Transcription / Notes THE SHELF LIFE OF THE IRANIAN WAR Hello, this is Darrell Castle with today's Castle Report. This is Friday the 13th day of March in the year of our Lord 2026. Once again my beat is war and Friday the 13th seems like a good day to talk about something so unpleasant. I will be talking about the fact, proven over centuries, that war is easy to start but hard to get out of and if one gets out the continuing effects exist long into the future. Yes, this is Friday the 13th once again and we are only two days from the Ides of March which was the date of Julius Ceasar's assassination on 15 March 44 BC. He was born in 100 BC so by my rough calculations that would make him 2126 years old today. Why talk about Ceasar more than 2000 years after his death, because he has been the gold standard for leaders who became emperors since then. Emperors in Rome, for example, continued to be called Ceasar after his death and today we ask as did Shakespear, upon what meat doth this our Ceasar feed that he has grown so strong. Our Ceasar today has certainly grown strong. But I submit that his meat is money and especially oil. Reports say that the U.S. has borrowed more than $50 billion per month for the last 5 months and that this war is costing in excess of $1 billion per day. The more important problem is oil and how much does it cost. The price of oil at the pump for the American consumer is what will determine if the U.S. can stay at war indefinitely or will have to declare victory and come home. It's really hard to just come home when you have demanded unconditional surrender in an undeclared war. Gas prices have accelerated but there are still a few desperation moves to temporarily moderate them. For the average American rising gas prices mean lowered standard of living because the real effect is similar to a cut in pay. The employer doesn't increase wages because it costs more to get to work so you have a lower standard of living and lowered optimism. Oil effects virtually everything price wise because everything has to be made or harvested and transported and that drives up the cost of living for each American. Iran is a major source of oil for the world or at least it was. Iran was a major supplier of oil to China with about 45% of China's 11 million barrels per day of imports passing through the Gulf and the Strait of Hormuz. That Strait is now closed by Iranian mines and anti-ship missiles. The administration has made it clear that the Iranian navy and air force are no more so this closing is a little hard to understand. Reports stated that the U.S. Navy sank 10 or 11 mine layers earlier this week so my question is why not sink them before they laid the mines. The end of Iranian oil has created opportunities for others to step up production which I imagine has made Russia and Vladimir Putin quite happy. The world price of oil is rising rapidly which puts money into Rusia for domestic use and to prosecute the Ukrainian war. Trump is very aware of the effect of rising oil prices on his poll numbers and on the upcoming mid-term elections so he has taken steps to alleviate it. He and Mr. Putin had what was described as a very good call on the 9th and Russia has been freed from sanctions to sell its oil on the world market. It will be a little higher in price for China than the cheap Iranian oil but nevertheless it will help. Trump is also freeing up several million barrels from the strategic petroleum reserve something he criticized Biden for doing to help his poll numbers. China has to be hurting from all this war and oil strangulation because reports are that about 90% of Chinese oil came from Venezuela, Iran and Russia and now only Russia is left. Perhaps telling China that it will no longer get energy from the Western Hemisphere or from U.S. allies is the point of the war. That is just speculation on my part but with the attack on the cartels in collaboration with other South and Latin American countries it makes sense. Rising energy prices will be hard for China's export driven economy to absorb because it makes all their exports less affordable worldwide. Trump is scheduled to travel to China later this month to meet with Xi Jinping so we'll see how that goes. So, as the title of this Castle Report suggests the U.S. and Israel need to end the war before prices in the U.S. rise so much they effect public support for the administration. There are many events from war that can be hidden or censored from the public but it is hard to hide price increases at the pump. Casualties can be hidden to a certain extent for example getting real casualty figures has been close to impossible so far. We learned this week that at least 8 Americans and today 4 crewmen in a refueling tanker died so 12 are now dead and about 150 wounded. Some of those are gravely wounded with brain injuries and burns and may not recover. Those numbers have been censored but they leak out so accurate or not, we can't be certain. The other thing that is easier to censor from Americans is damage to