The $100,000 H-1b visa fee landed in September 2025 like a fire alarm in the hallways of medicine—hospitals panicked, advocacy groups mobilized, and a lot of people predicted the international resident pipeline would collapse. Dr. Bryan Carmody, The Sheriff of Sodium rejoins co-hosts David Lee, Mukund Viswanadha, and Isa Perez-Sandi to ask the question nobody was asking: was the panic grounded in reality? More interesting to think about: why rural physician shortages are a compensation and incentive problem, not a numbers problem; why loan forgiveness alone probably isn't moving the needle; and what separates effective physician advocacy from "just expressing emotion and hoping facts do the work." Then graduating high-school listener Aditi asks whether community college is a viable launch pad for medicine.

They talk about Addicted to the Knicks, the idea of “KA,” Knicks fandom as church, childhood obsession, superstition, dads, recovery, and why caring about the Knicks feels weirdly connected to getting sober. Kevin talks about being hooked on the Knicks before drugs, his first brutal Knicks memory — leaving the Garden early and hearing Reggie Miller's eight points in nine seconds from a cab — and how addiction made him lose interest in everything, including basketball. Dave tells his legendary story of sneaking into Madison Square Garden during a Knicks/Bulls playoff game through a Calypso festival and a stoner kid in concessions. They hit Anthony Mason, John Starks, Patrick Ewing, Knicks Tape, David Lee, Nate Robinson, Brunson, KAT, Wemby, Mitchell Robinson, Tibbs, Mike Brown, and the insane feeling of New York City actually coming together around the Knicks.         Hosted by Simplecast, an AdsWizz company. For the Glory KC is back with the 181st episode of the show! Before we got to the regularly scheduled podcast content, we had to talk about the KC Soccer Journal's collab with the Olathe Indian Creek Library. That also led into Sheena's impromptu review of Nate Bukaty's new book, "Perfect Pitch" about Kansas City bidding and getting the World Cup in 2026. Nate will be a guest on a future For the Glory KC!But before then, back to our regularly scheduled content.Sporting Kansas City aren't playing matches since it's the World Cup break, but that doesn't mean there wasn't news. David Lee made waves with rumors of "four to seven, starter-level" additions being his goal for the secondary transfer window. Not only did Lee weigh in, but new majority owner Peter Mallouk chimed in as well.We talk about our thoughts on their statements then where the additions should be made. According to reports, Lee said three defenders, three attackers and a midfielder. I tried to get specific with my needs and pushed my co-host to do the same. There are so many possibilities, despite the uptick in results recently. Sporting KC are still only one spot up from the bottom of the whole league, so almost no position is safe.Beyond the roster, there was plenty of international call-up news, including the unfortunate news that no Sporting KC players made a World Cup roster. Also, SKC II won again, in a wild one against the Real Monarchs. The KC Current did play soccer this week and gutted out at 1-0 win over expansion side Boston Legacy FC. It wasn't always pretty and it was downright violent, but Kansas City found a way. Not without some injuries along the way. We talked about Kayla Sharples and Ally Sentnor's injuries, some up and down performances and of course, Temwa Chawinga's game winning goal that had plenty of players (Bethune, LaBonta, Izzy) pitching in.In the Digital Crawl, we hit on a few more topics, including:The USMNT's win over Senegal in a World Cup tune-upand the arrival of Argentina in Kansas CityHere is a rundown of sections and start times:Intro, Our Library Event and a Book Review - 0:00Sporting KC - 18:56KC Current - 52:17Digital Crawl - 1:10:38Upcoming GamesUSMNT vs Germany - SAT, June 6th @ 1:30PM CSTSKC II vs. MNUFC2 - MON, June 8th @ 7:00PMAs a special gift to For the Glory KC listeners and KC Soccer Journal readers, Backheeled.com is giving away 30 days of their amazing, independent American soccer coverage for free. The Co-Inventor of Tor on Why Your NHI Strategy Is Already BehindMost organizations have spent the last 20 years getting really good at human identity. 