Gazelka, M.D. (@hmgazelkamd) The path to approvals for a booster dose of the Pfizer COVID-19 vaccine has seemed complicated.  "This is a confusing set of recommendations," says Dr. Gregory Poland, an infectious diseases expert and head of Mayo Clinic's Vaccine Research Group. "So let's look at the four groups approved for only the Pfizer booster, thus far. If you got a primary series of the Pfizer vaccine, you're 65 and older, and it's been six months or more, you are eligible for a booster. If you're 50–64 and you have medical conditions that place you at high risk, you are eligible for the booster. The Secretary-General notes his appreciation of UNFPA's powerful advocacy to advance the sexual and reproductive health and rights agenda, to address population dynamics, and to empower youth, as well as UNFPA's focus on accountability and effective management, and its commitment to delivering transformative results in both development and humanitarian settings to end the unmet need for contraception, end preventable maternal deaths and end gender-based violence in all its forms. The Secretary-General looks forward to furthering this work, together with UNFPA and partners, and with urgency towards the 2030 deadline. Dr. Kanem brings more than 30 years of strategic leadership experience in the fields of preventive medicine, public and reproductive health, social justice and philanthropy to the role. SECRETARY-GENERALOn Saturday, the Secretary-General will be going to Barbados to take part in the opening ceremony of the UN Conference on Trade and Development, known as UNCTAD15. The opening ceremony will take place in Bridgetown, on 4 October. In his remarks, he is expected to underscore the challenges of tackling debt distress and the need to get down to the business of a sustainable and equitable recovery for all. He is also expected to highlight the need to re-ignite the engines of trade and investment, and ensure they benefit the poorest countries, as well as the need to build a global green economy. AFGHANISTAN In September the World Food Programme provided three million people with food assistance, double the number of people reached in August. This included 1.5 million women and girls, and 1.5 million men and boys. So far this year, 8.4 million people have received food assistance in the country's 34 provinces. WFP aims to provide emergency food and nutrition assistance to 14 million people by the end of the year. With winter approaching, humanitarian partners are working to get food and other non-food items prepositioned at strategic locations. You will recall that on 13 September, donors and Member States pledged more than $1.2 billion in humanitarian and development aid for Afghanistan. Since then, only $135 million has been received in support of the Afghanistan Flash Appeal seeking to help 11 million people before the end of year. We urge donors to fast-track the disbursement of funds to address the rapidly growing humanitarian needs that are set to worsen in winter. There is a risk of pipeline breaks in October and November in the provision of food, agriculture, health, nutrition, protection, education and water and sanitation hygiene assistance, due to funding shortfalls and import and transport delays. SYRIA Moving to Syria, we are concerned about the dire situation of civilians in the northwest where hostilities, an economic crisis and COVID have made the situation of already vulnerable people even more difficult. Hostilities continued to be reported across northwest Syria throughout September, with intensified airstrikes along the frontlines in southern Idlib and almost daily reports of violence. We are also extremely concerned about the spike in COVID-19 cases, with over 1,000 people per day testing positive in northwest Syria. There has been a 170 per cent increase in the total number of positive cases in the last month alone. Limited equipment to test for COVID-19 is a problem, on top of a severe oxygen shortage. In addition, less than 3 per cent of the population in the northwest is vaccinated. With 1.6 million people living in crowded camp settings the spread of COVID-19 will further tax an overburdened system. In addition, 97 per cent of the population in northwest Syria live in extreme poverty depending on humanitarian aid for food, medicine and other basic services. Progeria is a rare disease that causes premature aging in childhood; the FODMAP diet is explained as a treatment for IBS; J&J vaccine restarted; Question of the month: Fever and Cough.Introduction: Low FODMAP Diet and J&J COVID Vaccine is back.  