. Q8: How can pregnant people protect themselves and their fetus from COVID-19? While being fully vaccinated and boosted is the safest and best way to protect yourself from COVID-19, we recommend that people who are pregnant or nursing talk with their doctor about any doubts or worries they might have about the vaccines. Every person is different, with different needs to consider about their health care decisions.  Whatever people decide, we recommend that people continue to take precautions to prevent infection during pregnancy, such as wearing a mask in crowded indoor settings and distancing from others, especially when transmission levels are high.  Q9: Which COVID-19 vaccine should pregnant people receive?  In most situations, including for people who are pregnant or breastfeeding, the mRNA (Pfizer and Moderna) vaccines are preferred over the Johnson & Johnson vaccine for primary and booster vaccination.  The J&J COVID-19 vaccine can cause low platelets and blood clots in large blood vessels. The J&J/Janssen COVID-19 vaccine may be considered in some situations, including for persons who Had a severe reaction after an mRNA vaccine dose or who have a severe allergy to an ingredient of mRNA COVID-19 vaccines Would otherwise remain unvaccinated for COVID-19 due to limited access to mRNA COVID-19 vaccines Document accessibility: For individuals with disabilities or individuals who speak a language other than English, OHA can provide information in alternate formats such as translations, large print, or braille. The Food and Drug Administration and Centers for Disease Control and Prevention have made booster recommendations for all three COVID-19 vaccines available in the U.S., including authorizing a mix-and-match option for booster shots from Johnson & Johnson (J&J), Moderna or Pfizer.For people who received Moderna or Pfizer vaccines, booster vaccinations are recommended 6 months or more after finishing the initial two-dose series for those age 65 and older and for younger adults 18 and older who work or live in high-risk settings or have underlying medical conditions that increase their COVID-19 risk. For people who got the J&J COVID-19 vaccine, booster vaccinations are recommended for those 18 and older and who were vaccinated two or more months ago."What mix-and-match means is that regardless of what you got for your primary series, you could get any of the other three vaccines available for use in the U.S. as your booster if you're eligible for a booster," says Dr. Gregory Poland, head of Mayo Clinic's Vaccine Research Group. So how do you know which booster vaccination to choose?"All of the boosters will dramatically boost your antibody response," says Dr. Poland. "I would make the decision about a booster based on how did you respond to whatever you got originally? And are there any unique risk factors that you have?"People who responded well to the first vaccine with minimal side effects can choose to get the same brand for their booster vaccination. But there are reasons someone might choose a different vaccine. For example, a younger man who initially got the Moderna or Pfizer vaccine might want a J&J booster, because the mRNA vaccines are linked with a slight risk of heart inflammation called myocarditis. The pharmacist administered the J&J COVID-19 vaccine to a patient as closing. When billing after hours, the DUR showed “do not mix vaccines.” The pharmacist refuses to do further vaccines in the current conditions out of concern for patient welfare. Hypothetical Scenario #3 A pharmacist decided to “take a break” when COVID immunizations began, saying “I have heard that the vaccine is worse than the disease, and I can't do harm to patients.” The pharmacist has now requested a return to work and is claiming a religious exemption from administering immunizations. Hypothetical Scenario #4 A new patient comes to the pharmacy and states they previously received ChAdOx1 nCov-19 in the UK but would like to get a dose of BNT162b2 now.  Hypothetical Scenario #5 A regular patient presents a ‘prescription' to the pharmacy, and it states the following: “‘Patient' has previously received J&J Covid vaccine. Unfortunately, she was a chemotherapy patient at that time. She has no measurable antibodies. I have recommended that she be vaccinated as is she'd never been vaccinated before. She should receive 2 doses of either the Pfizer or Moderna vaccine. Thank you.”  Hypothetical Scenario #6A pharmacy's staff all have young children (ages 5 - 7 years old).  The staff conduct their own research, and they conclude that the mRNA vaccines are safe for young children. They consider using leftover doses of vaccine for their own children, as opposed to wasting these valuable doses. Hypothetical Scenario #7Your neighbor, a physician, has been caring for her elderly parents and children as her fulltime job. She was fully vaccinated in January during the first phase and comes in the pharmacy for a booster on mRNA1793. You know she is not immunocompromised.  