CardioNerds (Dr. Dan Ambinder and guest host, Dr. Pooja Prasad) join Dr. Donny Mattia from Phoenix Children's pediatric cardiology fellowship, Dr. Sri Nayak from the Mayo Clinic – Arizona adult cardiology fellowship, and Dr. Harrison VanDolah from the University of Arizona College of Medicine - Phoenix Med/Peds program for a sunrise hike of Piestewa Peak, followed by some coffee at Berdena's in Old Town Scottsdale (before the bachelorette parties arrive), then finally a stroll through the Phoenix Desert Botanical Gardens to discuss a thought-provoking case series full of clinical cardiology pearls. Expert commentary is provided by Dr. Tabitha Moe. Episode audio was edited by Dan Ambinder. They discuss the following case: Cardiology is consulted by the OB team for a 27-year-old female G1, now P1, who has just delivered a healthy baby boy at 34 weeks gestation after going into premature labor. She is experiencing shortness of breath and is found to have a significant past cardiac history, including atrial fibrillation and preexcitation, now with a pacemaker and intracardiac defibrillator. We review the differential diagnosis for peripartum cardiomyopathy (PPCM) and then combine findings from her infant son, who is seen by our pediatric cardiology colleagues and is found to have severe hypertrophic cardiomyopathy (HCM). Genetic testing for both ultimately reveals a LAMP2 mutation consistent with Danon Disease. The case discussion focuses on the differential diagnosis for PPCM, HCM, pearls on Danon Disease and other HCM “phenocopies,” and the importance of good history. Be on the lookout for features of non-sarcomeric HCM – as Dr. Michelle Kittleson said in Episode 166, “LVH plus” states. HCM with preexcitation, heart block, strong family history, or extracardiac symptoms such as peripheral neuropathy, myopathy, or cognitive impairment should be evaluated for infiltrative/inherited cardiomyopathies! As an X-linked dominant disorder, Danon disease will present differently in males vs females, with males having much more severe and earlier onset disease with extracardiac features. Making the diagnosis for genetic disorders such as Danon disease is important for getting the rest of family members tested as well as the opportunity for specialized treatments such as gene therapy Up to 5% of Danon disease cases may be due to copy number variants, which may be missed in genetic testing that does not do targeted deletion/duplication analysis!). Notes What is the differential diagnosis for peripartum cardiomyopathy? Peripartum cardiomyopathy is a diagnosis of exclusion – we must exclude other possible etiologies of heart failure! First, ensure that you are not missing an acute life-threatening etiology of acute decompensated heart failure – pulmonary embolism, amniotic fluid embolism, ACS, and SCAD should all be ruled out. Second, a careful history can identify underlying heart disease or risk factors for the development of heart failure, such as substance use, high-risk behaviors that put one at risk for HIV infection, and family history that suggests an inheritable cardiomyopathy. Lastly, a careful review of echocardiographic imaging may also identify underlying etiologies that warrant a change in management. Diagnosis of peripartum cardiomyopathy is important to consider as within 7 days of onset, patients may be eligible for treatment with bromocriptine – consider referring ...

On July 29, 1994, 21-year-old Angela Maher was killed in a crash while going to pick up her friend from Old Town Scottsdale as the designated driver. The woman who hit and killed her, 31-year-old Gloria Schulze, was charged with manslaughter and endangerment after admitting she had been drinking and smoking weed. But after her first court appearance, Schulze vanished and was never seen again. Now, 30 years later, the fugitive was unexpectedly found and questions about where she fled and how she lived since are finally answered, officially closing the case.

Peter Volny was a kid in Australia who had a passion for cars. Skipping college to try his career as a race car driver may not have lasted long, but that passion and love for cars continued to grow. Josh Hebert is a well-known figure in the Arizona culinary world. Before owning his own restaurants he worked in a variety of premiere kitchens across the central valley, including North at Kierland. Several years ago he opened his first restaurant, Posh Kitchen in Old Town Scottsdale. Posh was an innovative omakase style fine dining establishment, creating a plethora of creative dishes. Eventually Josh moved on from Posh and created his current restaurant, Hot Noodles, Cold Sake. Hot Noodles, Cold Sake is a ramen restaurant in northern Scottsdale. This small cozy spot makes some of Arizona's best traditional ramen. Their house specialty, the Goma sesame ramen, is absolutely delicious. Their more American versions include chicken and seafood ramens that are just as tasty.