By P. Eresha Perera, MS3, and Sherika Adams, MS3.Today is May 10, 2021.Irritable Bowel Syndrome. Patients with IBS frequently have other conditions such as anxiety, depression, somatization, fibromyalgia, chronic fatigue syndrome, GERD, dyspepsia, non-cardiac chest pain, chronic pain, and other mental illness. A common triad we see in the clinic is: Anxiety + Fibromyalgia + IBS. Treating these conditions is hard, and even more so when they are combined. Let’s focus for now on IBS treatment. Recently we had a patient with IBS who had a laparoscopic cholecystectomy and of course was complaining of abdominal pain and constipation. We mentioned the low FODMAP diet as part of the treatment. The low FODMAP diet has been proven for the treatment of irritable bowel syndrome (IBS) and or small intestinal bacterial overgrowth (SIBO). It has decreased symptoms in 86% of people. FODMAP is an acronym that stands for fermentable oligosaccharides, disaccharides, monosaccharides, and polyols. This diet attempts to restrict these short-chain carbs that are poorly absorbed by the small intestine, resulting in cramping, constipation, diarrhea, bloating, and gas or flatulence.You can recommend your patients to follow 3 steps: Step 1: Eliminate foods that are high on FODMAP, Step 2. Determine which foods cause symptoms by reintroducing eliminated foods slowly, and Step 3. After identification of the FODMAP foods that cause symptoms, remove them completely from the patient’s diet. Dr. Hazel Galon Veloso, John Hopkins's gastroenterologist, recommends doing step 1 for 2-6 weeks and step 2 reintroducing a high FODMAP food back into diet every 3 days. Example of HIGH FODMAP foods: Dairy-based milk, yogurt, ice cream, wheat products (cereal, bread, and crackers), beans, lentils, vegetables like artichokes, asparagus, onions, and garlic, and fruits such as apples, cherries, pears, and peaches. Example of LOW FODMAP foods: Eggs, meat, cheese such as Brie, cheddar, and feta; almond milk, rice, quinoa, oats, potatoes, tomatoes, cucumbers, zucchini, grapes, oranges, and strawberries.If available, Fodmap should be initiated with the advice of a nutritionist that can help with the transition, prevent over-restriction and nutritional replete diet. Consider this diet as an initial treatment for your patients with IBS.Vaccination with J&J COVID 19 Vaccination has been restarted.On a different note, On April 23, 2021, the CDC’s Advisory Committee on Immunization Practices (ACIP) has recommended to restart vaccination with the Janssen/Jonson & Jonson COVID-19 vaccine after a pause on April 13, 2021[2]. After giving the J&J vaccine to almost 8 million patients, 15 cases of Thrombosis with Thrombocytopenia Syndrome (TTS) were reported and three of them died. The recommendation was given after a risk-benefit analysis that determined that the benefits of the vaccine outweigh the risks. The risk of TTS in women age 18-49 still exists, but it is considered very low when compared to all the risks carried by COVID 19 itself. Under the emergency use authorization, the Jonson & Jonson vaccine is considered highly effective and safe. It’s progressive and causes children to age very quickly within the first few years of their life. The disease is not evident at birth. S. Sadiq-Ali: Exactly! Usually, kids will start developing symptoms within their first year of life with the first symptom being failure to thrive. Other common features include a disproportionately large head for their face, narrow nasal ridge and tip, small mouth, retro and micrognathia, little to no subQ fat with small outpouchings, delayed eruption of primary teeth, progressive joint contractures, and essentially all geriatric conditions like alopecia, osteoarthritis, and hearing loss. One interesting tidbit though is that their motor and mental development is normal! H. Arreaza: A child getting old quickly, that’s so interesting. What’s the pathophysiology of this condition?V. Phung: So, it’s due to a genetic mutation - a single nucleotide polymorphism - in the LMNA gene known as lamin A. This gene codes for the lamin A protein which holds the cell’s nucleus together. A mutation causes your body to make a much smaller protein called progerin. Progerin is not stable so it doesn’t hold the cell’s nucleus together properly. This instability is thought to be the cause of premature aging. S. Sadiq-Ali: That’s right Veronica! There are two common mutations – the classic form and the non-classic form. The difference between the two forms is where in the gene the mutation occurs. H. Arreaza: So, if I suspect my patient has progeria, I should do a genetic test for the LMNA gene mutation. How common is progeria?S. Sadiq-Ali: About 1 in every 4 to 8 million births is affected by progeria. Unlike many other conditions, there aren’t any predisposing factors - such as gender, location, or ethnicity. It’s completely random! Right now, about 179 children across 53 different countries have been diagnosed with progeria. 18 of those cases are here in the US. One family in India, has had 5 children with progeria. Another interesting fact is that there have been only 2 known cases of a completely healthy person carrying the mutated gene!  