2FA. Biometrics. Face ID. Ephemeral tokens. They did the work. And the whole time, they were quietly pushing every ounce of that compressed risk onto the non-human side of the house.Service accounts with username and password. API keys that never rotate. Credentials hardcoded in pipelines. Long-lived tokens that were supposed to be temporary.Eventually is here.In this episode, David Lee sits down with David Goldschlag, CEO and co-founder of Aembit and one of the original inventors of onion routing — the technology that became Tor. With 20+ years building security companies, David G brings a perspective on non-human identity and AI agent security that very few people in this industry can match. They get into why NHI is not a new problem but a neglected one, what it actually means to build a zero trust framework for AI agents, the concept of blended identity and why your existing IAM stack is only part of the answer, why workforce agents and customer agents are fundamentally different and why treating them the same is a mistake, and why data is still the new oil and why that matters more now than ever.If your org is spinning up agents and hasn't had a real strategic conversation about what those agents can access, who they're acting on behalf of, and what happens when something goes wrong, this episode is exactly where you need to start.Topics CoveredThe origins of Tor and why onion routing still matters 30 years laterHow Aembit went from "Okta for workloads" to purpose-built AI agent identityThe three types of agents: autonomous, workforce, and customer-facingBlended identity and blended policy in practiceWhy ephemeral credentials are non-negotiable for agent accessZero trust for AI: the three pillars (identity, prompt security, data security)Non-repudiation in the age of agentic AIWhy vibe coders are making the NHI problem exponentially harderData security as the ultimate endpoint for every breach scenarioStay ConnectedSubscribe to the Identity Jedi newsletter at theidentityjedi.comFollow on LinkedIn, YouTube, and SpotifyRate, review, and share if this episode hit different

The Doctor Doesn't Know Either: Inside the Diagnostic Crisis

The Short Coat

Play Episode Listen Later Apr 30, 2026 70:21

The feedback loop that would make doctors better diagnosticians doesn’t exist. Louise walked five minutes. Then her legs turned to stone. She stood at the side of the road, waiting for them to work again—and nobody figured out why for thirty years. Author and New York Times Health and Science Opinion Editor Alexandra Sifferlin has spent years as a journalist fielding emails from patients who couldn’t get a straight answer from medicine—not because their doctors were incompetent, but because diagnosis is harder, messier, and more difficult to do in 15 minutes than anyone wants to admit. Her book The Elusive Body: Patients, Doctors, and the Diagnosis Crisis traces the problem from rural Kentucky to the NIH’s Undiagnosed Diseases Network, and her conversation with M4 Jeff Goddard, M1 Madelyn Klemmensen, and M2 Zach Goddard goes deep on the mechanics of how diagnostic errors actually happen: availability bias, the missing feedback loop, specialty tunnel vision, and the slow erosion of trust that pushes patients toward people selling them supplements. The students here aren’t just asking sympathetic questions, although Jeff is literally a character in the book, something Dave found out in real time on this episode. They push on the hard stuff: when is a placeholder diagnosis ethical, whether AI will save us or become a crutch, and what do you actually do about a healthcare system where the patient bounces between docs who don’t have answers. What they keep landing on is uncomfortable—medicine doesn’t have great solutions to this, but the relationship between patient and physician might matter more than the technology of medicine. Solving the diagnostic crisis might mean uncomfortable, expensive changes. What if you answer a trivia question about 2,600-year-old cataract surgery correctly, and your reward is reaching into a box of jelly beans that might taste like dead fish — and you can’t lie about it? Ah, the question no one asked. Well, the answer is here! Blechardy is back, this time in Jelly Bean Mode — where every correct answer earns you a mystery BeanBoozled jelly bean that could be peach or could be barf. PA professor Jeremy Nelson and learning community manager Cody Pritchard team up against first-year Physician Associate student Tyler Mills, second-year med student Alexis Baker, and PA1 student Megan Renner. The categories: ancient medical procedures, misunderstood body parts, legends of medicine, internet memes from 2010–2020, and general knowledge. The chaos starts immediately when nobody can figure out what ancient healers packed wounds with (honey and moldy bread, for the record), and it only escalates from there. Fixing cataracts in 600 BCE, the ancient theory about what the other testicle does, the gland who’s name means “mucus” — mixed in with memes and the Mandela Effect. Jeremy buzzes in confidently and wrong more than once, Tyler steers the ship repeatedly into icebergs, and Alexis can’t quite tell if her jelly bean is genuinely bad or just a flavor she doesn’t like. The