infrastructure and casualty figures from others targeted because they host Americans. The U.S. bases and embassies in several of the Gulf States including Saudi Arabia have been seriously damaged by missile and drone attacks. In addition, the attacks have hit infrastructure and civilians which have nothing to do with Americans. These problems are much harder to solve because no one seems to know who is currently in charge in Iran. The Supreme Leader is dead and his son was appointed in his place. He might be dead or wounded but for some reason he seems to no longer be in charge. The very religious and new Islamic supreme leader apparently owns about $140 million of prime real estate in London's billionaire row. Does he control the military, the Iranian Revolutionary Guard Corps or (IRGC) and if not who does. There are indications that no one does and the IRGC is acting on orders of individual officers. That would explain why the new leader apologizes to the Arab states which Iran attacked and said it was a mistake which would not be repeated. The attacks continued after his apology as if it had not been made. The U.S. cost of living and that of the world for that matter is why the Strait of Hormuz seems likely to be the deciding point of the length of U.S. involvement. A war of conquest with a ground invasion of Iran would probably be long, bloody, and very expensive and therefore can't happen. Oh, wait I'm sorry I mean it can't happen if there are rational, non-insane people making the decisions. The key to the war, then, is the battle to keep the Strait open and with it the flow of oil. Air power can probably destroy the IRGC and that should make it somewhat easier, but we have known since the end of World War ll that air power alone will not make determined people give up their homeland. The options then seem to me to be destroy the IRGC very quickly, which may or may not be possible, somehow get a new regime into power which can control them and which also may not be possible, or just fight it out however long it takes in the Strait. I guess some combination of all three is also possible. I am certain that Trump wants out of this mess, but I doubt if the Israelis do. He held a meeting this week with the heads of various defense firms and told them to ramp up production of missiles both cruise and Patriot air defense missiles so the U.S. is running low. The Iranians answer by making 100,000 suicide drones at $20,000 each instead of the multi-million missiles to attempt to shoot them down. The Iranian tactic seems to be the rope-a-dope whereby the U.S. fires all its missiles then the Kamikaze drones counterattack. People tried to warn Trump but apparently he wasn't listening or had bad advisers as well. My own view, and this is pure speculation on my part, is that Trump believes that World Warlll has already started but it is a cold war right now pitting many forces against each other. We have what Reagan called the axis of evil consisting of China, Russia, Venezuela, Iran and North Korea. Two of those are gone and the rest are nuclear armed. Trump is apparently trying to cozy up to Putin which I imagine makes Xi a little nervous. Then we have the forces of the European global elite as managed by the World Economic Forum and its great reset of global bureaucracy and its unelected world government. Opposing those we have the American way of individual nation states living in freedom so that seems like the battle to me. In conclusion, I remember the wise words of a man I once heard say that if I am proud of anything about my life I am proudest of my humility. Well, I echo that now and admit that I know very little firsthand just what logic and research produce so I use my best guess powered by some experience. I know from history that the Middle East contains a lot of people with long memories. Apparently they are still settling disputes that happened over 1000 years ago. Finally, folks, children don't start wars they just die in them. It is a terrible thing to go to war and this one seems especially brutal. It appears that this attack was unprovoked and that makes it even sadder. I pray that it ends soon. We then take a deeper look at the idea of 'oaths' or 'swearing by something'. Jesus says that this comes from the evil one and we show that oaths are manipulative and undermine your credibility. Als jong musicus uit Halle in Saksen-Anhalt werd hij al bejubeld in Rome en Venetië. Hij reisde langs alle hoven en concertzalen en belandde in Londen, waar hij de beroemdste componist, organist én kunstpaus werd, ook die voor het koningshuis. Het koninklijk publiek verwachtte dan ook opera's met politieke boodschappen en portretten uit verhalen uit alle eeuwen en windstreken. En Händel leverde. Van stukken uit het oude Rome, Perzië, Mongolië en tot muziektheater dat speelt in de diepste middeleeuwen, aan de Olympus en vertelt uit ridderromans tijdens de Kruistochten. Jakub Józef Orliński levert niet minder. Hij vertelt over zijn visie op 'Cesare' als veroveraar en uitbuiter van het