V. Phung: Since they’re aging so rapidly and prematurely, their life expectancy is about 14.5 years. However, the oldest believed survivor - Tiffany from Ohio – has lived up to the age of 43! H. Arreaza: And she is still alive, as far as I know. What can be done in terms of management to ensure these children and adults can live their best, most comfortable life? S. Sadiq-Ali: There’s no cure so you’d want to manage any symptoms and make sure the child is getting proper nutrition. Generally, the recommendation is to have small frequent meals, maintain good hydration, do routine PT and exercises, use shoe pads since they don’t have much body fat to provide cushioning, use plenty of sunscreen, prescribe anticoagulation as needed for geriatric conditions like CAD/CVD, and manage any fractures or dislocations that may occur. It requires a multidisciplinary care team.H. Arreaza: So, you mentioned Tiffany Wedekind, the person with progeria who has lived the longest. Now, I want to mention Sam Berns, maybe the most famous person with progeria. “Life According to Sam” is an HBO documentary directed by Sean Fine and Andrea Nix Fine. It was presented in January 2013 at the Sundance Film Festival (I love Park City, Utah). The documentary explains the impact of progeria on the lives of Sam Berns and his parents, Dr. Leslie Gordon and Dr. Scott Berns. Sadiq-Ali: These kids are aging so quickly they have geriatric conditions; do they die from natural causes or from heart disease and stroke? V. Phung: That’s a great question. Unfortunately, yes. Death is commonly due to complications from atherosclerosis, cardiac disease, and cerebrovascular disease - like a heart attack or stroke. S. Sadiq-Ali: “The Curious Case of Benjamin Button” is usually thought to be an example of progeria, but it’s actually the opposite: A child born as an adult who dies as a baby.H. Arreaza: That was really educational. Progeria, a rare disease that you should know about, in case someone asks you. Thrombosis and thrombocytopenia caused by COVID-19 vaccines: How to identify and treat VITT, VIPIT, or TTS

At least 17 cases of thrombosis and thrombocytopenia have been reported in patients who received the Johnson & Johnson COVID-19 vaccine in the United States. Such events have been reported in patients who received the AstraZeneca vaccine as well. In this episode, Adam C. Cuker, MD, of the University of Pennsylvania, Philadelphia, tells host David H. Henry, MD, how to identify and manage patients with these vaccine-induced events. What’s in a name? The phenomenon of vaccine-induced thrombosis and thrombocytopenia has been given different names, including: Vaccine-induced immune thrombotic thrombocytopenia (VITT) Vaccine-induced prothrombotic immune thrombocytopenia (VIPIT) Thrombosis and thrombocytopenia syndrome (TTS). Dr. Cuker’s preferred acronym is VITT. VITT is an immune-mediated reaction to the Johnson & Johnson and AstraZeneca vaccines that “results in thrombocytopenia and a strong propensity for thrombosis,” Dr. Cuker explained. Dr. Henry noted that VITT is reminiscent of heparin-induced thrombocytopenia (HIT). Incidence unclear VITT appears to be “very rare,” but “we still don't have a great sense of how common it is” because additional cases may not have been recognized or have yet to present, Dr. Cuker said. VITT occurs about 5-30 days after vaccination. VITT appears to be mediated by IgG antibodies, which take time to build up. The exact mechanism is unknown, but VITT could be related to the adenovirus vector used in the Johnson & Johnson and AstraZeneca vaccines, Dr. Cuker said. The first 15 cases of VITT associated with the Johnson & Johnson vaccine occurred in women, and most patients were aged under 50 years. In Canada, where the AstraZeneca vaccine is available, cases of VITT have been reported in patients in their 80s and 90s. Diagnosing VITT Symptoms of VITT can include severe, unrelenting headache; severe abdominal pain; nausea and vomiting; as well as typical signs and symptoms of deep vein thrombosis or pulmonary embolism. To determine if a patient has VITT, Dr. Cuker recommends ordering a disseminated intravascular coagulation panel – prothrombin time, partial thromboplastin time, fibrinogen, and D-dimer – as well as a standard HIT enzyme-linked immunosorbent assay (ELISA). Rapid immunoassays for HIT are not reliable for VITT, so HIT ELISA must be used, Dr. Cuker emphasized. Most patients with VITT have a “strongly positive” ELISA with optical density values “well in excess of 100 or 1.0,” depending on the scale, Dr. Cuker said. Manage VITT like HIT Patients should receive an anticoagulant, but not heparin, Dr. Cuker said. It isn’t clear if heparin will be harmful in patients with VITT, but current guidelines recommend avoiding heparin. He also advised against using warfarin or vitamin K antagonists in patients with VITT “at least until their platelet count recovers.” High-dose intravenous immunoglobulin (e.g., 1 g/kg for 2 consecutive days) is recommended, as it is believed to interfere with platelet activation. Gazelka, M.D. (@hmgazelkamd)  Dr. Gregory Poland, an infectious diseases expert and head of Mayo Clinic's Vaccine Research Group says COVID-19mutations and the virus spread are happening because of people who don't wear masks, who don't get vaccinated and who don't adhere to safety recommendations. "I believe that we should be radically transparent and honest," says Dr. Poland. "The more time