real winner? Probably the audience, who gets to watch five adults negotiate whether toothpaste-flavored candy counts as yummy. Oh, and Dave. Dave wins, too, because he only had to eat one gross bean at the end or people would have rioted. Everyone else? Hosed. For the Glory KC is back with the 176th episode of the show!Sporting Kansas City had what was potentially the lowest moment on the field in club history. At the very least, in recent memory. So, a week after talking about expectations, Sheena and I are trying to align on the state of the team. Fans are rightfully upset with the performances of the week, particularly the showing in the US Open Cup against the Colorado Springs Switchbacks.Fans are calling for Raphel Wicky's job, David Lee's job, Peter Mallouk to (already) sell the team, and for the release or purge of almost every player on the roster. What is rational and what is irrational? We try to break it all down.Plus, we weave through the moments in the USOC game as well as the loss to the Vancouver Whitecaps on Friday night. We knew this season would start slow and have some lows, but I'm not sure anyone thought it would be quite this low. CMS' Health Technology Ecosystem Initiative (HTE) is an ambitious effort announced by the White House and CMS to modernize the nation's digital health infrastructure and expand data sharing beyond traditional clinical settings into the broader health and wellness ecosystem. At the center of this initiative is the CMS Interoperability Framework, which builds on prior federal interoperability efforts and emphasizes standardized data exchange, defined participant roles, secure and privacy-respecting data sharing, and patient-centric access. Ann Tiffany, Compliance Consultant, Clearwater, speaks with Melissa Soliz, Partner, Coppersmith Brockelman, and David Lee, Principal, Leavitt Partners, about what the HTE means for privacy, data governance, stakeholder obligations, enforcement, and liability risk. How Away Rotations play into getting that dream residency Among all the strange things about medical school, there’s the so-called “away” or “audition” rotation. Recently matched M4s Aditi Katwala, Hend Al-Kaylani, Lena Volfson, and Kristin Davis talk about what it’s like to leave CCOM for weeks at a time to visit another hospital. Maybe they want to experience some new things there they wouldn’t have seen at Iowa. But also, it’s often about showing off their med-student skills for a residency program they might match with in another part of the country. Spoiler–that’s not exactly how it worked out for them, but they learned a whole lot and ultimately that’s the point. This episode will clue you into the strategies, reasons, benefits and limitations of doing an advanced rotation away your home medical school. In addition, we have our usual laughs along the way. Also, we play our own special med school edition of That Escalated Quickly, in which the crew give their creative answers to a prompt based on their secret numbers from 1 to 10, then an organizer try to rank those responses from lowest to highest intensity. It’s a game where thoughtful discussion and pandemonium hold hands! For the Glory KC is back with the 172nd episode of the show!Sporting Kansas City returned home and right back down to earth after picking up a win last week in Los Angeles. The Colorado Rapids, who lost to Sporting KC in both meetings last year, crushed KC in a 4-1 outcome at Sporting Park. The internet was aflame with John Pulskamp slander, so we get into how much was him and how much was others (after all, there is plenty of blame to go around). We also talked about the lineup changes (and if they were the right ones), the rash of injuries on the team and of course -- the brutal night of officiating from Fotis Bazakos. After the game, Jake Davis pulled me aside in the locker room with a message for the fans, so Sheena and I talk through our thoughts on his words.Outside of the game, Sporting KC had other things going on in the week, including the addition of Brazilian center back Diego Borges as their newest (and David Lee's first) U22 Initiative signing. Plus, there was a spattering of other news around the team, including their games being aired for free over the air on tape delay. To make matters worse, the KC Current also lost this weekend. That's something that only happened three times during the entire 2025 NWSL regular season and it's already happened just two games into the 2026 season. To make matters worse, it was against the Mallory Swanson-less Chicago Stars, who had just gotten shellacked 4-0 the week prior. I break down what I saw from that game.In the Digital Crawl, we hit on a few more topics, including:KC Comets making the playoffs!SKC II win a shootout but then lose the next