onmetelijk rijke Egypte. Ze sluiten een deal, alsof ze onderhandelen in Davos of Mar-a-Lago. Hij maakt haar Farao, in plaats van haar gemene broer. Zij baart hem de opvolger die hem ontbrak. Orlínski zong in januari bij de Nationale Opera nog een heel andere rol, die van de naïeve prins Athamas. Bij zijn feestelijke royal wedding slaan de wrede goden toe. Of zijn dat de spookbeelden van zijn bruid Semele? Is haar dromen van oppergod Jupiter als haar ware echtgenoot niet ook weer zo'n politiek fata morgana? Zo'n droom van tomeloze macht om te ontsnappen aan dit huwelijk? Desnoods een waaraan zijzelf en de brave Athamas ten onder gaan? Bij Händel draait het steeds weer om macht, wellust, sluwheid en de hoop op oprechte menselijkheid. Zijn opera's worden bevolkt door Poetins, Von der Leyens, Ruttes, Jettens en Wilders uit alle eeuwen. Orlínski droomt van sommige van die rollen voor zijn komende carrière, vertelt hij. Het gesprek met de Poolse barokster gaat over zijn avonturen op het wereldtoneel. De opening van de Olympiade in Parijs van 2024 bijvoorbeeld. Hoe was het om als verrassende breakdancer annex zanger twee miljard kijkers te veroveren, alsof hijzelf een Caesar van de muziek was? Wat maakt dat hij zo dol is op Amsterdam, maar niettemin één dagelijkse ergernis heeft? Orlínski blijkt een zanger die zijn vak buitengewoon professioneel uitdiept. "Ik heb wel zeven lange termijnprojecten voor mezelf en gezelschappen en operahuizen die ik zorgvuldig probeer uit te bouwen." Zo ontstond ook de tournee en straks in Warschau de opname van 'Giulio Cesare', een gedurfde stap. Maar hij noemt zich ook ‘een soort archeoloog’. Met zijn researcher speurt hij in archieven naar verloren gewaande muziek, opera's waarvan we alleen nog de naam kennen. Eén ontdekking van een vergeten stuk is zo'n toekomstproject dat hij hoopt te lanceren als nieuwe opera op het wereldrepertoire. Händels work is deeply political, already 300 years. Händels operas show how rulers dominate their age and world, how to be masters of wisdom, mildness and justice. But no less how cruel and merciless they can be, how they do not shrink to destroy people, fool and ruin them. These weeks Amsterdam is 'the George Frederic Handel capital of Europe'. Most of all thanks to the spectacular musical debut of a young global Baroque-star who will sing such a cunning, cruel and powerful ruler. Julius Caesar, no less. With worldwide recognized Polish countertenor, Jakub Józef Orliński, the hosts of the podcast 'Trusted Sources' look for the many layers of the fabulous music of Händel (1685 - 1759). His "Giulio Cesare in Eigtto' is an opera as a diplomatic summit of two political animals of the Ancient World, Cleopatra and Ceasar. Händels operas again and again adress the dilemmas of power, cruelty, lust, mercy and hope for humanity. They are filled with Putins, Melania's, Berlusconi's, Ursula von der Leyens, with you and me. Talking with Orlínski is sharing wonderful stories of his adventures on a world stage. His live role at the Olympics in Paris, watched by an audience of billions around the world. But getting soaking wet there while breakdancing through a French Renaissance aria. His bike in Amsterdam. His ambitions as "almost an archeologist of music", going through archives to dig up unknown diamonds of Baroque music, forgotten over 300 years. La prévention des accidents Grand nombre de ces accidents de la main sont évitables. Des dispositifs de prévention (gants, carters, écrans, détecteurs...) et notices d'informations existent, tant dans le cadre professionnel, que dans le cadre domestique avec, par exemple, les mises en garde pour un certain nombre d'outils électriques ou de produits chimiques. La fatigue ou l'empressement peuvent conduire à négliger ces mesures ou dispositifs et constituent autant de facteurs de risque pour l'intégrité des mains. En cas d'accident, il est important de s'adresser à un service d'urgence dédié si possible et d'orienter le patient vers des services d‘accompagnement et de réadaptation, pour faire face aux séquelles éventuelles, en limiter l'impact tant fonctionnel que psychologique. Romans Overview and Chapter Focus Review of Romans chapters 1–11: Paul presented biblical/theological foundations for salvation. Chapter 12: Practical application—responsibility to God and society. Chapter 13 (today's focus): Responsibility to authority and neighbors. The Issue of Authority Authority is a challenging topic in modern times. Emphasizes a biblical rather than natural or emotional perspective on authority. Jesus' Teaching: Give to Caesar, Give to God Story from the Gospels (Matthew, Mark, Luke): Religious leaders try to trap Jesus with a question about paying taxes to Caesar. Jesus' response: Give Caesar what belongs to Caesar and give to God what belongs to God. The intent was to expose their motives and teach about priorities. Romans 13:1–10—Responsibility to Authority God's Established Order (Verses 1–4) God has instituted human governments for societal order and protection from wrongdoing. Government