this virus passes through one person after another, the more likely it continues to mutate. As a result of those mutations, two things are happening. Some of the mutations are making vaccines and plasma monoclonal antibodies less effective. The other thing is that the virus will likely become something that we have to live with for the rest of our lives." Dr. Poland reminds people that vaccine protection is not 100%. Breakthrough infections can occur. "Remember that in the clinical trials, 95% means that compared to unvaccinated people, your risk is reduced by 95% — not 100%," he says. "You might have a mild case of COVID-19,but you can still spread it to others, including those who are immune compromised, such as cancer patients. That's why we continue to wear masks until we get very widespread immunization." We have two brief segments in today’s episode: News and analysis of the Johnson & Johnson COVID vaccine pause, and the widespread hack of Microsoft Exchange Servers across the country (and the dramatic and unusual steps the FBI took in response).Johnson & Johnson: Federal health officials last week revealed that six women who received the vaccine had developed rare and severe blood clots in their brain, in one case fatally. Even more recently, a panel of expert advisors to the Centers for Disease Control determined that they needed more time to assess the risk of the drug, which was approved by the FDA under Emergency Use Authorization, or EUA.Our experts are General Partner Jorge Conde, who previously appeared on a episode on J&J efficacy rates, and General Partner Vineeta Agarwala, who is also a practicing clinician at Stanford Hospital, and recently joined us on "16 Minutes" with Dr. Bob Wachter of UCSF to analyze the vaccine rollout in the U.S. They address the clinical facts about the six J&J cases (and contrast it to the social media conversations and headlines), the incidence rate, and what the J&J vaccine shares with the Astra Zeneca vaccine, which has been halted or limited in Europe and elsewhere over similar blood clot concerns.FBI and Microsoft Exchange Servers: The Department of Justice recently announced that the FBI, after getting court authorization, had removed malicious code from hundreds of computers running on-premises versions of Microsoft Exchange Server software used to provide email services. In March, Microsoft had announced the initial hack and released detection tools and patches to help owners of the compromised computers, but the latest government announcement revealed that the FBI had taken the step of removing the malicious code, in this case web shells that enable remote administration, from computers that had not mitigated the risk. US health authorities on Tuesday recommended pausing the Johnson & Johnson Covid shot over blood clot fears, with the company quickly announcing it would delay its European rollout, in a setback for global immunization campaigns.Official says out of nearly seven million Americans who have so far received the single-dose vaccine, six women aged between 18 and 48 developed a rare type of clot in the brain along with low platelets.Food and Drug Administration scientist Peter Marks says the disorder might be triggered by a rare immune response to the vaccine similar to that seen among a few hundred recipients of the AstraZeneca jab in Europe.Both vaccines are based on adenovirus vector technology.The European Union has signed a deal for 200 million shots of the vaccine with an option for 200 million more.US authorities are now carrying out an investigation which could end up leading to tough regulatory choices, such as restricting the J&J shot to subsets of the population.People who have received a J&J shot within the past three weeks were asked to report to their doctors if they experienced severe headaches, abdominal pain, leg pain, or shortness of breath.

Protesters and police clashed again last night in Brooklyn Center, Minnesota, over the death of Daunte Wright.  It's been just over 100 days since Brexit began in earnest, and British exporters aren't happy with how things are going. Trade with Europe has taken a major hit, with exports of goods to the European Union plummeting by more than 41% in January and recovering only modestly after that.

South Africa’s joint lead investigator for the Johnson & Johnson COVID-19 vaccine trial on Tuesday said that a government regulator was processing an application for the vaccine to be granted emergency use authorisation.The official, Glenda Gray said discussions were underway with regulatory authorities to roll out the single-dose vaccine to health workers, after the government on Sunday halted distribution of the AstraZeneca vaccine over efficacy concerns.South Africa had given fast-track approval to AstraZeneca’s COVID-19 vaccine for emergency use last month and had said it was reviewing applications by Pfizer and Johnson & Johnson.Gray said J&J also submitted a rolling submission to SAHPRA to expedite the regulatory processes in South Africa, so this AD26 vaccine is being processed for emergency use.