gameMLS 2026 playoffs and 2027 schedule formats revealedAnd Antoine Griezman and Catarina Macaro are coming to MLS and NWSL respectivelyHere is a rundown of sections and start times:Sporting KC - 10:06KC Current - 1:01:46Digital Crawl - 1:15:09Upcoming GamesKC Current @ Seattle Reign - WED, March 25 at 8:00PM CTSKC II vs. St. Louis City 2 - SAT, March 28 at 2:00PMUSMNT vs Belgium - SAT, March 28 at 2:30PMKC Current @ Portland Thorns - SAT, March 28 at 3:00PM As a special gift to For the Glory KC listeners and KC Soccer Journal readers, Backheeled.com is giving away 30 days of their amazing, independent American soccer coverage for free. For the Glory KC is back with the 171st episode of the show!Sporting Kansas City did what it took much longer to do in the prior three seasons. They won a game. Sporting KC earned a 2-1 win over the LA Galaxy on the road last night in California. And they did it on the back of their superstar striker Dejan Joveljic and their newest players, Lasse Berg Johnsen.Dejan was the first on the scoreboard, after missing a golden chance to assist Calvin Harris on a goal just minutes earlier. On the game winning goal in the second half, Dejan was also involved. He first found Stephen Afrifa on a nice pace and then had eyes in the back of his head to dummy the ball to Berg Johnsen for the game winner.Lasse was making his debut, cleaned up a lot of the midfield and found a way to score. It was a quality start for a player looked at to be a big addition to the roster.Sheena and I talk through those moments, the 5-2-3 shape the team was in with Shapi Suleymanov at left wingback (!!!) and the other positions changes needed to make that move work. We also talk culture, Rapha Wicky, and the team starting to believe. We also discuss Sporting KC signing Capita Capemba and David Lee's promise to add at least one more player in the transfer window. The KC Current started the 2026 season with something they rarely had to do in 2025; come from behind. They pulled off a 2-1 victory as well, but they needed a comeback to pull it off. It also took a debut goal from Croix Bethune and Ally Sentnor scoring for the first time in KC Current colors. In the Digital Crawl, we hit on a few more topics, including:SKC II with an inauspicious start to the seasonNWSL's 18th team expected this yearHere is a rundown of topics and start times:Sheena's CPKC Experience - 1:11Sporting KC - 13:17KC Current - 52:12Digital Crawl - 1:07:31Upcoming GamesSporting KC vs Colorado Rapids, Saturday, March 21st at 7:30PM CTKC Current @ Chicago Stars, Sunday, March 22nd at 1:00PMSKC II vs. MNUFC2, Sunday, March 22nd at 7:00PM As a special gift to For the Glory KC listeners and KC Soccer Journal readers, Backheeled.com is giving away 30 days of their amazing, independent American soccer coverage for free. Prioritize Fun! What should you do with the time between getting accepted to medical school and orientation week? Fallon Jung, Anna Royer, Jonah Albrecht, and Charis Edwards tell their stories about finding out they got in (including one bathroom cry session and a Colorado NICU celebration), what they actually did to prepare, and why you absolutely shouldn’t pre-study. If you’re headed to medical school next year, this episode is basically your older sibling giving you the honest advice nobody else will—like why floor time in windowless study rooms is underrated, how to fill your cup before classes start, and the surprising truth about how much fun you can still have during M1 year. You’ll hear why these students think the admissions committee already believes you can do this, practical tips on setting up your study space without buying every resource known to humankind, and honest talk about mental health, sleep, and remembering why you wanted to be a doctor in the first place. The crew plus show photographer David Lee also debuts a med school edition of “That Escalated Quickly,” a party game that somehow involves electrocuting your brain’s pleasure centers, peppermint ice cream debates, and the very specific hell of planning to study instead of actually studying. Whether you’re pre-med, pre-PA, or just curious what actually happens before med school starts, hang out with your SCP friends who genuinely want you to succeed—and who aren’t afraid to laugh at themselves along the way. In this episode, host Kalie Moore talks with David Lee, Co-founder & CEO of Nex, to unpack one of the rarest success stories in modern gaming: launching a new consumer console and winning. In a category dominated for decades by the same three players, Nex broke through by rethinking who gaming hardware is for. Often compared to Wii or Kinect, Nex's real innovation isn't motion-based play alone, but a family-first platform built around physical activity, kid safety, and