LUIS MIRANDA BIO Luis Antonio Miranda Concepción is a political strategist, philanthropist, advocacy consultant and author, his latest book: "Relentless: My Story of the Latino Spirit That Is Transforming America” with a foreword by his son, Lin-Manuel Miranda.  Luis was born and raised in Puerto Rico and moved to NYC at 19 when he received a scholarship from NYU.  In the 1980s, Miranda was a special advisor for Hispanic Affairs to the Mayor of New York City, Ed Koch, serving as Director of the Mayor's Office for Hispanic Affairs. In this week's episode, host Dr. Mark Hoffman discusses quality and safety in OBGYN and medicine as a whole with Dr. Komal Bajaj, a professor of OBGYN at Albert Einstein College of Medicine, reproductive geneticist and Chief Quality Officer at NYC Health and Hospitals/Jacobi in North Central Bronx. In addition, Dr. Bajaj is the clinical director of the NYC Health Simulation Center. --- SHOW NOTES The episode begins by discussing the core principles of a Chief Quality Officer and how a typical week demonstrates those values. Dr. Bajaj emphasizes the importance of measurement as a key tool for quality and safety assessment. Information is helpful, however data literacy and transparency is also important to help the healthcare community to better understand a set of data. A significant challenge in ensuring quality and safety is identifying a precise dataset and developing an effective measurement methodology, which may be costly and labor intensive. Dr. Bajaj discusses how a culture of safety adopted by the entire healthcare team protects the patient and adds to the quality of care. An additional key principle is learning from events through risk management; each event should be thoroughly discussed as an opportunity for future changes. According to Dr. Bajaj, a crucial aspect of her role as the quality and safety officer is to take into account regulatory and accreditation standards. These guidelines aid healthcare systems in identifying areas of care that require improvement. Dr. Bajaj often looks at her hospital as a functioning ecosystem, with multiple interplaying parts, each which interact with one another to help make patient care of the best quality. She stresses the importance of physicians as leaders understanding the interlinking between the different parts of the system to improve patient safety and outcomes. Dr. Hoffman and Dr. Bajaj engage in a discussion about the prevalent issue of resistance when it comes to finding solutions for enhancing quality and safety. They strongly advocate for empowering providers to take initiative in identifying problems within their institutions and seeking necessary changes. The two doctors believe that unanimity is not always essential to make a positive impact on quality and safety. Rather, they acknowledge that some providers may require more time to embrace change in their routines. Dr. Hoffman and Dr. Bajaj then delve into the relationship between quality, safety, and the financial aspects of healthcare. They reach a conclusion that emphasizes how quality and safety initiatives in a healthcare system provide financial incentives for all stakeholders, including staff, patients, and the entire ecosystem. The level of care patients receive directly influences crucial factors such as hospital funding, ratings, and assessments by national organizations. The future of quality and safety in OBGYN is explored last. Dr. Bajaj introduced the idea of a person-centeredness approach to quality and safety that incorporates staff and patients' voices into initiatives. Then she shared how an emphasis on the basics, safety, an empowering culture, and strong leadership are being re-examined as key goals for the future. In the first half of 2020, New York City quickly became the American epicenter of the COVID-19 pandemic, with over 200,000 cases reported in the first few months. The city came to a standstill as thousands of people died alone in hospitals and bodies piled up in freezer trucks that could not transport them away fast enough. In August 2020, amid this cataclysm, Dr. Dave Chokshi assumed position as New York City's Health Commissioner and began the arduous task of repairing a broken city and restoring public trust among its residents. Prior to this work, Dr. Chokshi led the NYC Health and Hospitals Corporation and was a White House Fellow at the US Department of Veterans Affairs. My next guest, Dr. Amanda K. Johnson, MD, MBA is Assistant Vice President of Care Models in the Office of Ambulatory Care and Population Health for NYC Health + Hospitals. In this role, she leads a team to deliver enhanced primary care, outreach, and engagement models for people experiencing structural barriers to accessing health care, such as homelessness or involvement in the criminal legal system. She also serves as Director of the Take Care pillar of the Test & Trace Corps, which provides New Yorkers with the resources needed to effectively isolate or quarantine due to COVID-19 and connects people with Long COVID with physical health, mental health, financial, and community supports.  She completed her residency in internal medicine at UCSF's primary care program based at San Francisco General Hospital. She received her medical degree from Harvard Medical School and her MBA from Harvard Business School. Those of us in the New York City area saw our classmate, Oxiris Barbot, as a leader at the beginning of COVID. A doctor and life-long advocate for health care equity, Oxiris helped all of us through those first difficult first months. Here is a bit more about Oxiris from ChangeLab Solutions, where she is a board member: With more than 25 years' experience in public health and health care delivery, Dr. Oxiris Barbot has dedicated her career to achieving health equity. When she was commissioner of health for New York City, she led the nation's largest municipal health department in centering an equity agenda on communities, bridging public health and health care delivery, and leveraging data for action and policy. She successfully guided the city's responses to the first wave of the COVID-19 pandemic and to New York City's largest measles outbreak in 30 years. As an innovative public health leader, Dr. Barbot has championed addressing health inequities in major cities along the East Coast. In 2010, Dr. Barbot was appointed commissioner of health for Baltimore City. During her tenure in Baltimore, she led the development of Healthy Baltimore 2015, a robust health agenda dedicated to ensuring that all Baltimore residents realize their full health potential. Under her leadership, the City of Baltimore achieved a record reduction in the rate of infant death, among many achievements in health. From 2003 to 2010, Dr. Barbot served as medical director of New York City's public schools. In this capacity, she spearheaded development and implementation of an electronic health record system that improved delivery of health services for more than 1 million children. Prior to her work in New York City, Dr. Barbot served as chief of pediatrics and community medicine at Unity Health Care, Inc., a federally qualified health center in Washington, DC. Dr. Barbot holds a bachelor's degree from Yale University and a medical degree from the University of Medicine and Dentistry of New Jersey. She completed her pediatric residency at George Washington University's Children's National Medical Center.