is meant to be a servant for people's good and well-being. Calling vs. Command: Respect and Cooperation Christians are called to respect and cooperate with government, not to unconditional obedience. The Greek word for "submit" implies voluntary cooperation. When state laws conflict with God's commands, believers must obey God's word (Acts 4–5). Importance of interpreting scripture in context. The Nature of Authority (Verses 5–7) All authority comes from God, whether we understand or like it. God places people in authority to fulfill larger purposes (examples: Pharaoh, Nebuchadnezzar, Cyrus, Herod). Followers are called to submit for the Lord's sake and pray for those in authority (1 Peter 2:13, 1 Timothy 2:1–2). Accountability for Leaders and Followers Leaders are expected to rule justly and will be held accountable by God. Followers must live righteously; everyone is responsible for their own choices. Example: Roman Centurion's Understanding of Authority Roman centurion understood both earthly and divine authority. Authority flows from a higher source: God. Jesus commended the centurion's great faith. Love as the Divine Motivator (Verses 8–10) Love is the solution to authority issues. Ongoing debt of love ("let your only outstanding debt be your debt of love"). John 3:16 cited as the ultimate example of divine love. Greatest commandments: Love God and love your neighbor. Love fulfills the law and breaks the chains of authority issues. Conclusion and Application Culture distorts concepts like authority and love; scripture renews perspective. God's authority is ultimate; believers are called to please and live for Him. Prayer that misunderstandings and misconceptions would be resolved through relationship with God. Encouragement to let Christ's image be formed in us, to be diligent, light in darkness, and to be salt in the world.

Visit your primary care provider for additional medical advice.IntroWelcome to episode 203 of Rio Bravo qWeek, your weekly dose of knowledge.Today, we're tackling one of the biggest health challenges of our time: opioid use disorder, or OUD. Nearly every community in America has been touched by it: families, friends, even healthcare providers themselves. For decades, treatment has been surrounded by barriers, painful withdrawals, stigma, and strict rules that often do more harm than good. Too many people who need help never make it past those walls. But here's the hopeful part, new approaches are rewriting the story. They are less about rigid rules and more about meeting people where they are. Two of the most promising strategies for treatment of OUD are buprenorphine microinduction and harm reduction. Let's learn why these two connected strategies could change the future of addiction recovery. Background information of treatment: The X-waiver (short for DATA 2000 waiver) was a special DEA requirement for prescribing buprenorphine for opioid use disorder. Doctors used to take extra training (8 hours) and apply for it. Then, they could prescribe buprenorphine to a very limited number of patients. The X-waiverhelped regulate buprenorphine but also created barriers to access treatment to OUD. It was eliminated in January 2023 and now all clinicians with a standard DEA registration no longer need a waiver to prescribe buprenorphine for OUD. Why buprenorphine?Buprenorphine is one of the safest and most effective medications for opioid use disorder. It has some key attributes that make it both therapeutic and extremely safe: 1) As a partial agonist at mu-opioid receptors, it binds and provides enough partial stimulation to prevent cravings and withdrawal symptoms without producing strong euphoria associated with full agonists. 2) Because it has a strong binding affinity compared to full agonists, it easily displaces other opioids that may be occupying the receptor. 3) As an antagonist at kappa-opioid receptors, it contributes to improved mood and reduced stress-induced cravings. 4) The “ceiling effect”: increasing the dosage past a certain point does not produce a stronger opioid effect. This ceiling effect reduces the risk of respiratory depression and overdose, making it a safer option than full agonists. 5) It also had mild analgesic effects, reducing pain. 6) Long duration of action: The strong binding affinity and slow dissociation from the mu-opioid receptor are responsible for buprenorphine's long half-life of 24–60 hours. This prolonged action allows for once-daily dosing in medication-assisted treatment for OUD. Induction vs microinduction:The problem is, starting it—what's called “induction”—can be really tough. Patients usually need to stop opioids and go through a period of withdrawal first. Drugs like fentanyl, which can cause precipitated withdrawal —a sudden, severe crash may push people back to using opioids. Because buprenorphine binds so tightly to the mu-opioid receptor, it can displace other opioids, such as heroin or methadone. If buprenorphine is taken while a person still has other opioids in their system, it can trigger