Jill: It popped up- Jack: You crack me up with this stuff. Jill: I know. It popped up. I was looking for a long term rental. Jack: I have no control over my life at all. Jill: Here at the beach. I didn't realize it. I'm like this place looks kind of good. I'm zeroing in on the address and I'm like, oh this is going to be really bad. You can't do that. Could you [imagine 00:00:43] if we live- who would live above a bar, number one, and number two, that can't be good if you hang out there. Baby I'll be downstairs. That's never going to work. You never make it upstairs. Jack: Pretty much plan on going to sleep around three in the morning every morning. Jill: Yeah. Right. Then on the weekends, even if you're trying to get a good night sleep, that's not going to happen. Jack: Our home based office is in Scottsdale Arizona. Our house is there and all that stuff. Jill and I are going back and forth from California to Scottsdale and we're trying to find a place. It's pretty funny. This little journey about finding a place in itself, from real estate standpoint, is pretty darn funny. Jill: It is. It's hilarious. You know what it makes me think of too, you and I once a long time ago looked at a place in Old Town Scottsdale, if you remember that. It was a new remodeled building way up high. It was like on the third floor, remember that? Jack: Yeah. Mm-hmm (affirmative) Jill: We looked at this going, oh how cool would that be? We could live there. Then we realize oh no. Every one of our friends would be on our couch every weekend. We'd never have any- we're like no that's not going to work. Jack: Right. Jill: That's my funny thing. Jack: I'm in the process of buying a fairly large boat. We're using the same way that we buy property. We're sending a letter to everybody who owns one and offering way less than even half of what it's worth. If you don't know this, every single boat is for sale. The owner may not know it, but it is for sale. Jill: Yes. Jack: Anyway. I'm not telling our friends about that either. Jill: No don't. Jack: I'm going to take a picture of a rotting wooden boat somewhere and say, yeah come on stay on our boat. Jill: That's what we decided we're going to do. Jack: Don't tell anyone. Jill: We're going to show them a horrible awful picture of this beat up, junky, diesel smelling- Jack: Yes. Exactly. Jill: They're going to be like, "No we're all good. “What are you supposed to do … do the same thing your whole life until you croak? I'm not doing that.” Grant Woods is talking about his second act. The former Arizona Attorney General is getting in touch with his creative side. After spending years in the courtroom as a successful trial attorney, he is now spending time writing and creating music. “I think that is one of the downfalls of a civilization, if you stop respecting or learning from music, painting, dance, theater, or film.” Woods started writing a diverse collection of songs and attracting well known singers. Then it turned into something more than music. Grant's collection of music turned into “The Project”. Woods was encouraged by some to take his mix of blues, rock, jazz, country and folk music and make an album in NYC or LA. Instead, he stayed true to his roots, reaching out to musicians in Arizona and compiling a body of work to benefit The Arizona School For The Arts. Woods likes to tell students, “You live in a place where we still value the arts. We respect the arts. If you respect the arts that means you respect each other. We are not a narrow minded bigoted place.” “Not just in Arizona but across the country we see the arts devalued,” Woods told us when he visited our studios in Old Town Scottsdale. Music and the arts are two of his great passions — politics is another. Woods has been deeply involved in politics for a long time. He has a close relationship with Senator John McCain, having served as his Chief Of Staff when McCain was a Congressman. “I've been a Republican my whole life and I guess for the last 20 years or so I wonder why,” he says. “We don't attract the best and brightest anymore. It is a bloodsport. Normal people, very talented people who should be running the country have no interest because who needs it.” Woods advocates for less government but says when government does do something, they should do it well. Citing public education, infrastructure, national defense as examples. We asked for his reaction to comments made by Donald Trump questioning whether or not Senator McCain is a war hero. “He was totally dismissive of it,” Woods says of his phone call with McCain when the controversy broke this past summer, “But the rest of us were pretty pissed. When you start questioning John McCain's heroism, that is offensive to me.” During our 20 minute Podcast, Woods talked about how he put the exploration of his creative self on hold earlier in his life so that he could focus on courtroom litigation and politics. “Politics is almost antithetical to the creative process,” he says, “negative, confrontational, and combative — that is not great space for you to be able to create.” About that second act he says, “If you've had success in the first act, it is a challenge to start at the bottom.” On the other hand, if you get yourself out of the box, who knows what you can create.