Interested in the learning more? Nigeria is expected to receive 176,000 doses of the Johnson and Johnson COVID-19 vaccines on Wednesday. Executive Director, National Primary Health Care Development Agency, Faisal Shuaib, said at a news conference in Abuja on Tuesday.He stated the J&J single shot vaccine is safe and efficient against the coronavirus disease, including the deadlier Delta variant. It is expected to boost Nigeria's COVID-19 fight with the Delta variant sparking fears of a third wave of the pandemic. Dr. Shuaib had recently announced that the second phase of the COVID-19 vaccination programme would commence soon.The exercise had earlier been scheduled to begin on Tuesday, but it was postponed due to “unforeseen circumstances,” according to a spokesperson in the Office of the Secretary to the Government of the Federation, Willie Bassey. The need for the rollout of the second batch of the vaccines has, however, become crucial as Nigeria gradually returns to seeing a spike in infections. As of Monday, 422 new cases of COVID-19 were recorded in the country, with five more deaths.According to the Nigeria Centre for Disease Control (NCDC), the infections were recorded in nine states. Lagos maintained the lead with 190 cases, followed by Rivers (86) and Ogun (85). Others are Oyo (22), FCT (20), Kwara (7), Edo (5), Abia (4), and Bayelsa (3). So far, a total of 178,508 cases have been confirmed, 165,983 patients have recovered and 2,192 deaths have been recorded in 36 states, including the Federal Capital Territory.

It was another tragic weekend in the midst of our national epidemic of gun violence, with mass shootings taking place in Wisconsin, Texas, Nebraska, and Ohio. That was after the shooting last week at a FedEx warehouse in Indiana, which many have called to investigate as a hate crime against Sikhs. We discuss the shootings, as well as what’s happening in Minnesota in relation to the police killings of Daunte Wright and George Floyd. The worldwide death toll from COVID reached 3 million over the weekend. In lighter news, half of the U.S. population aged 18 and older has gotten at least one dose of a vaccine. The White House announced a plan to track variants.  And in headlines: Republican lawmakers in Florida aim to make many forms of protest illegal, Biden walks back his decision to cap the number of refugees allowed into the U.S., and NASA will attempt to fly a helicopter on Mars. Nigeria primary healthcare chief told Reuters that the country hopes to receive up to 70 million doses of the Johnson & Johnson COVID-19 vaccine this year through the African Union amid concerns about delayed deliveries of AstraZeneca shots.Nigeria has recorded 2,049 deaths from COVID-19 and began vaccinations this month, plans to inoculate 40% of its people this year and another 30% in 2022.Johnson & Johnson says it will supply the AU with up to 400 million doses of its single-dose vaccine beginning in the third quarter.The head of Nigeria’s National Primary Health Care Development Agency, Faisal Shuaib told Reuters that Nigeria expects to initially receive 30 million doses of the Johnson & Johnson vaccine in July through the AU.

The Food and Drug Administration and Centers for Disease Control and Prevention have made booster recommendations for all three COVID-19 vaccines available in the U.S., including authorizing a mix-and-match option for booster shots from Johnson & Johnson (J&J), Moderna or Pfizer.For people who received Moderna or Pfizer vaccines, booster vaccinations are recommended 6 months or more after finishing the initial two-dose series for those age 65 and older and for younger adults 18 and older who work or live in high-risk settings or have underlying medical conditions that increase their COVID-19 risk. For people who got the J&J COVID-19 vaccine, booster vaccinations are recommended for those 18 and older and who were vaccinated two or more months ago."What mix-and-match means is that regardless of what you got for your primary series, you could get any of the other three vaccines available for use in the U.S. as your booster if you're eligible for a booster," says Dr. Gregory Poland, head of Mayo Clinic's Vaccine Research Group. So how do you know which booster vaccination to choose?"All of the boosters will dramatically boost your antibody response," says Dr. Poland. "I would make the decision about a booster based on how did you respond to whatever you got originally? And are there any unique risk factors that you have?"People who responded well to the first vaccine with minimal side effects can choose to get the same brand for their booster vaccination. But there are reasons someone might choose a different vaccine. For example, a younger man who initially got the Moderna or Pfizer vaccine might want a J&J booster, because the mRNA vaccines are linked with a slight risk of heart inflammation called myocarditis.