parental trust. In a down year for console sales, Nex sold over 650,000 units, expanded into thousands of retail stores, and captured meaningful market share by designing specifically for families. The conversation traces Nex's nearly decade-long journey from mobile-first products to a high-stakes pivot into living-room hardware - and the leadership decisions required to make that leap under uncertainty. Kalie and David dig into why most motion-gaming platforms struggled to last, what Nex designed differently for long-term engagement, and how retail, subscriptions, and trusted IP shaped its growth. The episode closes with a look ahead to Nex's 2026 roadmap, from international expansion to connected play designed with strict family controls, and David's long-term vision for what Nex could mean to families ten or twenty years from now. We'd like to thank Overwolf for making this episode possible! In an age where digital entertainment often leads to sedentary lifestyles, active gaming systems are emerging as a transformative solution for families seeking to combine fun with physical activity. These innovative platforms leverage technology to create engaging experiences that encourage movement, making them an ideal choice for parents looking to foster healthy habits in their children while also spending quality time together. One of the most exciting developments in this space is the Nex Playground, which was highlighted by David Lee, the company's CEO and Co-Founder.Bridging Fun and FitnessThe Nex Playground is an active play system designed specifically for kids and families, featuring a motion-tracking camera that connects to a television. This system not only provides entertainment but also promotes physical activity through a diverse library of games. With nearly 60 games available, families can choose from a variety of interactive experiences that challenge players to use their bodies in dynamic ways. The design of the Nex Playground is user-friendly, featuring a wide-angle camera that tracks movement within a 172-degree field, allowing for a comprehensive view of the living space. This capability enables the system to create immersive gaming experiences that respond to players' movements in real-time. For example, in games like "Fruit Ninja," players can slice virtual food with their body movements, while "Home Run Heroes" allows them to swing a bat to hit virtual baseballs. Such games not only entertain but also encourage kids to be active, promoting cardiovascular health and coordination.Active Gaming System for FamiliesOne of the standout features of active gaming systems like Nex Playground is their ability to engage families in a shared experience. Unlike traditional video games that often isolate players, these active systems can require participation from all members of the family. Parents can join in on the fun, making it a bonding activity that fosters teamwork and communication. The interactive nature of these games also helps to develop essential motor skills in children, as they learn to navigate their physical space while engaging with the digital world.Affordability is another significant aspect of the Nex Playground. With an annual subscription of $89, families gain access to an extensive library of games, with new titles added regularly. This model not only provides value but also ensures that families have a continuous stream of fresh content to keep them engaged. The inclusion of beloved intellectual properties (IPs) from companies like Hasbro and NBCUniversal further enhances the appeal, as families can enjoy games featuring characters and themes they already love. Moreover, the simplicity of the system's interface, which includes a basic remote control for navigation, ensures that users of all ages can easily access and enjoy the games. This accessibility is crucial in making active gaming systems a viable option for families, as it reduces the barriers to entry often associated with more complex gaming platforms.ConclusionIn conclusion, active gaming systems like the Nex Playground represent a significant advancement in family entertainment, combining the thrill of gaming with the benefits of physical activity. By encouraging movement, fostering family interaction, and providing a diverse range of engaging games, these systems help families create lasting memories while promoting healthier lifestyles. As technology continues to evolve, it is likely that we will see even more innovative solutions that bring families together through active play, proving that gaming can indeed be a pathway to fitness and fun.You can get yours at Amazon, Walmart, Target, Best Buy, and Sam's Club.Interview by Don Baine, The Gadget Professor.Sponsored by: Get $5 to protect your credit card information online with Privacy.