sudden and severe withdrawal symptoms.Opioid withdrawal sign sand symptoms:Opioid withdrawal symptoms are very uncomfortable; patients may even get aggressive during withdrawals. As a provider, once you meet one of these patients you never forget how uncomfortable and nasty they can be. The symptoms are lacrimation or rhinorrhea, piloerection "goose flesh," myalgia, diarrhea, nausea/vomiting, pupillary dilation, photophobia, insomnia, autonomic hyperactivity (tachypnea, hyperreflexia, tachycardia, sweating, hypertension, hyperthermia), and yawning. Think about all the symptoms you run for COWS (Clinical Opiate Withdrawal Scale). It is estimated 85 % of opioid-using patients who inject drugs (PWID) reported opioid withdrawal. Fortunately, even though opioid withdrawal is very uncomfortable, it is not life-threatening (unlike alcohol or benzodiazepine withdrawal, which can be fatal).Many patients who start the journey treating opioid use disorder experience “bumps in the road” --they avoid treatment or drop out early. What is Microinduction? Microinduction is a fairly new strategy started in Switzerland around 2016. It is also known as the “Bernese method” (named after the city of Bern, Switzerland). With this method, instead of stopping opioids cold turkey, patients start with tiny doses of buprenorphine—fractions of a milligram. These doses gradually increase over several days while the patient continues their regular opioid use. While they begin this titer, they can continue use of the full agonist they were previously using–methadone, fentanyl, or heroin, while the buprenorphine begins to take effect. Once the buprenorphine builds up to a therapeutic level, the full agonist is stopped. This method uses buprenorphine's unique pharmacology to stabilize the brain's opioid system without triggering those really nasty withdrawal symptoms.Early studies and case reports suggest this is safe, tolerable, and effective method to do. Microinduction is changing the game, and it has been spreading quickly in North America. Instead of forcing patients to stop opioids completely, the dose is slowly increased over the next three to seven days, while the patient keeps using their usual opioids.By the end of that week, the buprenorphine has built up to a therapeutic level and the full agonist is stopped. The difference is really dramatic. Instead of a painful crash into withdrawal, patients describe the process as a gentle step down, or a ramp instead of a cliff. It's a flexible method. It can be done in a hospital, a clinic, or even outpatient with good follow-up. Once a patient and doctor develop a strong relationship built on the principles of patient autonomy and patient-centered care, microinduction can be closely monitored on a monthly basis including televisits. Microinduction has been shown to help more patients stay in treatment. The Role of Harm Reduction Instead of demanding perfection, harm reduction focuses on best practices providers can implement to reduce risk and keep patients safe. Harm reduction can vary from providing naloxone to reverse overdoses, giving out clean syringes, or offering safer injection education. It also means allowing patients to stay in treatment even if they keep using other substances, and tailoring care for groups like adolescents, parents, or people recently released from incarceration. Harm reduction says that instead of demanding perfection, let's focus on progress. Instead of all-or-nothing, let's devote resources to keeping people alive and safe. As mentioned,an option is providing naloxone kits so overdoses can be reversed in the moment. Also, giving out clean syringes so the risk of HIV or hepatitis infection is reduced while injecting heroin. Another way to reduce harm is teaching safer injection practices so people can protect themselves until they're ready for that next step in their treatment. It also means keeping the doors open, even when patients slip. If someone is still using other substances, they still deserve care. And it means tailoring support for groups who oftentimes get left behind. For people like adolescents, parents balancing childcare, or people coming out of incarceration who are at the highest risk of overdose. Harm reduction recognizes that recovery isn't a straight line. It's about meeting people where they are and walking with them forward. Conclusion:Microinduction is itself a harm reduction strategy. It lowers barriers by removing the need for painful withdrawal.When paired with a harm reduction culture in clinics, patients are more likely to enter care, stay engaged, and build trust with doctors for continued care. Managing opioid use disorder is one of the greatest health challenges of our time. But solutions like buprenorphine microinduction and harm reduction strategies are reshaping treatment—making it safer, more humane, and more accessible. If we embrace these approaches, we can turn barriers into bridges and help more people find recovery. Even without trying, every night you go to bed a little wiser.
